    Navigating surgical anatomy of the Denonvilliers' fascia and dissection planes of the anterior mesorectum with a cadaveric simulation model

    Anterior dissection of the rectum in the male pelvis represents one of the most complex phases of total meso-rectal excision. However, the possible existence of different anatomical planes is controversial and the exact anatomical topography of Denonvilliers' fascia is still debated. The aim of the study is to accurately define in a cadaveric simulation model the existence and boundaries of Denonvilliers' fascia, identifying the anatomical planes suitable for surgical dissection. The pelvises of 31 formalin-preserved male cadavers were dissected. Careful and detailed dissection was carried out to visualize the anatomical structures and the potential dissection planes, simulating an anterior meso-rectum dissection. Denonvilliers' fascia was identified in 100% of the pelvises, as a single-layer fascia that originates from the peritoneal reflection and descends until its firm adhesion to the prostate capsule. The fascia divides the space providing an anterior and a posterior plane. Anteriorly to the fascia, during the caudal dissection, its firm adhesion to the prostate capsule forces to section it sharply. The cadaveric simulation model allowed an accurate description of Denonvilliers' fascia, defining several planes for anterior dissection of the meso-rectum

    Mitigation of coupled wind-wave-earthquake responses of a 10 MW fixed-bottom offshore wind turbine

    © 2020 Elsevier Ltd In this paper we present a study on the mitigation of dynamic responses of a 10 MW monopile offshore wind turbine under coupled wind-wave-earthquake excitations. We have developed and validated the generic seismic coupled analysis and structural control architecture tool to overcome the limitation of numerical tools when examining the wind-wave-earthquake coupling effects. We investigated the dynamic responses of a 10 MW monopile offshore wind turbine under different loading combinations and found that the earthquake loading increases the tower-top displacement and pile-cap moment by 47.6% and 95.1%, respectively, compared to the wind-wave-only condition. It is found that the earthquake-induced vibration in the fore-aft direction is mitigated by the wind and wave loadings due to the energy dissipated by the aerodynamic and hydrodynamic damping. In addition, the tower responses are dominated by the earthquake excitation. In order to alleviate the tower vibration induced by the earthquake, we implemented the structural control capability within the tool using tuned mass dampers. The tuned mass dampers with appropriately selected design parameters achieve a larger mitigation on the tower-top displacement for the earthquake-only condition compared to the coupled-loading scenario. The reason is that the tuned mass damper is only effective in mitigating tower vibration, and it is not capable of reducing the tower elastic deformation which is the major contribution of the tower displacement for the coupled-loading condition. In addition, we have found that a heavier tuned mass damper requires a lower tuned frequency to achieve a larger mitigation. A configuration for the mitigation control of the 10 MW offshore wind turbine is suggested by using a 5% mass ratio of the tuned mass damper

    Experimental and theoretical studies on the effect of the oxo group in 1,4-benzodiazepines

    Two families of regioisomeric 1,4-benzodiazepines, 4-benzyl-3H-benzo[e][1,4]diazepin-5-ones and 4-benzoyl-4,5-dihydro-3H-benzo[e][1,4]diazepines, have been synthesized through a similar Ugi/reduction cyclization sequence. Their conformation and stability depend on the position of the tautomeric imine/enamine equilibrium present in the diazepine nucleus, which in turn depends on the relative position of the carbonyl group adjacent to the nitrogen at the 4-position in the benzodiazepine system. Moreover, the electrophilic center on the imine tautomer is essential for the antitumor activity of some benzodiazepines as a DNA binding position.     Introduction: the process of vocational training towards Technical Education Studies from Pinar del Rio University of Medical Sciences for students of Pre-University Institutes, is a spontaneous and non-systemic manifestation, and it does not achieve school-family-community integration, showing itself by the insufficient  scheduling of activities into the university to increase motivation of pre-university students interested in entering the technical education, resulting in  a high degree of demotivation, which is associated with academic failure.Objective: to design a strategy for the development of vocational training towards higher studies of health profiles in the technical education, from the university to the pre-university students taking advantage of the school-family-community relationship.Methods: materialist dialectical method as the articulator of the rest of the methods, using among the empirical methods: surveys, interviews with experts, and the documentary analysis; as methods from the theoretical level: historical-logical, modeling and structural systemic were used. Results: the strategy for professional orientation towards studies in technical education from the university encompassing a conscious, systemic and comprehensive approach based on the relationship school - family - community, considering the development of professional performance.Conclusions: a strategy was designed to increase professional motivation towards the studies in Technical Education from the university, taking into account a conscious, systemic and comprehensive approach based on the relationship school - family - community, supported on the development of professional performance, where each one assumes the role consciously, together with a dynamic multiplier of actions.Introducción: el proceso de orientación profesional hacia las carreras de la Enseñanza Técnica Profesional desde la Universidad de Ciencias Médicas. Pinar del Río, para estudiantes de Institutos Preuniversitarios, se manifiesta espontáneo, asistémico y no logra la integración de la escuela-familia-comunidad, manifestándose en: Insuficientes actividades de la universidad hacia los estudiantes de preuniversitarios interesados en ingresar a una carrera de la enseñanza técnica alto grado de desmotivación, asociado al fracaso académico.Objetivo: diseñar una estrategia para el desarrollo de la orientación profesional hacia carreras de los perfiles de salud de la enseñanza técnica, desde la universidad, en estudiantes de preuniversitario aprovechando la vinculación escuela–familia–comunidad.Método: método dialéctico materialista como articulador del resto de los métodos empleados, entre los métodos empíricos se emplearon: encuestas, entrevistas a expertos, análisis documental y como métodos del nivel teórico: histórico-lógico, modelación y sistémico estructural. Resultados: se propone una estrategia para la orientación profesional hacia carreras de la enseñanza técnica desde la universidad desde un enfoque consciente, sistémico e integrado a partir de la vinculación escuela – familia – comunidad, basada en el desarrollo del modo de actuación profesional. Conclusiones: se diseñó una estrategia para la orientación profesional hacia carreras de la Enseñanza Técnica desde la universidad desde un enfoque consciente, sistémico e integrado a partir de la vinculación escuela – familia – comunidad, basada en el desarrollo del modo de actuación profesional, donde cada uno asume su rol de manera consciente y en una dinámica multiplicadora de acciones.

