    Exposure to carcinogenic polycyclic aromatic compounds and health risk assessment for diesel-exhaust exposed workers

    Objectives: Workers' exposure to diesel exhaust in a bus depot, a truck repair workshop and an underground tunnel was determined by the measuring of elemental carbon (EC) and 15 carcinogenic polycyclic aromatic compounds (PACs) proposed by the US Department of Health and Human Services/National Toxicology Program (NTP). Based on these concentration data, the genotoxic PAC contribution to the diesel-exhaust particle (DEP) lung-cancer risk was calculated. Method: Respirable particulate matter was collected during the summer and winter of 2001 (except for in the underground situation) and analysed by coulometry for EC and by GC-MS methods for PACs. The use of potency equivalence factors (PEFs) allowed the studied PAC concentrations to be expressed as benzo[a]pyrene equivalents (B[a]Peq). We then calculated the lung-cancer risk due to PACs and DEPs by multiplying the B[a]Peq and EC concentrations by the corresponding unit risk factor. The ratio of these two risks values has been considered as an estimate of the genotoxic contribution to the DEP cancer risk. Results: For the bus depot and truck repair workshop, exposure to EC and PACs has been shown to increase by three to six times and ten times, respectively, during winter compared to summer. This increase has been attributed mainly to a decrease in ventilation during the cold. With the PEF approach, the B[a]Peq concentration is five-times higher than if only benzo[a]pyrene (B[a]P) is considered. Dibenzopyrenes contribute an important part to this increase. A simple calculation based on unit risk factors indicates that the studied PAC contribution to the total lung-cancer risk attributed to DEPs is in the range of 3-13%. Conclusions: The 15 NTP PACs represent a small but non-negligible part of lung-cancer risk with regard to diesel exposure.     This paper presents the results of an experimental study of three samples containing various elements in the near-surface layers. The depth profiles of all the elements of different atomic masses from hydrogen to silver were investigated by Rutherford Backscattering Spectrometry (RBS) and Elastic Recoil Detection Analysis (ERDA). The experiments were performed by using the low-energy (about 2 MeV) 4He+ ion beams. The obtained results demonstrate the possibility of the RBS and ERDA methods in the investigation of depth profiles of any mass element with an atomic concentration of about 0.01 at.% and a depth resolution close to 10 nm

    Development and Validation of a New Bitumen Fume Generation System which Generates Polycyclic Aromatic Hydrocarbon Concentrations Proportional to Fume Concentrations

    Bitumen fumes emitted during road paving and roofing contain polycyclic aromatic compounds (PACs) of potential health concern. Little information is available for an experimental device devoted to inhalation experiments with animals exposed to bitumen fumes, and in all studies the systems were never validated for a range of fume concentrations, which prohibited their use for toxicological concentration-effect studies. Therefore, the purpose of this study was to validate a new experimental device able to generate bitumen fumes at different total particulate matter (TPM) concentrations with a linear correlation between TPM and the concentrations of different PACs, thus allowing toxicological dose-response studies with fumes representative of those in the field. Atmosphere samples collected from an animal exposure chamber allowed the determination of TPM, toluene soluble matter, polycyclic aromatic hydrocarbons (PAHs) and semi-volatiles. The particulate size distributions were determined in order to assess the deposition pattern in the respiratory tract. The temperature of 170°C was chosen by analogy with the upper range of the temperature used during paving operations. The temperature of the air passing over the fume emission area was regulated to 20°C and stirring of the heated bitumen was restricted to 90 r.p.m. The data show that the objective of developing a static fume generation system that reproducibly produces fumes in the inhalation chamber for specified target concentrations (TPM) were successful. The within-day variation coefficients for TPM were between 2.5 and 6.1%. The day-to-day variations for TPM concentration were between 4.1 and 5.8%. The concentrations of the 4-5 ring PAHs and the polycyclic aromatic sulphur heterocycles were proportional to the TPM concentration. The 2 and 3 ring PAH concentrations showed a deviation from proportionality with the TPM, probably due to their re-evaporation during sampling. The mass median aerodynamic diameter of airborne particles varied from 1.4 µm at a fume concentration of 5 mg/m3 to 3.2 µm at 100 mg/m3. In conclusion, this equipment was suitable for nose-only inhalation studies in the 5-100 mg/m3 range of TPM. Bitumen fumes were generated with a good reproducibility under well-controlled conditions.     Seasonality of respiratory viruses causing hospitalizations for acute respiratory infections in children in Nha Trang, Vietnam.

