    Retorno de la fertilidad en un perro con un absceso prostático

    Se presenta en la consulta un macho reproductor, Pastor Alemán, de 6 años por infertilidad tras las dos últimas cubriciones. No presenta ningún otro síntoma clínico y carece de antecedentes patológicos. Ha realizado anteriormente 14 montas satisfactorias. Se realiza un espermiograma que muestra una astenoteratozoospermia junto con una hematospermia de la tercera fase del eyaculado. La ecografía del aparato génito-urinario permite observar una prostatitis junto con dos cavidades intraprostáticas, que son drenadas mediante punción ecoguiada, la cual revela un contenido purulento. La asociación de un tratamiento médico con antibióticos y finasteride, junto con una omentalización de la próstata, permite una recuperación de la fertilidad, objetivada mediante espermiograma, sin necesidad de una castración quirúrgica

    Objective: To analyze health care costs of patients with diabetes mellitus and hypertension, and to estimate the cost of medium complexity outpatient procedures, compared with the standard reimbursement values used in Brazil. Materials and methods: We analyzed direct health costs in a public health reference unit in Recife/PE, in 2007. Costs were determined and allocated using the techniques of absorption costing and apportionment. Results: Direct costs and the amount reimbursed by the SUS totaled R4,855,291.82andR 4,855,291.82 and R 2.118.893,56, respectively. The greatest groups of expenditure were medications, with R1,762,424.42(36.3 1,762,424.42 (36.3%), outsourced services, with R 996,637.82 (20.5%); and personnel, with R$ 978,096.10 (20.1%). All procedures had higher estimated costs than what is reimbursed by the SUS. Conclusions: Drugs were associated with the highest health care costs, a considerable difference was observed between estimated costs and the amount reimbursed by the SUS.     Elements Discrimination in the Study of Super-Heavy Elements using an Ionization Chamber

    Dedicated ionization chamber was built and installed to measure the energy loss of very heavy nuclei at 2.7 MeV/u produced in fusion reactions in inverse kinematics (beam of 208Pb). After going through the ionization chamber, products of reactions on 12C, 18O targets are implanted in a Si detector. Their identification through their alpha decay chain is ambiguous when their half-life is short. After calibration with Pb and Th nuclei, the ionization chamber signal allowed us to resolve these ambiguities. In the search for rare super-heavy nuclei produced in fusion reactions in inverse or symmetric kinematics, such a chamber will provide direct information on the nuclear charge of each implanted nucleus.Comment: submitted to NIMA, 10 pages+4 figures, Latex, uses elsart.cls and grahpic

    The SERVIDIAH study (Evaluation of Health Services for Diabetic and Hypertensive Subjects) was conducted in 2010 in the State of Pernambuco, Brazil. A multi-stage random sample of 785 hypertensive and 823 diabetic patients was drawn from 208 Family Health Strategy (FHS) units selected throughout 35 municipalities. Patients underwent a structured interview and weight, height, blood pressure and HbA1c levels (for diabetic patients) were measured. Mean age was approximately 60 years, and women were overrepresented in the sample (70%). 43.7% of hypertensive subjects and 25.8% of diabetic subjects achieved adequate blood pressure control and 30.5% of diabetic subjects had HbA1c levels below 7%. Despite 70% of the patients being overweight or obese, few had adhered to a weight-loss diet. The study of this representative sample of hypertensive and diabetic patients attended by the FHS in the State of Pernambuco shows that improvements in the management of hypertension and diabetes are needed in order to prevent the occurrence of serious and costly complications, especially given the context of increasing incidence of these two conditions

    Interés de las biopsias de útero como método diagnóstico de la infertilidad de origen inexplicado en la perra

    Las lesiones uterinas están consideradas como una de las causas principales de la infertilidad canina. Sin embargo, hasta el momento, escasas publicaciones han evaluado su verdadero impacto en la perra. En este estudio se describen los resultados histológicos de 20 perras con infertilidad de origen indeterminado a las que se les practicó una biopsia uterina por laparotomía durante el periodo de diestro, en el 75% de los casos. Se definieron 3 grupos: perras cuya gestación nunca fue confirmada (GNC), perras con interrupción de la gestación (IG) y perras con subfertilidad. En total, el 89.5% de las biopsias fueron anormales. Las patologías más frecuentemente diagnosticadas fueron la endometritis (31.5%), la HGQ (Hiperplasia Glándulo-Quística) (31.5%) y la FDG (fibrosis asociada a la degeneración de las glándulas endometriales), (21%), que pudieron encontrarse aisladas o asociadas a otras anomalías. En ninguno de los casos se aisló un agente infeccioso. Mientras que la HGQ y la FDG probablemente jueguen un papel importante en la infertilidad, las endometritis podrían ser, en ciertos casos la causa primaria, y en otros, la consecuencia de la interrupción precoz de la gestación

    Multidisciplinary intensive functional restoration versus outpatient active physiotherapy in chronic low back pain: a randomized controlled trial.

    STUDY DESIGN: Randomized parallel group comparative trial with a 1-year follow-up period. OBJECTIVE: To compare in a population of patients with chronic low back pain, the effectiveness of a functional restoration program (FRP), including intensive physical training and a multidisciplinary approach, with an outpatient active physiotherapy program at 1-year follow-up. SUMMARY OF BACKGROUND DATA: Controlled studies conducted in the United States and in Northern Europe showed a benefit of FRPs, especially on return to work. Randomized studies have compared these programs with standard care. A previously reported study presented the effectiveness at 6 months of both functional restoration and active physiotherapy, with a significantly greater reduction of sick-leave days for functional restoration. METHODS: A total of 132 patients with low back pain were randomized to either FRP (68 patients) or active individual therapy (64 patients). One patient did not complete the FRP; 19 patients were lost to follow-up (4 in the FRP group and 15 in the active individual treatment group). The number of sick-leave days in 2 years before the program was similar in both groups (180 ± 135.1 days in active individual treatment vs. 185 ± 149.8 days in FRP, P = 0.847). RESULTS: In both groups, at 1-year follow-up, intensity of pain, flexibility, trunk muscle endurance, Dallas daily activities and work and leisure scores, and number of sick-leave days were significantly improved compared with baseline. The number of sick-leave days was significantly lower in the FRP group. CONCLUSION: Both programs are efficient in reducing disability and sick-leave days. The FRP is significantly more effective in reducing sick-leave days. Further analysis is required to determine if this overweighs the difference in costs of both programs

    Monitoring Of 14 Mev Neutrons

    Long-lived fission products and minor actinides produced in nuclear power plants are the most radiotoxic nuclear wastes. They can be transmuted into stable nuclei or into nuclei with shorter lifetime thanks to the so-called Accelerator Driven Systems (ADS), consisting of the coupling of an intense high energy proton beam, a spallation target and a sub-critical reactor core. For safety reasons, an on-line and robust measurement of the reactivity during loading and power operation is mandatory. The investigation of the relationship between the current of the accelerator and the power level (or neutron flux) of the reactor appears to be powerful, any change in reactivity being accessible through the measurement of the current and the flux. Such a relationship will be studied in an experiment to be performed at the YALINA facility (JIPNR Sosny - Belarus) in the framework of the EUROTRANS IP (6th^{th} FP). At this installation, 14 MeV neutrons are produced in T(d,n)4^{4}He reactions by a deuteron beam impinging on a TiT target. Due to the tritium consumption over time, the intensity of the deuteron beam cannot be used for the monitoring of the neutron beam. The source neutron yield itself has to be accessed.
