    This report is a fire safety evaluation of Västralid nursing home in Olofström. The object consist of three connected housing bodies, first built in the 1970’s. At first, the buildings were used as regular apartments, but they were converted into nursing apartments for elderly people in the 1990’s. In the report, an analysis of fire scenarios are compared to egress scenarios to evaluate the personal safety in case of a fire. The results are that today, egress takes substantially more time than the time it takes for the conditions to become critical. The conclusion is that the fire protection is poor in the perspective of personal safety. A specific risk observed in the buildings is substandard fire compartmentation, since the ventilation system and elevator shaft break through the fire compartments. The means of egress are limited because the escape routes in many cases are blocked and not adapted for the facility. To achieve a satisfying personal fire safety a number of measures are proposed. It is important that all of the measures are undertaken as a whole, and not individually. If all of the measures are undertaken the fire safety is deemed satisfactory

    On the biological relevance of MHC class II and B7 expression by tumour cells in melanoma metastases

    A large number of studies have indicated that specific immune reactivity plays a crucial role in the control of malignant melanoma. In this context, expression of MHC I, MHC II and B7 molecules by melanoma cells is seen as relevant for the immune response against the tumour. For a better understanding of the biological relevance of MHC II and B7 expression by tumour cells in metastatic melanoma, we studied the expression of these molecules in melanoma metastases in relation to the inflammatory response, regression of the tumour and survival from 27 patients treated with biochemotherapy (30 mg m−2 Cisplatin and 250 mg m−2 decarbazine (dimethyl-triazene-imidazole-carboxamide, DTIC) on days 1–3 i.v., and 107 IU IFN-α2b 3 days a week s.c., q. 28d). In 19 out of 27 lesions studied, we found expression of MHC II by the tumour cells, while only in one out of 11 tumour biopsies obtained from untreated metastatic melanoma patients, MHC II expression was detected. Expression of B7.1 and B7.2 by tumour cells was found in nine out of 24 and 19 out of 24 lesions, respectively. In all cases where B7.1 expression was found, expression of B7.2 by the tumour cells was also seen. In general, no or only few inflammatory cells positive for B7 were found. Expression of MHC II by tumour cells was positively correlated with the presence of tumour-infiltrating lymphocytes, regression of the lesion, and with time to progression (TTP) and overall survival (OS) of the patient. However, no significant correlation between B7.1 or B7.2 expression and regression of the tumour, TTP or OS was found. In light of other recent findings, these data altogether do support a role as biomarker for MHC II expression by tumour cells; however, its exact immunological pathomechanism(s) remain to be established

    Towards a dynamic reference frame in Iceland

    There is a growing need for geodetic reference frames that on a national level support the increas-ing use of global positioning services. Today, the vast majority of countries have their own national ref-erence frame. In Europe this frame is normally aligned to ETRS89. This system is co-moving with theEurasian tectonic plate. Global Navigation Satellite Systems (GNSS) and global positioning services arenormally aligned to the Earth as a whole through a global reference frame like ITRF2014. Consequently,global positioning services does not give direct access to the national reference frame without a time-dependent transformation.A solution is to align the national reference frame directly to a global reference frame. In such aframe, the coordinates of a point fixed to the ground will change with time, - a fact leading to the expres-sion dynamic reference frame (DRF).To be prepared for future challenges, the Nordic Geodetic Commission (NKG) initiated a pilot-project on DRF in Iceland. Iceland has a very active and complex geodynamic situation. It is located atthe boundary of two tectonic plates and affected by seismic and volcanic activity, recent ice loadingchanges as well as glacial isostatic adjustment (GIA). Due to this, the traditional concept of a static geo-detic reference frame is difficult to maintain at the uncertainty level required by modern applications.Iceland was therefore a natural place to investigate the concept of DRF.This paper focuses on the outcome and conclusions of the DRF project in Iceland. We give tenpreconditions for a DRF. Living on an ever-changing Earth, we see that many of these preconditionshave to be in place regardless of type of reference frame. Through the work in the Nordic countries andNKG, the Nordic area will be well prepared for the future challenges. However, some legal issues forinstance, can be challenging. A two-frame solution combining static- and dynamic- reference framesseems like the best alternative in the foreseeable future

    Country-Specific vs. Common Birthweight-for-Gestational Age References to Identify Small for Gestational Age Infants Born at 24-28 weeks: An International Study

    BACKGROUND Controversy exists as to whether birthweight-for-gestational age references used to classify infants as small for gestational age (SGA) should be country specific or based on an international (common) standard. We examined whether different birthweight-for-gestational age references affected the association of SGA with adverse outcomes among very preterm neonates. METHODS Singleton infants (n = 23 788) of 24(0) -28(6) weeks' gestational age in nine high-resource countries were classified as SGA (<10th centile) using common and country-specific references based on birthweight and estimated fetal weight (EFW). For each reference, the adjusted relative risk (aRR) for the association of SGA with composite outcome of mortality or major morbidity was estimated. RESULTS The percentage of infants classified as SGA differed slightly for common compared with country specific for birthweight references [9.9% (95% CI 9.5, 10.2) vs. 11.1% (95% CI 10.7, 11.5)] and for EFW references [28.6% (95% CI 28.0, 29.2) vs. 24.6% (95% CI 24.1, 25.2)]. The association of SGA with the composite outcome was similar when using common or country-specific references for the total sample for birthweight [aRRs 1.47 (95% CI 1.43, 1.51) and 1.48 (95% CI 1.44, 1.53) respectively] and for EFW references [aRRs 1.35 (95% CI 1.31, 1.38) and 1.39 (95% CI 1.35, 1.43) respectively]. CONCLUSION Small for gestational age is associated with higher mortality and morbidity in infants born <29 weeks' gestational age. Although common and country-specific birthweight/EFW references identified slightly different proportions of SGA infants, the risk of the composite outcome was comparable

