    "Fat but powerful'' paradox: association of muscle power and adiposity markers with all-cause mortality in older adults from the EXERNET multicentre study

    Objectives: To assess the influence of muscle power and adiposity on all-cause mortality risk and to evaluate the fat but powerful'' (F+P) (or fat but fit'') paradox in older adults. Methods: A total of 2563 older adults (65 €''91 years old) from the EXERNET multicentre study were included. Adiposity (body mass index (BMI), waist circumference, body fat percentage (BF%) and fat index), allometric and relative power (sit-to-stand muscle power test) and various covariates (age, sex, hypertension, smoking status and walking and sitting times per day) were registered at baseline. All-cause mortality was recorded during a median follow-up of 8.9 years. Participants were classified into four groups: lean and powerful (L+P), F+P, lean but weak and fat and weak (F+W). Cox proportional hazard regression models and adjusted HRs were calculated. Results: According to BMI and waist circumference, all-cause mortality risk was reduced in the F+P (HR=0.55 and 0.63, p=0.044 and 0.049, respectively) and L+P (HR=0.57 and 0.58, p=0.043 and 0.025, respectively) groups. According to BF%, all-cause mortality decreased in the L+P group (HR=0.53; p=0.021), and a trend for a reduction was reported in the F+P group (HR=0.57; p=0.060). According to fat index, a survival benefit was only noted in the L+P group (HR=0.50; p=0.049). Higher levels of relative power reduced all-cause mortality risk among older people (HR=0.63 and 0.53, p=0.006 and 0.011, respectively). Conclusion: Powerful older people exhibited a reduced 9-year all-cause mortality regardless of BMI, waist circumference and BF%.     Dolor crónico: relación con sintomatología prefrontal y estrés percibido

    ABSTRACT Objective: The quality of perceived stress coping in chronic pain is related to the performance of the prefrontal cortex as the main structure of control and coordination of superior behavior control. The present study explores the presence of prefrontal symptomatology, in the form of problems to manage cognition, emotions and behavior, and perceived stress in a sample of people suffering from chronic pain. Material and methods: We studied 78 participants with a diagnosis of chronic pain ranging in age from 27 to 81 years (mean 54.2 and d.t 13.4). Sociodemographic and clinical variables were analyzed together with the results in the 11-item Short Brief Pain Questionnaire (CBD) to assess the intensity and interference of pain, the Prefrontal Symptom Inventory (ISP), both in its complete version of 46 items and in the abbreviated of 20, and the 10-itme Perceived Scale of Perceived Stress (EEP). Results and discussion: Significant correlations between the prefrontal symptomatology and the intensity (r = 0.32) and the interference (r = 0.53) of the pain, as well as between the stress and the interference of the pain in the mood (r = 0.36). People report more painful feelings when they refer more cognitive and emotional management problems surrounding the environment. The general interference of pain is related to more motivational and attention problems, while the interference that the pain produces in the mood also increases the problems with executive and emotional control. A preliminary structural equation explaining the effect is proposed. Conclusion: The data suggest that the stress perceived by people with chronic pain depends on the inability of the prefrontal cortex to cope with a changing or threatening situation and this problem is fed back over and over as the person is less able to cope with the environment. Therefore, comprehensive treatment of chronic pain should include psychological interventions focused on coping with stress and cognitive optimization of skills related to prefrontal functioning.RESUMEN Objetivo: La calidad del afrontamiento del estrés percibido en el dolor crónico está relacionada con el rendimiento de la corteza prefrontal como estructura principal de control y coordinación de control superior de la conducta. El presente trabajo estudia la presencia de sintomatología prefrontal, en forma de problemas para gestionar la cognición, las emociones y el comportamiento, y de estrés percibido en una muestra de personas que sufren dolor crónico. Material y métodos: Se estudiaron 78 participantes con diagnóstico de dolor crónico con edades comprendidas entre 27 y 81 años (media 54,2 y d.t. 13,4). Se analizaron variables sociodemográficas y clínicas junto con los resultados en el Cuestionario Breve de Dolor (CBD) abreviado de 11 ítems para valorar la intensidad e interferencia de dolor, el Inventario de Síntomas Prefrontales (ISP), tanto en su versión completa de 46 ítems como en la abreviada de 20, y la Escala de Estrés Percibido (EEP) abreviada de 10 ítems. Resultados y discusión: Aparecen correlaciones significativas entre la sintomatología prefrontal y la intensidad (r = 0,32) y la interferencia (r = 0,53) del dolor, así como entre el estrés y la interferencia del dolor en el estado del ánimo (r = 0,36). Las personas declaran más sensación dolorosa cuando refieren más problemas de gestión cognitiva y emocional del entorno que las rodea. La interferencia general del dolor se relaciona con más problemas motivacionales y de atención, mientras que la interferencia que el dolor produce en el estado del ánimo incrementa, además, los problemas con el control ejecutivo y emocional. Se propone una ecuación estructural preliminar explicativa del efecto. Conclusión: Los datos sugieren que el estrés percibido por las personas con dolor crónico depende de la incapacidad de la corteza prefrontal para afrontar una situación cambiante o amenazante, y este problema se retroalimenta una y otra vez al ser la persona cada vez menos capaz de afrontar el ambiente. Por tanto, el tratamiento integral del dolor crónico debe incluir intervenciones psicológicas centradas en el afrontamiento del estrés y la optimización cognitiva de las habilidades relacionadas con el funcionamiento prefrontal

