Cecchi, Italy Introduction: Nowadays, pollinosis is affecting a large percentage of population in countries with a western life style. The existence of allergenic activity in the atmosphere is not only associated to pollen grains and fungal spores, but also to submicronic and paucimicronic biological particles. The origin of these allergens can be due to the rupture of pollen transported in the atmosphere or to the presence of allergens from other parts of the plant making amorphous material with an allergen load. Poaceae pollen is recognized as one of the main causes of allergic disease in all Europe. In this study we have tried to compare Poaceae pollen counts in the air and Phl p 5, one of the major allergens of this family, through the use of a high-volume cascade impactor (Chemvol). This study was done in the frame of the European project HIALINE and it compares the results obtained in 2009 by 4 different partners participating in this project: in Córdoba (Spain), Évora (Portugal), Lyon (France) and Parma (Italy). Methodology: Pollen grains were sampled using a 7-day volumetric Hirst type spore trap. Chemvol high-volume cascade impactor equipped with stages PM>10µm, 10 µm>PM>2.5µm was used for detecting aeroallergens. In each stage polyurethane filters were use as an impacting substrate. Phl p 5 allergen was determined using an allergen specific ELISA. Antibodies for analysis were delivered by Allergopharma Joachim Ganzer KG, the industrial partner in this project. At each location both samplers were placed side-by-side. Results: Most of the allergen was collected in the PM>10µm fraction. Similar profiles between airborne pollen and the total allergenic load was observed during the pollen season. A good correlation was obtained between pollen count and allergen content of the air and a value of 2.5 pg/pollen grain of Poaceae was estimated. Discussion: This is the first year of this project. Nevertheless, results suggest that the allergenic load in outdoor air might be mainly due to pollen bursts. It supports the hypothesis that monitoring the allergens itself in ambient air might be an improvement in allergen exposure assessment.     Airborne olive pollen counts are not representative of exposure to the major olive allergen Ole e 1

    Pollen is routinely monitored, but it is unknown whether pollen counts represent allergen exposure. We therefore simultaneously determined olive pollen and Ole e 1 in ambient air in C ordoba, Spain, and Evora, Portugal, using Hirst-type traps for pollen and high-volume cascade impactors for allergen. Pollen from different days released 12-fold different amounts of Ole e 1 per pollen (both locations P < 0.001). Average allergen release from pollen (pollen potency) was much higher in C ordoba (3.9 pg Ole e 1/pollen) than in Evora (0.8 pg Ole e 1/pollen, P = 0.004). Indeed, yearly olive pollen counts in C ordoba were 2.4 times higher than in Evora, but Ole e 1 concentrations were 7.6 times higher. When modeling the origin of the pollen, >40% of Ole e 1 exposure in Evora was explained by high-potency pollen originating from the south of Spain. Thus, olive pollen can vary substantially in allergen release, even though they are morphologically identical

    A global analysis of the comparability of winter chill models for fruit and nut trees

    Many fruit and nut trees must fulfill a chilling requirement to break their winter dormancy and resume normal growth in spring. Several models exist for quantifying winter chill, and growers and researchers often tacitly assume that the choice of model is not important and estimates of species chilling requirements are valid across growing regions. To test this assumption, Safe Winter Chill (the amount of winter chill that is exceeded in 90% of years) was calculated for 5,078 weather stations around the world, using the Dynamic Model [in Chill Portions (CP)], the Chilling Hours (CH) Model and the Utah Model [Utah Chill Units (UCU)]. Distributions of the ratios between different winter chill metrics were mapped on a global scale. These ratios should be constant if the models were strictly proportional. Ratios between winter chill metrics varied substantially, with the CH/CP ratio ranging between 0 and 34, the UCU/CP ratio between −155 and +20 and the UCU/CH ratio between −10 and +5. The models are thus not proportional, and chilling requirements determined in a given location may not be valid elsewhere. The Utah Model produced negative winter chill totals in many Subtropical regions, where it does not seem to be useful. Mean annual temperature and daily temperature range influenced all winter chill ratios, but explained only between 12 and 27% of the variation. Data on chilling requirements should always be amended with information on the location and experimental conditions of the study in which they were determined, ideally including site-specific conversion factors between winter chill models. This would greatly facilitate the transfer of such information across growing regions, and help prepare growers for the impact of climate change

    Threat of allergenic airborne grass pollen in Szczecin, NW Poland: the dynamics of pollen seasons, effect of meteorological variables and air pollution

    The dynamics of Poaceae pollen season, in particularly that of the Secale genus, in Szczecin (western Poland) 2004–2008 was analysed to establish a relationship between the meteorological variables, air pollution and the pollen count of the taxa studied. Consecutive phases during the pollen season were defined for each taxon (1, 2.5, 5, 25, 50, 75, 95, 97.5, 99% of annual total), and duration of the season was determined using the 98% method. On the basis of this analysis, the temporary differences in the dynamics of the seasons were most evident for Secale in 2005 and 2006 with the longest main pollen season (90% total pollen). The pollen season of Poaceae started the earliest in 2007, when thermal conditions were the most favourable. Correlation analysis with meteorological factors demonstrated that the relative humidity, mean and maximum air temperature, and rainfall were the factors influencing the average daily pollen concentrations in the atmosphere; also, the presence of air pollutants such as ozone, PM10 and SO2 was statistically related to the pollen count in the air.     Ragweed as an Example of Worldwide Allergen Expansion

