    Laparoscopic, three-port and SILS cholecystectomy: a retrospective study.

    Introduction. The aim of this study was to compare the results of classic laparoscopic, three-port and SILS cholecystectomy. Materials and methods. We conducted a retrospective study of da- ta collected between January 2010 and December 2012 pertaining to 159 selected patients with symptomatic gallstones. 57 underwent lapa- roscopic cholecystectomy, 51 three-port cholecystectomy and 48 SILS cholecystectomy. We then compared the groups with respect to mean ope- rating time, intraoperative complications, postoperative pain, duration of hospitalization and final aesthetic result. Introduction The first laparoscopic cholecystectomy was carried out in 1987 in France by Philippe Mouret (1). The progressive evolution of the technique has led this procedure to be- come the gold standard in the treatment of symptoma- tic gallstones (2). As the technology improved, many sur- geons began to reduce the number and size of the ports with the aim of achieving ever lower invasiveness, con- sequently reducing trauma and postoperative pain and improving the cosmetic results. There was thus a pro- Results. The mean operating time was significantly higher in the SILS cholecystectomy group (93 minutes) than in the other two groups. There were no intraoperative complications. There were no significant differences in the duration of hospitalization among the three groups. Patients in the SILS cholecystectomy group reported significantly less pain 3, 6 and 12 hours after surgery. The aesthetic results at 1 and 6 months’ follow-up were also decidedly better. Conclusions. On the basis of this study, SILS cholecystectomy is a feasible, safe procedure. In any case, it should be used in selected patients only and carried out by a dedicated team with strong experience in laparoscopy. The main advantages of this technique are a reduction in post-operative pain and improved aesthetic result, at the price, howe- ver, of its greater technical difficulty and longer operating times. Future studies are in any case necessary to evaluate any other benefits of this method

    Propiedades microbiológicas en un suelo de la Patagonia Argentina bajo la influencia de diferentes especies forestales

    191-197In forest systems, leaf litter is usually the main source of nutrients for the vegetation and microorganisms. Leaf litter composition may vary according to the biomass and quality of the materials produced by different species. Tree roots affect the development and activity of microflora. The aim of this study was to evaluate the influence of two implanted tree species, with different composition of plant residues, on the soil characteristics. The soil is classified as an Andisol and is located in the Trevelín Forest Station, Chubut, Argentina, Lat. 43° S, Long 71°31' West. The samples were extracted from the surface soil of two forest plots with a dominant species each: Radiata Pine (Pinus radiata D. Don.) and European Oak (Quercus robur). The following analytical determinations were carried out on soil samples: organic carbon, microbial respiration, deshydrogenase activity, microbial count (bacterial communities, amilolytic, actinomyces and fungi) as well as the functional diversity of associated microbial communities. The Shannon-Weaver index (H) was calculated. The soil under Oak trees showed the highest values of microbial respiration, organic carbon and amilolytic and bacterial community counts. No significant differences were found in deshydrogenase activity, actinomyces and fungi counts between species. The principal component analysis showed significant variations in the physiology of microbial communities associated with these two tree species. The microorganism H diversity index was significantly greater for Pine. Plants residues of broadleaf species such as Oak have lower contents of recalcitrant substances than conifer species, and this fact, associated with the higher development of total and amylolytic bacteria, could result in an increase in the decomposition of plant residues in the soil and in higher organic carbon and microbial respiration values

    Strategies and Alliances into Action to Improve National Collaboration

    The Italian NILDE network of libraries continues to grow through the use of the NILDE system and currently comprises more than 600 Italian librarians and about 10.000 registered end-users.The system allows to daily manage and to record all the Inter-Library-Loan (ILL) operations, with a high national coverage. This paper presents the NILDE network governance and evolution and the strategies that have been put into action to improve collaboration in resource sharing among the participants. These strategies include: − release of best practices and worst practices; − activities to promote the knowledge about the network; − cooperation with the Italian national catalogs and consortia; − data analysis about ILL and its performance, related to: turn around time, reciprocity factor, requested/supplied documents imbalance analysis, analysis of ILL requested serial titles and their relationship with consortial e-only acquisitions. The availability of such a high volume of ILL data has allowed for the first time to analyze the trends and gaps of ILL and to help future cooperative acquisitions planning

    Resumo Vários sistemas para a produção de proteínas recombinantes têm sido utilizados para a otimização da produção das mesmas. A glândula mamária é considerada um sistema muito interessante para a produção destas proteínas devido ao seu alto nível de expressão e à sua capacidade para realizar modificações pós-traducionais. Caprinos podem produzir elevadas quantidades de leite durante um longo período de lactação e, portanto, tornam-se importantes candidatos como biorreatores para a expressão de proteínas recombinantes no leite. No entanto, caprinos transgênicos não são de fácil obtenção devido à baixa eficiência dos métodos e à expressão imprevisível da proteína recombinante. O incremento na eficiência dos métodos na transgênese em caprinos é um grande desafio a superar. Esta revisão tem por objetivo apresentar o estado atual dos métodos tradicionais, bem como apresentar técnicas emergentes para obtenção eficiente de caprinos transgênicos

    Enhanced Processing of Threat Stimuli under Limited Attentional Resources

    The ability to process stimuli that convey potential threat, under conditions of limited attentional resources, confers adaptive advantages. This study examined the neurobiology underpinnings of this capacity. Employing an attentional blink paradigm, in conjunction with functional magnetic resonance imaging, we manipulated the salience of the second of 2 face target stimuli (T2), by varying emotionality. Behaviorally, fearful T2 faces were identified significantly more than neutral faces. Activity in fusiform face area increased with correct identification of T2 faces. Enhanced activity in rostral anterior cingulate cortex (rACC) accounted for the benefit in detection of fearful stimuli reflected in a significant interaction between target valence and correct identification. Thus, under conditions of limited attention resources activation in rACC correlated with enhanced processing of emotional stimuli. We suggest that these data support a model in which a prefrontal “gate” mechanism controls conscious access of emotional information under conditions of limited attentional resources

    Diagnostic accuracy of computed tomography coronary angiography in patients with a zero calcium score

    To evaluate the diagnostic accuracy of 64-slice CT coronary angiography (CT-CA) for the detection of significant coronary artery stenosis in patients with zero on the Agatston Calcium Score (CACS). We enrolled 279 consecutive patients (96 male, mean age 48±12 years) with suspected coronary artery disease. Patients were symptomatic (n=208) or asymptomatic (n=71), and underwent conventional coronary angiography (CAG). For CT-CA we administered an IV bolus of 100 ml of iodinated contrast material. CT-CA was compared to CAG using a threshold for significant stenosis of ≤50%. The prevalence of disease demonstrated at CAG was 15% (1.4% in asymptomatic). The population at CAG showed no or non-significant disease in 85% (238/279), single vessel disease in 9% (25/279), and multi-vessel disease in 6% (16/279). Sensitivity, specificity, and positive and negative predictive values of CT-CA vs. CAG on the patient level were 100%, 95%, 76%, and 100% in the overall population and 100%, 100%, 100%, and 100% in asymptomatic patients, respectively. CT-CA proves high diagnostic performance in patients with or without symptoms and with zero CACS. The prevalence of significant disease detected by CT-CA was not negligible in asymptomatic patients.
