    Specialist Outreach to Isolated and Disadvantaged Communities: A Population-Based Study

    Background Visiting-specialist clinics (specialist outreach) have the potential to overcome some of the substantial access barriers faced by disadvantaged rural, remote, and Indigenous communities, but the effectiveness of outreach clinics has not been assessed outside urban and non-disadvantaged settings. We aimed to assess the effects of outreach clinics on access, referral patterns, and care outcomes in remote communities in Australia. Methods We undertook a population-based observational study of regular surgical, ophthalmological, gynaecological, and ear, nose, and throat outreach visits, compared with hospital clinics alone, on access, referral practices, and outcomes for the populations of three remote Indigenous communities in northern Australia for 11 years. We assessed all new non-emergency potential specialist surgical cases who presented initially between Jan 1, 1990, and Jan 1, 2001. The effects of outreach clinics on the proportion of patients referred, the time from referral to initial specialist consultation, and the rates of community-based and hospital-based procedures were analysed using logic regression and Cox proportional hazard models. Findings 2339 new surgical problems presented in 2368 people between 1990 and 2001. Outreach improved the rate of referral completion (adjusted hazard ratio 1.41, 95% CI 1.07-1.86) and the risk of timely completion according to the urgency of referral (adjusted relative risk 1.30, 1.05-1.53). Outreach had no significant effect on initiation of elective referrals, but there were 156 opportunistic presentations on outreach clinic days. Specialist investigations and procedures in community clinics removed the need for many patients to travel to hospital, and outreach consultations were associated with a reduced rate of procedures that needed hospital admission (adjusted hazard ratio 0.67, 0.43-.03). Interpretation Specialist outreach visits to remote disadvantaged Indigenous communities in Australia improve access to specialist consultations and procedures without increasing elective referrals or demands for hospital inpatient services

It argues that due to conceptual and practical flaws with VI, conscientious consumers should adopt VA. It considers and rejects the so-called compassionate carnivore movement. It then explores arguments denying the casual efficacy of adopting any form of veganism. It concludes that VA can make a difference, and those in consumer cultures are obligated to adopt and practice it

    Simultaneous dual-element analyses of refractory metals in naturally occurring matrices using resonance ionization of sputtered atoms

    The combination of secondary neutral mass spectrometry (SNMS) and resonance ionization spectroscopy (RIS) has been shown to be a powerful tool for the detection of low levels of elemental impurities in solids. Drawbacks of the technique have been the laser-repetition-rate-limited, low duty cycle of the analysis and the fact that RIS schemes are limited to determinations of a single element. These problems have been addressed as part of an ongoing program to explore the usefulness of RIS/SNMS instruments for the analysis of naturally occurring samples. Efficient two-color, two-photon (1+1) resonance ionization schemes were identified for Mo and for four platinum-group elements (Ru, Os, Ir, and Re). Careful selection of the ionization schemes allowed Mo or Ru to be measured simultaneously with Re, Os, or Ir, using two tunable dye lasers and an XeCl excimer laser. Resonance frequencies could be switched easily under computer control, so that all five elements can be rapidly analyzed. In situ measurements of these elements in metal grains from five meteorites were conducted. From the analyses, estimates of the precision and the detection limit of the instrument were made.     Explaining Forward Discount Bias: Is it Anchoring?

    Anchoring is a well-documented behaviour pattern. It occurs when agents form their expectations of an objective variable by only partially adjusting from some given starting value. We present a model of the foreign exchange market in which there are two types of traders: those who are fully rational and those whose expectations are anchored to the forward exchange rate. Under plausible conditions, a significant proportion of the anchored traders survive in the market in the long-run. The model explains both forward discount bias in the direction consistently observed in foreign exchange markets and the results of surveys of market participants’ exchange rate expectations.

    1. A necessary component of elimination programmes for wildlife disease is effective surveillance. The ability to distinguish between disease freedom and non‐detection can mean the difference between a successful elimination campaign and new epizootics. Understanding the contribution of different surveillance methods helps to optimize and better allocate effort and develop more effective surveillance programmes. 2. We evaluated the probability of rabies virus elimination (disease freedom) in an enzootic area with active management using dynamic occupancy modelling of 10 years of raccoon rabies virus (RABV) surveillance data (2006–2015) collected from three states in the eastern United States. We estimated detection probability of RABV cases for each surveillance method (e.g. strange acting reports, roadkill, surveillance‐trapped animals, nuisance animals and public health samples) used by the USDA National Rabies Management Program. 3. Strange acting, found dead and public health animals were the most likely to detect RABV when it was present, and generally detectability was higher in fall– winter compared to spring–summer. Found dead animals in fall–winter had the highest detection at 0.33 (95% CI: 0.20, 0.48). Nuisance animals had the lowest detection probabilities (~0.02). 4. Areas with oral rabies vaccination (ORV) management had reduced occurrence probability compared to enzootic areas without ORV management. RABV occurrence was positively associated with deciduous and mixed forests and medium to high developed areas, which are also areas with higher raccoon (Procyon lotor) densities. By combining occupancy and detection estimates we can create a probability of elimination surface that can be updated seasonally to provide guidance on areas managed for wildlife disease. 5. Synthesis and applications. Wildlife disease surveillance is often comprised of a combination of targeted and convenience‐based methods. Using a multi‐method analytical approach allows us to compare the relative strengths of these methods, providing guidance on resource allocation for surveillance actions. Applying this multi‐method approach in conjunction with dynamic occupancy analyses better informs management decisions by understanding ecological drivers of disease occurrence