    BACKGROUND: Acute respiratory infections (ARIs) are the most common causes of death in children under 5 years of age. While the etiology of most pneumonia and ARI episodes is undiagnosed, a broad range of ARI-causing viruses circulate widely in South East Asia. However, the patterns and drivers of the seasonal transmission dynamics are largely unknown. Here we identify the seasonal patterns of multiple circulating viruses associated with hospitalizations for ARIs in Nha Trang, Vietnam. METHODS: Hospital based enhanced surveillance of childhood ARI is ongoing at Khanh Hoa General Hospital in Nha Trang. RT-PCR was performed to detect 13 respiratory viruses in nasopharyngeal samples from enrolled patients. Seasonal patterns of childhood ARI hospital admissions of various viruses were assessed, as well as their association with rainfall, temperature, and dew point. RESULTS: Respiratory syncytial virus peaks in the late summer months, and influenza A in April to June. We find significant associations between detection of human parainfluenza 3 and human rhinovirus with the month's mean dew point. Using a cross-wavelet transform we find a significant out-of-phase relationship between human parainfluenza 3 and temperature and dew point. CONCLUSIONS: Our results are important for understanding the temporal risk associated with circulating pathogens in Southern Central Vietnam. Specifically, our results can inform timing of routing seasonal influenza vaccination and for when observed respiratory illness is likely viral, leading to judicious use of antibiotics in the region

    Estimation of methane emissions from local and crossbreed beef cattle in Daklak province of Vietnam

    Objective: This study was aimed at evaluating effects of cattle breed resources and alternative mixed-feeding practices on meat productivity and emission intensities from household farming systems (HFS) in Daklak Province, Vietnam. Methods: Records from Local Yellow×Red Sindhi (Bos indicus; Lai Sind) and 1/2 Limousin, 1/2 Drought Master, and 1/2 Red Angus cattle during the growth (0 to 21 months) and fattening (22 to 25 months) periods were used to better understand variations on meat productivity and enteric methane emissions. Parameters were determined by the ruminant model. Four scenarios were developed: (HFS1) grazing from birth to slaughter on native grasses for approximately 10 h plus 1.5 kg dry matter/d (0.8% live weight [LW]) of a mixture of Guinea grass (19%), cassava (43%) powder, cotton (23%) seed, and rice (15%) straw; (HFS2) growth period fed with elephant grass (1% of LW) plus supplementation (1.5% of LW) of rice bran (36%), maize (33%), and cassava (31%) meals; and HFS3 and HFS4 computed elephant grass, but concentrate supplementation reaching 2% and 1% of LW, respectively. Results: Results show that compared to HFS1, emissions (72.3±0.96 kg CH 4 /animal/life; least squares means± standard error of the mean) were 15%, 6%, and 23% lower (p < 0.01) for the HFS2, HFS3, and HFS4, respectively. The predicted methane efficiencies (CO 2 eq) per kg of LW at slaughter (4.3±0.15), carcass weight (8.8±0.25 kg) and kg of edible protein (44.1±1.29) were also lower (p < 0.05) in the HFS4. In particular, irrespective of the HSF, feed supply and ratio changes had a more positive impact on emission intensities when crossbred 1/2 Red Angus cattle were fed than in their crossbred counterparts. Conclusion: Modest improvements on feeding practices and integrated modelling frameworks may offer potential trade-offs to respond to climate change in Vietnam

    Culturally adaptive storytelling intervention versus didactic intervention to improve hypertension control in Vietnam: a cluster-randomized controlled feasibility trial

