    Efficacy and safety of a double-coated paclitaxel-eluting coronary stent: The EUCATAX trial

    Objectives: The aim of this study was the comparison of a new double-coated paclitaxel-eluting coronary stent with bare-metal stent (BMS) in patients undergoing percutaneous coronary intervention. Background: Stent coating with biodegradable polymers as a platform for elution of drugs has the potential for complete elution of drugs and for decreasing the risk of late complications. Methods: Multicenter randomized trial comparing a paclitaxel-eluting stent (PES) coated with a biodegradable polymer and glycocalyx with the equivalent BMS. We randomly assigned 422 patients with de novo coronary lesions to PES (211 patients) or to BMS (211 patients). Primary end point was target vessel failure (TVF) defined as cardiac death, myocardial infarction, and target vessel revascularization. Clinical secondary end points were target vessel revascularization, target lesion revascularization, stent thrombosis (ST), and major adverse cardiovascular events (MACE). Angiographic secondary end points were late loss and binary restenosis. Results: At 1 year of follow-up, TVF rate was 9.5% in the PES group and 17.1% in the BMS group (P = 0.02), and MACE rate was 10% in PES and 19% in BMS arm (P = 0.009). All other secondary end points were reached but ST. ST rate was low and similar in both study arms. Conclusions: The study shows that patients treated with PES with dual coating technology had significantly lower incidence of TVF and MACE than those treated with BMS design; however, longer follow-up should be necessary to assess true advantages of this technology compared with the previous one.Fil: Rodriguez, Alfredo E..     Assessment on the Achievable Throughput of Multi-band ITU-T G.652.D Fiber Transmission Systems

    Fiber-optic multi-band transmission (MBT) aims at exploiting the low-loss spectral windows of single-mode fibers (SMFs) for data transport, expanding by ∌11× the available bandwidth of C-band line systems and by ∌5× C+L-band line systems'. MBT offers a high potential for cost-efficient throughput upgrades of optical networks, even in absence of available dark-fibers, as it utilizes more efficiently the existing infrastructures. This represents the main advantage compared to approaches such as multi-mode/-core fibers or spatial division multiplexing. Furthermore, the industrial trend is clear: the first commercial C+L-band systems are entering the market and research has moved toward the neighboring S-band. This article discusses the potential and challenges of MBT covering the ITU-T optical bands O → L. MBT performance is assessed by addressing the generalized SNR (GSNR) including both the linear and non-linear fiber propagation effects. Non-linear fiber propagation is taken into account by computing the generated non-linear interference by using the generalized Gaussian-noise (GGN) model, which takes into account the interaction of non-linear fiber propagation with stimulated Raman scattering (SRS), and in general considers wavelength-dependent fiber parameters. For linear effects, we hypothesize typical components' figures and discussion on components' limitations, such as transceivers,' amplifiers' and filters' are not part of this work. We focus on assessing the transmission throughput that is realistic to achieve by using feasible multi-band components without specific optimizations and implementation discussion. So, results are meant to address the potential throughput scaling by turning-on excess fiber transmission bands. As transmission fiber, we focus exclusively on the ITU-T G.652.D, since it is the most widely deployed fiber type worldwide and the mostly suitable to multi-band transmission, thanks to its ultra-wide low-loss single-mode high-dispersion spectral region. Similar analyses could be carried out for other single-mode fiber types. We estimate a total single-fiber throughput of 450 Tb/s over a distance of 50 km and 220 Tb/s over regional distances of 600 km: ∌ 10 × and 8× more than C-band transmission respectively and ∌ 2.5× more than full C+L

    Metabolomic profile of cancer stem cell-derived exosomes from patients with malignant melanoma

    Malignant melanoma (MM) is the most aggressive and life-threatening form of skin cancer. It is characterized by an extraordinary metastasis capacity and chemotherapy resistance, mainly due to melanoma cancer stem cells (CSCs). To date, there are no suitable clinical diagnostic, prognostic or predictive biomarkers for this neoplasia. Therefore, there is an urgent need for new MM biomarkers that enable early diagnosis and effective disease monitoring. Exosomes represent a novel source of biomarkers since they can be easily isolated from different body fluids. In this work, a primary patient-derived MM cell line enriched in CSCs was characterized by assessing the expression of specific markers and their stem-like properties. Exosomes derived from CSCs and serums from patients with MM were characterized, and their metabolomic profile was analysed by highresolution mass spectrometry (HRMS) following an untargeted approach and applying univariate and multivariate statistical analyses. The aim of this study was to search potential biomarkers for the diagnosis of this disease. Our results showed significant metabolomic differences in exosomes derived from MM CSCs compared with those from differentiated tumour cells and also in serum-derived exosomes from patients with MM compared to those from healthy controls. Interestingly, we identified similarities between structural lipids differentially expressed in CSC-derived exosomes and those derived from patients with MM such as the glycerophosphocholine PC 16:0/0:0. To our knowledge, this is the first metabolomic-based study aimed at characterizing exosomes derived from melanoma CSCs and patients’ serum in order to identify potential biomarkers for MM diagnosis.     CAR density influences antitumoral efficacy of BCMA CAR T cells and correlates with clinical outcome