Trabajando sobre ellas y combinando unas con otras podemos extraer una serie de productos muy útiles para el pronóstico inmediato o nowcasting. Con este fin, el NWC SAF (Satellite Application Facility on support to Nowcasting) genera y difunde los productos y servicios operativos de EUMETSAT, para lo que pone a disposición de los usuarios un software con el que poder generar las herramientas que a todo predictor le serán de suma utilidad cuando haya que tomar decisiones en un periodo de una a tres o seis horas

    Mild and short-term caloric restriction prevents obesity-induced cardiomyopathy in young zucker rats without changing in metabolites and fatty acids cardiac profile

    Caloric restriction (CR) ameliorates cardiac dysfunction associated with obesity. However, most of the studies have been performed under severe CR (30-65% caloric intake decrease) for several months or even years in aged animals. Here, we investigated whether mild (20% food intake reduction) and short-term (2-weeks) CR prevented the obese cardiomyopathy phenotype and improved the metabolic profile of young (14 weeks of age) genetically obese Zucker fa/fa rats. Heart weight (HW) and HW/tibia length ratio was significantly lower in fa/fa rats after 2 weeks of CR than in counterparts fed ad libitum. Invasive pressure measurements showed that systolic blood pressure, maximal rate of positive left ventricle (LV) pressure, LV systolic pressure and LV end-diastolic pressure were all significantly higher in obese fa/fa rats than in lean counterparts, which were prevented by CR. Magnetic resonance imaging revealed that the increase in LV end-systolic volume, stroke volume and LV wall thickness observed in fa/fa rats was significantly lower in animals on CR diet. Histological analysis also revealed that CR blocked the significant increase in cardiomyocyte diameter in obese fa/fa rats. High resolution magic angle spinning magnetic resonance spectroscopy analysis of the LV revealed a global decrease in metabolites such as taurine, creatine and phosphocreatine, glutamate, glutamine and glutathione, in obese fa/fa rats, whereas lactate concentration was increased. By contrast, fatty acid concentrations in LV tissue were significantly elevated in obese fa/fa rats. CR failed to restore the LV metabolomic profile of obese fa/fa rats.     E3 ubiquitin ligase Atrogin-1 mediates adaptive resistance to KIT-targeted inhibition in gastrointestinal stromal tumor

    KIT/PDGFRA oncogenic tyrosine kinase signaling is the central oncogenic event in most gastrointestinal stromal tumors (GIST), which are human malignant mesenchymal neoplasms that often feature myogenic differentiation. Although targeted inhibition of KIT/PDGFRA provides substantial clinical benefit, GIST cells adapt to KIT/PDGFRA driver suppression and eventually develop resistance. The specific molecular events leading to adaptive resistance in GIST remain unclear. By using clinically representative in vitro and in vivo GIST models and GIST patients’ samples, we found that the E3 ubiquitin ligase Atrogin-1 (FBXO32)—the main effector of muscular atrophy in cachexia—resulted in the most critical gene derepressed in response to KIT inhibition, regardless the type of KIT primary or secondary mutation. Atrogin-1 in GISTs is transcriptionally controlled by the KIT-FOXO3a axis, thus indicating overlap with Atrogin-1 regulation mechanisms in nonneoplastic muscle cells. Further, Atrogin-1 overexpression was a GIST-cell-specific pro-survival mechanism that enabled the adaptation to KIT-targeted inhibition by apoptosis evasion through cell quiescence.     Clinical value of next generation sequencing of plasma cell-free DNA in gastrointestinal stromal tumors

    [Background] Gastrointestinal stromal tumor (GIST) initiation and evolution is commonly framed by KIT/PDGFRA oncogenic activation, and in later stages by the polyclonal expansion of resistant subpopulations harboring KIT secondary mutations after the onset of imatinib resistance. Thus, circulating tumor (ct)DNA determination is expected to be an informative non-invasive dynamic biomarker in GIST patients.[Methods] We performed amplicon-based next-generation sequencing (NGS) across 60 clinically relevant genes in 37 plasma samples from 18 GIST patients collected prospectively. ctDNA alterations were compared with NGS of matched tumor tissue samples (obtained either simultaneously or at the time of diagnosis) and cross-validated with droplet digital PCR (ddPCR).[Results] We were able to identify cfDNA mutations in five out of 18 patients had detectable in at least one timepoint. Overall, NGS sensitivity for detection of cell-free (cf)DNA mutations in plasma was 28.6%, showing high concordance with ddPCR confirmation. We found that GIST had relatively low ctDNA shedding, and mutations were at low allele frequencies. ctDNA was detected only in GIST patients with advanced disease after imatinib failure, predicting tumor dynamics in serial monitoring. KIT secondary mutations were the only mechanism of resistance found across 10 imatinib-resistant GIST patients progressing to sunitinib or regorafenib.[Conclusions] ctDNA evaluation with amplicon-based NGS detects KIT primary and secondary mutations in metastatic GIST patients, particularly after imatinib progression.
