Colorectal Cancer Stage at Diagnosis Before vs During the COVID-19 Pandemic in Italy
IMPORTANCE Delays in screening programs and the reluctance of patients to seek medical
attention because of the outbreak of SARS-CoV-2 could be associated with the risk of more advanced
colorectal cancers at diagnosis.
OBJECTIVE To evaluate whether the SARS-CoV-2 pandemic was associated with more advanced
oncologic stage and change in clinical presentation for patients with colorectal cancer.
DESIGN, SETTING, AND PARTICIPANTS This retrospective, multicenter cohort study included all
17 938 adult patients who underwent surgery for colorectal cancer from March 1, 2020, to December
31, 2021 (pandemic period), and from January 1, 2018, to February 29, 2020 (prepandemic period),
in 81 participating centers in Italy, including tertiary centers and community hospitals. Follow-up was
30 days from surgery.
EXPOSURES Any type of surgical procedure for colorectal cancer, including explorative surgery,
palliative procedures, and atypical or segmental resections.
MAIN OUTCOMES AND MEASURES The primary outcome was advanced stage of colorectal cancer
at diagnosis. Secondary outcomes were distant metastasis, T4 stage, aggressive biology (defined as
cancer with at least 1 of the following characteristics: signet ring cells, mucinous tumor, budding,
lymphovascular invasion, perineural invasion, and lymphangitis), stenotic lesion, emergency surgery,
and palliative surgery. The independent association between the pandemic period and the outcomes
was assessed using multivariate random-effects logistic regression, with hospital as the cluster
variable.
RESULTS A total of 17 938 patients (10 007 men [55.8%]; mean [SD] age, 70.6 [12.2] years)
underwent surgery for colorectal cancer: 7796 (43.5%) during the pandemic period and 10 142
(56.5%) during the prepandemic period. Logistic regression indicated that the pandemic period was
significantly associated with an increased rate of advanced-stage colorectal cancer (odds ratio [OR],
1.07; 95%CI, 1.01-1.13; P = .03), aggressive biology (OR, 1.32; 95%CI, 1.15-1.53; P < .001), and stenotic
lesions (OR, 1.15; 95%CI, 1.01-1.31; P = .03).
CONCLUSIONS AND RELEVANCE This cohort study suggests a significant association between the
SARS-CoV-2 pandemic and the risk of a more advanced oncologic stage at diagnosis among patients
undergoing surgery for colorectal cancer and might indicate a potential reduction of survival for
these patients
Solution speciation of potential anticancer metal complexes of salicylaldehyde semicarbazone and its bromo derivative
The stoichiometry and thermodynamic stability of copper(II), vanadium(IV/V), iron(II)/(III) and gallium(III) complexes of salicylaldehyde semicarbazone (SSC, HL) and its 5-bromo derivative (Br-SSC, HL) have been determined by pH-potentiometry, UVâVis spectrophotometry, EPR, 1H and 51V NMR spectroscopy in 30% (w/w) dimethyl sulfoxide/water solvent mixture. Proton dissociation processes and lipophilicity of the ligands were also studied in detail. Formation of mono-ligand complexes such as [ML], [MLHâ1], [MLHâ2] was found with copper(II), vanadium(IV/V), while bis-ligand species of iron(II)/(III) and gallium(III) such as [ML2], [ML2Hâ1] and [ML2Hâ2] were also detected, in which the ligands coordinate via monoanionic (Oâ,N1,O) or dianionic (Oâ,N1,Oâ) modes. The bromine substituent on the phenol ring has no significant impact on the stability and binding modes but provides a remarkably enhanced lipophilic character, which is advantageous for the bioactivity. The Ga(III)âsalicylaldehyde semicarbazone species show unambiguously higher stability; whereas Cu(II) species have somewhat lower stability relative to the corresponding thiosemicarbazone analogues, however no decomposition of the Cu(II) complex was observed even at micromolar concentrations at physiological pH
Masculinity and secessionism in Italy: An assessment
The masculinist rhetoric of the northern Italian separatist party, the Lega Nord, is an example of nationalist status affirmation. The stereotyped contrast between effeminate southerners and masculine northerners developed at the end of last century is used to affirm the superiority of the masculine North. The Lega's rhetoric parallels mainstream concerns on Italy's international status, and proposals for reforms to make Italy more âmasculineâ and European. The use of a gendered language for the affirmation of national status reveals a belief in a normative hierarchy of nations, but also introduces a tension between a âmasculinistâ status affirmation and a model of modernity which includes the emancipation of women as one of its principles.FLWINinfo:eu-repo/semantics/publishe
Genomic designing for biotic stress resistant grapevine
Grapevines are challenged by a range of diseases and pests, causing economic losses and requiring often costly approaches to mitigate damage. Public interest in reducing the use of chemicals is a related challenge, along with climate change. Yet, the Vitis gene pool provides vast resources for the development of genetic resistance in rootstock and scion cultivars. Traditional breeding approaches have made great strides in the development of adaptive traits, and recent access to âomic technologies has further facilitated the identification of useful loci along with rapid trait introgression from wild species. Moreover, marker technologies are now used to stack multiple genes for the same trait into a single genotype, a heretofore barely accessible technology. Genomic technologies are also impacting germplasm characterization, and thereby facilitating âBreeding by Designâ approaches. Genetic transformation and gene-editing technologies are also applicable for both cultivar improvement as well as functional studies of genes. The landscape for acceptance of new resistant cultivars is complex and with wine grapes, subject to high degrees of regulation especially in the European Union. With rootstocks, as well as table/raisin grapes, gaining acceptance in the marketplace for new cultivars developed through either traditional or marker-assisted approaches is routine. Yet even in the highly regulated EU environment, the adoption of new wine cultivars of interspecific origins is beginning to take place in both traditional wine growing regions as well as non-traditional regions nearby