    Complete response to anti-interleukin-5 biologics in a real-life setting:results from the nationwide Danish Severe Asthma Register

    BACKGROUND: Phase III regulatory trials show that anti-interleukin (IL)-5 biologics efficiently reduce exacerbations and the use of maintenance oral corticosteroids (mOCS) in patients with severe eosinophilic asthma. However, patients eligible for these trials differ significantly compared with real-life severe asthma populations. Therefore, our aim was to explore efficacy in a real-life setting. The Danish Severe Asthma Register (DSAR) is a complete, nationwide register that comprises all Danish patients on biological therapy for severe asthma. METHODS: This prospective study identified patients in the DSAR who were complete responders to anti-IL-5 biologics after 1 year of treatment. A complete response was defined as resolution of the parameter setting the indication, i.e. recurrent exacerbations and/or use of mOCS. RESULTS: A total of 289 out of 502 (58%) patients were complete responders to anti-IL-5 biologics after 12 months. Complete responders had greater improvements in forced expiratory volume in 1 s and Asthma Control Questionnaire (ACQ) score compared with noncomplete responders (Δ 210 versus 30 mL; p<0.0001 and Δ −1.04 versus −0.68; p=0.016, respectively). A complete response was predicted by age at onset, less severe disease at baseline (i.e. no mOCS and lower ACQ score) and higher blood eosinophils. CONCLUSIONS: More than half of Danish patients treated with anti-IL-5 biologics for severe asthma achieve a complete response to treatment, thereby becoming free from asthma exacerbations and the need for mOCS. Complete responders also achieved superior effects on lung function and symptoms compared with noncomplete responders

    Crystal structure of vaccinia virus mRNA capping enzyme provides insights into the mechanism and evolution of the capping apparatus

    [EN] Vaccinia virus capping enzyme is a heterodimer of D1 (844 aa) and D12 (287 aa) polypeptides that executes all three steps in m(7)GpppRNA synthesis. The D1 subunit comprises an N-terminal RNA triphosphatase (TPase)-guanylyltransferase (GTase) module and a C-terminal guanine-N7-methyltransferase (MTase) module. The D12 subunit binds and allosterically stimulates the MTase module. Crystal structures of the complete D1.D12 heterodimer disclose the TPase and GTase as members of the triphosphate tunnel metalloenzyme and covalent nucleotidyltransferase superfamilies, respectively, albeit with distinctive active site features. An extensive TPase-GTase interface clamps the GTase nucleotidyltransferase and OB-fold domains in a closed conformation around GTP. Mutagenesis confirms the importance of the TPase-GTase interface for GTase activity. The D1.D12 structure complements and rationalizes four decades of biochemical studies of this enzyme, which was the first capping enzyme to be purified and characterized, and provides new insights into the origins of the capping systems of other large DNA viruses.We are grateful for access to platforms of the Grenoble Partnership for Structural Biology, especially the High Throughput Crystallization (HTX) laboratory of the European Molecular Biology Laboratory (EMBL) for robotic crystallization.     Dark Matter Constraints from a Joint Analysis of Dwarf Spheroidal Galaxy Observations with VERITAS

    We present constraints on the annihilation cross section of WIMP dark matter based on the joint statistical analysis of four dwarf galaxies with VERITAS. These results are derived from an optimized photon weighting statistical technique that improves on standard imaging atmospheric Cherenkov telescope (IACT) analyses by utilizing the spectral and spatial properties of individual photon events. We report on the results of \sim230 hours of observations of five dwarf galaxies and the joint statistical analysis of four of the dwarf galaxies. We find no evidence of gamma-ray emission from any individual dwarf nor in the joint analysis. The derived upper limit on the dark matter annihilation cross section from the joint analysis is 1.35×1023cm3s11.35\times 10^{-23} {\mathrm{ cm^3s^{-1}}} at 1 TeV for the bottom quark (bbˉb\bar{b}) final state, 2.85×1024cm3s12.85\times 10^{-24}{\mathrm{ cm^3s^{-1}}} at 1 TeV for the tau lepton (τ+τ\tau^{+}\tau^{-}) final state and 1.32×1025cm3s11.32\times 10^{-25}{\mathrm{ cm^3s^{-1}}} at 1 TeV for the gauge boson (γγ\gamma\gamma) final state.Comment: 14 pages, 9 figures, published in PRD, Ascii tables containing annihilation cross sections limits are available for download as ancillary files with readme.txt file description of limit

    Self-reported Smoking and Urinary Cotinine Levels among Pregnant Women in Korea and Factors Associated with Smoking during Pregnancy

    This study examined urinary cotinine levels and self-reported smoking among pregnant women in Korea and the factors associated with smoking during pregnancy. The subjects were selected from pregnant women who visited 30 randomly sampled obstetric clinics and prenatal care hospitals in Korea in 2006. Smoking status was determined by self-reporting and urinary cotinine measurement. A total of 1,090 self-administered questionnaires and 1,057 urine samples were analyzed. The percentage of smoking revealed by self-reporting was 0.55% (95% confidence interval [CI], 0.11-0.99) and that revealed by urinary cotinine measurement (>100 ng/mL) was 3.03% (95% CI, 1.99-4.06). The kappa coefficient of agreement between self-reported smoking status and urinary cotinine measurement was 0.20 (95% CI, 0.03-0.37). Multiple logistic regression analysis revealed that early gestational period, low educational level, and being married to a smoker were significant risk factors for smoking during pregnancy. Smoking among pregnant women in Korea is not negligible, and those who are concerned to maternal and child health should be aware of this possibility among pregnant women in countries with similar cultural background
