    Sequential coupled simulation of a dual source heat pump and shallow geothermal reservoir

    The numerical simulation is an important tool for the assessment of exploiting geothermal energy. It can be used in shallow geothermal applications to evaluate the different production scenarios and the sustainability of the system (geothermal reservoir and heat pump) on long term. Moreover, in shallow geothermal projects, to simulate the real behaviour of the system, the load profiles of the end user and variations of the working mode of the heat pump should be taken into account. The present work introduces and describes a coupled numerical model, in which a dedicated Matlab\uae script has been realized to allow a sequential coupled simulation of a shallow geothermal reservoir exploited with a dual source heat pump. A mathematical model of a dual source heat pump that can work with the ground or the air as source/sink has been developed in Matlab\uae environment. Each component of the heat pump has been modelled considering the equations that govern the physical phenomena. The dynamic numerical simulator FEFLOW\uae has been used to simulate the behaviour of the geothermal reservoir, subjected to heat extraction/reinjection by a closed loop vertical heat exchangers field. This methodological approach is useful to evaluate the performance of the coupled system on the long term, and it is important for understanding the advantages and limits of the dual source heat pump in assuring the sustainability over time when heat is exchanged with the ground, avoiding the depletion of geothermal resources. The mathematical models have been validated with experimental data from a geothermal plant located in Tribano (Padova, IT). This is one of the four pilot sites realized within the framework of the H2020 GEOTeCH Project. It consists of eight coaxial borehole heat exchangers 30 m deep, connected to the 16 kW dual source heat pump prototype realized by HIREF S.p.A. The geothermal heat pump system has been working, and monitored, since October 2017 and it provides heating and air conditioning to an office area. Experimental results have been used to verify the new coupled model, and although the preliminary results are encouraging, further study and work are necessary to make it robust and stable for future routine work

    Lengthening Patients Previously Treated for Massive Lower Limb Reconstruction for Bone Tumors with the PRECICE 2 Nail

    The objective of this study was to determine the efficacy of the PRECICE 2® nail in the treatment of lower limb length discrepancy in patients with a history of bone tumors. This study reports on outcomes, complications, and the safety of the PRECICE 2 limb lengthening nail in a cohort of pediatric patients with limb length discrepancy after surgery for bone tumors. Seventeen patients were treated with intramedullary magnetic nails. The average patient age at the time of surgery was 19 (range 11–32). The PRECICE 2 nail was used on 14 femurs (6 retrograde and 8 anterograde) and 3 tibias. The average consolidation time was 141 days (range 50–360) with a mean CI of 31 ± 12 days/cm. The ASAMI bone score showed 14 (82%) excellent results, 1 (6%) good result, and 2 (12%) poor results. The ASAMI functional score showed 13 (84.6%) excellent results, 3 (11.5%) good results, and 1 (3.8%) fair result. Patients treated with chemotherapy for bone cancer did not show any increase in distraction time or consolidation time. A total of 3 (17%) problems, 1 obstacle (5.5%), and 1 complication (5.5%) were encountered in our case series. The PRECICE 2 nail allows for effective and accurate lengthening preserving the range of motion in patients treated for bone tumors

    Operational observing and forecasting system for dissolved oxygen and environmental parameters in the Northern Adriatic Sea

    The northern Adriatic Sea (NA) is affected by strong anthropogenic pressure, superimposed to a large river runoff. The consequent pressure exerted on the NA ecosystem either triggers or worsens negative phenomena like anoxic/hypoxic events. During the summer-autumn period, the NA is often exposed to these events, which can be categorised as either coastal (relatively frequent south of the Po River delta during the summer) and offshore (rare, affecting wider areas). An operational system for monitoring and forecasting anoxic/hypoxic events has been set up in the framework of the EU LIFE “EMMA” project. The system is composed of a meteo-oceanographic buoy; a numerical prediction system based on the Regional Ocean Modelling System (ROMS), including a Fashamtype module for biogeochemical fluxes; and oceanographic surveys. Every day since June 2007, the system provides 3-hourly forecasts of marine currents, thermohaline and biogeochemical fields for the incoming three days. The system demonstrated its ability to produce accurate temperature forecasts and relatively good salinity and dissolved oxygen forecasts. The Root Mean Square Error of the dissolved oxygen forecast was largely due to the mean bias. The system is currently being improved to include a better representation of benthic layer biogeochemical processes and several adjustments of the model. While developing model improvements, dissolved oxygen forecasts were improved with the removal of the 10-day mean bias

    BACKGROUND: The timing of cardiac surgery after stroke in infective endocarditis (IE) remains controversial. We examined the relationship between the timing of surgery after stroke and the incidence of in-hospital and 1-year mortalities. METHODS: Data were obtained from the International Collaboration on Endocarditis-Prospective Cohort Study of 4794 patients with definite IE who were admitted to 64 centers from June 2000 through December 2006. Multivariate logistic regression and Cox regression analyses were performed to estimate the impact of early surgery on hospital and 1-year mortality after adjustments for other significant covariates. RESULTS: Of the 857 patients with IE complicated by ischemic stroke syndromes, 198 who underwent valve replacement surgery poststroke were available for analysis. Overall, 58 (29.3%) patients underwent early surgical treatment vs 140 (70.7%) patients who underwent late surgical treatment. After adjustment for other risk factors, early surgery was not significantly associated with increased in-hospital mortality rates (odds ratio, 2.308; 95% confidence interval [CI], .942-5.652). Overall, probability of death after 1-year follow-up did not differ between 2 treatment groups (27.1% in early surgery and 19.2% in late surgery group, P = .328; adjusted hazard ratio, 1.138; 95% CI, .802-1.650). CONCLUSIONS: There is no apparent survival benefit in delaying surgery when indicated in IE patients after ischemic stroke. Further observational analyses that include detailed pre- and postoperative clinical neurologic findings and advanced imaging data (eg, ischemic stroke size), may allow for more refined recommendations on the optimal timing of valvular surgery in patients with IE and recent stroke syndromes

    Impact of early valve surgery on outcome of staphylococcus aureus prosthetic valve infective endocarditis: Analysis in the international collaboration of endocarditis-prospective cohort study

    Background. The impact of early valve surgery (EVS) on the outcome of Staphylococcus aureus (SA) prosthetic valve infective endocarditis (PVIE) is unresolved. The objective of this study was to evaluate the association between EVS, performed within the first 60 days of hospitalization, and outcome of SA PVIE within the International Collaboration on Endocarditis-Prospective Cohort Study. Methods. Participants were enrolled between June 2000 and December 2006. Cox proportional hazards modeling that included surgery as a time-dependent covariate and propensity adjustment for likelihood to receive cardiac surgery was used to evaluate the impact of EVS and 1-year all-cause mortality on patients with definite left-sided S. aureus PVIE and no history of injection drug use. Results. EVS was performed in 74 of the 168 (44.3%) patients. One-year mortality was significantly higher among patients with S. aureus PVIE than in patients with non-S. aureus PVIE (48.2% vs 32.9%; P = .003). Staphylococcus aureus PVIE patients who underwent EVS had a significantly lower 1-year mortality rate (33.8% vs 59.1%; P = .001). In multivariate, propensity-adjusted models, EVS was not associated with 1-year mortality (risk ratio, 0.67 [95% confidence interval, .39-1.15]; P = .15). Conclusions. In this prospective, multinational cohort of patients with S. aureus PVIE, EVS was not associated with reduced 1-year mortality. The decision to pursue EVS should be individualized for each patient, based upon infection-specific characteristics rather than solely upon the microbiology of the infection causing PVIE
