    Increased cerebral blood flow velocities assessed by transcranial Doppler examination is associated with complement activation after cardiopulmonary bypass

    The role of complement activation on the cerebral vasculature after cardiopulmonary bypass (CPB) is unclear. The goal of the study was to assess whether heparin-coated CPB reduces complement activation, and influences cerebral blood flow velocities (CBFV). Twenty-four patients undergoing coronary surgery were randomly allocated to non-coated (NC-group) or heparin-coated (HC-group) CPB. Complement activation was assessed by measuring sC5b-9. Transcranial Doppler (TCD) was performed on middle cerebral arteries before and after CPB. Systolic (SV), diastolic (DV) and mean (MV) CBFV were measured. Significant increase of sC5b-9 (p=0.003) was observed in the NC-group and CBFV increased after CPB (SV by 27%, p=0.05; DV by 40%, p=0.06; MV by 33%, p=0.04) whereas no changes were detected in the HC-group. TCD values were higher in the NC-group than in the HC-group (SV, p=0.04; DV, p=0.03; MV, p=0.03) although cardiac index, systemic vascular resistance, haematocrit and pCO2 were similar. Postoperative SV, DV and MV were significantly correlated with sC5b-9 (r=0.583, p=0.009; r=0.581, p=0.009; r=0.598, p=0.007, respectively). Increased CBFV after CPB are correlated to the level of complement activation and may be controlled by heparin-coated circuits

    The Genetic Basis of Heterosis: Multiparental Quantitative Trait Loci Mapping Reveals Contrasted Levels of Apparent Overdominance Among Traits of Agronomical Interest in Maize (Zea mays L.)

    Understanding the genetic bases underlying heterosis is a major issue in maize (Zea mays L.). We extended the North Carolina design III (NCIII) by using three populations of recombinant inbred lines derived from three parental lines belonging to different heterotic pools, crossed with each parental line to obtain nine families of hybrids. A total of 1253 hybrids were evaluated for grain moisture, silking date, plant height, and grain yield. Quantitative trait loci (QTL) mapping was carried out on the six families obtained from crosses to parental lines following the “classical” NCIII method and with a multiparental connected model on the global design, adding the three families obtained from crosses to the nonparental line. Results of the QTL detection highlighted that most of the QTL detected for grain yield displayed apparent overdominance effects and limited differences between heterozygous genotypes, whereas for grain moisture predominance of additive effects was observed. For plant height and silking date results were intermediate. Except for grain yield, most of the QTL identified showed significant additive-by-additive epistatic interactions. High correlation observed between heterosis and the heterozygosity of hybrids at markers confirms the complex genetic basis and the role of dominance in heterosis. An important proportion of QTL detected were located close to the centromeres. We hypothesized that the lower recombination in these regions favors the detection of (i) linked QTL in repulsion phase, leading to apparent overdominance for heterotic traits and (ii) linked QTL in coupling phase, reinforcing apparent additive effects of linked QTL for the other traits

    Introduction Guidelines for the treatment of steroid-dependent nephrotic syndrome (SDNS) and frequently relapsing nephrotic syndrome (FRNS) are lacking. Given the substantial impact of SDNS/FRNS on quality of life, strategies aiming to provide long-term remission while minimising treatment side effects are needed. Several studies confirm that rituximab is effective in preventing early relapses in SDNS/FRNS; however, the long-term relapse rate remains high (~70% at 2 years). This trial will assess the association of intravenous immunoglobulins (IVIgs) to rituximab in patients with SDNS/FRNS and inform clinicians on whether IVIg’s immunomodulatory properties can alter the course of the disease and reduce the use of immunosuppressive drugs and their side effects. Methods and analysis We conduct an open-label multicentre, randomised, parallel group in a 1:1 ratio, controlled, superiority trial to assess the safety and efficacy of a single infusion of rituximab followed by IVIg compared with rituximab alone in childhood-onset FRNS/SDNS. The primary outcome is the occurrence of first relapse within 24 months. Patients are allocated to receive either rituximab alone (375 mg/m²) or rituximab followed by IVIg, which includes an initial Ig dose of 2 g/kg, followed by 1.5 g/kg injections once a month for the following 5 months (maximum dose: 100 g). Ethics and dissemination The study has been approved by the ethics committee (Comité de Protection des Personnes) of Ouest I and authorised by the French drug regulatory agency (Agence Nationale de Sécurité du Médicament et des Produits de Santé). Results of the primary study and the secondary aims will be disseminated through peer-reviewed publications

    Influence of inflammation on total energy expenditure in hemodialysis patients.

