    Multilevel Surgery Improves Gait in Spastic Hemiplegia But Does Not Resolve Hip Dysplasia

    Background: Multilevel orthopaedic surgery may improve gait in Type IV hemiplegia, but it is not known if proximal femoral osteotomy combined with adductor release as part of multilevel surgery in patients with hip dysplasia improves hip development. Questions/purposes: We asked whether varus derotational osteotomy of the proximal femur, combined with adductor release, influenced hip development in patients with Type IV hemiplegia having multilevel surgery. Patients and Methods: We retrospectively reviewed 11 children and adolescents with Type IV hemiplegia who had a proximal femoral osteotomy due to unilateral hip displacement to correct gait dysfunction between 1999 and 2006. The mean age at the time of surgery was 11.1years (range, 7 to 16 years). We obtained the Movement Analysis Profile and Gait Profile Score before and after surgery. We also measured the Migration Percentage of Reimers and applied the Melbourne Cerebral Palsy Hip Classification System (MCPHCS). The minimum followup was 2years 3months (mean, 6years 6months; range, 2years 3months to 10years 8months). Results: The majority of gait parameters improved but hip development was not normalized. According to the MCPHCS at last followup, no hips were classified as Grade I, two hips were classified as Grade II, and the remainder were Grade III and IV. Conclusions: Unilateral surgery including a proximal femoral osteotomy improved gait and walking ability in individuals with spastic hemiplegic cerebral palsy. However, hip dysplasia persists.     Annotation Studio: multimedia text annotation for students

    Annotation Studio will be a web-based application that actively engages students in interpreting literary texts and other humanities documents. While strengthening students' new media literacies, this open source web application will develop traditional humanistic skills including close reading, textual analysis, persuasive writing, and critical thinking. Initial features will include: 1) easy-to-use annotation tools that facilitate linking and comparing primary texts with multi-media source, variation, and adaptation documents; 2) sharable collections of multimedia materials prepared by faculty and student users; 3) multiple filtering and display mechanisms for texts, written annotations, and multimedia annotations; 4) collaboration functionality; and 5) multimedia composition tools. Products of the start-up phase will include a working prototype, feedback from students and instructors, and a white paper summarizing lessons learned

    The genomic landscape of balanced cytogenetic abnormalities associated with human congenital anomalies

    Despite the clinical significance of balanced chromosomal abnormalities (BCAs), their characterization has largely been restricted to cytogenetic resolution. We explored the landscape of BCAs at nucleotide resolution in 273 subjects with a spectrum of congenital anomalies. Whole-genome sequencing revised 93% of karyotypes and demonstrated complexity that was cryptic to karyotyping in 21% of BCAs, highlighting the limitations of conventional cytogenetic approaches. At least 33.9% of BCAs resulted in gene disruption that likely contributed to the developmental phenotype, 5.2% were associated with pathogenic genomic imbalances, and 7.3% disrupted topologically associated domains (TADs) encompassing known syndromic loci. Remarkably, BCA breakpoints in eight subjects altered a single TAD encompassing MEF2C, a known driver of 5q14.3 microdeletion syndrome, resulting in decreased MEF2C expression. We propose that sequence-level resolution dramatically improves prediction of clinical outcomes for balanced rearrangements and provides insight into new pathogenic mechanisms, such as altered regulation due to changes in chromosome topology

    Detecting Children's Lies: Comparing True Accounts About Highly Stressful Injuries with Unprepared, Prepared, and Coached Lies

    In this investigation, 514 university students judged whether children were telling the truth about highly emotional events. Eight children (half female, half 8–9 and the remainder 12–14 years old) had been injured seriously enough to require emergency room treatment and were interviewed a few days later. Each was yoked to three other children matched in age and gender who fabricated accounts under one of three conditions: lies that were unprepared, prepared (24 hours to prepare), and coached by parents. Participants were at chance when judging true accounts as well as unprepared and prepared lies. However, 74% of the coached lies were judged as true. Participants' confidence in their judgments, age, experience with children, and relevant coursework/training did not improve judgments

    Cognition and Freezing of Gait in Parkinson's Disease: A Systematic Review and Meta-Analysis

    Freezing of gait (FOG) is a common and disabling symptom in people with Parkinson's Disease (PwPD). Although cognition is thought to be worse in PwPD who freeze, a comprehensive analysis of this relationship will inform future research and clinical care. This systematic review and meta-analysis compared cognition between PwPD who do and do not exhibit FOG across a range of cognitive domains and assessed the impact of disease severity and medication status on this relationship. 145 papers (n=9010 participants) were included in the analysis, with 144 and 138 articles meeting the criteria to assess moderating effects of disease severity and medication status, respectively. PwPD who freeze exhibited worse cognition than PwPD without FOG across global cognition, executive function/attention, language, memory, and visuospatial domains. Greater disease severity and "ON" levodopa medication status moderated the FOG status-cognition relationship in global cognitive performance but not in other cognitive domains. This meta-analysis confirmed that cognition is worse in PwPD with FOG and highlights the importance of disease severity and medication status in this relationship