    Uso y abuso de las Tecnologías de la Información y la Comunicación por adolescentes: un estudio representativo de la ciudad de Madrid

    La Cátedra para el Desarrollo Social de la Universidad Camilo José Cela (UCJC) y el Organismo Autónomo Madrid Salud del Ayuntamiento de Madrid han presentado el estudio ‘Uso y abuso de las Tecnologías de la Información y la Comunicación por adolescentes. Un estudio representativo de la ciudad de Madrid’ que tiene como resultado la gran prevalencia de problemas asociados al abuso de las TIC en esta población de adolescentes escolarizados. El estudio ha utilizado el MULTICAGE-TIC aplicado a una muestra de 2.341 estudiantes de 4º de la ESO (15 y 16 años) de centros educativos públicos, concertados y privados de la ciudad de Madrid. Se trata del primer estudio que analiza una muestra representativa –con bajo grado de error– de adolescentes escolarizados en una gran ciudad occidental para explorar el uso y abuso de las TIC y su relación con indicadores de salud mental. Los resultados apuntan a una gran prevalencia de problemas asociados al abuso de las TIC entre los adolescentes de 15 y 16 años. Según los resultados obtenidos, solo un 32% de los adolescentes hacen un uso adecuado de Internet, mientras que el 31,5% muestran señales de riesgo, un 23,3% mantienen una conducta de uso abusiva y un 13,2% muestran una clara dependencia comportamental en el uso de la red. En definitiva, más de un tercio de la muestra desarrolla un uso problemático de Internet y casi otro tercio está en riesgo de desarrollarlo. El estudio revela que más de la mitad de los adolescentes hacen un uso inadecuado del teléfono móvil: un 28,4% muestra un uso de riesgo, un 21% hace un uso abusivo y un 8% presenta dependencia de su smartphone. En cuanto al uso de Whatsapp, un 43,5% de los adolescentes presenta una conducta problemática en su uso. Concretamente, un 21% de los adolescentes muestra un uso de riesgo, un 14,5% un abuso y un 8% una dependencia. El estudio alerta de que el hecho de que casi la mitad de la muestra de adolescentes presente en mayor o menor grado un uso problemático de estas aplicaciones puede establecer patrones que persistan a lo largo del tiempo. Estos comportamientos tienen una relación directa con indicadores de mal aprovechamiento académico. En cuanto a las redes sociales, los adolescentes madrileños mostraron en casi un 40% un uso problemático: un 19% mostró un uso de riesgo, un 13% un abuso y un 7% una dependencia. En el caso de los videojuegos, un 24% de la muestra mostró un uso de riesgo, un 12,5% un abuso y el 5,7% una dependencia. El estudio evidencia diferencias entre los chicos y las chicas, ya que las adolescentes presentan puntuaciones de abuso o dependencia mucho más frecuentemente que los varones en todas las conductas problemáticas salvo en los videojuegos. El tamaño de las diferencias es muy considerable, especialmente en el uso de aplicaciones de mensajería instantánea y de redes sociales. Más del 90% de los participantes hacen uso de Whatsapp (un 96% de las chicas y un 92% de los chicos). La segunda aplicación más utilizada por los jóvenes es Youtube, con un 90% de las muestras y sin diferencias por sexo. En tercer lugar, Instagram es utilizado por el 87% de las chicas y significativamente menos por los chicos, un 74%. Lejos queda el uso de Snapchat, al que recurren más las chicas (71%) que los chicos (41%). Más de un 40% utiliza Spotify y menos de la tercera parte de la muestra hace uso de Spotify. Facebook y Twitter no evidencian un uso significativo por los adolescentes. El estudio concluye que, a mayor grado de uso problemático de las TIC, la puntuación en riesgo de mala salud mental es mayor. El incremento de los problemas asociados al uso y abuso de las TIC contribuye a la pérdida de control superior de la conducta. La inmadurez cerebral propia de la adolescencia es un importante factor de vulnerabilidad para la pérdida de control superior de la conducta. Sin embargo, estos factores solo explican una parte pequeña del abuso de las TIC, pues debe atenderse a variables no estudiadas como la presión de grupo, factores culturales o modas. El 5% de los adolescentes declara jugar con mucha frecuencia en webs de apuestas deportivas y casi un 12% lo ha hecho en alguna ocasión. Éste es un problema mayoritariamente de los adolescentes varones.     MULTICAGE CAD-4 para la detección de adicciones conductuales: validez estructural después de la inclusión de una escala en el abuso de teléfonos inteligentes