    Background and Objectives: Studies show that inflammation can contribute to an increase in resting energy expenditure in patients with chronic kidney disease; however, findings about total energy expenditure (TEE) have not been reported. The aim of this study was to evaluate the effects of inflammation on TEE and physical activity energy expenditure in hemodialysis (HD) patients.Design: This was a cross-sectional study.Setting: This study was conducted from Hopital Edouard Herriot, Lyon, France.Patients: This study included 24 HD patients and 18 healthy subjects.Main Outcome Measure: TEE and step counts were measured over a 7-day period by the SenseWear Pro2 Armband in 24 HD patients (15 patients with C-reactive protein,5 mg/L, aged 67.0 +/- 6 14.7 years, and 9 with C-reactive protein >5 mg/L, aged 69.0 +/- 6 18.0 years) and compared with 18 healthy subjects (62.3 +/- 6 15.3 years).Results: Mean estimated TEE measured with SenseWear Pro2 Armband was significantly lower (25.5 +/- 4.1 kcal/kg/day) in patients with inflammation when compared with those without inflammation (32.0 +/- 6.7 kcal/kg/day) and with healthy subjects (31.8 +/- 6 7.0 kcal/kg/day) (P = .012). There was a difference in the physical activity (step counts) between patient groups (P < .05). Healthy subjects and patients without inflammation walked more (8,107 +/- 5,419 and 6,016 +/- 3,752 steps/day, respectively) as compared with patients with inflammation (2,801 +/- 2,754 steps/day, P = .001).Conclusion: Our findings suggest that patients with inflammation have a lower TEE when compared with healthy subjects and patients without inflammation.     Acquisition d'images thermiques par drone : corrections radiométriques à partir de données terrain

    National audienceThermal images have many applications in agronomy, including crop water stress status assessment. Nowadays, the miniaturization of thermal cameras allows installing them onboard the Unmanned Aerial Vehicles (UAV), but this miniaturization leads to some difficulties: the miniaturized thermal cameras have no temperature control system of their sensor. The instability of the miniaturized camera makes a high drift in the acquisition of temperature data so that acquired thermal images don't fit the real temperature of the studying object, so data have to be continuously corrected. We need to have stable reference on field in order to compute the actual temperature value. In this article we present a method for radiometric correction of UA V remote sensed thermal images. We have implemented a device in order to retrieve ground temperature measurements. This device is composed with four targets (cold, hot, dry soil, wet soil) which measured continuously the target temperature thanks to IR120 (Campbell ®) radio-thermometer. A meteorological station is included in this ground system and acquires air temperature and moisture, solar radiation, wind speed and direction every 10 seconds. The images are radiometrically corrected by linear regression from on ground thermal data collected. Corrected images have been compared with mean canopy surface temperature of a sample of 10 trees measured with radio-thermometers. The results showed a good link between data from on ground radio-thermometer and data from thermal camera after radiometric correction. We can conclude that images obtained by this method are of sufficient quality to be used in vegetation water stress studies.Les images thermiques ont de nombreuses applications dans le domaine agronomique, notamment pour informer sur la réponse des plantes au stress hydrique. La miniaturisation des caméras thermiques permet aujourd'hui de les installer sur des drones. Cependant, les caméras thermiques miniaturisées embarquées à bord de drones n'ont pas de système de contrôle de la température des microbolomètres, ce qui conduit à la dérive continue des données, avec la difficulté de mesurer correctement la température absolue des objets étudiés sur le terrain. Nous présenterons ici une méthode de correction radiométrique des images thermiques acquises par drone afin de pallier cette difficulté. La proposition consiste à cibler périodiquement des références de terrain stables, mesurées indépendamment, pour apprécier la dérive et calculer la température absolue. Un dispositif d'acquisition de mesures au sol consistant en un système d'enregistrement en continu de la température de quatre cibles (froide, chaude, sol sec et sol humide) via des thermo-radiomètres IR 120 (Campbell®Scientific) a été installé les jours d'acquisition des images. Une station météorologique complète le dispositif et permet d'acquérir des données de température et d'humidité de l'air, de rayonnement solaire, et de vitesse du vent, toutes les 10 secondes. Les images acquises sont corrigées radiométriquement par régression linéaire à partir des données thermiques collectées sur la batterie de cibles. Les températures extraites des images corrigées ont été comparées aux températures moyennes de surface d'un échantillon de 1 O arbres mesurées par thermo-radiomètres. Les résultats montrent une bonne corrélation entre les données issues des thermo-radiomètres et celles issues de la caméra thermique et corrigées radiométriquement. Ce système offre une précision suffisante pour être utilisé dans des études sur le stress hydrique de la végétation