    BACKGROUND: Vietnam is experiencing an epidemiologic transition with an increased prevalence of non-communicable diseases. Novel, large-scale, effective, and sustainable interventions to control hypertension in Vietnam are needed. We report the results of a cluster-randomized feasibility trial at 3 months follow-up conducted in Hung Yen province, Vietnam, designed to evaluate the feasibility and acceptability of two community-based interventions to improve hypertension control: a storytelling intervention, We Talk about Our Hypertension, and a didactic intervention. METHODS: The storytelling intervention included stories about strategies for coping with hypertension, with patients speaking in their own words, and didactic content about the importance of healthy lifestyle behaviors including salt reduction and exercise. The didactic intervention included only didactic content. The storytelling intervention was delivered by two DVDs at 3-month intervals; the didactic intervention included only one installment. The trial was conducted in four communes, equally randomized to the two interventions. RESULTS: The mean age of the 160 study patients was 66 years, and 54% were men. Most participants described both interventions as understandable, informative, and motivational. Between baseline and 3 months, mean systolic blood pressure declined by 8.2 mmHg (95% CI 4.1-12.2) in the storytelling group and by 5.5 mmHg (95% CI 1.4-9.5) in the didactic group. The storytelling group also reported a significant increase in hypertension medication adherence. CONCLUSIONS: Both interventions were well accepted in several rural communities and were shown to be potentially effective in lowering blood pressure. A large-scale randomized trial is needed to compare the effectiveness of the two interventions in controlling hypertension. TRIAL REGISTRATION: ClinicalTrials.gov, NCT02483780

    On Random Subspace Optimization-Based Hybrid Computing Models Predicting the California Bearing Ratio of Soils

    The California Bearing Ratio (CBR) is an important index for evaluating the bearing capacity of pavement subgrade materials. In this research, random subspace optimization-based hybrid computing models were trained and developed for the prediction of the CBR of soil. Three models were developed, namely reduced error pruning trees (REPTs), random subsurface-based REPT (RSS-REPT), and RSS-based extra tree (RSS-ET). An experimental database was compiled from a total of 214 soil samples, which were classified according to AASHTO M 145, and included 26 samples of A-2-6 (clayey gravel and sand soil), 3 samples of A-4 (silty soil), 89 samples of A-6 (clayey soil), and 96 samples of A-7-6 (clayey soil). All CBR tests were performed in soaked conditions. The input parameters of the models included the particle size distribution, gravel content (G), coarse sand content (CS), fine sand content (FS), silt clay content (SC), organic content (O), liquid limit (LL), plastic limit (PL), plasticity index (PI), optimum moisture content (OMC), and maximum dry density (MDD). The accuracy of the developed models was assessed using numerous performance indexes, such as the coefficient of determination, relative error, MAE, and RMSE. The results show that the highest prediction accuracy was obtained using the RSS-based extra tree optimization technique

    BackgroundUnderstanding the relationship between serotype epidemiology and antimicrobial susceptibility of Streptococcus pneumoniae is essential for the effective introduction of pneumococcal conjugate vaccines (PCVs) and control of antimicrobial-resistant pneumococci.MethodsWe conducted a community-based study in Nha Trang, central Vietnam, to clarify the serotype distribution and pattern of S. pneumoniae antimicrobial susceptibility in children under 5 years of age and to identify risk factors for carrying antimicrobial-resistant strains. Nasopharyngeal swabs collected from children with acute respiratory infections (ARIs) hospitalized between April 7, 2008, and March 30, 2009, and from healthy children randomly selected in July 2008 were subjected to bacterial culture. Minimum inhibitory concentrations (MICs) against S. pneumoniae were determined, and multiplex-polymerase chain reaction (PCR) serotyping assays were performed. Logistic regression was applied to identify risk factors.ResultsWe collected 883 samples from 331 healthy children and 552 ARI cases; S. pneumoniae was isolated from 95 (28.7%) healthy children and 202 (36.6%) ARI cases. Age and daycare attendance were significantly associated with pneumococcal carriage. In total, 18.0, 25.8 and 75.6% of the isolates had high MICs for penicillin (≥4 μg/ml), cefotaxime (≥2 μg/ml) and meropenem (≥0.5 μg/ml), respectively. The presence of pneumococci non-susceptible to multiple beta-lactams was significantly associated with serotype 19F (Odds Ratio: 4.23) and daycare attendance (Odds Ratio: 2.56) but not ARIs, age or prior antimicrobial use. The majority of isolates non-susceptible to multiple beta-lactams (90%) were PCV13 vaccine serotypes.ConclusionsS. pneumoniae serotype 19F isolates non-susceptible to multiple beta-lactams are widely prevalent among Vietnamese children. Vaccine introduction is expected to significantly increase drug susceptibility
