    Modeling Horizontal Ultraviolet Irradiance for All Sky Conditions by Using Artificial Neural Networks and Regression Models

    In the present study, different models constructed with meteorological variables are proposed for the determination of horizontal ultraviolet irradiance (IUV), on the basis of data collected at Burgos (Spain) during an experimental campaign between March 2020 and May 2022. The aim is to explore the effectiveness of a range of variables for modelling horizontal ultraviolet irradiance through a comparison of supervised artificial neural network (ANN) and regression model results. A preliminary feature selection process using the Pearson correlation coefficient was sufficient to determine the variables for use in the models. The following variables and their influence on horizontal ultraviolet irradiance were analyzed: horizontal global irradiance (IGH), clearness index (kt), solar altitude angle (α), horizontal beam irradiance (IBH), diffuse fraction (D), temperature (T), sky clearness (ε), cloud cover (Cc), horizontal diffuse irradiance (IDH), and sky brightness (∆).     Characterization of Campylobacter species in Spanish retail from different fresh chicken products and their antimicrobial resistance

    Contaminated chicken products have been recognized as the primary vehicles of Campylobacter transmission to human. Pulsed-field gel electrophoresis (PFGE) and antimicrobial resistance of Campylobacter isolates from fresh chicken products at retail were studied. A total of 512 samples including: thigh, breast, marinated and minced chicken were purchased from different retail stores. Half of the samples were packed and the other half were unpacked. The 39.4% of the samples were Campylobacter positive; being unpacked chicken products (45.3%) more contaminated than packed chicken (33.6%). PFGE typing showed a high diversity among isolates; clustering 204 isolates into 76 PFGE types: 55 clusters of C. jejuni, 19 of C. coli and 2 of C. lari. C. coli genotypes showed higher resistance than other Campylobacter species. Although modified atmosphere packaging can reduce the prevalence of Campylobacter spp., it does not avoid their presence in at least 33.6% of packed chicken products analyzed. Some pulsotypes might persist in the processing plant or butcher shops environment for longer than previously thought.     [ES] Introducción: Los meningoceles transetmoidales son raros. El diagnóstico suele ser tardío y se suele realizar por la presencia de meningitis recurrente. Caso Clínico: Paciente de 10 años con antecedentes de meningitis neumocócica en dos ocasiones. Endoscopia nasal: tumoración en fosa nasal derecha entre cornete medio y septum. TAC: lesión con densidad de partes blandas con disrupción de la cortical adyacente a la "crista gali" derecha. RNM: lesión de componente quístico, con pedículo en fosa craneal anterior. Se realiza craneotomía frontal derecha, extirpando meningocele con orificio de salida en lámina cribosa, posterior cierre con duramadre, hueso propio y pericráneo. Estudio anatomopatológico: tejido fibrovascular a modo de membranas y tejido de aspecto glial. Discusión: El meningocele intranasal es una malformación rara caracterizada por una herniación de las meninges, a través de un defecto en la lámina cribiforme, hacia la cavidad nasal, debido generalmente a una anomalía congénita, pero que también secundario a traumatismos o hipertensión crónica intracraneal. Más frecuente en determinadas áreas geográficas, sobre todo en el sudeste asiático. Clínicamente, lo más frecuente es la obstrucción o masa nasal con o sin meningitis de repetición. En el caso de localización etmoidal debe hacerse diagnóstico diferencial con quiste dermoide, hemangioma, glioma y pólipo nasal. La TC y la RM son fundamentales en el diagnóstico y deben preceder a la biopsia. El tratamiento es quirúrgico con abordaje intracraneal o endoscópico nasal. Conclusiones: Ante todo caso de meningitis aguda recidivante, deberían descartarse malformaciones ocultas de la base de cráneo. [EN] Introduction: Transethmoidal meningoceles are rare. The diagnosis is usually delayed and often made by the presence of recurrent meningitis. Case report: A 10-year boy with history of recurrent pneumococcal meningitis. Nasal endoscopy: mass in the right nostril between middle turbinate and septum. CT: soft tissue density lesion with disruption of the cortex adjacent to the "crista gali". MRI: cystic component injury with pedicle in anterior cranial fossa. Right frontal craniotomy was performed, removing meningocele with exit point on the cribriform plate, posterior dural closure, bone itself and scalp. Pathologic Study: fibrovascular tissue as an aspect membranes and glial tissue. Discussion: Intranasal meningocele is a rare malformation characterized by herniation of the meninges into the nasal cavity through a defect in the cribriform plate, due to a congenital abnormality, but also secondary to trauma or chronic intracranial hypertension. More common in certain geographic areas, especially in Southeast Asia. Usually cause nasal obstruction with or without recurrent meningitis. Differential diagnosis must be made with dermoid cyst, hemangioma, glioma and nasal polyp. CT and MRI are essential in the diagnosis and should precede the biopsy. The treatment is surgical or endoscopic nasal intracranial approach. Conclusions: In the presence of recurrent acute meningitis, it is necessary to rule out occult malformations of the skull base