    Addictive behaviors are not limited to drugs use, but also include certain daily behaviors that can cause gratification. Their progression to more severe pathological patterns entails grave consequences for the individual, including multiple psychopathological manifestations. The early detection of this type of behavior is of concern to primary health care. Therefore, in order to detect risk at early stages, reliable and valid tools for daily practice are essential. The MULTICAGE CAD-4 questionnaire is a screening tool for simultaneously detecting addictive behaviors. This study includes a new scale for the detection of smartphone abuse. The objective is to evaluate the adequacy of its psychometric properties. A sample of 2,074 subjects that were recruited from primary care centers ofMadrid(Spain) completed the MULTICAGE CAD-4 questionnaire. A confirmatory factor analysis, using unweighted least squares method, was performed. The test showed good internal consistency both at item and scale levels. The questionnaire structure was consistent with theoretical expectations. The MULTICAGE CAD-4, including the new smartphone scale, is a robust, reliable tool with a valid structure for assessing the presence of dysfunctional or potentially addictive behaviors, and especially useful in primary health care services.Los comportamientos adictivos no se limitan al uso de drogas, sino que también incluyen ciertos comportamientos diarios que pueden causar gratificación. Su progresión a patrones patológicos más severos conlleva graves consecuencias para el individuo, incluidas múltiples manifestaciones psicopatológicas. La detección temprana de este tipo de comportamiento es de interés para la atención primaria de salud. Por lo tanto, para detectar riesgos en etapas tempranas, las herramientas confiables y válidas para la práctica diaria son esenciales. El cuestionario MULTICAGE CAD-4 es una herramienta de detección para detectar simultáneamente conductas adictivas. Este estudio incluye una nueva escala para la detección del abuso de teléfonos inteligentes. El objetivo es evaluar la adecuación de sus propiedades psicométricas. Una muestra de 2.074 sujetos reclutados en centros de atención primaria de Madrid (España) completó el cuestionario MULTICAGE CAD-4. Se realizó un análisis factorial confirmatorio, utilizando el método de mínimos cuadrados no ponderados. La prueba mostró una buena consistencia interna tanto a nivel del ítem como de las escalas. La estructura del cuestionario fue consistente con las expectativas teóricas. El MULTICAGE CAD-4, incluida la nueva escala para teléfonos inteligentes, es una herramienta robusta y confiable con una estructura válida para evaluar la presencia de comportamientos disfuncionales o potencialmente adictivos, y especialmente útil en servicios de atención primaria de salud

