Case Report: Graft Versus Tumor Effect After Non-Myeloablative Allogeneic Stem-Cell Transplantation in a Patient With Brentuximab-Vedotin Refractory Sezary Syndrome
Sezary Syndrome (SS) is a rare leukemic variant of primary cutaneous T-cell lymphoma.
Relapsed or refractory disease is generally considered incurable by conventional
therapeutic approaches, although durable responses can be achieved with novel
monoclonal antibodies. Allogeneic hematopoietic stem cell transplantation (alloHSCT)
may have potential value by inducing graft vs-lymphoma (GvL) effects, but there is
currently no consensus regarding the timing of alloHSCT or type of conditioning
regimen. Here we present the case of a male patient who achieved a complete
remission (CR) of primary refractory SS after non-myeloablative alloHSCT. Patient: Two
years prior to HSCT, the patient had been refractory to CHOEP-based chemotherapy,
interferon, extracorporeal photopheresis (ECP), and bexarotene. Directly prior to
alloHSCT brentuximab-vedotin (BV) was applied resulting in a partial remission of the
skin compartment and overall in a stable disease. Prior to HSCT, flow cytometry of the
bone marrow and peripheral blood showed an infiltration with T-cells positive for CD5,
CD4, low CD3, low CD2 and negative for CD7, CD38, HLA-DR and CD8. The trephine
biopsy showed a 7% infiltration of SS cells. The CD4:CD8 ratio in peripheral blood (pb)
was massively increased at 76.67, with 63.5% of white blood cells expressing a SS
immune phenotype. The conditioning regimen included 30 mg/m2 fludarabine on days -5,
-4 and -3 and total body irradiation with 2 Gy on day -1. Immunosuppression consisted of
cyclosporine A from day-1 and mycophenolate mofetil from day 0. The patient received
6.55x106 CD34+ cells and 1.11x108 CD3+ cells/kg body weight. Bone marrow
evaluation on day 28 still showed persistent SS cells by flow cytometry. After tapering
immunosuppression until day 169, the CD4:CD8 ratio in pb normalized. CR was
documented on day 169 after alloHSCT and is now ongoing for almost 3 years after
alloHSCT. Conclusions: We confirm that an alloHSCT can be a curative option for
refractory patients with SS. The achievement of a CR after tapering the
immunosuppressive therapy indicates a significant role of the GvL effect. In present
treatment algorithms for patients with SS, the timing of an alloHSCT and the intensity of
conditioning should be further explored
Allogeneic Hematopoietic Stem Cell Transplantation in a Rare Case of Tonsillar Mast Cell Sarcoma
Mast cell sarcoma comprises a rare aggressive mast cell neoplasia with histological, clinical, and genetic features distinct from other mast cell neoplasm. Until now, prognosis is still poor due to high rates of progression to mast cell leukemia and failure of conventional chemotherapies. Our here presented first report about successful allogeneic hematopoietic stem cell transplantation leading to remission in a case of tonsillar MCS represents a promising potential curative treatment option for this rare and often fatal disease.

Glutamatergic medication in the treatment of obsessive compulsive disorder (OCD) and autism spectrum disorder (ASD) - study protocol for a randomised controlled trial
BACKGROUND: Compulsivity is a cross-disorder trait underlying phenotypically distinct psychiatric disorders that emerge in childhood or adolescence. Despite the effectiveness of serotonergic compounds in the treatment of obsessive-compulsive disorder, treatment-resistant symptoms remaining in 40 to 60 % of patients present a pressing clinical problem. There are currently no medications that effectively treat the core impairments of autism spectrum disorder. There is an urgent need for the development of conceptually novel pharmacological strategies. Agents targeting glutamate neurotransmission, such as memantine, represent promising candidates. This proof-of-concept clinical study will allow pilot-testing of memantine for both clinical effectiveness and tolerability/safety. Memantine is an N-methyl-D-aspartate receptor antagonist, approved for the treatment of Alzheimer's dementia in a number of countries. METHODS/DESIGN: This 12-week study has an add-on, randomised, double-blind, placebo-controlled design of treatment with memantine, including an up-titration phase (forced flexible dose design, 5-15 mg/day), in patients aged 6-17 years and 9 months with obsessive-compulsive disorder or autism spectrum disorder. It is planned to include patients with obsessive-compulsive disorder (N = 50) or autism spectrum disorder (N = 50) across four centres in three European countries. Patients will be randomly assigned to memantine or placebo in a 1:1 ratio. Primary objectives are the investigation of the effectiveness of memantine in paediatric patients for improving symptoms of compulsivity (primary outcome measure: total score on the Children's Yale-Brown Obsessive-Compulsive Scale) and to explore its tolerability and safety. Secondary objectives are to explore the effects of memantine at the level of structure, function and biochemistry of the fronto-striatal circuits, and to collect blood for genetic analyses and biomarkers. Tertiary objectives are to explore the role of new candidate genes and pathways for compulsivity by linking genes to clinical phenotypes, response to treatment, neurocognitive test performance, and key structural and functional neuroimaging measures of the fronto-striatal circuits and to explore biomarkers/proteomics for compulsivity traits. DISCUSSION: This study is part of the large, translational project TACTICS ( http://www.tactics-project.eu/ ) that is funded by the European Union and investigates the neural, genetic and molecular factors involved in the pathogenesis of compulsivity. Its results will provide clinically relevant solid information on potential new mechanisms and medication treatment in obsessive-compulsive and autism spectrum disorders. Parameter Estimation and Quantitative Parametric Linkage Analysis with GENEHUNTER-QMOD
Objective: We present a parametric method for linkage analysis of quantitative phenotypes. The method provides a test for linkage as well as an estimate of different phenotype parameters. We have implemented our new method in the program GENEHUNTER-QMOD and evaluated its properties by performing simulations. Methods: The phenotype is modeled as a normally distributed variable, with a separate distribution for each genotype. Parameter estimates are obtained by maximizing the LOD score over the normal distribution parameters with a gradient-based optimization called PGRAD method. Results: The PGRAD method has lower power to detect linkage than the variance components analysis (VCA) in case of a normal distribution and small pedigrees. However, it outperforms the VCA and Haseman-Elston regression for extended pedigrees, nonrandomly ascertained data and non-normally distributed phenotypes. Here, the higher power even goes along with conservativeness, while the VCA has an inflated type I error. Parameter estimation tends to underestimate residual variances but performs better for expectation values of the phenotype distributions. Conclusion: With GENEHUNTER-QMOD, a powerful new tool is provided to explicitly model quantitative phenotypes in the context of linkage analysis. Увеличение степени интеграции элементной базы предъявляет все более жесткие требования к уменьшению концентрации загрязняющих примесей и окислительных дефектов упаковки в исходных кремниевых пластинах с ее сохранением в технологическом цикле изготовления ИМС. Это обуславливает высокую актуальность применения геттерирования в современной технологии микроэлектроники. В статье рассмотрены существующие методы геттерирования кремниевых пластин и механизмы их протекания.Збільшення ступеня інтеграції елементної бази пред'являє все більш жорсткі вимоги до зменшення концентрації забруднюючих домішок та окислювальних дефектів упаковки у вихідних кремнієвих пластинах за її збереження у технологічному циклі виготовлення ІМС.