    Prevalence of vertebral fractures and their prognostic significance in the survival in patients with chronic kidney disease stages 3-5 not on dialysis

    The prevalence of vertebral fractures (VF) and their association with clinical risk factors and outcomes are poorly documented in chronic kidney disease (CKD) cohorts. The aim of the study was to evaluate the prevalence of VF in patients with non-dialysis dependent CKD (NDD-CKD), their value in predicting mortality and its correlation with parameters of bone mineral metabolism and vascular calcification. 612 NDD 3-5 stage CKD patients participating in the OSERCE-2 study, a prospective, multicenter, cohort study, were prospectively evaluated and categorized into two groups according to presence or absence of VF at enrollment. VF were assessed with lateral radiographs and Genant semi-quantitative method was applied. Three radiologists specialized in musculoskeletal radiology performed consensual reading of individual images obtained using a Raim DICOM Viewer and a Canon EOS 350 camera to measure with Java Image software in those who had traditional acetate X-ray. Factors related to VF were assessed by logistic regression analysis. Association between VF and death over a 3-year follow-up was assessed by Kaplan-Meier survival curves and Cox-proportional hazard models. VF were detected in 110patients(18%). Serumphosphatelevels(OR0.719,95%CI0.532to0.972,p = 0.032),ankle-brachial index 3 and serum phosphate, the presence of VF (HR 1.983, 95% CI 1.009-3.898, p = 0.047) were an independent predictor of all-cause mortality. In our study 18% of patients with NDD-CKD have VF. Factors associated with VF were age, low serum phosphate levels and peripheral vascular disease. The presence of VF was an independent risk factor for mortality in stages 3-5 NDD-CKD patients. Clinical trials are needed to confirm whether this relationship is causal and reversible with treatment for osteoporosis

    Dynamic profiles and predictive values of some biochemical and haematological quantities in COVID-19 inpatients

    Introduction: Severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection in some hospitalized patients has shown some important alterations in laboratory tests. The aim of this study was to establish the most relevant quantities associated with the worst prognosis related to COVID-19. Materials and methods: This was a descriptive, longitudinal, observational and retrospective study, in a cohort of 845 adult inpatients from Bellvitge University Hospital (L'Hospitalet de Llobregat, Barcelona, Spain). A multivariate regression analysis was carried out in demographic, clinical and laboratory data, comparing survivors (SURV) and non-survivors (no-SURV). A receiver operating characteristic analysis was also carried out to establish the cut-off point for poor prognostic with better specificity and sensibility. Dynamic changes in clinical laboratory measurements were tracked from day 1 to day 28 after the onset of symptoms. Results: During their hospital stay, 18% of the patients died. Age, kidney disease, creatinine (CREA), lactate-dehydrogenase (LD), C-reactive-protein (CRP) and lymphocyte (LYM) concentration showed the strongest independent associations with the risk of death in the multivariate regression analysis. Established cut-off values for poor prognosis for CREA, LD, CRP and LYM concentrations were 75.0 mu mol/L, 320 U/L, 80.9 mg/L and 0.69 x10(9)/L. Dynamic profile of laboratory findings, were in agreement with the consequences of organ damage and tissue destruction. Conclusions: Age, kidney disease, CREA, LD, CRP and LYM concentrations in COVID-19 patients from the southern region of Catalonia provide important information for their prognosis. Measurement of LD has demonstrated to be very good indicator of poor prognosis at initial evaluation because of its stability over time

    COVID-19 Severity and Survival over Time in Patients with Hematologic Malignancies: A Population-Based Registry Study

    Mortality rates for COVID-19 have declined over time in the general population, but data in patients with hematologic malignancies are contradictory. We identified independent prognostic factors for COVID-19 severity and survival in unvaccinated patients with hematologic malignancies, compared mortality rates over time and versus non-cancer inpatients, and investigated post COVID-19 condition. Data were analyzed from 1166 consecutive, eligible patients with hematologic malignancies from the population-based HEMATO-MADRID registry, Spain, with COVID-19 prior to vaccination roll-out, stratified into early (February–June 2020; n = 769 (66%)) and later (July 2020–February 2021; n = 397 (34%)) cohorts. Propensity-score matched non-cancer patients were identified from the SEMI-COVID registry. A lower proportion of patients were hospitalized in the later waves (54.2%) compared to the earlier (88.6%), OR 0.15, 95%CI 0.11–0.20. The proportion of hospitalized patients admitted to the ICU was higher in the later cohort (103/215, 47.9%) compared with the early cohort (170/681, 25.0%, 2.77; 2.01–3.82). The reduced 30-day mortality between early and later cohorts of non-cancer inpatients (29.6% vs. 12.6%, OR 0.34; 0.22–0.53) was not paralleled in inpatients with hematologic malignancies (32.3% vs. 34.8%, OR 1.12; 0.81–1.5). Among evaluable patients, 27.3% had post COVID-19 condition.     Genome-wide association analysis of dementia and its clinical endophenotypes reveal novel loci associated with Alzheimer's disease and three causality networks : The GR@ACE project

