Comparing the Determinants of Mode Choice across Travel Purposes
This paper considers travel mode choice for a range of journey purposes in Ireland using micro-data for 2009. Results suggest that demographic and socio-economic variables, location and public transport availability are important determinants of mode choice. The results also indicate an attachment to the car as a mode of transport for non-commuting journeys when available, and especially when used regularly for work journeys. Importantly, the determinants of mode choice are found to differ across journey purposes suggesting that it is not valid to generalise the results from studies considering only one journey purpose
Anemia de células falciformes en pediatría: Revisión de la literatura
Introduction: Sickle cell anemia is a structural hemoglobinopathy of genetic origin, characterized by the presence of sickle hemoglobin. Abnormal hemoglobin is unstable and tends to polymerize and can occlude the microcirculation. Also, it produces both acute and chronic multi system manifestations associated with an increased risk of infection. Objective: To describe the information of the exposed topic highlighting the most relevant aspects such as diagnosis and treatment. Methodology: A literature review with electronic search was carried out in the following databases: PubMed, MEDLINE, Medscape, Scopus; different types of articles were included that addressed sickle cell anemia in pediatrics such as original articles, reviews of topics and management. Results: A review of 22 articles was obtained which describes the subject of sickle cell anemia, guidelines and treatments based on the management and control of symptoms. It is evident that the use of transcranial Doppler ultrasound and transfusions proved to be preventive strategies or effective treatments for the complications related to this pathology in children. Conclusions: In Colombia, the prevalence of the pathology is not established yet. On the other hand, the main manifestations are related to complications of vaso-occlusion in different organs and functional asplenia which predisposes to infectious conditions. La hemoglobina anormal es inestable, tiende a polimerizarse y puede ocluir la microcirculación, produciendo manifestaciones multisistémicas tanto agudas como crónicas relacionándose con mayor riesgo de contraer infecciones. Objetivo: Describir la información del tema expuesto resaltando los aspectos más relevantes como diagnóstico y tratamiento. Metodología: Se efectuó una revisión bibliográfica con búsqueda electrónica en las siguientes bases de datos: PubMed, MEDLINE, Medscape, Scopus; y se incluyó diferentes tipos de artículo (artículos originales, revisiones de temas y guías de manejo) que abordaran la anemia de células falciformes en pediatría. Resultados: Se obtuvo una revisión de 22 artículos, donde se describe el tema de anemia de células falciformes, pautas y tratamientos basados en el manejo y control de los síntomas; se evidencia que el uso de ecografía doppler transcraneal y las transfusiones demostraron ser estrategias preventivas o de tratamiento eficaces para las complicaciones relacionadas con esta patología en los niños. Conclusiones: En Colombia, la prevalencia de la patología no se encuentra establecida. Por otra parte, las principales manifestaciones se relacionan con complicaciones de vaso-oclusión en los diferentes órganos y la asplenia funcional, la cual predispone a cuadros infecciosos. Upper Respiratory Tract Co-detection of Human Endemic Coronaviruses and High-density Pneumococcus Associated With Increased Severity Among HIV-Uninfected Children Under 5 Years Old in the PERCH Study.
BACKGROUND: Severity of viral respiratory illnesses can be increased with bacterial coinfection and can vary by sex, but influence of coinfection and sex on human endemic coronavirus (CoV) species, which generally cause mild to moderate respiratory illness, is unknown. We evaluated CoV and pneumococcal co-detection by sex in childhood pneumonia. METHODS: In the 2011-2014 Pneumonia Etiology Research for Child Health study, nasopharyngeal and oropharyngeal (NP/OP) swabs and other samples were collected from 3981 children <5 years hospitalized with severe or very severe pneumonia in 7 countries. Severity by NP/OP detection status of CoV (NL63, 229E, OC43 or HKU1) and high-density (≥6.9 log10 copies/mL) pneumococcus (HDSpn) by real-time polymerase chain reaction was assessed by sex using logistic regression adjusted for age and site. RESULTS: There were 43 (1.1%) CoV+/HDSpn+, 247 CoV+/HDSpn-, 449 CoV-/HDSpn+ and 3149 CoV-/HDSpn- cases with no significant difference in co-detection frequency by sex (range 51.2%-64.0% male, P = 0.06). More CoV+/HDSpn+ pneumonia was very severe compared with other groups for both males (13/22, 59.1% versus range 29.1%-34.7%, P = 0.04) and females (10/21, 47.6% versus 32.5%-43.5%, P = 0.009), but only male CoV+/HDSpn+ required supplemental oxygen more frequently (45.0% versus 20.6%-28.6%, P < 0.001) and had higher mortality (35.0% versus 5.3%-7.1%, P = 0.004) than other groups. For females with CoV+/HDSpn+, supplemental oxygen was 25.0% versus 24.8%-33.3% (P = 0.58) and mortality was 10.0% versus 9.2%-12.9% (P = 0.69). CONCLUSIONS: Co-detection of endemic CoV and HDSpn was rare in children hospitalized with pneumonia, but associated with higher severity and mortality in males. Findings may warrant investigation of differences in severity by sex with co-detection of HDSpn and SARS-CoV-2

Aluminum toxicity in childhood
Aluminum intoxication is an iatrogenic disease caused by the use of aluminum compounds for phosphate binding and by the contamination of parenteral fluids. Although organ aluminum deposition was noted as early as 1880 and toxicity was documented in the 1960s, the inability to accurately measure serum and tissue aluminum prevented delineation of its toxic effects until the 1970s.
