Cytarabine-related lung infiltrates on high resolution computerized tomography: a possible complication with benign outcome in leukemic patients.
Potentially fatal lung toxicity occurs in 12-20% of leukemic patients treated with cytarabine especially at intermediate to high doses, usually presenting as noncardiogenic pulmonary edema (NCPE). Anecdotally the association between cytarabine and the onset of bronchiolitis obliterans organizing pneumonia (BOOP) has been reported. We describe here three cases of patients affected by acute myeloid leukemia (AML) treated with chemotherapeutic regimens including high dose cytarabine, who developed early onset of fever, mild dyspnea, moderate hypoxemia on arterial blood gas analysis and lung infiltrates documented by high-resolution computerized tomography (HRCT), with a more indolent behaviour and a benign clinical outcome, compared with similar cases previously reported in the literature. Our cases widen the spectrum of clinical features of cytarabine-related toxicity in leukemic patients
Accurate Closed-Form Real-Time EGN Model Formula Leveraging Machine-Learning over 8500 Thoroughly Randomized Full C-Band Systems
We derived an approximate non-linear interference (NLI) closed-form model (CFM), capable of handling a very broad range of optical WDM system scenarios. We tested the CFM over 8500 randomized C-band WDM systems, of which 6250 were fully-loaded and 2250 were partially loaded. The systems had highly diversified channel formats, symbol rates, fibers, as well as other parameters. We improved the CFM accuracy by augmenting the formula with simple machine-learning factors, optimized by leveraging the system test-set. We further improved the CFM by adding a term which models special situations where NLI has high self-coherence. In the end, we obtained a very good match with the results found using the numerically-integrated Enhanced GN-model (or EGN-model). We also checked the CFM accuracy by comparing its predictions with full-C-Band split-step simulations of 300 randomized systems. The combined high accuracy and very fast computation time (milliseconds) of the CFM potentially make it an effective tool for real-time physical-layer-aware optical network management and control
YP10-03 Cross-cultural psychiatry, liaison psychiatry and other major challenges for modern psychiatrists
Beyond the awareness that psychiatry and contemporary medicine have undergone a dramatic change in recent years, walking the first steps as recently-qualified consultants in psychiatry is a major challenge. As a consequence of changes in society, technological progress and restricted funding availability, modern psychiatrists have to face problems that are new, and difficult to be faced, but also representing an opportunity to grow and contribute massively to medicine.This is particularly true in two specific fields, cross-cultural psychiatry and consultation-liaison psychiatry. The former is defined as psychiatry of disorders influenced by the cultural background; the latter is defined as the care of psychiatric disturbances in the medically ill. Though generic, these definitions highlight what is challenging in these branches of psychiatry: the disposition towards an inter-disciplinary approach to human illnesses. Both deepen their roots in the bio-psycho-social paradigm of George Engel and were in fact frequently mentioned in Engel's writings as the future of psychiatry. Training and clinical experience in cross-cultural and consultation-liaison psychiatry are to be major components of the curriculum of psychiatric trainees.Therefore, the role of psychiatrists and psychiatrists-to-be in the contemporary scientific community and society must be a strong one: on the side of scientific knowledge, by marking the complex, systemic nature of physiopathology and therapy; on the side of epistemology, by marking the paradoxes of bio-medicine; on the side of organization of health care, by promoting the need for a person-oriented approach to illness; and finally on the side of culture, society and ethics
Machine Learning for Power Profiles Prediction in Presence of Inter-channel Stimulated Raman Scattering
Two artificial neural network (ANN) models are presented to predict power profiles over C+L–band in presence of inter-channel stimulated Raman scattering (ISRS) and to support non-linear interference (NLI) modeling. Deterministic Raman crosstalk effects in amplified wavelength division multiplexing transmission
We study the deterministic effects of Raman-induced crosstalk in amplified
wavelength division multiplexing (WDM) optical fiber transmission lines. We
show that the dynamics of pulse amplitudes in an N-channel transmission system
is described by an N-dimensional predator-prey model. We find the equilibrium
states with non-zero amplitudes and prove their stability by obtaining the
Lyapunov function. The stability is independent of the exact details of the
approximation for the Raman gain curve. Furthermore, we investigate the impact
of cross phase modulation and Raman self and cross frequency shifts on the
dynamics and establish the stability of the equilibrium state with respect to
these perturbations. Our results provide a quantitative explanation for the
robustness of differential-phase-shift-keyed WDM transmission against Raman

Genomic and clinical findings in myeloid neoplasms with PDGFRB rearrangement
Platelet-derived growth factor receptor B (PDGFRB) gene rearrangements define a unique subgroup of myeloid and lymphoid neoplasms frequently associated with eosinophilia and characterized by high sensitivity to tyrosine kinase inhibition. To date, various PDGFRB/5q32 rearrangements, involving at least 40 fusion partners, have been reported. However, information on genomic and clinical features accompanying rearrangements of PDGFRB is still scarce. Here, we characterized a series of 14 cases with a myeloid neoplasm using cytogenetic, single nucleotide polymorphism array, and next-generation sequencing. We identified nine PDGFRB translocation partners, including the KAZN gene at 1p36.21 as a novel partner in a previously undescribed t(1;5)(p36;q33) chromosome change. In all cases, the PDGFRB recombination was the sole cytogenetic abnormality underlying the phenotype. Acquired somatic variants were mainly found in clinically aggressive diseases and involved epigenetic genes (TET2, DNMT3A, ASXL1), transcription factors (RUNX1 and CEBPA), and signaling modulators (HRAS). By using both cytogenetic and nested PCR monitoring to evaluate response to imatinib, we found that, in non-AML cases, a low dosage (100–200 mg) is sufficient to induce and maintain longstanding hematological, cytogenetic, and molecular remissions
