    An applied paradigm for simple analysis of the lower limb kinematic chain in explosive movements: an example using the fencing foil attacking lunge

    A simple method to quantify the kinematic chain in a propulsive task would facilitate assessment of athlete effectiveness. The study’s aim was to assess if the kinematic chain distinguishes between skill levels. Fencers were separated into two groups based on attacking lunge ability (7 skilled; 8 novices). Rear leg 3D joint angular extension velocity magnitudes and timings, sword kinematics and rear leg kinetics were obtained in the propulsion phase of the attacking lunge. Skilled fencers obtained greater sword velocity (3.24 ± 0.24 m∙s−1 vs. 2.69 ± 0.29 m∙s−1; p = 0.02). The skilled group had a greater sequential kinematic chain of the hip, knee and ankle, demonstrated by significantly greater ankle angular velocity (9.1 ± 2.1 rad·s−1 skilled; 5.4 ± 2.9 rad·s−1 novice). Ankle plantarflexion velocity showed a strong positive correlation with horizontal peak force (r = 0.81; p < 0.01). The skilled group demonstrated greater horizontal impulse (1.85 ± 0.29 N·s·kg−1 skilled; 1.45 ± 0.32 N·s·kg−1 novice), suggesting greater effectiveness in applying the kinematic chain towards horizontal propulsion. Analysis of the kinematic chain, which was able to distinguish between skill levels in a propulsive task, is an effective and simple paradigm to assess whole limb contributions to propulsive movements

    Effect of acute tensile loading on gender-specific tendon structural and mechanical properties.

    Stretching is commonly used prior to exercise, as it is thought to reduce the risk of injury, and it is also used in the preconditioning of tendon grafts. As tendon properties have been shown to be different between genders, it is proposed that stretching will differentially affect the structure. Here we examine the effect of acute stretch on the mechanical properties of both male and female medial gastrocnemius tendon. Female [20 years ± 1 (SEM), n = 17] and male (22 years ±1, n = 18) subjects underwent a 5-min passive dorsiflexion stretch. Prior to and post stretch medial gastrocnemius tendon stiffness (K), length (1) and cross-sectional area (csa) were measured using ultrasonography and dynamometry. Stiffness and Young's modulus (Δ) were significantly reduced with stretch for both genders (p < 0.05). Females showed significantly (p < 0.05) greater pre- to poststretch decreases in K (22.4 vs. 8, 8%) and e (20.5 vs. 8.4%) in comparison to males. The present results show that stretching acutely reduces stiffness of the medial gastrocnemius tendon in females and males, with females showing significantly greater change. The observed disparity between genders may be due in part to variations in tendon moment arm and intrinsic differences in tendon composition. These differential changes in tendon mechanical properties have functional, motor control, and injury risk implications, as well as possible implications for preconditioning of tendon grafts

    Which anthropometric and lower body power variables are predictive of professional and amateur playing status in male rugby union players?

    The purpose of this study was to compare anthropometric and lower body power measurements between current professional and amateur male rugby union players. The present study also sought to determine which anthropometric and physical performance variables were predictive of playing standard. Thirty professional and 30 amateur RU players performed Wattbike 6 s maximal effort (WB6S) and countermovement (CMJ) and squat jump (SJ) assessments, anthropometric measures were also taken. Dependant variables recorded and analysed including: body mass, stature, Σ8 site skinfolds, WB6S absolute and relative peak power, CMJ and SJ average concentric force, jump height, peak velocity, time to peak force, rate of force development (RFD) and absolute and relative peak force and power. Professional players were heavier, taller and leaner than their amateur counterparts (p < 0.05). Professional players performed significantly better in all physical performance measures except CMJ and SJ time to peak force, CMJ RFD and SJ relative peak force. Variables which were predictive of playing standard were: Σ8 skinfolds, CMJ peak velocity and WB6S absolute and relative peak power (p < 0.05). These findings indicate that the current body of male professional RU players is anthropometrically and physically superior to their amateur counterparts, although not all variables assessed here were predictive of playing standard. Data presented here indicate that Σ8 skinfolds, WB6S absolute and relative power and CMJ peak velocity are predictive of playing standard, whereas other anthropometric and strength and power variables are not

    What is the role of culture, diversity, and community engagement in transdisciplinary translational science?

