    The impact of mental health and psychosocial support programmes on children and young people's mental health in the context of humanitarian emergencies in low- and middle-income countries: A systematic review and meta-analysis.

    Humanitarian emergencies pose a significant global health challenge for children and young people's mental and psychological health. This systematic review investigates the effectiveness of mental health and psychosocial support (MHPSS) programmes delivered to children and young people affected by humanitarian emergencies in low- and middle-income countries (LMICs). Twelve electronic databases, key websites and citation checking were undertaken. Forty-three randomised controlled trials (RCTs) published in English between January 1980 and May 2023 were included in the review. Overall, the findings suggest that cognitive behavioural therapy may improve depression symptoms in children and young people affected by humanitarian emergencies. Narrative exposure therapy may reduce feelings of guilt. However, the impact of the other MHPSS modalities across outcomes is inconsistent. In some contexts, providing psychosocial programmes involving creative activities may increase the symptoms of depression in children and young people. These findings emphasise the need for the development of MHPSS programmes that can safely and effectively address the diverse needs of children and young people living in adversarial environments

    Double Exponential Instability of Triangular Arbitrage Systems

    If financial markets displayed the informational efficiency postulated in the efficient markets hypothesis (EMH), arbitrage operations would be self-extinguishing. The present paper considers arbitrage sequences in foreign exchange (FX) markets, in which trading platforms and information are fragmented. In Kozyakin et al. (2010) and Cross et al. (2012) it was shown that sequences of triangular arbitrage operations in FX markets containing 4 currencies and trader-arbitrageurs tend to display periodicity or grow exponentially rather than being self-extinguishing. This paper extends the analysis to 5 or higher-order currency worlds. The key findings are that in a 5-currency world arbitrage sequences may also follow an exponential law as well as display periodicity, but that in higher-order currency worlds a double exponential law may additionally apply. There is an "inheritance of instability" in the higher-order currency worlds. Profitable arbitrage operations are thus endemic rather that displaying the self-extinguishing properties implied by the EMH.Comment: 22 pages, 22 bibliography references, expanded Introduction and Conclusion, added bibliohraphy reference

    Estimates of measles case fatality ratios: a comprehensive review of community-based studies.

    BACKGROUND: Global deaths from measles have decreased notably in past decades, due to both increases in immunization rates and decreases in measles case fatality ratios (CFRs). While some aspects of the reduction in measles mortality can be monitored through increases in immunization coverage, estimating the level of measles deaths (in absolute terms) is problematic, particularly since incidence-based methods of estimation rely on accurate measures of measles CFRs. These ratios vary widely by geographic and epidemiologic context and even within the same community from year-to-year. METHODS: To understand better the variations in CFRs, we reviewed community-based studies published between 1980 and 2008 reporting age-specific measles CFRs. RESULTS: The results of the search consistently document that measles CFRs are highest in unvaccinated children under age 5 years; in outbreaks; the lowest CFRs occur in vaccinated children regardless of setting. The broad range of case and death definitions, study populations and geography highlight the complexities in extrapolating results for global public health planning. CONCLUSIONS: Values for measles CFRs remain imprecise, resulting in continued uncertainty about the actual toll measles exacts

    Mass transfer dynamics in double degenerate binary systems

    We present a numerical study of the mass transfer dynamics prior to the gravitational wave-driven merger of a double white dwarf system. Recently, there has been some discussion about the dynamics of these last stages, different methods seemed to provide qualitatively different results. While earlier SPH simulations indicated a very quick disruption of the binary on roughly the orbital time scale, more recent grid-based calculations find long-lived mass transfer for many orbital periods. Here we demonstrate how sensitive the dynamics of this last stage is to the exact initial conditions. We show that, after a careful preparation of the initial conditions, the reportedly short-lived systems undergo mass transfer for many dozens of orbits. The reported numbers of orbits are resolution-biased and therefore represent only lower limits to what is realized in nature.     CD4 memory T cells divide poorly in response to antigen because of their cytokine profile

    Immunological memory is a hallmark of adaptive immunity, and understanding T cell memory will be central to the development of effective cell-mediated vaccines. The characteristics and functions of CD4 memory cells have not been well defined. Here we demonstrate that the increased size of the secondary response is solely a consequence of the increased antigen-specific precursor frequency within the memory pool. Memory cells proliferated less than primary responding cells, even within the same host. By analyzing the entry of primary and memory cells into the cell cycle, we found that the two populations proliferated similarly until day 5; after this time, fewer of the reactivated memory cells proliferated. At this time, fewer of the reactivated memory cells made IL-2 than primary responding cells, but more made IFNγ. Both these factors affected the low proliferation of the memory cells, because either exogenous IL-2 or inhibition of IFNγ increased the proliferation of the memory cells