    Identification of new markers associated with long-term efficacy in patients treated with CAR T cells is a current medical need, particularly in diseases such as multiple myeloma. In this study, we address the impact of CAR density on the functionality of BCMA CAR T cells. Functional and transcriptional studies demonstrate that CAR T cells with high expression of the CAR construct show an increased tonic signaling with up-regulation of exhaustion markers and increased in vitro cytotoxicity but a decrease in in vivo BM infiltration. Characterization of gene regulatory networks using scRNA-seq identified regulons associated to activation and exhaustion up-regulated in CARHigh T cells, providing mechanistic insights behind differential functionality of these cells. Last, we demonstrate that patients treated with CAR T cell products enriched in CARHigh T cells show a significantly worse clinical response in several hematological malignancies.     Surface indicators are correlated with soil multifunctionality in global drylands

    Multiple ecosystem functions need to be considered simultaneously to manage and protect the several ecosystem services that are essential to people and their environments. Despite this, cost effective, tangible, relatively simple and globally relevant methodologies to monitor in situ soil multifunctionality, that is, the provision of multiple ecosystem functions by soils, have not been tested at the global scale. We combined correlation analysis and structural equation modelling to explore whether we could find easily measured, field-based indicators of soil multifunctionality (measured using functions linked to the cycling and storage of soil carbon, nitrogen and phosphorus). To do this, we gathered soil data from 120 dryland ecosystems from five continents. Two soil surface attributes measured in situ (litter incorporation and surface aggregate stability) were the most strongly associated with soil multifunctionality, even after accounting for geographic location and other drivers such as climate, woody cover, soil pH and soil electric conductivity. The positive relationships between surface stability and litter incorporation on soil multifunctionality were greater beneath the canopy of perennial vegetation than in adjacent, open areas devoid of vascular plants. The positive associations between surface aggregate stability and soil functions increased with increasing mean annual temperature. Synthesis and applications. Our findings demonstrate that a reduced suite of easily measured in situ soil surface attributes can be used as potential indicators of soil multifunctionality in drylands world-wide. These attributes, which relate to plant litter (origin, incorporation, cover), and surface stability, are relatively cheap and easy to assess with minimal training, allowing operators to sample many sites across widely varying climatic areas and soil types. The correlations of these variables are comparable to the influence of climate or soil, and would allow cost-effective monitoring of soil multifunctionality under changing land-use and environmental conditions.     Experimental Demonstration of Multivendor and Multidomain EON With Data and Control Interoperability Over a Pan-European Test Bed

    The operation of multidomain and multivendor EONs can be achieved by interoperable sliceable bandwidth variable transponders (S-BVTs), a GMPLS/BGP-LS-based control plane, and a planning tool. The control plane is extended to include the control of S-BVTs and elastic cross connects, which combine a large port-count fiber-switch (optical backplane) and bandwidth-variable wavelength-selective switches, enabling the end-to-end provisioning and recovery of network services. A multipartner testbed is built to demonstrate and validate the proposed end-to-end architecture. Interoperability among S-BVTs is experimentally tested between different implementations. In this case, transponders are configured using the proposed control plane. The achieved performance with hard-decision and soft-decision FECs using only the information distributed by the control plane is measured against the performance of the single-vendor implementation, where proprietary information is used, demonstrating error-free transmission up to 300 km.Peer ReviewedPostprint (author's final draft

    Relationship between olive oil consumption and ankle-brachial pressure index in a population at high cardiovascular risk

    The aim of this study was to ascertain the association between the consumption of different categories of edible olive oils (virgin olive oils and olive oil) and olive pomace oil and ankle-brachial pressure index (ABI) in participants in the PREDIMED-Plus study, a trial of lifestyle modification for weight and cardiovascular event reduction in individuals with overweight/obesity harboring the metabolic syndrome. Methods: We performed a cross-sectional analysis of the PREDIMED-Plus trial. Consumption of any category of olive oil and olive pomace oil was assessed through a validated food-frequency questionnaire. Multivariable linear regression models were fitted to assess associations between olive oil consumption and ABI. Additionally, ABI ≀1 was considered as the outcome in logistic models with different categories of olive oil and olive pomace oil as exposure. Results: Among 4330 participants, the highest quintile of total olive oil consumption (sum of all categories of olive oil and olive pomace oil) was associated with higher mean values of ABI (beta coefficient: 0.014, 95% confidence interval [CI]: 0.002, 0.027) (p for trend = 0.010). Logistic models comparing the consumption of different categories of olive oils, olive pomace oil and ABI ≀1 values revealed an inverse association between virgin olive oils consumption and the likelihood of a low ABI (odds ratio [OR] 0.73, 95% CI [0.56, 0.97]), while consumption of olive pomace oil was positively associated with a low ABI (OR 1.22 95% CI [1.00, 1.48]). Conclusions: In a Mediterranean population at high cardiovascular risk, total olive oil consumption was associated with a higher mean ABI. These results suggest that olive oil consumption may be beneficial for peripheral artery disease prevention, but longitudinal studies are needed