    Concepts of culture and diversity are necessary considerations in the scientific application of theory generation and developmental processes of preventive interventions; yet, culture and/or diversity are often overlooked until later stages (e.g., adaptation [T3] and dissemination [T4]) of the translational science process. Here, we present a conceptual framework focused on the seamless incorporation of culture and diversity throughout the various stages of the translational science process (T1-T5). Informed by a community-engaged research approach, this framework guides integration of cultural and diversity considerations at each phase with emphasis on the importance and value of "citizen scientists" being research partners to promote ecological validity. The integrated partnership covers the first phase of intervention development through final phases that ultimately facilitate more global, universal translation of changes in attitudes, norms, and systems. Our comprehensive model for incorporating culture and diversity into translational research provides a basis for further discussion and translational science development

    A Complete Expression Profile of Matrix-Degrading Metalloproteinases in Dupuytren’s Disease

    Dupuytren’s disease (DD) is a common fibrotic condition of the palmar fascia, leading to deposition of collagen-rich cords and finger contractions. The metzincin superfamily contains key enzymes in the turnover of collagen and other extracellular matrix macromolecules. A number of broad-spectrum matrix metalloproteinase inhibitors, used in cancer clinical trials, caused side effects of DD-like contractures. We tested the hypothesis that changes in the expression of specific metalloproteinases underlie or contribute to the fibrosis and contracture seen in DD. We collected tissue from patients with DD and used normal palmar fascia as a control. We profiled the expression of the entire matrix metalloproteinase (MMP), tissue inhibitor of metalloproteinases (TIMP), and a disintegrin and metalloproteinase domain with thrombospondin motif (ADAMTS) gene families in these tissues using real-time reverse-transcription polymerase chain reaction. A number of metalloproteinases and inhibitors are regulated in DD. The expression of 3 key collagenases, MMP1, MMP13, and MMP14 is increased significantly in the DD nodule, as is the expression of the collagen biosynthetic enzyme ADAMTS14. The expression of MMP7, an enzyme with broad substrate specificity, is increased in the DD nodule and remains equally expressed in the DD cord. TIMP1 expression is increased significantly in DD nodule compared with normal palmar fascia. This study measured the expression of all MMP, ADAMTS, and TIMP genes in DD. Contraction and fibrosis may result from: (1) increased collagen biosynthesis mediated by increased ADAMTS-14; (2) an increased level of TIMP-1 blocking MMP-1– and MMP-13–mediated collagenolysis; and (3) contraction enabled by MMP-14–mediated pericellular collagenolysis (and potentially MMP-7), which may escape inhibition by TIMP-1. The complete expression profile will provide a knowledge-based approach to novel therapeutics targeting these genes

PcP incidence was highest in patients with the lowest CD4 counts but uncommon compared with incidences of pulmonary tuberculosis and bacterial pneumonia

    Lactobacillus-depleted vaginal microbiota in pregnant women living with HIV-1 infection are associated with increased local inflammation and preterm birth