    <p/> <p>Multiple factors are contributing to the expansion of ragweed on a worldwide scale. This review seeks to examine factors that may contribute to allergen expansion with reference to ragweed as a well-studied example. It is our hope that increased surveillance for new pollens in areas not previously affected and awareness of the influence the changing environment plays in allergic disease will lead to better outcomes in susceptible patients.</p

    Stress, ageing and their influence on functional, cellular and molecular aspects of the immune system

    The immune response is essential for keeping an organism healthy and for defending it from different types of pathogens. It is a complex system that consists of a large number of components performing different functions. The adequate and controlled interaction between these components is necessary for a robust and strong immune response. There are, however, many factors that interfere with the way the immune response functions. Stress and ageing now consistently appear in the literature as factors that act upon the immune system in the way that is often damaging. This review focuses on the role of stress and ageing in altering the robustness of the immune response first separately, and then simultaneously, discussing the effects that emerge from their interplay. The special focus is on the psychological stress and the impact that it has at different levels, from the whole system to the individual molecules, resulting in consequences for physical health

    Severe Asthma Standard-of-Care Background Medication Reduction With Benralizumab: ANDHI in Practice Substudy

    peer reviewedBackground: The phase IIIb, randomized, parallel-group, placebo-controlled ANDHI double-blind (DB) study extended understanding of the efficacy of benralizumab for patients with severe eosinophilic asthma. Patients from ANDHI DB could join the 56-week ANDHI in Practice (IP) single-arm, open-label extension substudy. Objective: Assess potential for standard-of-care background medication reductions while maintaining asthma control with benralizumab. Methods: Following ANDHI DB completion, eligible adults were enrolled in ANDHI IP. After an 8-week run-in with benralizumab, there were 5 visits to potentially reduce background asthma medications for patients achieving and maintaining protocol-defined asthma control with benralizumab. Main outcome measures for non–oral corticosteroid (OCS)-dependent patients were the proportions with at least 1 background medication reduction (ie, lower inhaled corticosteroid dose, background medication discontinuation) and the number of adapted Global Initiative for Asthma (GINA) step reductions at end of treatment (EOT). Main outcomes for OCS-dependent patients were reductions in daily OCS dosage and proportion achieving OCS dosage of 5 mg or lower at EOT. Results: For non–OCS-dependent patients, 53.3% (n = 208 of 390) achieved at least 1 background medication reduction, increasing to 72.6% (n = 130 of 179) for patients who maintained protocol-defined asthma control at EOT. A total of 41.9% (n = 163 of 389) achieved at least 1 adapted GINA step reduction, increasing to 61.8% (n = 110 of 178) for patients with protocol-defined EOT asthma control. At ANDHI IP baseline, OCS dosages were 5 mg or lower for 40.4% (n = 40 of 99) of OCS-dependent patients. Of OCS-dependent patients, 50.5% (n = 50 of 99) eliminated OCS and 74.7% (n = 74 of 99) achieved dosages of 5 mg or lower at EOT. Conclusions: These findings demonstrate benralizumab's ability to improve asthma control, thereby allowing background medication reduction. © 202

    Evaluation of appendicitis risk prediction models in adults with suspected appendicitis

    Background Appendicitis is the most common general surgical emergency worldwide, but its diagnosis remains challenging. The aim of this study was to determine whether existing risk prediction models can reliably identify patients presenting to hospital in the UK with acute right iliac fossa (RIF) pain who are at low risk of appendicitis. Methods A systematic search was completed to identify all existing appendicitis risk prediction models. Models were validated using UK data from an international prospective cohort study that captured consecutive patients aged 16–45 years presenting to hospital with acute RIF in March to June 2017. The main outcome was best achievable model specificity (proportion of patients who did not have appendicitis correctly classified as low risk) whilst maintaining a failure rate below 5 per cent (proportion of patients identified as low risk who actually had appendicitis). Results Some 5345 patients across 154 UK hospitals were identified, of which two‐thirds (3613 of 5345, 67·6 per cent) were women. Women were more than twice as likely to undergo surgery with removal of a histologically normal appendix (272 of 964, 28·2 per cent) than men (120 of 993, 12·1 per cent) (relative risk 2·33, 95 per cent c.i. 1·92 to 2·84; P < 0·001). Of 15 validated risk prediction models, the Adult Appendicitis Score performed best (cut‐off score 8 or less, specificity 63·1 per cent, failure rate 3·7 per cent). The Appendicitis Inflammatory Response Score performed best for men (cut‐off score 2 or less, specificity 24·7 per cent, failure rate 2·4 per cent). Conclusion Women in the UK had a disproportionate risk of admission without surgical intervention and had high rates of normal appendicectomy.
