    Swimming training repercussion on metabolic and structural bone development; benefits of the incorporation of whole body vibration or pilometric training; the RENACIMIENTO project

    Introduction: Enviromental factors such as exercise participation and nutrition have often been linked to bone improvements. However, not all sports have the same effects, being non-osteogenic sports such as swimming defined as negative or neutral sports to practice regarding bone mass by some authors, similarly exercisediet interaction in especific groups is still not clear. Objective: To present the methodology of the RENACIMENTO project that aims to evaluate body composition and more specifically bone mass by several techniques in adolescent swimmers and to observe the effects and perdurability of whole body vibration (WBV) and jumping intervention (JIN) on body composition and fitness on this population and explore posible diet interactions. Design: Randomized controlled trial. Methods: 78 swimmers (12-17 y) and 26 sex- and agematched controls will participate in this study. Dual energy X-ray, peripheral Quantitative Computed Tomography, Quantitative Ultrasound, Bioelectrical Impedance Analysis, and anthropometry measurements will be performed in order to evaluate body composition. Physical activity, nutrition, pubertal development and socio-economical status may act as confounders of body composition and therefore will also be registered. Several fitness factors regarding strength, endurance, performance and others will also be registered to evaluate differences with controls and act as confounders. A 7- month WBV therapy will be performed by 26 swimmers consisting of a training of 15 minutes 3 times per week. An 8 month JIM will also be performed by 26 swimmers 3 times per week. The remaining 26 swimmers will continue their normal swimming training. Four evaluations will be performed, the first one in order to describe differences between swimmers and controls. The second one to describe the effects of the interventions and the third and fourth evaluations to describe the perdurability of the effects of the WBV and JIN. Conclusion: The RENACIMIENTO project will allow to answer several questions regarding body composition, fitness, bone mass and interaction with diet of adolescent swimmers, describe swimming as a positive, negative or neutral sport to practice regarding these parameters and elucidate the effects and perdurability of WBV and JIM on body composition. Introducción: En la actualidad se ha demostrado que el ejercicio físico y la nutrición mejoran la masa ósea. Sin embargo, existen deportes de bajo impacto como la natación que no presentan efectos positivos en su desarrollo. Además, la interacción ejercicio-dieta y su efecto osteogénico sigue sin estar clara. Objetivo: Presentar la metodología del proyecto RENACIMIENTO que tiene por objetivo evaluar la composición corporal del nadador adolescente y más concretamente la masa ósea a través de diversas técnicas. Además se pretenden determinar los efectos y la perdurabilidad que pueden tener el entrenamiento vibratorio (WBV) y una intervención con saltos (JIN) sobre la composición corporal y condición física de estos adolescentes, explorando la posible interacción con la dieta. Diseño: Ensayo clínico aleatorizado. Metodología: 78 nadadores (12-17 años) y 26 controles del mismo sexo y edad participarán en el estudio. Se utilizarán absorciometría dual de rayos-x, tomografía axial computerizada, ultrasonidos cuantitativo, bioimpedancia eléctrica, y medidas antropométricas para evaluar la composición corporal. La actividad física, nutrición, desarrollo puberal y status socio-económico podrían actuar como covariables de la composición corporal y por lo tanto también serán registradas. Se evaluarán diversos factores de la condición física relacionados con la fuerza, resistencia, rendimiento y otros para definir las diferencias con los controles y para que sirvan como covariables. 26 nadadores realizarán una intervención de WBV 7 meses 15 minutos 3 veces por semana. Además otros 26 nadadores realizarán una JIN 3 veces por semana durante 8 meses. Los 26 nadadores restantes continuarán con su entrenamiento habitual de natación. Se realizaran 4 evaluaciones, la primera de ellas para describir las diferencias existentes entre nadadores y controles, la segunda para describir los efectos de las intervenciones realizadas y la tercera y cuarta para evaluar la perdurabilidad de estas intervenciones. Conclusión: El proyecto RENACIMIENTO permitirá contestar a diversas preguntas relacionadas con la composición corporal, condición física y masa ósea de los nadadores adolescentes y el posible efecto combinado dieta-ejercicio en esta población. Definirá la natación como un deporte positivo, neutral o negativo en relación con estos parámetros y esclarecerá los efectos y perdurabilidad de la WBV y JIN sobre la composición corporal