    Water stress assessment at tree scale: High-resolution thermal UAV imagery acquisition and processing

    International audienceWater stress assessment can be performed by analyzing thermal images taken onboard Unmanned Aerial Vehicles (UAVs). This study focuses on the acquisition and data extraction of high-resolution UAV-sensed thermal images. The datasets obtained, through computation of spectral indices and image classification, allowed to assess the response to drought of an apple hybrid population submitted to different water regimes. Studies were performed in an experimental apple orchard located in southern France. Flights were planned at solar noon on four successive dates during summer 2013 (40 m altitude, 0.10 m spatial resolution) and at five successive hours of the day, once water stress was established. The high temporal and spatial resolution of images allowed acquiring data at canopy and intra-canopy scales, with a short revisit time. As the miniaturized uncooled thermal camera carried onboard the UAV needs careful correction of radiometry, this was performed by continuous reference to ground thermal targets, while an automatized image processing was carried out. Thanks to the high resolution of the remote images obtained, and the capacity to efficiently delineate each individual tree within the whole trial, it was possible to analyze inter- and intra-canopy thermal variations. Indices extracted from thermal images showed significantly higher canopy temperatures in water restricted trees compared to well-irrigated ones. These differences were related to the severity of water deficit. However, responses also varied significantly according to the genotype. The image-based variables in apple trees constitute a basis for a further finely tuned analysis of the differential response to water stress

    New measurements of energy expenditure and physical activity in chronic kidney disease.

    The accurate estimation of total daily energy expenditure (TEE) in chronic kidney patients is essential to allow the provision of nutritional requirements; however, it remains a challenge to collect actual physical activity and resting energy expenditure in maintenance dialysis patients. The direct measurement of TEE by direct calorimetry or doubly labeled water cannot be used easily so that, in clinical practice, TEE is usually estimated from resting energy expenditure and physical activity. Prediction equations may also be used to estimate resting energy expenditure; however, their use has been poorly documented in dialysis patients. Recently, a new system called SenseWear Armband (BodyMedia, Pittsburgh, PA) was developed to assess TEE, but so far no data have been published in chronic kidney disease patients.     International audienceBackground: In patients with cast nephropathy and acute kidney injury (AKI) requiring dialysis, the reduction of serum free light chains (FLC) using chemotherapy and intensive hemodialysis (IHD) with a high cut-off filter may improve renal and patient outcomes. We evaluated the effectiveness of a combination of chemotherapy and IHD with an adsorbent polymethylmethacrylate membrane (IHD-PMMA) on renal recovery and survival. Methods: A single-center retrospective cohort-study was conducted. Between 2007 and 2014, patients with dialysis-dependent acute cast nephropathy treated with chemotherapy and IHD-PMMA were included. Patients had six 6-h hemodialysis sessions a week, until pre-dialysis serum FLC fell below 200 mg/L, for a maximum of 3 weeks. Primary outcomes were renal recovery, defined as dialysis independence, and survival. Results: Seventeen patients were included, all with stage 3 AKI. All received chemotherapy, mostly based on bortezomib and steroids (88%). Twelve patients (71%) achieved renal recovery, usually within 60 days (92%). At 3 months, the overall hematological response rate was 57%; hematological response was maintained for at least 2 years in 86% of responders. At 6, 12, and 24 months, 76, 75, and 62% of patients were alive, respectively. Higher reduction in involved FLC by day 12 (p = 0.022) and day 21 (p = 0.003) was associated with renal recovery. Patients with FLC reduction rate \textgreater 50% by day 21 experienced a lower mortality (hazard ratio 0.10, 95% CI 0.02-0.63). Conclusion: In patients with dialysis-dependent myeloma cast nephropathy, early FLC removal by IHD-PMMA combined with chemotherapy was associated with high rates of renal recovery and survival.
