    SERUM IONIZED MAGNESIUM IN DIABETIC OLDER PERSONS

    Objective. Several alterations of magnesium metabolism have been associated with type 2 diabetes pathophysiology, a condition particularly frequent in older persons. We aimed to evaluate serum total (Mg-tot) and serum ionized magnesium (Mg-ion) in older persons with type 2 diabetes in order to explore clinically applicable methods for the detection of magnesium deficit. Material/Methods: Mg-tot and Mg-ion were measured in 105 fasting subjects with type 2 diabetes (mean age: 71.10.8 years; M/F: 45/60) and in 100 age-matched non-diabetic control persons (mean age: 72.20.8 years; M/F: 42/58). Results: Mg-ion concentrations were significantly lower in diabetic persons compared with controls (0.490.05 mmol/L vs. 0.550.05 mmol/L; p<0.001). Mg-tot was also slightly but significantly lower in diabetic patients (0.820.007 mmol/L vs. 0.840.006 mmol/L; p<0.05). There was an almost complete overlap in the values of Mg-tot in older diabetic patients and controls; conversely, 44.8% of diabetic patients had Mg-ion values below 0.47 mmol/L, while none of the controls did. After adjustment for age, sex, BMI, and triglycerides, Mg-tot was significantly associated with FBG in all the participants (p<0.05) and Mg-ion was significantly associated with FBG in all the participants (p<0.01) and with HbA1c in diabetic participants (p<0.001). Conclusions: Alterations of magnesium serum concentrations are common in type 2 diabetic older adults; Mg-ion evaluation may help to identify subclinical magnesium depletion (i.e. in patients with normal Mg-tot); the close independent associations of Mg-tot and Mg-ion with FBG and with HbA1c reinforce the possible link between magnesium homeostasis and altered glucose metabolism

    The Interplay between Magnesium and Testosterone in Modulating Physical Function in Men

    The role of nutritional status as determinant of successful aging is very well recognized. There is recent evidence that nutrition may exert its beneficial effects through the modulation of the hormonal anabolic milieu. Under-nutrition and anabolic hormonal deficiency frequently coexist in older individuals determining an increased risk of mobility impairment and adverse outcomes. Mineral dietary assessment has received attention as key component of the nutritional modulation of anabolic status and physical performance. There is evidence that several minerals, including magnesium, exert a positive influence on Insulin-like Growth Factor-1 (IGF-1) secretion in both sexes, and Testosterone (T) in men. In this review we summarize the existing knowledge about the mechanisms by which magnesium can affect T bioactivity in older men. Particular attention will be also devoted to the preliminary observational and intervention studies addressing the relationship between magnesium and T in adult and older individuals. We believe that, if larger studies will confirm these pivotal data, hormonal and mineral strategies might be adopted as synergistic treatment to approach the multifactorial nature of accelerated aging

    Ancestry of the Brazilian TP53 c.1010G>A (p.Arg337His, R337H) founder mutation : clues from haplotyping of short tandem repeats on Chromosome 17p

    Rare germline mutations in TP53 (17p13.1) cause a highly penetrant predisposition to a specific spectrum of early cancers, defining the Li-Fraumeni Syndrome (LFS). A germline mutation at codon 337 (p.Arg337His, c1010G>A) is found in about 0.3% of the population of Southern Brazil. This mutation is associated with partially penetrant LFS traits and is found in the germline of patients with early cancers of the LFS spectrum unselected for familial his- tory. To characterize the extended haplotypes carrying the mutation, we have genotyped 9 short tandem repeats on chromosome 17p in 12 trios of Brazilian p.Arg337His carriers. Results confirm that all share a common ancestor haplotype of Caucasian/Portuguese-Ibe- ric origin, distant in about 72–84 generations (2000 years assuming a 25 years intergenera- tional distance) and thus pre-dating European migration to Brazil. So far, the founder p. Arg337His haplotype has not been detected outside Brazil, with the exception of two resi- dents of Portugal, one of them of Brazilian origin. On the other hand, increased meiotic recombination in p.Arg337His carriers may account for higher than expected haplotype diversity.     Identification and Characterization of a New Orthoreovirus from Patients with Acute Respiratory Infections

    First discovered in the early 1950s, reoviruses (respiratory enteric orphan viruses) were not associated with any known disease, and hence named orphan viruses. Recently, our group reported the isolation of the Melaka virus from a patient with acute respiratory disease and provided data suggesting that this new orthoreovirus is capable of human-to-human transmission and is probably of bat origin. Here we report yet another Melaka-like reovirus (named Kampar virus) isolated from the throat swab of a 54 year old male patient in Kampar, Perak, Malaysia who was suffering from high fever, acute respiratory disease and vomiting at the time of virus isolation. Serological studies indicated that Kampar virus was transmitted from the index case to at least one other individual and caused respiratory disease in the contact case. Sequence analysis of the four small class genome segments indicated that Kampar and Melaka viruses are closely related. This was confirmed by virus neutralization assay, showing an effective two-way cross neutralization, i.e., the serum against one virus was able to neutralize the other. Although the exact origin of Kampar virus is unknown, epidemiological tracing revealed that the house of the index case is surrounded by fruit trees frequently visited by fruit bats. There is a high probability that Kampar virus originated from bats and was transmitted to humans via bat droppings or contaminated fruits. The discovery of Kampar virus highlights the increasing trend of emergence of bat zoonotic viruses and the need to expand our understanding of bats as a source of many unknown viruses

    Phytostabilization of mine tailings using compost-assisted direct planting: Translating greenhouse results to the field

