    Mouth rinsing with a sweet solution increases energy expenditure and decreases appetite during 60 minutes of self-regulated walking exercise

    Carbohydrate mouth rinsing can improve endurance exercise performance and is most ergogenic when exercise is completed in the fasted state. This strategy may also be beneficial to increase exercise capacity and the energy deficit achieved during moderate intensity exercise relevant to weight control when performed after an overnight fast. Eighteen healthy men (mean(SD); age 23(4)years, body mass index 23.1(2.4)kg.m-2 ) completed a familiarisation trial and three experimental trials. After an overnight fast, participants performed 60-minutes of treadmill walking at a speed that equated to a rating of perceived exertion of 13 (“fairly hard”). Participants manually adjusted the treadmill speed to maintain this exertion. Mouth rinses for the experimental trials contained either a 6.4% maltodextrin solution with sweetener (CHO), a taste-matched placebo (PLA) or water (WAT). Appetite ratings were collected using visual analogue scales and exercise energy expenditure and substrate oxidation were calculated from online gas analysis. Increased walking distance during CHO and PLA induced greater energy expenditure compared with WAT (mean difference (90% CI); 79(60)kJ; P=0.035; d=0.24 and 90(63)kJ; P=0.024; d=0.27, respectively). Appetite area under the curve was lower in CHO and PLA than WAT (8(6)mm; P=0.042; d=0.43 and 6(8)mm; P=0.201; d=0.32, respectively). Carbohydrate oxidation was higher in CHO than PLA and WAT (7.3(6.7)g; P=0.078; d=0.47 and 10.1(6.5)g; P=0.015; d=0.81, respectively). This study provides novel evidence that mouth rinsing with a sweetened solution may promote a greater energy deficit during moderate exertion walking exercise by increasing energy expenditure and decreasing appetite. A placebo effect may have contributed to these benefits

    Scenario-Based Design Theorizing:The Case of a Digital Idea Screening Cockpit

    As ever more companies encourage employees to innovate, a surplus of ideas has become reality in many organizations – often exceeding the available resources to execute them. Building on insights from a literature review and a 3-year collaboration with a banking software provider, the paper suggests a Digital Idea Screening Cockpit (DISC) to address this challenge. Following a design science research approach, it suggests a prescriptive design theory that provides practitioner-oriented guidance for implementing a DISC. The study shows that, in order to facilitate the assessment, selection, and tracking of ideas for different stakeholders, such a system needs to play a dual role: It needs to structure decision criteria and at the same be flexible to allow for creative expression. Moreover, the paper makes a case for scenario-based design theorizing by developing design knowledge via scenarios

    Climate-sensitive health priorities in Nunatsiavut, Canada

    Background: This exploratory study used participatory methods to identify, characterize, and rank climate-sensitive health priorities in Nunatsiavut, Labrador, Canada. Methods: A mixed method study design was used and involved collecting both qualitative and quantitative data at regional, community, and individual levels. In-depth interviews with regional health representatives were conducted throughout Nunatsiavut (n = 11). In addition, three PhotoVoice workshops were held with Rigolet community members (n = 11), where participants took photos of areas, items, or concepts that expressed how climate change is impacting their health. The workshop groups shared their photographs, discussed the stories and messages behind them, and then grouped photos into re-occurring themes. Two community surveys were administered in Rigolet to capture data on observed climatic and environmental changes in the area, and perceived impacts on health, wellbeing, and lifestyles (n = 187). Results: Climate-sensitive health pathways were described in terms of inter-relationships between environmental and social determinants of Inuit health. The climate-sensitive health priorities for the region included food security, water security, mental health and wellbeing, new hazards and safety concerns, and health services and delivery. Conclusions: The results highlight several climate-sensitive health priorities that are specific to the Nunatsiavut region, and suggest approaching health research and adaptation planning from an EcoHealth perspective

    Virtual reality crowd simulation: effects of agent density on user experience and behaviour

    Agent-based crowd simulations are used for modelling building and space usage, allowing designers to explore hypothetical real-world scenarios, including extraordinary events such as evacuations. Existing work which engages virtual reality (VR) as a platform for crowd simulations has been primarily focussed on the validation of simulation models through observation; the use of interactions such as gaze to enhance a sense of immersion; or studies of proxemics. In this work, we extend previous studies of proxemics and examine the effects of varying crowd density on user experience and behaviour. We have created a simulation in which participants walk freely and perform a routine manual task, whilst interacting with agents controlled by a typical social force simulation model. We examine and report the effects of crowd density on both affective state and behaviour. Our results show a significant increase in negative affect with density, measured using a self-report scale. We further show significant differences in some aspects of user behaviours, using video analysis, and discuss how our results relate to VR simulation design for mixed human–agent scenarios

    Uncertainties in radon related to house-specific factors and proximity to geological boundaries in England

    Data collected as a part of a survey on radon concentrations from about 40 000 dwellings in England for six contrasting geological units were analysed to evaluate the impact of house-specific factors (building characteristics and construction dates) and of proximity to geological boundaries. After adjusting for temperature and outdoor radon, geological unit, house type, double glazing and date of building were found to have a statistically significant influence on indoor radon concentrations and explained about 29 % of the total variation between dwellings in logarithmically transformed radon values. In addition, there were statistically significant differences in radon concentrations according to proximity to geological boundaries categories for most of the geological units, but no consistent pattern could be detected

    Steroid sparing maintenance immunosuppression in highly sensitised patients receiving Alemtuzumab induction

    This analysis reports on the outcomes of two different steroid sparing immunosuppression protocols used in the management of 120 highly sensitised patients (HSPs) with cRF>85% receiving Alemtuzumab induction, 53 maintained on tacrolimus (FK) monotherapy and 67 tacrolimus plus mycophenolate mofetil (FK+MMF). There was no difference in the median cRF or mode of sensitisation between the 2 groups, although the FK+MMF cohort received more poorly matched grafts. There was no difference in one-year patient or allograft survival, however rejection free survival was inferior with FK monotherapy compared with FK+MMF at 65.4% and 91.4% respectively, p<0.01. DSA-free survival was comparable. Whilst there was no difference in rates of BK between the cohorts, CMV-free survival was inferior in the FK+MMF group at 86.0% compared with 98.1% in the FK group, p=0.026. One-year post-transplant diabetes free survival was 89.6% and 100.0% in the FK and FK+MMF group respectively, p=0.027, the difference attributed to the use of prednisolone to treat rejection in the FK cohort, p=0.006. We report good outcomes in HSPs utilising a steroid sparing protocol with Alemtuzumab induction and FK+MMF maintenance and provide granular data on immunological and infectious complications to inform steroid avoidance in these patient groups
