    Angioplastie coronaire percutanée chez la femme : particularités cliniques, procédurales et pronostiques

    AbstractX Les résultats de l´angioplastie transluminale (ATL) des coronaires chez la femme ont été pendant de nombreuses années controversés et longtemps considérés comme moins favorable que chez l´homme. Le but de notre travail était d’évaluer les caractéristiques de l’angioplastie coronaire chez la femme, ainsi que les résultats immédiats et à moyen terme et de les comparer à ceux chez l’homme. Nous avons comparé 200 patients dont 100 femmes, ayant bénéficié d’une angioplastie coronaire, colligés au service de cardiologie de l’hôpital Hédi Chaker de Sfax entre 2002 et 2007. Les femmes étaient significativement plus âgées que les hommes. La comparaison de la fréquence des facteurs de risque d’athérosclérose chez les deux sexes, a permis de noter une co-morbidité franchement plus importante chez la femme, avec une différence statiquement significative. L’ATL a été plus motivée chez la femme devant un angor stable sévère (p<0,05) et un SCA ST- (p=NS). Les femmes avaient plus d’atteinte polytronculaire (p<0,05), d’atteintes des segments moyens et distaux (p<0,05) et plus des lésions serrées (p=NS), longues et calcifiées (p<0,05). 259 stents ont été déployés, un stenting direct était plus fréquent dans la population féminine (p<0,05). Le diamètre des artères féminines, assimilé à celui du stent et/ou ballon utilisé, a été significativement moins important que celui des hommes. La longueur des stents et/ou ballon utilisés, ainsi que les pressions de larguage des stents ont été plus importantes chez la population féminine (p<0,05). Le succès angiographique global a été obtenu chez 94% de la population générale, sans différence significative entre les deux sexes. Les évènements cardiaques majeurs (MACE) hospitaliers ont été plus fréquents chez la femme (p=0,05). Après un suivi moyen de 31 mois, le taux de MACE global a été significativement plus important chez la femme (39% vs. 28%, p<0,05), portant surtout sur la mortalité globale (13% vs. 3%, p<0,05). Le taux de resténose a été comparable chez les deux sexes. L’angioplastie coronaire constitue actuellement un moyen thérapeutique efficace et sur chez la femme, au prix de complications plus fréquentes.Key words: Angioplastie coronaire percutanée, syndrome coronarien aigu, sexes, mortalité, Tunisi

    Kinetic modelling of methanol synthesis over commercial catalysts: A critical assessment

    Kinetic modelling of methanol synthesis over commercial catalysts is of high importance for reactor and process design. Literature kinetic models were implemented and systematically discussed against a newly developed kinetic model based on published kinetic data. Deviations in the sensitivities of the kinetic models were explained by means of the experimentally covered parameter range. The simulation results proved that an extrapolation of the working range of the kinetic models can lead towards significant simulation errors especially with regard to pressure, stoichiometric number and CO/CO2_{2}-ratio considerably limiting the applicability of kinetic models frequently applied in scientific literature. Therefore, the validated data range for kinetic models should be considered when detailed reactor simulations are carried out. With regard to Power-to-Methanol processes special attention should be drawn towards the rate limiting effect of water at high CO2_{2} contents in the syngas. Moreover, it was shown that kinetic models based on data measured over outdated catalysts show significantly lower activity than those derived from state-of-the-art catalysts and should therefore be applied with caution for reactor and process simulations. The plausible behavior of the herein proposed kinetic model was demonstrated by a systematic comparison towards established kinetic approaches within both, an ideal kinetic reactor and an industrial steam cooled tubular reactor. Relative to the state-of-the-art kinetic models it was proven that the herein proposed kinetic model can be applied over the complete industrially relevant working range for methanol synthesis

    Electrosprayed shrimp and mushroom nanochitins on cellulose tissue for skin contact application

    Cosmetics has recently focused on biobased skin-compatible materials. Materials from natural sources can be used to produce more sustainable skin contact products with enhanced bioactivity. Surface functionalization using natural-based nano/microparticles is thus a subject of study, aimed at better understanding the skin compatibility of many biopolymers also deriving from biowaste. This research investigated electrospray as a method for surface modification of cellulose tissues with chitin nanofibrils (CNs) using two different sources—namely, vegetable (i.e., from fungi), and animal (from crustaceans)—and different solvent systems to obtain a biobased and skin-compatible product. The surface of cellulose tissues was uniformly decorated with electrosprayed CNs. Biological analysis revealed that all treated samples were suitable for skin applications since human dermal keratinocytes (i.e., HaCaT cells) successfully adhered to the processed tissues and were viable after being in contact with released substances in culture media. These results indicate that the use of solvents did not affect the final cytocompatibility due to their effective evaporation during the electrospray process. Such treatments did not also affect the characteristics of cellulose; in addition, they showed promising anti-inflammatory and indirect antimicrobial activity toward dermal keratinocytes in vitro. Specifically, cellulosic substrates decorated with nanochitins from shrimp showed strong immunomodulatory activity by first upregulating then downregulating the pro-inflammatory cytokines, whereas nanochitins from mushrooms displayed an overall anti-inflammatory activity via a slight decrement of the pro-inflammatory cytokines and increment of the anti-inflammatory marker.     Liquid and solid functional bio-based coatings