    Background: Pregnant women living with HIV-1 infection (PWLWH) have an elevated risk of preterm birth (PTB) of unknown aetiology, which remains after successful suppression of HIV. Women at high risk for HIV have a common bacterial profile which has been associated with poor birth outcomes. We set out to explore factors associated with gestational age at delivery of PWLWH in a UK population. Methods: Prospective study of PWLWH (n = 53) in whom the vaginal microbiota and cervicovaginal cytokine milieu were assessed using metataxonomics and multiplexed immunoassays, respectively. Cross-sectional characterisation of vaginal microbiota in PWLWH were compared with 22 HIV uninfected pregnant women (HUPW) at a similar second trimester timepoint. Within PWLWH the relationships between bacterial composition, inflammatory response, and gestational age at delivery were explored. Findings: There was a high rate of PTB among PWLWH (12%). In the second trimester the vaginal microbiota was more diverse in PWLWH than in HUPW (Inverse Simpson Index, p = 0.0004 and Species Observed, p = 0.009). PWLWH had a lower prevalence of L. crispatus dominant vaginal microbiota group (VMB I, 15 vs 54%) than HUPW and higher prevalence of L. iners dominant (VMB III, 36 vs 9% and VMB IIIB, 15 vs 5%) and mixed anaerobes (VMB IV, 21 vs 0%). Across the second and third trimesters in PWLWH, VMB III/IIIB and IV were associated with PTB and with increased local inflammation [cervicovaginal fluid (CVF) cytokine concentrations in upper quartile]. High bacterial diversity and anaerobic bacterial abundance were also associated with CVF pro-inflammatory cytokines, most notably IL-1ÎČ. Interpretation: There is an association between local inflammation, vaginal dysbiosis and PTB in PWLWH. Understanding the potential of antiretroviral therapies to influence this cascade will be important to improve birth outcomes in this population

    Comparative analysis of vaginal microbiota sampling using menstrual cups and high vaginal swabs in pregnant women living with HIV-1 infection

    Background: Menstrual cups (MCs) are increasingly used to collect cervicovaginal secretions to characterise vaginal mucosal immunology, in conjunction with high vaginal swabs (HVS) for metataxonomics, particularly in HIV transmission studies. We hypothesised that both methods of collecting bacterial biomass are equivalent for 16S rRNA gene sequencing. Material and Methods: Cervicovaginal fluid (CVF) samples from 16 pregnant women with HIV-1 (PWWH) were included to represent the major vaginal bacterial community state types (CST I-V). Women underwent sampling during the second trimester by liquid amies HVS followed by a MC (Soft discℱ) and samples were stored at -80°C. Bacterial cell pellets obtained from swab elution and MC (500 ”L, 1 in 10 dilution) were resuspended in 120 ”L PBS for DNA extraction. Bacterial 16S rRNA gene sequencing was performed using V1-V2 primers and were analysed using MOTHUR. Paired total DNA, bacterial load, amplicon read counts, diversity matrices and bacterial taxa were compared by sampling method using MicrobiomeAnalyst, SPSS and R. Results: The total DNA eluted from one aliquot of diluted CVF from an MC was similar to that of a HVS (993ng and 609ng, p=0.18); the mean bacterial loads were also comparable for both methods (MC: 8.0 log10 16S rRNA gene copies versus HVS: 7.9 log10 16S rRNA gene copies, p=0.27). The mean number of sequence reads generated from MC samples was lower than from HVS (MC: 12730; HVS:14830, p=0.05). The α-diversity metrices were similar for both techniques; MC Species Observed: 41 (range 12-96) versus HVS: 47 (range 16-96), p=0.15; MC Inverse Simpson Index: 1.98 (range 1.0-4.0) versus HVS: 0.48 (range 1.0-4.4), p=0.22). The three most abundant species observed were: Lactobacillus iners, Lactobacillus crispatus and Gardnerella vaginalis. Hierarchical clustering of relative abundance data showed that samples obtained using different techniques in an individual clustered in the same CST group. Conclusion: These data demonstrate that despite sampling slightly different areas of the lower genital tract, there was no difference in bacterial load or composition between methods. Both are suitable for characterisation of vaginal microbiota in PWWH. The MC offers advantages, including a higher volume of sample available for DNA extraction and complimentary assays

    Background: Pregnant women living with HIV infection (PWLWH) have elevated rates of preterm birth (PTB) in which HIV and cART are implicated. PWLWH also have a high prevalence of adverse vaginal microbiota, which associate with genital tract inflammation. The mechanism underlying PTB in PWLWH is unknown. We present the first data in PWLWH on genital-tract matrix-metalloproteinase-9(MMP-9), an important collagenase implicated in labour onset, and tissue inhibitor of metalloproteinases-1(TIMP-1) and explore correlations with local inflammation and vaginal bacteria