    Popularity versus Similarity in Growing Networks

    Popularity is attractive -- this is the formula underlying preferential attachment, a popular explanation for the emergence of scaling in growing networks. If new connections are made preferentially to more popular nodes, then the resulting distribution of the number of connections that nodes have follows power laws observed in many real networks. Preferential attachment has been directly validated for some real networks, including the Internet. Preferential attachment can also be a consequence of different underlying processes based on node fitness, ranking, optimization, random walks, or duplication. Here we show that popularity is just one dimension of attractiveness. Another dimension is similarity. We develop a framework where new connections, instead of preferring popular nodes, optimize certain trade-offs between popularity and similarity. The framework admits a geometric interpretation, in which popularity preference emerges from local optimization. As opposed to preferential attachment, the optimization framework accurately describes large-scale evolution of technological (Internet), social (web of trust), and biological (E.coli metabolic) networks, predicting the probability of new links in them with a remarkable precision. The developed framework can thus be used for predicting new links in evolving networks, and provides a different perspective on preferential attachment as an emergent phenomenon

    Private patient perceptions about a public programme; what do private Indian tuberculosis patients really feel about directly observed treatment?

    <p>Abstract</p> <p>Background</p> <p>India accounts for one-fifth of the global incident cases of tuberculosis(TB). The country presently has the world's largest directly observed treatment, short course (DOTS) programme, that has shown impressive results and covers almost 100% of the billion-plus Indian population. Despite such a successful programme, the majority of Indian patients with tuberculosis prefer private healthcare, although repeated audits of this sector have shown the quality to be poor.</p> <p>We aimed to ascertain the level of awareness and knowledge of private patients with tuberculosis attending our clinic at a tertiary private healthcare institute with regards to the DOTS programme, understanding the reasons behind their preference for private healthcare, and evaluating their perceptions and reasons for accepting or failing to accept directly observed therapy as a treatment option.</p> <p>Methods</p> <p>A structured interview schedule was administered to private patients with tuberculosis at the P.D. Hinduja Hospital and Medical Research Centre, Mumbai, India between January 2006 to November 2007.</p> <p>Results</p> <p>Only 30 of 200 patients (15%) were aware of the DOTS programme. After being explained what directly observed therapy was, 136 patients (68%) found this form of treatment unacceptable.183 patients (91.5%) preferred buying the drugs themselves to visiting a DOTS centre. 90 patients (45%) were not prepared to be observed while swallowing their TB drugs, finding it an intrusion of privacy.</p> <p>Conclusions</p> <p>Our study reveals a poor knowledge and awareness of the DOTS programme among the cohort of TB patients that we interviewed. The control of TB in India will undoubtedly benefit from more patients being attracted to and treated by the existing DOTS programmes. However, directly observed treatment, in its present form, is considered too rigid and intrusive and is unlikely to be accepted by a majority of patients seeking private healthcare. Novel strategies and more flexible options will have to be devised to ensure higher cure rates without compromising patient choice.</p

    Effect of hydraulic coefficient on membrane performance for rejection of emerging contaminants

    The efficient removal of Endocrine Disruptive Chemicals (EDCs) namely oxybenzone and atrazine using polysulfone (PSf) composite membranes is reported in this article. A negatively charged hydrophilic mixed matrix membrane was prepared by using Polyaniline modified halloysite nanotubes (PANi-HNT) and polysulfone. The X-ray diffraction (XRD) and attenuated total reflectance infrared (ATR-IR) technique confirm the conversion of Halloysite nanotubes to Polyaniline modified halloysite nanotubes and their presence in membrane matrix. The microvoids observable in the SEM images depict the internal structure of the membrane. Further, increasing in the water uptake and decreasing in the contact angle with respect to increasing the concentration of Polyaniline modified halloysite nanotubes confirm the enhancement of the membrane hydrophilicity. This hydrophilicity increases for higher concentrations of PANi-HNT leads to a significant improvement of the water flux. The removal efficiency of prepared mixed matrix membranes was found to be 98% for oxybenzone and 50% for atrazine. The separation process was discussed in terms of adsorption coefficient, membrane charge and mean pore size. Considering the performances highlighted in this study, the proposed membranes appear usable for the removal of EDCs from contaminated solutions
