    Consumo de alimentos ultraprocessados no Brasil: distribuição e evolução temporal 2008–2018

    OBJECTIVE To evaluate sociodemographic factors associated with the consumption of ultra-processed foods and the temporal evolution of their consumption in Brazil between 2008 and 2018. METHODS The study used food consumption data of individuals aged ≥ 10 years from 2008–2009 and 2017–2018 Pesquisas de Orçamentos Familiares (POF – Household Budget Surveys), grouping the foods according to the Nova classification. We used crude and adjusted linear regression models to assess the association between sociodemographic characteristics and consumption of ultra-processed foods in 2017–2018 and the temporal variation in their consumption between 2008 and 2018. RESULTS Ultra-processed foods accounted for 19.7% of calories in 2017–2018. The adjusted analysis showed that their consumption was higher in women (versus men) and the South and Southeast regions (versus North) and lower in blacks (versus whites) and rural areas (versus urban), in addition to decreasing with the increased age and increasing with higher education and income. Consumption of ultra-processed foods increased by 1.02 percentage points (pp) from 2008–2009 to 2017–2018. This increase was significantly higher among men (+1.59 pp), black people (+2.04 pp), indigenous (+5.96 pp), in the rural area (+2.43 pp), those with up to 4 years of schooling (+1.18 pp), in the lowest income quintile (+3.54 pp), and the North (+2.95 pp) and Northeast (+3.11 pp) regions. On the other hand, individuals in the highest level of schooling (-3.30 pp) and the highest income quintile (-1.65 pp) reduced their consumption. CONCLUSIONS The socioeconomic and demographic segments with the lowest relative consumption of ultra-processed foods in 2017–2018 are precisely those that showed the most significant increase in the temporal analysis, pointing to a trend towards national standardization at a higher level of consumption.OBJETIVO: Avaliar fatores sociodemográficos associados ao consumo de alimentos ultraprocessados e a evolução temporal do consumo no Brasil entre 2008 e 2018. MÉTODOS: Foram utilizados dados do consumo alimentar de indivíduos com idade ≥ 10 anos das Pesquisas de Orçamentos Familiares (POF) 2008–2009 e 2017–2018. Os alimentos foram agrupados segundo a classificação Nova. Modelos de regressão linear brutos e ajustados foram utilizados para avaliar a associação entre características sociodemográficas e o consumo de ultraprocessados em 2017–2018 e a variação temporal de seu consumo entre 2008 e 2018. RESULTADOS: Alimentos ultraprocessados representaram 19,7% das calorias em 2017–2018. A análise ajustada mostrou que seu consumo foi maior no sexo feminino (versus masculino) e nas regiões Sul e Sudeste (versus Norte), e menor em negros (versus brancos) e na área rural (versus urbana), além de diminuir com o aumento da idade e aumentar com escolaridade e renda. O consumo de ultraprocessados aumentou 1,02 pontos percentuais (pp) de 2008–2009 a 2017–2018, sendo este aumento mais expressivo em homens (+1,59 pp), negros (+2,04 pp), indígenas (+5,96 pp), na área rural (+2,43 pp), naqueles com até 4 anos de estudo (+1,18 pp), no quinto mais baixo de renda (+3,54 pp) e nas regiões Norte (+2,95 pp) e Nordeste (+3,11 pp). Por outro lado, seu consumo se reduziu na maior faixa de escolaridade (-3,30 pp) e no quinto mais alto de renda (-1,65 pp). CONCLUSÕES: Os segmentos socioeconômicos e demográficos que tiveram menor consumo relativo de ultraprocessados em 2017–2018 são justamente os que apresentaram um aumento mais expressivo na análise temporal, apontando para uma tendência de padronização nacional em um patamar de consumo mais alto

    Rare Variants Associated with Arrhythmogenic Cardiomyopathy: Reclassification Five Years Later.

    Genetic interpretation of rare variants associated with arrhythmogenic cardiomyopathy (ACM) is essential due to their diagnostic implications. New data may relabel previous variant classifications, but how often reanalysis is necessary remains undefined. Five years ago, 39 rare ACM-related variants were identified in patients with features of cardiomyopathy. These variants were classified following the American College of Medical Genetics and Genomics' guidelines. In the present study, we reevaluated these rare variants including novel available data. All cases carried one rare variant classified as being of ambiguous significance (82.05%) or likely pathogenic (17.95%) in 2016. In our comprehensive reanalysis, the classification of 30.77% of these variants changed, mainly due to updated global frequencies. As in 2016, nowadays most variants were classified as having an uncertain role (64.1%), but the proportion of variants with an uncertain role was significantly decreased (17.95%). The percentage of rare variants classified as potentially deleterious increased from 17.95% to 23.07%. Moreover, 83.33% of reclassified variants gained certainty. We propose that periodic genetic reanalysis of all rare variants associated with arrhythmogenic cardiomyopathy should be undertaken at least once every five years. Defining the roles of rare variants may help clinicians obtain a definite diagnosis

    COVID-19 and stem cell transplantation; results from an EBMT and GETH multicenter prospective survey