    MC2^2: Subaru and Hubble Space Telescope Weak-Lensing Analysis of the Double Radio Relic Galaxy Cluster PLCK G287.0+32.9

    The second most significant detection of the Planck Sunyaev Zel'dovich survey, PLCK~G287.0+32.9 (z=0.385z=0.385) boasts two similarly bright radio relics and a radio halo. One radio relic is located 400\sim 400 kpc northwest of the X-ray peak and the other 2.8\sim 2.8 Mpc to the southeast. This large difference suggests that a complex merging scenario is required. A key missing puzzle for the merging scenario reconstruction is the underlying dark matter distribution in high resolution. We present a joint Subaru Telescope and {\it Hubble Space Telescope} weak-lensing analysis of the cluster. Our analysis shows that the mass distribution features four significant substructures. Of the substructures, a primary cluster of mass $M_{200\text{c}}=1.59^{+0.25}_{-0.22}\times 10^{15} \ h^{-1}_{70} \ \text{M}_{\odot}dominatestheweaklensingsignal.Thisclusterislikelytobeundergoingamergerwithone(ormore)subclusterwhosemassisapproximatelyafactorof10lower.Onecandidateisthesubclusterofmass dominates the weak-lensing signal. This cluster is likely to be undergoing a merger with one (or more) subcluster whose mass is approximately a factor of 10 lower. One candidate is the subcluster of mass M_{200\text{c}}=1.16^{+0.15}_{-0.13}\times 10^{14} \ h^{-1}_{70} \ \text{M}_{\odot}located located \sim 400kpctothesoutheast.ThelocationofthissubclustersuggeststhatitsinteractionwiththeprimaryclustercouldbethesourceoftheNWradiorelic.Anothersubclusterisdetected kpc to the southeast. The location of this subcluster suggests that its interaction with the primary cluster could be the source of the NW radio relic. Another subcluster is detected \sim 2MpctotheSEoftheXraypeakwithmass Mpc to the SE of the X-ray peak with mass M_{200\text{c}}=1.68^{+0.22}_{-0.20}\times 10^{14} \ h^{-1}_{70} \ \text{M}_{\odot}.ThisSEsubclusterisinthevicinityoftheSEradiorelicandmayhavecreatedtheSEradiorelicduringapastmergerwiththeprimarycluster.Thefourthsubcluster,. This SE subcluster is in the vicinity of the SE radio relic and may have created the SE radio relic during a past merger with the primary cluster.     Multiphoton Ionization Followed by Time-of-Flight Mass Spectroscopy of Sputtered Neutrals

    Multiphoton ionization (MPI) by pulsed, tunable lasers provides a sensitive means for detection of neutral atoms, resulting from the high probability achievable in both the ionization and subsequent detection steps. Substantial selectivity is achieved by excitation between energy levels of the atom of interest. This resonant MPI technique can access all atomic states including ground and metastable levels. The high efficiency of MPI technique permits detailed sputtering data to be obtained with minimal target damage. The goal is to obtain velocity and angular distributions for each energy level of every sputtered species. In practice, two types of experimental configurations have been employed. In one method, the photoionized atoms are allowed to strike a spatially resolved detector near the target, with extraction fields that preserve the angular distribution information. Velocity information is obtained by time of flight (TOF). This method is most suitable for majority species in the sputtered flux. In the case of minority species (either very dilute surface constituents or highly excited states produced), additional noise reduction is necessary. A suitable configuration involves extraction of the photoions into a sector-field TOF mass spectrometer. In standard, isochronous operation, energy and angular spreads at the point of ionization are compensated in flight to produce sharp TOF mass spectra. Noise sources (photons, metastable and scattered atoms) escaping through transparent grids are strongly suppressed. Angular distributions can be mapped pointwise by varying the relation between the point of ion beam impact and the photoionization volume. Velocity data can be obtained from the TOF spectra or by Doppler scanning on any resonant step of the laser excitation. Recent data are discussed

    A purple patch for evidence-based health policy?

    The global focus on nation states' responses to the COVID-19 pandemic has rightly highlighted the importance of science and evidence as the basis for policy action. Those with a lifelong passion for evidence-based policy (EBP) have lauded Australia's and other nations' policy responses to COVID-19 as a breakthrough moment for the cause. This article reflects on the complexity of the public policy process, the perspectives of its various actors, and draws on Alford's work on the Blue, Red and Purple zones to propose a more nuanced approach to advocacy for EBP in health. We contend that the pathway for translation of research evidence into routine clinical practice is relatively linear, in contrast to the more complex course for translation of evidence to public policy - much to the frustration of health researchers and EBP advocates. Cairney's description of the characteristics of successful policy entrepreneurs offers useful guidance to advance EBP and we conclude with proposing some practical mechanisms to support it. Finally, we recommend that researchers and policy makers spend more time in the Purple zone to enable a deeper understanding of, and mutual respect for, the unique contributions made by research, policy and political actors to sound public policy
