    Low molecular weight Adiponectin increases the mortality risk in very old patients

    Despite its beneficial role on insulin resistance and atherosclerosis, adiponectin has been frequently reported as an independent positive predictor of cardiovascular mortality. Very few information is available regarding adiponectin isoforms and mortality, in particular in advanced aging. Baseline serum levels of Total Adiponectin and its circulating isoforms (HMW-, MMW-, LMW-Adiponectin) were measured in 97 old patients (mean age: 79 years). Patients were followed up for all-cause mortality (study end-point) for an average of 76.4 Ā±37.3 months. A positive association was observed for LMW-Ad and all-cause mortality (HR: 1.13, 95% CI: 1.05-1,22, p: 0.002). After multivariate adjustment for age, sex and a previous history of myocardial infarction, higher levels of LMW-Ad were significantly associated with all-cause mortality (HR: 1.11, 95% CI: 1.02-1.21; p: 0.017). Interestingly neither total adiponectin neither the other two circulating isoforms (MMW- and HMW-Ad) showed any significant association with the study end-point. Our data suggest that the association between high serum adiponectin levels and increased mortality rate in elderly is contingent to an unbalanced circulating levels of adiponectin isoforms. The present results support the hypothesis that high levels of Low Molecular Weight adiponectin are a biomarker for mortality risk in very old patients

    Modulated linear dynamics of nanobeams accounting for higher gradient effects

    We present some numerical results for the linear dynamics of nanobeams modulated by an axial force, basing on a recent proposal of literature that encompasses both the standard nonlocal elasticity, according to Eringen, and second-order strain elasticity. Three different possibilities for the elastic potential energy provide different responses that highlight the contributions of nonlocality and strain gradient, plus their combination. An axial force affects the linear stationary dynamics of such nanobeams, inducing suitable variation of the natural angular frequencies for benchmark cases, until static buckling occurs when the natural angular frequency vanishes. Effects of the various elastic potentials on this modulation are investigated and thoroughly commented

    Effect of plant foods and beverages on plasma non-enzymatic antioxidant capacity in human subjects: a meta-analysis

    Non-enzymatic antioxidant capacity (NEAC) represents a sensitive biomarker measuring the in vivo antioxidant potential of vegetable foods. To evaluate the effectiveness of plant-derived foods and beverages on the plasma non-enzymatic antioxidant system, we analysed all literature published upto May 2010. Data were extracted by two authors independently, and the effect size was summarised using standardised mean differences by a random-effects model. For the analysis, eighty-eight studies were included, reporting a total number of 122 interventions and involving 2890 subjects. There was overall evidence of the effectiveness of fruit, vegetables, dietary patterns based on plant foods, red wine and tea in increasing plasma NEAC. No changes were found for chocolate and fruit juices. We observed an overall effect size three times higher in subjects with risk factors when compared with healthy subjects. Total radical-trapping antioxidant parameter, oxygen radical absorbance capacity and ferric-reducing antioxidant power methods showed a similar increase in plasma NEAC following dietary supplementation, whereas Trolox equivalent antioxidant capacity did not respond to dietary supplementation. Data from the present meta-analysis show that plant-derived foods represent an effective strategy to enhance an endogenous antioxidant network in humans. This is particularly evident in the presence of oxidative stress-related risk factors

    Multi-Year Concentrations, Health Risk, and Source Identification, of Air Toxics in the Venice Lagoon

    This work presents and discusses the results of multi-year measurements of air toxics concentrations from different sites of Venice Lagoon. The aim of the study is the characterization of the air quality of the area, in terms of PM10, PM10-bound metals, and polycyclic aromatic hydrocarbons (PAHs) concentrations, even with the individuation of the related inhalation risk and the identification of the main contaminants' sources. The study moreover provides an important multi-years trend of chemical characterization of the Venice Lagoon. Sampling for PM10 and its metal content was carried out between 2010 and 2015, using low volume sequential aerosol samplers; V, Cr, Fe, Co, Ni, Cu, Zn, As, Mo, Cd, Sb, Tl, and Pb were measured by ICP-MS. Samples for PAHs concentration in gas and particle phases were collected from 2010 to 2014, using a high-volume air sampler that allows to sample both phases simultaneously. Samples for PAHs determinations were analyzed by GC-HRMS. Concentrations of air toxics have been studied with the use of enrichment factor, the coefficient of divergence, and the Mann-Kendal test, to individuate eventual local divergences, seasonal and other temporal trends. The inhalation risk assessment has been achieved by the calculation of the cumulative incremental carcinogenic risk for all the air toxics analyzed. The sources' identification and impact evaluation have been carried out using the atmospheric vanadium concentration, to calculate the primary contribution of ship traffic to PM10, the Positive Matrix Factorization, and the Diagnostic Ratios, with the aim of evaluating the impact of construction activities of M.o.S.E, (Modulo Sperimentale Elettromeccanico-Electromechanical Experimental Module) as well as of ship traffic and other possible air toxics' sources. Temporal trends indicate a slight decrease of PAHs and PM10 and of its content of Pb, Zn, Sb. Sharp increases of As and Cd during 2014 and 2015 may be due to local sources like emissions from the construction yard, ship, and road traffic. The cumulative incremental carcinogenic risk was below the unconditionally acceptable risk; Co and As are the most important contributors among metals, followed by Cd and Ni while the PAH congeners that most contribute to the carcinogenic risk were benzo(a)pyrene and dibenzo(A,H)anthracene