    Standard practice in reclamation of mine tailings is the emplacement of a 15 to 90 cm soil/gravel/rock cap which is then hydro-seeded. In this study we investigate compost-assisted direct planting phytostabilization technology as an alternative to standard cap and plant practices. In phytostabilization the goal is to establish a vegetative cap using native plants that stabilize metals in the root zone with little to no shoot accumulation. The study site is a barren 62-hectare tailings pile characterized by extremely acidic pH as well as lead, arsenic, and zinc each exceeding 2000 mg kg(-1). The study objective is to evaluate whether successful greenhouse phytostabilization results are scalable to the field. In May 2010, a 0.27 ha study area was established on the Iron King Mine and Humboldt Smelter Superfund (IKMHSS) site with six irrigated treatments; tailings amended with 10, 15, or 20% (w/w) compost seeded with amix of native plants (buffalo grass, arizona fescue, quailbush, mountain mahogany, mesquite, and catclaw acacia) and controls including composted (15 and 20%) unseeded treatments and an uncomposted unseeded treatment. Canopy cover ranging from 21 to 61% developed after 41 months in the compost-amended planted treatments, a canopy cover similar to that found in the surrounding region. No plants grew on unamended tailings. Neutrophilic heterotrophic bacterial counts were 1.5 to 4 orders of magnitude higher after 41 months in planted versus unamended control plots. Shoot tissue accumulation of various metal(loids) was at or below Domestic Animal Toxicity Limits, with some plant specific exceptions in treatments receiving less compost. Parameters including % canopy cover, neutrophilic heterotrophic bacteria counts, and shoot uptake of metal(loids) are promising criteria to use in evaluating reclamation success. In summary, compost amendment and seeding, guided by preliminary greenhouse studies, allowed plant establishment and sustained growth over 4 years demonstrating feasibility for this phytostabilization technology.     Construindo a participação social no SUS: um constante repensar em busca de equidade e transformação

    Trata-se de um ensaio que expõe reflexões sobre a busca e o exercício da participação social no Sistema Único de Saúde (SUS), compreendendo seu conceito como as diferentes ações dos grupos sociais que influenciam a formulação, execução, fiscalização e avaliação das políticas públicas. Por isso o fato de a comunidade participar no sistema de saúde é visto como condição fundamental para o exercício pleno da saúde, capaz de promover equidade e de transformar a atenção à saúde. As ações de construir a cidadania e formar sujeitos coletivos são tomadas como fundamentos para a conquista de espaços democráticos e de direitos sociais. Mapeou-se a normatização atual referente à participação popular, considerando seus avanços, mas explicitando também suas fragilidades, principalmente no que tange aos conselhos e conferências de saúde. No ambiente dos espaços institucionais de participação, foram destacados elementos necessários para consolidar e efetivar a participação popular, como a relação entre os atores e a comunicação para a socializar as informações e conhecimentos em saúde. Busca-se também problematizar o assunto, trazendo à tona alguns dos obstáculos e críticas relativos à participação social. Pretende-se, com este ensaio, colaborar para a democratização dos diferentes espaços de escuta da sociedade civil, tornando-os locais cada vez mais apropriados para as tomadas de decisão sobre a construção de políticas e serviços de saúde, dos quais a população se apropria coletivamente, superando a simples obrigatoriedade burocrática da participação e contribuindo para a saúde e qualidade de vida.It concerns an essay that presents reflections on the search and the practice of the social participation in the Brazilian Unified Health System (SUS), including the understanding of its concept as to how the different actions of the social groups influence the formularization, execution, fiscalization and evaluation of the public policies. Therefore the participation of the community in the health system is considered an essential condition for the full exercise of the health, capable of promoting fairness and to transforming the health attention. The building of citizenship and the formation of collective citizens are taken as beddings for the conquest of democratic spaces and social rights. The current normalization of the public participation has been mapped considering its advances, but also stating its fragilities, mainly those that concerning the participation out of health councils and conferences. In the institutional environment, some elements where pointed as necessary to consolidate and effect the communitarian participation, like the relation between the actors and the communication for the socialization of health information and knowledge. Questions of the subject are also sought by bringing to attention some of the critical obstacles in relation to the social participation. This essay is intended to collaborate in behalf of the real democratization of the different spaces where the civil society can be heard, transforming these places in more appropriate locations to make decisions concerning the construction of health policies and services. The population can benefit from these spaces collectively surpassing the simple bureaucratic obligatoriness of the participation and contributing for the health and quality of life

    Brazilian Consensus on Photoprotection

    Brazil is a country of continental dimensions with a large heterogeneity of climates and massive mixing of the population. Almost the entire national territory is located between the Equator and the Tropic of Capricorn, and the Earth axial tilt to the south certainly makes Brazil one of the countries of the world with greater extent of land in proximity to the sun. The Brazilian coastline, where most of its population lives, is more than 8,500 km long. Due to geographic characteristics and cultural trends, Brazilians are among the peoples with the highest annual exposure to the sun. Epidemiological data show a continuing increase in the incidence of nonmelanoma and melanoma skin cancers. Photoprotection can be understood as a set of measures aimed at reducing sun exposure and at preventing the development of acute and chronic actinic damage. Due to the peculiarities of Brazilian territory and culture, it would not be advisable to replicate the concepts of photoprotection from other developed countries, places with completely different climates and populations. Thus the Brazilian Society of Dermatology has developed the Brazilian Consensus on Photoprotection, the first official document on photoprotection developed in Brazil for Brazilians, with recommendations on matters involving photoprotection
