    Increased HCMV seroprevalence in patients with hepatocellular carcinoma.

    International audienceABSTRACT: BACKGROUND: Hepatocellular carcinoma (HCC) is the most common primary liver cancer, usually arising after years of chronic liver inflammation that could result from viral infections such as hepatitis B virus (HBV) and hepatitic C virus (HCV) infections. Human cytomegalovirus (HCMV) infects primary human hepatocytes and remains an important cause of morbidity in immunocompromised persons where it may manifest as symptomatic end-organ disease including hepatitis. The goal of the present study was to determine a potential correlation between HCMV infection and the appearance of HCC. METHODS: First, we analyzed the seroprevalence of HCMV in a cohort of 11,318 patients hospitalized between 2003 and 2009 in different departments of a French University Hospital. Second, we studied HCMV seroprevalence in a cohort of 190 subjects who were stratified on the basis of age, gender, HCC, cirrhosis (Cir), and the exposition to hepatotropic viruses (HCV, HBV). We further determined whether HCMV DNA was present specifically in tumour area in liver biopsies from HCC-positive patients by using nested PCR. RESULTS: We found that the HCMV seroprevalence was high in the Hepatology department. The HCMV seroprevalence was significantly higher in patients infected with HCV and/or HBV than in patients who were not infected by those later viruses (76.2% versus 56.5%, p < 0.001). The HCMV seroprevalence was significantly higher in patients with HCC (74%) and lower in patients without HCC (54% for HCC-/Cir+ patients, 57% for HCC-/Cir- subjects). We observed a positive correlation between serum IL-6 levels and HCMV seroprevalence in cirrhotic patients, but not in HCC patients. Serum IL-6 levels correlated positively with C-reactive protein (CRP) levels. Preliminary histological studies from liver biopsies from HCC-positive patients highlighted that HCMV DNA can be detected in tumour area of some of the patients studied. CONCLUSIONS: Our results indicate that HCMV seroprevalence in patients with HCC is significantly higher than in patients without HCC, is positively correlated with serum IL-6 levels in cirrhotic patients, and is positively associated with the presence of other hepatotropic viruses such as HCV and HBV

    Cytomegalovirus and Tumors: Two Players for One Goal-Immune Escape

    Cytomegalovirus (CMV) and the human tumor cell share the same objectives: escape the recognition and destruction by the immune system and establish a state of immune tolerance conducive for their development. For early tumor development, the escape of the first lines of defense of the immune surveillance is a critical step which determines survival or destruction. The presence of CMV on the tumor site and its involvement in carcinogenesis as initiator or promoter is increasingly documented. In this article, we highlight the similarity between mechanisms used by tumors and CMV to circumvent the immune defenses and evade from immune surveillance. We suggest that CMV and tumors help one another for their common objective. CMV gets shelter in immunologically poor environment of the tumor cells. In return CMV, by acting directly on the cancer cell and/or on the tumor microenvironment, provides the tumor cell the ways to promote its immune escape and development of immune tolerance

    New distances to RAVE stars

    Probability density functions are determined from new stellar parameters for the distance moduli of stars for which the RAdial Velocity Experiment (RAVE) has obtained spectra with S/N>=10. Single-Gaussian fits to the pdf in distance modulus suffice for roughly half the stars, with most of the other half having satisfactory two-Gaussian representations. As expected, early-type stars rarely require more than one Gaussian. The expectation value of distance is larger than the distance implied by the expectation of distance modulus; the latter is itself larger than the distance implied by the expectation value of the parallax. Our parallaxes of Hipparcos stars agree well with the values measured by Hipparcos, so the expectation of parallax is the most reliable distance indicator. The latter are improved by taking extinction into account. The effective temperature absolute-magnitude diagram of our stars is significantly improved when these pdfs are used to make the diagram. We use the method of kinematic corrections devised by Schoenrich, Binney & Asplund to check for systematic errors for general stars and confirm that the most reliable distance indicator is the expectation of parallax. For cool dwarfs and low-gravity giants tends to be larger than the true distance by up to 30 percent. The most satisfactory distances are for dwarfs hotter than 5500 K. We compare our distances to stars in 13 open clusters with cluster distances from the literature and find excellent agreement for the dwarfs and indications that we are over-estimating distances to giants, especially in young clusters.Comment: 20 pages accepted by MNRAS.     Influence of neutropenia on mortality of critically ill cancer patients : results of a meta-analysis on individual data

    Background: The study objective was to assess the influence of neutropenia on outcome of critically ill cancer patients by meta-analysis of individual data. Secondary objectives were to assess the influence of neutropenia on outcome of critically ill patients in prespecified subgroups (according to underlying tumor, period of admission, need for mechanical ventilation and use of granulocyte colony stimulating factor (G-CSF)). Methods: Data sources were PubMed and the Cochrane database. Study selection included articles focusing on critically ill cancer patients published in English and studies in humans from May 2005 to May 2015. For study selection, the study eligibility was assessed by two investigators. Individual data from selected studies were obtained from corresponding authors. Results: Overall, 114 studies were identified and authors of 30 studies (26.3% of selected studies) agreed to participate in this study. Of the 7515 included patients, three were excluded due to a missing major variable (neutropenia or mortality) leading to analysis of 7512 patients, including 1702 neutropenic patients (22.6%). After adjustment for confounders, and taking study effect into account, neutropenia was independently associated with mortality (OR 1.41; 95% CI 1.23-1.62; P = 0.03). When analyzed separately, neither admission period, underlying malignancy nor need for mechanical ventilation modified the prognostic influence of neutropenia on outcome. However, among patients for whom data on G-CSF administration were available (n = 1949; 25.9%), neutropenia was no longer associated with outcome in patients receiving G-CSF (OR 1.03; 95% CI 0.70-1.51; P = 0.90). Conclusion: Among 7512 critically ill cancer patients included in this systematic review, neutropenia was independently associated with poor outcome despite a meaningful survival. Neutropenia was no longer significantly associated with outcome in patients treated by G-CSF, which may suggest a beneficial effect of G-CSF in neutropenic critically ill cancer patients