    © The Author(s) 2021.This study reports on 382 COVID-19 patients having undergone allogeneic (n = 236) or autologous (n = 146) hematopoietic cell transplantation (HCT) reported to the European Society for Blood and Marrow Transplantation (EBMT) or to the Spanish Group of Hematopoietic Stem Cell Transplantation (GETH). The median age was 54.1 years (1.0–80.3) for allogeneic, and 60.6 years (7.7–81.6) for autologous HCT patients. The median time from HCT to COVID-19 was 15.8 months (0.2–292.7) in allogeneic and 24.6 months (−0.9 to 350.3) in autologous recipients. 83.5% developed lower respiratory tract disease and 22.5% were admitted to an ICU. Overall survival at 6 weeks from diagnosis was 77.9% and 72.1% in allogeneic and autologous recipients, respectively. Children had a survival of 93.4%. In multivariate analysis, older age (p = 0.02), need for ICU (p < 0.0001) and moderate/high immunodeficiency index (p = 0.04) increased the risk while better performance status (p = 0.001) decreased the risk for mortality. Other factors such as underlying diagnosis, time from HCT, GVHD, or ongoing immunosuppression did not significantly impact overall survival.     Introdução: Os Hemangioblastomas são tumores de origem vascular que podem prejudicar o sistema nervoso central, com&nbsp; um pico de incidência na quarta década de vida. Apresentação do caso: Paciente de 24 anos, feminino,&nbsp; procedente do centro-oeste do Brasil, apresenta há 2 anos, pequena massa na região cervical com crescimento lento. Há 1 ano, associa-se forte dor nos ombros e cefaleia, além de redução gradual da sensibilidade em mãos e pés. Ao exame, redução da sensibilidade tátil, térmica e dolorosa em membros. Realizada Ressonância magnética com evidência de lesões&nbsp; nodulares de realce homogêneo, associando-se a cisto. Discussão: A clínica dos hemangiomas varia de acordo com a localização do tumor e do efeito de massa, sendo a cefaléia o sintoma mais comum, seguido por náuseas, vertigem, e diplopia. O diagnóstico é realizado através de angiografia, tomografia computadorizada e ressonância magnética. A terapia empregada é a remoção cirúrgica por excisão tumoral. Conclusão: Por meio de uma identificação imediata de infiltração tumoral ou a presença de células tumorais residuais, em que se observa a frequência de uma pequena quantidade de células tumorais pode levar à recorrência tumoral sendo por isso importante remover completamente os hemangioblastomas

    Introdução: A síndrome de Bruns é uma manifestação rara de uma neurocisticercose (NCC) intraventricular.&nbsp; A NCC é causada pela larva da Tenia solium, por meio da contaminação fecal-oral, e caracteriza-se como uma das possíveis infecções helmínticas do sistema nervoso central. A doença possui maior prevalência em países de baixa renda, onde há precariedade no sistema de saneamento básico, além de ser uma das causas de convulsões no mundo. Apresentação do caso: Paciente do sexo masculino, 47 anos, admitido no Pronto Socorro da Santa Casa de Campo Grande, após quadro de crise convulsiva tônico-clonica. História pregressa de início há 6 meses de episódios semanais de cefaleia holocraniana intensa, em aperto, associado a náuseas, vômitos e vertigem. Discussão: A síndrome de Bruns consiste em episódios recorrentes de cefaleia intensa, vômitos e vertigens provocadas por movimentos abruptos da cabeça ou pela manobra de Valsava. Tais agravos levam a obstruções periódicas na circulação do líquor e aumentos paroxísticos da pressão intracraniana, devido a uma massa ventricular móvel que provoca hidrocefalia não comunicante. O diagnóstico pode ser difícil, visto que o quadro é inespecífico e depende de fatores como localização&nbsp; das lesões e sistema imune do hospedeiro. Conclusão: Por se tratar de uma forma rara de evolução da neurocisticercose, é necessário atenção quanto a sintomatologia inespecífica. Pois, os casos podem ser negligenciados e evoluírem de forma desfavorável por atraso da terapêutica

    ARTEFACTS: How do we want to deal with the future of our one and only planet?

    The European Commission’s Science and Knowledge Service, the Joint Research Centre (JRC), decided to try working hand-in-hand with leading European science centres and museums. Behind this decision was the idea that the JRC could better support EU Institutions in engaging with the European public. The fact that European Union policies are firmly based on scientific evidence is a strong message which the JRC is uniquely able to illustrate. Such a collaboration would not only provide a platform to explain the benefits of EU policies to our daily lives but also provide an opportunity for European citizens to engage by taking a more active part in the EU policy making process for the future. A PILOT PROGRAMME To test the idea, the JRC launched an experimental programme to work with science museums: a perfect partner for three compelling reasons. Firstly, they attract a large and growing number of visitors. Leading science museums in Europe have typically 500 000 visitors per year. Furthermore, they are based in large European cities and attract local visitors as well as tourists from across Europe and beyond. The second reason for working with museums is that they have mastered the art of how to communicate key elements of sophisticated arguments across to the public and making complex topics of public interest readily accessible. That is a high-value added skill and a crucial part of the valorisation of public-funded research, never to be underestimated. Finally museums are, at present, undergoing something of a renaissance.