    Extracellular vesicles from pristane-treated CD38-deficient mice express an antiinflammatory neutrophil protein signature, which reflects the mild lupus severity elicited in these mice

    In CD38-deficient (Cd38-/-) mice intraperitoneal injection of pristane induces a lupus-like disease, which is milder than that induced in WT mice, showing significant differences in the inflammatory and autoimmune processes triggered by pristane. Extracellular vesicles (EV) are present in all body fluids. Shed by cells, their molecular make-up reflects that of their cell of origin and/or tissue pathological situation. The aim of this study was to analyze the protein composition, protein abundance, and functional clustering of EV released by peritoneal exudate cells (PECs) in the pristane experimental lupus model, to identify predictive or diagnostic biomarkers that might discriminate the autoimmune process in lupus from inflammatory reactions and/or normal physiological processes. In this study, thanks to an extensive proteomic analysis and powerful bioinformatics software, distinct EV subtypes were identified in the peritoneal exudates of pristane-treated mice: 1) small EV enriched in the tetraspanin CD63 and CD9, which are likely of exosomal origin; 2) small EV enriched in CD47 and CD9, which are also enriched in plasma-membrane, membrane-associated proteins, with an ectosomal origin; 3) small EV enriched in keratins, ECM proteins, complement/coagulation proteins, fibrin clot formation proteins, and endopetidase inhibitor proteins. This enrichment may have an inflammation-mediated mesothelial-tomesenchymal transition origin, representing a protein corona on the surface of peritoneal exudate EV; 4) HDL-enriched lipoprotein particles. Quantitative proteomic analysis allowed us to identify an anti-inflammatory, Annexin A1- enriched pro-resolving, neutrophil protein signature, which was more prominent in EV from pristane-treated Cd38-/- mice, and quantitative differences in the protein cargo of the ECM-enriched EV from Cd38-/- vs WT mice. These differences are likely to be related with the distinct inflammatory outcome shown by Cd38-/- vs WT mice in response to pristane treatment. Our results demonstrate the power of a hypothesis-free and data-driven approach to transform the heterogeneity of the peritoneal exudate EV from pristanetreated mice in valuable information about the relative proportion of different EV in a given sample and to identify potential protein markers specific for the different small EV subtypes, in particular those proteins defining EV involved in the resolution phase of chronic inflammation.Proyecto del plan estatal, Ministerio de Ciencia e Innovacion PT13/0001/011CSIC PT17/0019/0010 PID2020-119567RB-I0

    Physical Activity Patterns of the Spanish Population Are Mostly Determined by Sex and Age: Findings in the ANIBES Study

    Background Representative data for the Spanish population regarding physical activity (PA) behaviors are scarce and seldom comparable due to methodological inconsistencies. Aim Our objectives were to describe the PA behavior by means of the standardized self-reported International Physical Activity Questionnaire (IPAQ) and to know the proportion of the Spanish population meeting and not meeting international PA recommendations. Material and Methods PA was assessed using the IPAQ in a representative sample of 2285 individuals (males, 50.4%) aged 9–75 years and living in municipalities of at least 2,000 inhabitants. Data were analyzed according to: age groups 9–12, 13–17, 18–64, and 65–75 years; sex; geographical distribution; locality size and educational levels. Results Mean total PA was 868.8±660.9 min/wk, mean vigorous PA 146.4±254.1 min/wk, and mean moderate PA 398.1±408.0 min/wk, showing significant differences between sexes (p<0.05). Children performed higher moderate-vigorous PA than adolescents and seniors (p<0.05), and adults than adolescents and seniors (p<0.05). Compared to recommendations, 36.2%of adults performed <150 min/week of moderate PA, 65.4% <75 min/week of vigorous PA and 27.0%did not perform any PA at all, presenting significant differences between sexes (p<0.05). A total of 55.4%of children and adolescents performed less than 420 min/week of MVPA, being higher in the later (62.6%) than in the former (48.4%). Highest non-compliance was observed in adolescent females (86.5%). Conclusion Sex and age are the main influencing factors on PA in the Spanish population. Males engage in more vigorous and light PA overall, whereas females perform more moderate PA. PA behavior differs between age groups and no clear lineal increase with age could be observed. Twenty-seven percent of adults and 55.4% of children and adolescents do not meet international PA recommendations.     Ideal cardiovascular health and inflammation in European adolescents: The HELENA study