    EFECTOS DEL ENTRENAMIENTO MULTICOMPONENTE “ELDERFIT” EN PERSONAS MAYORES CON SARCOPENIA

    Introducción: La disminución de la capacidad funcional es común a la fragilidad y la sarcopenia. El ejercicio físico de tipo multicomponente parece ser un método eficaz en su mejora, pudiendo constituir una herramienta de prevención y tratamiento de ambas. Objetivo: Evaluar los cambios en la capacidad funcional, fragilidad y riesgo de sarcopenia en personas de al menos en riesgo de sarcopenia y sin sarcopenia mediante un programa de ejercicio multicomponente “ELDERFIT” en la población adulta mayor.Metodología: Un total de 92 adultos mayores (69 mujeres y 23 hombres) (80,6 6,2 años) participaron en este estudio. Se dividieron en grupo intervención (GI; n= 49) y grupo control (GC; n= 43) por conveniencia. El GI realizó un programa de entrenamiento físico multicomponente supervisado de 6 meses y 3 días a la semana, mientras que el GC continuó con sus actividades habituales y se realizaron 2 evaluaciones; una al inicio (pre) y otra tras los 6 meses de entrenamiento (post).Estadística: Tras analizar la normalidad (Saphiro-Wilk), se realizó un ANOVA de medidas repetidas para estudiar si había cambios significativos tras los seis meses de entrenamiento entre los grupos (GxT) y dentro de cada grupo.Resultados: Tras los 6 meses de entrenamiento el GI con riesgo de sarcopenia y sin sarcopenia mejoraron en las variables usadas en los criterios de sarcopenia, en handgrip y velocidad de la marcha (todas p0,05). En el GI hay un mayor número de personas que mejoran su estado de sarcopenia, siendo estas las personas que consiguen mejorar su masa muscular (todas pConclusiones: El programa de entrenamiento multicomponente de 6 meses parece mejorar las variables asociadas a la sarcopenia y fragilidad en adultos mayores y por tanto revertir el grado de sarcopenia. Se recomienda este tipo de entrenamiento para mejorar fuerza, velocidad de marcha y la capacidad funcional en esta población. <br /

    Influence of different playing surfaces on bone mass accretion in male adolescent football players: A one-season study

    There are different surfaces on which football is played, but their influence on bone mass accretion still remains unknown. The aims of this study were to compare bone mass accretion between football players and controls, and evaluate the influence of two different playing surfaces on bone accretion. A total of 27 male football players (13.2 +/- 0.5 years) and 15 controls (12.6 +/- 1.1 years) participated in this study. Football players were classified into two groups according to the surface they trained on: 14 on third-generation artificial turf with elastic layer and 13 on third-generation artificial turf without elastic layer. Bone mineral content and areal bone mineral density were measured using dual-energy X-ray absorptiometry. Bone mineral apparent density variables were calculated. Bone geometry and strength of the non-dominant tibia were assessed with peripheral quantitative computed tomography. For both football players and controls, bone variables measured at subtotal body, lumbar spine, legs and tibia (p < 0.05) significantly increased. Based on the time spent practicing football, the increase in areal bone mineral density for the legs (p < 0.05) was higher in football players than controls. Moreover, lumbar spine bone mineral apparent density increased more in third-generation artificial turf without elastic layer players in comparison with third-generation artificial turf with elastic layer players (p < 0.05). Playing football on third-generation artificial turf with elastic layer and third-generation artificial turf without elastic layer seems to positively affect bone mass during growth. After playing for one season on these playing surfaces, football practice on third-generation artificial turf without elastic layer with the lower shock absorption seems to have produced the highest increment in areal bone mineral density at lumbar spine. Thus, football practice on surfaces with lower shock absorption could provide an extra benefit on bone health

    Harmonization Process and Reliability Assessment of Anthropometric Measurements in the Elderly EXERNET Multi-Centre Study

    Background: The elderly EXERNET multi-centre study aims to collect normative anthropometric data for old functionally independent adults living in Spain.Purpose: To describe the standardization process and reliability of the anthropometric measurements carried out in the pilot study and during the final workshop, examining both intra- and inter-rater errors for measurements.Materials and Methods: A total of 98 elderly from five different regions participated in the intra-rater error assessment, and 10 different seniors living in the city of Toledo (Spain) participated in the inter-rater assessment. We examined both intra- and inter-rater errors for heights and circumferences.Results: For height, intra-rater technical errors of measurement (TEMs) were smaller than 0.25 cm. For circumferences and knee height, TEMs were smaller than 1 cm, except for waist circumference in the city of Ca´ceres. Reliability for heights and circumferences was greater than 98% in all cases. Inter-rater TEMs were 0.61 cm for height, 0.75 cm for knee-height and ranged between 2.70 and 3.09 cm for the circumferences measured. Inter-rater reliabilities for anthropometric measurements were always higher than 90%.Conclusion: The harmonization process, including the workshop and pilot study, guarantee the quality of the anthropometric measurements in the elderly EXERNET multi-centre study. Method: 1092 participants (80.3±5.6y;66.5%women) from two Spanish cohorts were included. Telephone-based questionaries were used to evaluate health risk behaviours and lifestyle during lockdown and 7-months later. Self-reported physician-based diagnosis of chronic diseases was also reported. Cox-proportional models adjusted for main confounders were applied. Results: Improvements concerning lifestyle were found in older people with chronic diseases, although they worsened the physical component (except cancer). When they were compared to those without these diseases, hypertension was associated with a lower frequency of increased alcohol consumption (Hazard ratio:0.73[95% confidence interval:0.55;0.99]). Pulmonary diseases were associated with a lower risk of both decreased sedentary time (0.58[0.39;0.86]) and worsening sleep quality (0.56[0.36;0.87]), while CVD was only associated with a lower frequency of decreased sedentary time (0.58[0.38;0.88]). Depression was linked to a higher risk of increasing diet quality (1.53[1.00;2.36]). Cancer was less likely to worsen sleep quality (0.44[0.22;0.89]), but more likely to worsen their social contact frequency (2.05[1.05;3.99]). No significant association related to musculoskeletal diseases. Conclusions: Beneficial changes in health risk behaviours and lifestyle after the COVID-19 lockdown in older people with chronic diseases were found. Particularly, older people with hypertension, pulmonary disease and cancer showed beneficial changes after lockdown compared to their counterparts without diseases. Those with CVD and depression showed lifestyles that could involve a health risk