    The rose of the Sainte-Chapelle in Paris: sophisticated stained glasses for late medieval painters

    The restoration of the rose (15th century) of the Sainte-Chapelle in Paris, France, offered a unique opportunity to investigate the color and chemical composition of these emblematic medieval French stained glasses with non-destructive analyses. The obtained results are aimed at complementing the knowledge from art historians and thus together trying to compensate for the total absence of archives on the construction of the rose. Comparison with the glasses of the nave (13th century) reveals an important evolution of the aesthetics based on new types of glasses: new colors and extensive use of flashed glass. The systematic study of the chemical composition of both sides of each glass piece revealed that about half of the studied glasses were flashed. For non-flashed glasses, this comparison allowed evaluating the influence of glass surface weathering, although very moderate, on the composition variability. In light of the variability criteria, the multivariate analysis of the chemical composition allowed inferring that most glasses originate from the same production glasshouse. The new colors result from the original composition of flashed glass, allowing superimposing otherwise incompatible redox states of the coloring transition elements. The comparison with the glasses of the nave reveals the glass technology evolution that occurred over two centuries and allowed the production of new glasses for the medieval glaziers at the eve of the Parisian Renaissance

    Big data, small concepts:Histosophy as an approach to longue-durée history

    In this essay, we sketch out a method, histosophy, which makes possible the study of intellectual history and conceptual genealogy both in depth and over long periods of time. Histosophy uses digital tools to survey ‘large issues within small compasses.’ A genealogy of signifiers, it considers metonymic parts of a problem in order to contribute precisely and coherently to a larger perspective. We outline the theoretical contours of our approach. We exemplify how it works in practice by looking at the signifier ‘esprit de corps’, the study of which is presented in detail in the histosophical book The Genealogy of Esprit de Corps(Edinburgh University Press, 2019). The phrase ‘esprit de corps’ has been widely used since the eighteenth century in different discourses (political, military, sociological, etc.), but it is sufficiently limited that its genealogy can be traced across centuries and nations with precision, coherence, clarity, and with the help of automated search engines. By contrast, related but bigger concepts like freedom, individualism or solidarity are part of dozens of disparate and fuzzy discourses, so often uttered that the analysis of modern uses is problematic.     Improved distances and ages for stars common to TGAS and RAVE

    ABSTRACT We combine parallaxes from the first Gaia data release with the spectrophotometric distance estimation framework for stars in the fifth RAVE survey data release. The combined distance estimates are more accurate than either determination in isolation – uncertainties are on average two times smaller than for RAVE-only distances (three times smaller for dwarfs), and 1.4 times smaller than TGAS parallax uncertainties (two times smaller for giants). We are also able to compare the estimates from spectrophotometry to those from Gaia, and use this to assess the reliability of both catalogues and improve our distance estimates.We find that the distances to the lowest log g stars are, on average, overestimated and caution that they may not be reliable. We also find that it is likely that the Gaia random uncertainties are smaller than the reported values. As a byproduct we derive ages for the RAVE stars, many with relative uncertainties less than 20 percent. These results for 219 566 RAVE sources have been made publicly available, and we encourage their use for studies that combine the radial velocities provided by RAVE with the proper motions provided by Gaia. A sample that we believe to be reliable can be found by taking only the stars with the flag notification ‘flag_any=0’.     Correlations between age, kinematics, and chemistry as seen by the RAVE survey

    We explore the connections between stellar age, chemistry, and kinematics across a Galactocentric distance of 7.5 < R(kpc) < 9.0, using a sample of ∼12 000 intermediate-mass (FGK) turn-off stars observed with the RAdial Velocity Experiment (RAVE) survey. The kinematics of this sample are determined using radial velocity measurements from RAVE, and parallax and proper motion measurements from the Tycho–Gaia Astrometric Solution (TGAS). In addition, ages for RAVE stars are determined using a Bayesian method, taking TGAS parallaxes as a prior. We divide our sample into young (0 < τ < 3 Gyr) and old (8 < τ < 13 Gyr) populations, and then consider different metallicity bins for each of these age groups. We find significant differences in kinematic trends of young and old, metal-poor and metal-rich, stellar populations. In particular, we find a strong metallicity dependence in the mean Galactocentric radial velocity as a function of radius (∂VR/∂R) for young stars, with metal-rich stars having a much steeper gradient than metal-poor stars. For ∂Vϕ/∂R, young, metal-rich stars significantly lag the LSR with a slightly positive gradient, while metal-poor stars show a negative gradient above the LSR. We interpret these findings as correlations between metallicity and the relative contributions of the non-axisymmetries in the Galactic gravitational potential (the spiral arms and the bar) to perturb stellar orbits.∂Vϕ/∂R, young, metal-rich stars significantly lag the LSR with a slightly positive gradient, while metal-poor stars show a negative gradient above the LSR.
