    The past several decades have witnessed major cutbacks in the extent of government-provided entitlements as tax bases have been eroded by the growth of offshore competition for production and placement of funds. While the opportunities for minimizing costs by moving to lower wage and lower tax jurisdictions are a very real threat to the tax base required for the preservation of equitable institutions, globalization has been used as a battering ram to dismantle safety nets and to suppress sentiments of solidarity. Indeed, there has been a relentless campaign to shed social support which is summed up in one shorthand word and an associated phrase: 'neoliberalism' and a 'race to the bottom'

    Mesh Exposure and Associated Risk Factors in Women Undergoing Transvaginal Prolapse Repair with Mesh

    Objective. To determine frequency, rate, and risk factors associated with mesh exposure in women undergoing transvaginal prolapse repair with polypropylene mesh. Methods. Retrospective chart review was performed for all women who underwent Prolift Pelvic Floor Repair System (Gynecare, Somerville, NJ) between September 2005 and September 2008. Multivariable logistic regression was performed to identify risk factors for mesh exposure. Results. 201 women underwent Prolift. Mesh exposure occurred in 12% (24/201). Median time to mesh exposure was 62 days (range: 10–372). When mesh was placed in the anterior compartment, the frequency of mesh exposure was higher than that when mesh was placed in the posterior compartment (8.7% versus 2.9%, P=0.04). Independent risk factors for mesh exposure were diabetes (AOR = 7.7, 95% CI 1.6–37.6; P=0.01) and surgeon (AOR = 7.3, 95% CI 1.9–28.6; P=0.004). Conclusion. Women with diabetes have a 7-fold increased risk for mesh exposure after transvaginal prolapse repair using Prolift. The variable rate of mesh exposure amongst surgeons may be related to technique. The anterior vaginal wall may be at higher risk of mesh exposure as compared to the posterior vaginal wall

    Analysis of vaginal microbicide film hydration kinetics by quantitative imaging refractometry

    We have developed a quantitative imaging refractometry technique, based on holographic phase microscopy, as a tool for investigating microscopic structural changes in water-soluble polymeric materials. Here we apply the approach to analyze the structural degradation of vaginal topical microbicide films due to water uptake. We implemented transmission imaging of 1-mm diameter film samples loaded into a flow chamber with a 1.5×2 mm field of view. After water was flooded into the chamber, interference images were captured and analyzed to obtain high resolution maps of the local refractive index and subsequently the volume fraction and mass density of film material at each spatial location. Here, we compare the hydration dynamics of a panel of films with varying thicknesses and polymer compositions, demonstrating that quantitative imaging refractometry can be an effective tool for evaluating and characterizing the performance of candidate microbicide film designs for anti-HIV drug delivery. © 2014 Rinehart et al

    Clarifying the meaning of mantras in wildland fire behaviour modelling: reply to Cruz <i>et al.</i> (2017)

    International audienceIn a recent communication, Cruz et al. (2017) called attention to several recurring statements (mantras) in the wildland fire literature regarding empirical and physical fire behaviour models. Motivated by concern that these mantras have not been fully vetted and are repeated blindly, Cruz et al. (2017) sought to verify five mantras they identify. This is a worthy goal and here we seek to extend the discussion and provide clarification to several confusing aspects of the Cruz et al. (2017) communication. In particular, their treatment of what they call physical models is inconsistent, neglects to reference current research activity focussed on combined experimentation and model development, and misses an opportunity to discuss the potential use of physical models to fire behaviour outside the scope of empirical approaches