    Bone Mass and the CAG and GGN Androgen Receptor Polymorphisms in Young Men

    BACKGROUND: To determine whether androgen receptor (AR) CAG (polyglutamine) and GGN (polyglycine) polymorphisms influence bone mineral density (BMD), osteocalcin and free serum testosterone concentration in young men. METHODOLOGY/PRINCIPAL FINDINGS: Whole body, lumbar spine and femoral bone mineral content (BMC) and BMD, Dual X-ray Absorptiometry (DXA), AR repeat polymorphisms (PCR), osteocalcin and free testosterone (ELISA) were determined in 282 healthy men (28.6+/-7.6 years). Individuals were grouped as CAG short (CAG(S)) if harboring repeat lengths of < or = 21 or CAG long (CAG(L)) if CAG > 21, and GGN was considered short (GGN(S)) or long (GGN(L)) if GGN < or = 23 or > 23. There was an inverse association between logarithm of CAG and GGN length and Ward's Triangle BMC (r = -0.15 and -0.15, P<0.05, age and height adjusted). No associations between CAG or GGN repeat length and regional BMC or BMD were observed after adjusting for age. Whole body and regional BMC and BMD values were similar in men harboring CAG(S), CAG(L), GGN(S) or GGN(L) AR repeat polymorphisms. Men harboring the combination CAG(L)+GGN(L) had 6.3 and 4.4% higher lumbar spine BMC and BMD than men with the haplotype CAG(S)+GGN(S) (both P<0.05). Femoral neck BMD was 4.8% higher in the CAG(S)+GGN(S) compared with the CAG(L)+GGN(S) men (P<0.05). CAG(S), CAG(L), GGN(S), GGN(L) men had similar osteocalcin concentration as well as the four CAG-GGN haplotypes studied. CONCLUSION: AR polymorphisms have an influence on BMC and BMD in healthy adult humans, which cannot be explained through effects in osteoblastic activity

    Reliability and validity of the Adolescent Stress Questionnaire in a sample of European adolescents--the HELENA study

    BACKGROUND: Since stress is hypothesized to play a role in the etiology of obesity during adolescence, research on associations between adolescent stress and obesity-related parameters and behaviours is essential. Due to lack of a well-established recent stress checklist for use in European adolescents, the study investigated the reliability and validity of the Adolescent Stress Questionnaire (ASQ) for assessing perceived stress in European adolescents. METHODS: The ASQ was translated into the languages of the participating cities (Ghent, Stockholm, Vienna, Zaragoza, Pecs and Athens) and was implemented within the HELENA cross-sectional study. A total of 1140 European adolescents provided a valid ASQ, comprising 10 component scales, used for internal reliability (Cronbach α) and construct validity (confirmatory factor analysis or CFA). Contributions of socio-demographic (gender, age, pubertal stage, socio-economic status) characteristics to the ASQ score variances were investigated. Two-hundred adolescents also provided valid saliva samples for cortisol analysis to compare with the ASQ scores (criterion validity). Test-retest reliability was investigated using two ASQ assessments from 37 adolescents. RESULTS: Cronbach α-values of the ASQ scales (0.57 to 0.88) demonstrated a moderate internal reliability of the ASQ, and intraclass correlation coefficients (0.45 to 0.84) established an insufficient test-retest reliability of the ASQ. The adolescents' gender (girls had higher stress scores than boys) and pubertal stage (those in a post-pubertal development had higher stress scores than others) significantly contributed to the variance in ASQ scores, while their age and socio-economic status did not. CFA results showed that the original scale construct fitted moderately with the data in our European adolescent population. Only in boys, four out of 10 ASQ scale scores were a significant positive predictor for baseline wake-up salivary cortisol, suggesting a rather poor criterion validity of the ASQ, especially in girls. CONCLUSIONS: In our European adolescent sample, the ASQ had an acceptable internal reliability and construct validity and the adolescents' gender and pubertal stage systematically contributed to the ASQ variance, but its test-retest reliability and criterion validity were rather poor. Overall, the utility of the ASQ for assessing perceived stress in adolescents across Europe is uncertain and some aspects require further examination.The HELENA Study takes place with the financial support of the European Community Sixth RTD Framework Programme (Contract FOOD-CT-2005-007034)

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.