    Background and aims Inflammation plays a key role in atherosclerosis and this process seems to appear in childhood. The ideal cardiovascular health index (ICHI) has been inversely related to atherosclerotic plaque in adults. However, evidence regarding inflammation and ICHI in adolescents is scarce. The aim is to assess the association between ICHI and inflammation in European adolescents. Methods and results As many as 543 adolescents (251 boys and 292 girls) from the Healthy Lifestyle in Europe by Nutrition in Adolescence (HELENA) study, a cross-sectional multi-center study including 9 European countries, were measured. C-reactive protein (CRP), complement factors C3 and C4, leptin and white blood cell counts were used to compute an inflammatory score. Multilevel linear models and multilevel logistic regression were used to assess the association between ICHI and inflammation controlling by covariates. Higher ICHI was associated with a lower inflammatory score, as well as with several individual components, both in boys and girls (p < 0.01). In addition, adolescents with at least 4 ideal components of the ICHI had significantly lower inflammatory score and lower levels of the study biomarkers, except CRP. Finally, the multilevel logistic regression showed that for every unit increase in the ICHI, the probability of having an inflammatory profile decreased by 28.1% in girls. Conclusion Results from this study suggest that a better ICHI is associated with a lower inflammatory profile already in adolescence. Improving these health behaviors, and health factors included in the ICHI, could play an important role in CVD prevention

    Dietary animal and plant protein intakes and their associations with obesity and cardio-metabolic indicators in European adolescents: The HELENA cross-sectional study

    Background: Previous studies suggest that dietary protein might play a beneficial role in combating obesity and its related chronic diseases. Total, animal and plant protein intakes and their associations with anthropometry and serum biomarkers in European adolescents using one standardised methodology across European countries are not well documented. Objectives: To evaluate total, animal and plant protein intakes in European adolescents stratified by gender and age, and to investigate their associations with cardio-metabolic indicators (anthropometry and biomarkers). Methods: The current analysis included 1804 randomly selected adolescents participating in the HELENA study (conducted in 2006-2007) aged 12.5-17.5 y (47% males) who completed two non-consecutive computerised 24-h dietary recalls. Associations between animal and plant protein intakes, and anthropometry and serum biomarkers were examined with General linear Model multivariate analysis. Results: Average total protein intake exceeded the recommendations of World Health Organization and European Food Safety Authority. Mean total protein intake was 96 g/d (59% derived from animal protein). Total, animal and plant protein intakes (g/d) were significantly lower in females than in males and total and plant protein intakes were lower in younger participants (12.5-14.9 y). Protein intake was significantly lower in underweight subjects and higher in obese ones; the direction of the relationship was reversed after adjustments for body weight (g/(kg.d)). The inverse association of plant protein intakes was stronger with BMI z-score and body fat percentage (BF%) compared to animal protein intakes. Additionally, BMI and BF% were positively associated with energy percentage of animal protein. Conclusions: This sample of European adolescents appeared to have adequate total protein intake. Our findings suggest that plant protein intakes may play a role in preventing obesity among European adolescents. Further longitudinal studies are needed to investigate the potential beneficial effects observed in this study in the prevention of obesity and related chronic diseases