    The development of new bio-based coating materials to be applied on cellulosic and plastic based substrates, with improved performances compared to currently available products and at the same time with improved sustainable end of life options, is a challenge of our times. Enabling cellulose or bioplastics with proper functional coatings, based on biopolymer and functional materials deriving from agro-food waste streams, will improve their performance, allowing them to effectively replace fossil products in the personal care, tableware and food packaging sectors. To achieve these challenging objectives some molecules can be used in wet or solid coating formulations, e.g., cutin as a hydrophobic water-and grease-repellent coating, polysaccharides such as chitosan-chitin as an antimicrobial coating, and proteins as a gas barrier. This review collects the available knowledge on functional coatings with a focus on the raw materials used and methods of dispersion/application. It considers, in addition, the correlation with the desired final properties of the applied coatings, thus discussing their potential

    Splitting Arabic Texts into Elementary Discourse Units

    International audienceIn this article, we propose the first work that investigates the feasibility of Arabic discourse segmentation into elementary discourse units within the segmented discourse representation theory framework. We first describe our annotation scheme that defines a set of principles to guide the segmentation process. Two corpora have been annotated according to this scheme: elementary school textbooks and newspaper documents extracted from the syntactically annotated Arabic Treebank. Then, we propose a multiclass supervised learning approach that predicts nested units. Our approach uses a combination of punctuation, morphological, lexical, and shallow syntactic features. We investigate how each feature contributes to the learning process. We show that an extensive morphological analysis is crucial to achieve good results in both corpora. In addition, we show that adding chunks does not boost the performance of our system

    High Resolution Genotyping of Clinical Aspergillus flavus Isolates from India Using Microsatellites

    Contains fulltext : 124312.pdf (publisher's version ) (Open Access)BACKGROUND: Worldwide, Aspergillus flavus is the second leading cause of allergic, invasive and colonizing fungal diseases in humans. However, it is the most common species causing fungal rhinosinusitis and eye infections in tropical countries. Despite the growing challenges due to A. flavus, the molecular epidemiology of this fungus has not been well studied. We evaluated the use of microsatellites for high resolution genotyping of A. flavus from India and a possible connection between clinical presentation and genotype of the involved isolate. METHODOLOGY/PRINCIPAL FINDINGS: A panel of nine microsatellite markers were selected from the genome of A. flavus NRRL 3357. These markers were used to type 162 clinical isolates of A. flavus. All nine markers proved to be polymorphic displaying up to 33 alleles per marker. Thirteen isolates proved to be a mixture of different genotypes. Among the 149 pure isolates, 124 different genotypes could be recognized. The discriminatory power (D) for the individual markers ranged from 0.657 to 0.954. The D value of the panel of nine markers combined was 0.997. The multiplex multicolor approach was instrumental in rapid typing of a large number of isolates. There was no correlation between genotype and the clinical presentation of the infection. CONCLUSIONS/SIGNIFICANCE: There is a large genotypic diversity in clinical A. flavus isolates from India. The presence of more than one genotype in clinical samples illustrates the possibility that persons may be colonized by multiple genotypes and that any isolate from a clinical specimen is not necessarily the one actually causing infection.     A computational model of invasive aspergillosis in the lung and the role of iron

    BACKGROUND: Invasive aspergillosis is a severe infection of immunocompromised hosts, caused by the inhalation of the spores of the ubiquitous environmental molds of the Aspergillus genus. The innate immune response in this infection entails a series of complex and inter-related interactions between multiple recruited and resident cell populations with each other and with the fungal cell; in particular, iron is critical for fungal growth. RESULTS: A computational model of invasive aspergillosis is presented here; the model can be used as a rational hypothesis-generating tool to investigate host responses to this infection. Using a combination of laboratory data and published literature, an in silico model of a section of lung tissue was generated that includes an alveolar duct, adjacent capillaries, and surrounding lung parenchyma. The three-dimensional agent-based model integrates temporal events in fungal cells, epithelial cells, monocytes, and neutrophils after inhalation of spores with cellular dynamics at the tissue level, comprising part of the innate immune response. Iron levels in the blood and tissue play a key role in the fungus’ ability to grow, and the model includes iron recruitment and consumption by the different types of cells included. Parameter sensitivity analysis suggests the model is robust with respect to unvalidated parameters, and thus is a viable tool for an in silico investigation of invasive aspergillosis. CONCLUSIONS: Using laboratory data from a mouse model of invasive aspergillosis in the context of transient neutropenia as validation, the model predicted qualitatively similar time course changes in fungal burden, monocyte and neutrophil populations, and tissue iron levels. This model lays the groundwork for a multi-scale dynamic mathematical model of the immune response to Aspergillus species.