    Introduction: Large variability among Alzheimer's disease (AD) cases might impact genetic discoveries and complicate dissection of underlying biological pathways. Methods: Genome Research at Fundacio ACE (GR@ACE) is a genome-wide study of dementia and its clinical endophenotypes, defined based on AD's clinical certainty and vascular burden. We assessed the impact of known AD loci across endophenotypes to generate loci categories. We incorporated gene coexpression data and conducted pathway analysis per category. Finally, to evaluate the effect of heterogeneity in genetic studies, GR@ACE series were meta-analyzed with additional genome-wide association study data sets. Results: We classified known AD loci into three categories, which might reflect the disease clinical heterogeneity. Vascular processes were only detected as a causal mechanism in probable AD. The meta-analysis strategy revealed the ANKRD31-rs4704171 and NDUFAF6-rs10098778 and confirmed SCIMP-rs7225151 and CD33-rs3865444. Discussion: The regulation of vasculature is a prominent causal component of probable AD. GR@ACE meta-analysis revealed novel AD genetic signals, strongly driven by the presence of clinical heterogeneity in the AD series

    La Red de investigación en docencia universitaria “Universidad, docencia, genero e igualdad” persigue avanzar en la calidad e innovación de las enseñanzas universitarias a partir de la inclusión de la perspectiva de género. Se busca dar cumplimiento a las directrices generales de los nuevos planes de estudio respecto del principio de igualdad de oportunidades entre hombres y mujeres en la formación universitaria (Real Decreto 1393/2007. BOE nº 260, 30 de octubre de 2007). En la quinta edición de la Red, y dada su composición multidisciplinar, se ha trabajado en tres líneas de investigación: 1) mantenimiento del “Portal web con recursos docentes con perspectiva de género”, proyecto financiado por el Instituto de la Mujer (PACUI, 2012) e iniciado en el curso 2012-2013; 2) desarrollo de la primera versión de “iLengUA”, una herramienta informática para un discurso inclusivo e igualitario; y 3) diseño de una Guía de recomendaciones para la inclusión de la perspectiva de género en la docencia universitaria

    EDUCORE project: a clinical trial, randomised by clusters, to assess the effect of a visual learning method on blood pressure control in the primary healthcare setting

    <p>Abstract</p> <p>Background</p> <p>High blood pressure (HBP) is a major risk factor for cardiovascular disease (CVD). European hypertension and cardiology societies as well as expert committees on CVD prevention recommend stratifying cardiovascular risk using the SCORE method, the modification of lifestyles to prevent CVD, and achieving good control over risk factors. The EDUCORE (Education and Coronary Risk Evaluation) project aims to determine whether the use of a cardiovascular risk visual learning method - the EDUCORE method - is more effective than normal clinical practice in improving the control of blood pressure within one year in patients with poorly controlled hypertension but no background of CVD;</p> <p>Methods/Design</p> <p>This work describes a protocol for a clinical trial, randomised by clusters and involving 22 primary healthcare clinics, to test the effectiveness of the EDUCORE method. The number of patients required was 736, all between 40 and 65 years of age (n = 368 in the EDUCORE and control groups), all of whom had been diagnosed with HBP at least one year ago, and all of whom had poorly controlled hypertension (systolic blood pressure ≥ 140 mmHg and/or diastolic ≥ 90 mmHg). All personnel taking part were explained the trial and trained in its methodology. The EDUCORE method contemplates the visualisation of low risk SCORE scores using images embodying different stages of a high risk action, plus the receipt of a pamphlet explaining how to better maintain cardiac health. The main outcome variable was the control of blood pressure; secondary outcome variables included the SCORE score, therapeutic compliance, quality of life, and total cholesterol level. All outcome variables were measured at the beginning of the experimental period and again at 6 and 12 months. Information on sex, age, educational level, physical activity, body mass index, consumption of medications, change of treatment and blood analysis results was also recorded;</p> <p>Discussion</p> <p>The EDUCORE method could provide a simple, inexpensive means of improving blood pressure control, and perhaps other health problems, in the primary healthcare setting;</p> <p>Trial registration</p> <p>The trial was registered with ClinicalTrials.gov, number NCT01155973 [<url>http://ClinicalTrials.gov</url>].</p