    Valorisation of history and landscape for promoting the memory of WWI

    10 p.In recent years many activities were conducted to commemorate the 100th Anniversary of the FirstWorld War (WWI) outbreak. Among these, the valorisation of history and landscape (VAST) project(http://vast.fbk.eu) was part of the initiatives promoted by the Autonomous Province of Trento (Italy)as a tribute to WWI events in the region. The project was primarily aimed to document and promote,through 3D digitization approaches, ICT technologies and communication material, the memory of sites,theatre of the world conflict. The Trentoā€™s area was under the Austro-Hungarian Empire until the end ofWWI and on the border with the Italian Kingdom. The area represented a crucial and bloody war frontbetween the Austrian and Italian territories. It was thus constellated of military fortresses, trenches andtunnels, most of them now ruined and at risk to slowly disappear. 3D surveying and modelling techniqueswere exploited to produce 3D digital models of structures and objects, along with virtual tours, dissem-ination material and a WebGIS of the area.     TRPV1 channels are critical brain inflammation detectors and neuropathic pain biomarkers in mice

    The capsaicin receptor TRPV1 has been widely characterized in the sensory system as a key component of pain and inflammation. A large amount of evidence shows that TRPV1 is also functional in the brain although its role is still debated. Here we report that TRPV1 is highly expressed in microglial cells rather than neurons of the anterior cingulate cortex and other brain areas. We found that stimulation of microglial TRPV1 controls cortical microglia activation per se and indirectly enhances glutamatergic transmission in neurons by promoting extracellular microglial microvesicles shedding. Conversely, in the cortex of mice suffering from neuropathic pain, TRPV1 is also present in neurons affecting their intrinsic electrical properties and synaptic strength. Altogether, these findings identify brain TRPV1 as potential detector of harmful stimuli and a key player of microglia to neuron communication

    Weather-Related Flood and Landslide Damage: A Risk Index for Italian Regions

    The frequency of natural hazards has been increasing in the last decades in Europe and specifically in Mediterranean regions due to climate change. For example heavy precipitation events can lead to disasters through the interaction with exposed and vulnerable people and natural systems. It is therefore necessary a prevention planning to preserve human health and to reduce economic losses. Prevention should mainly be carried out with more adequate land management, also supported by the development of an appropriate risk prediction tool based on weather forecasts. The main aim of this study is to investigate the relationship between weather types (WTs) and the frequency of floods and landslides that have caused damage to properties, personal injuries, or deaths in the Italian regions over recent decades. In particular, a specific risk index (WT-FLARI) for each WT was developed at national and regional scale. This study has identified a specific risk index associated with each weather type, calibrated for each Italian region and applicable to both annual and seasonal levels. The risk index represents the seasonal and annual vulnerability of each Italian region and indicates that additional preventive actions are necessary for some regions. The results of this study represent a good starting point towards the development of a tool to support policy-makers, local authorities and health agencies in planning actions, mainly in the medium to long term, aimed at the weather damage reduction that represents an important issue of the World Meteorological Organization mission

    Gefitinib in Non Small Cell Lung Cancer

    Gefitinib is an oral, reversible, tyrosine kinase inhibitor of epidermal growth factor receptor (EGFR) that plays a key role in the biology of non small cell lung cancer (NSCLC). Phase I studies indicated that the recommended dose of gefitinib was 250ā€‰mg/day. Rash, diarrhea, and nausea were the most common adverse events. The positive results obtained in early phase 2 clinical trials with gefitinib were not confirmed in large phase 3 trials in unselected patients with advanced NSCLC. The subsequent discovery that the presence of somatic mutations in the kinase domain of EGFR strongly correlates with increased responsiveness to EGFR tyrosine kinase inhibitors prompted phase 2 and 3 trials with gefitinib in the first line-treatment of EGFR-mutated NSCLC. The results of these trials have demonstrated the efficacy of gefitinib that can be now considered as the standard first-line treatment of patients with advanced NSCLC harbouring activating EGFR mutations

    The association of financial difficulties with clinical outcomes in cancer patients: secondary analysis of 16 academic prospective clinical trials conducted in Italy

    Background: Cancer may cause financial difficulties, but its impact in countries with public health systems is unknown. We evaluated the association of financial difficulties with clinical outcomes of cancer patients enrolled in academic clinical trials performed within the Italian public health system. Patients and methods: Data were pooled from 16 prospective multicentre trials in lung, breast or ovarian cancer, using the EORTC quality of life (QOL) C30 questionnaire. Question 28 scores financial difficulties related to disease or treatment in four categories from 'not at all' to 'very much'. We defined financial burden (FB) as any financial difficulty reported at baseline questionnaire, and financial toxicity (FT) as score worsening in a subsequent questionnaire. We investigated (i) the association of FB with clinical outcomes (survival, global QOL response [questions 29/30] and severe toxicity), and (ii) the association of FT with survival. Multivariable analyses were performed using logistic regression models or the Cox model adjusting for trial, gender, age, region and period of enrolment, baseline global QOL and, where appropriate, FB and global QOL response. Results are reported as odds ratio (OR) or hazard ratio (HR) with 95% confidence intervals (CI). Results: At baseline 26% of the 3670 study patients reported FB, significantly correlated with worse baseline global QOL. FB was not associated with risks of death (HR 0.94, 95% CI 0.85-1.04, P = 0.23) and severe toxicity (OR 0.90, 95% CI 0.76-1.06, P = 0.19) but was predictive of a higher chance of worse global QOL response (OR 1.35, 95% CI 1.08-1.70, P = 0.009). During treatment, 2735 (74.5%) patients filled in subsequent questionnaires and 616 (22.5%) developed FT that was significantly associated with an increased risk of death (HR 1.20, 95% CI 1.05-1.37, P = 0.007). Several sensitivity analyses confirmed these findings. Conclusion: Even in a public health system, financial difficulties are associated with relevant cancer patients outcomes like QOL and survival
