    Gastric and Enteric Metastization of Invasive Lobular Breast Carcinoma

    O carcinoma lobular invasivo (CLI) representa 5-15% dos casos de cancro invasivo da mama, diferindo do carcinoma ductal invasivo(CDI), o tumor invasivo mais frequente da mama, quer na forma de apresentação clínica, quer nos aspectos imagiológicos e histológicos, assim como no padrão de metastização. O objectivo deste artigo passa pelo relato clínico de um padrão atípico de metastização de cancro da mama. Mulher de 80 anos, que apresentava queixas de dor abdominal, com 6 meses de evolução, de carácter generalizado, com maior intensidade nos quadrantes direitos, associada a episódios de sub-oclusão. O exame objectivo revelou volumosa hérnia incisional paramediana direita sem sinais de sofrimento e uma lesão ulcerada da mama esquerda nunca antes revelada pela doente. Realizou uma mamografia que mostrou uma lesão T4 e a biópsia da mesma revelou Carcinoma Ductal Invasivo. Por apresentar anemia microcítica hipocrómica de 7 g/dL realizou também uma Endoscopia Digestiva Alta que demonstrou uma úlcera da pequena curvatura gástrica cuja biópsia revelou um carcinoma difuso. Optou-se pela intervenção cirúrgica com ideação paliativa. Foi efectuada mastectomia simples esquerda e correcção de hérnia incisional com enterectomia segmentar. Os resultados histológicos das peças operatórias foram surpreendentes: Carcinoma lobular invasivo da mama e metástase do mesmo no segmento de intestino ressecado. Foram revistas as lâminas referentes à biópsia gástrica previamente realizada, tendo sido feito estudo imunohistoquímico que mostrou positividade para os receptores hormonais o que favorecia tratar-se de metástase de carcinoma lobular da mama no estômago. Como conclusão temos a referir que o CLI apresenta um padrão distinto de metastização em relação ao CDI, com diferentes órgãos-alvo, realçando o papel fundamental da suspeição clínica e de uma histologia exigente para o seu diagnóstico

    INTRODUCTION: Olfaction is frequently affected in chronic rhino-sinusitis with polyposis and has been recognised to have important impact on quality of life. Surgical resolution on cases of maximal medical therapy failure is an option to relieve symptoms, with debates as to how extensive surgery should be. A more radical approach will achieve better disease control with less relapse, but can also compromise olfaction. This decision about a more radical surgical approach should be shared with the patient. Thorough informed consent regarding disease control and hyposmia should be taken. MATERIAL AND METHODS: Literature review and consultation with a board of experts. RESULTS AND DISCUSSION: We propose some elements to be included in the informed consent discussion, in order to broadly address the surgical limitations regarding anosmia as a frequent complaint, as well as the different options and their associated consequences. CONCLUSION: Radical surgery decision making should be shared with the patient and the informed consent should be as thorough as possible regarding disease control and hyposmia resolution

    Mapping mania symptoms based on focal brain damage

    BACKGROUND. Although mania is characteristic of bipolar disorder, it can also occur following focal brain damage. Such cases may provide unique insight into brain regions responsible for mania symptoms and identify therapeutic targets. METHODS. Lesion locations associated with mania were identified using a systematic literature search (n = 41) and mapped onto a common brain atlas. The network of brain regions functionally connected to each lesion location was computed using normative human connectome data (resting-state functional MRI, n = 1000) and contrasted with those obtained from lesion locations not associated with mania (n = 79). Reproducibility was assessed using independent cohorts of mania lesions derived from clinical chart review (n = 15) and of control lesions (n = 490). Results were compared with brain stimulation sites previously reported to induce or relieve mania symptoms. RESULTS. Lesion locations associated with mania were heterogeneous and no single brain region was lesioned in all, or even most, cases. However, these lesion locations showed a unique pattern of functional connectivity to the right orbitofrontal cortex, right inferior temporal gyrus, and right frontal pole. This connectivity profile was reproducible across independent lesion cohorts and aligned with the effects of therapeutic brain stimulation on mania symptoms. CONCLUSION. Brain lesions associated with mania are characterized by a specific pattern of brain connectivity that lends insight into localization of mania symptoms and potential therapeutic targets. FUNDING.     The Enhanced Efficacy of Intracellular Delivery of Doxorubicin/C6-Ceramide Combination Mediated by the F3 Peptide/Nucleolin System Is Supported by the Downregulation of the PI3K/Akt Pathway

    Targeting multiple cellular populations is of high therapeutic relevance for the tackling of solid tumors heterogeneity. Herein, the ability of pegylated and pH-sensitive liposomes, functionalized with the nucleolin-binding F3 peptide and containing doxorubicin (DXR)/C6-ceramide synergistic combination, to target, in vitro, ovarian cancer, including ovarian cancer stem cells (CSC), was assessed. The underlying molecular mechanism of action of the nucleolin-mediated intracellular delivery of C6-ceramide to cancer cells was also explored. The assessment of overexpression of surface nucleolin expression by flow cytometry was critical to dissipate differences identified by Western blot in membrane/cytoplasm of SKOV-3, OVCAR-3 and TOV-112D ovarian cancer cell lines. The former was in line with the significant extent of uptake into (bulk) ovarian cancer cells, relative to non-targeted and non-specific counterparts. This pattern of uptake was recapitulated with putative CSC-enriched ovarian SKOV-3 and OVCAR-3 sub-population (EpCAMhigh/CD44high). Co-encapsulation of DXR:C6-ceramide into F3 peptide-targeted liposomes improved cytotoxic activity relative to liposomes containing DXR alone, in an extent that depended on the intrinsic resistance to DXR and on the incubation time. The enhanced cytotoxicity of the targeted combination was mechanistically supported by the downregulation of PI3K/Akt pathway by C6-ceramide, only among the nucleolin-overexpressing cancer cells presenting a basal p-Akt/total Akt ratio lower than 1

    Detection of anti-HspX antibodies and HspX protein in patient sera for the identification of recent latent infection by Mycobacterium tuberculosis

    Mycobacterium tuberculosis is a pathogen causing tuberculosis (TB) a spectrum of disease including acute and asymptomatic latent stages. Identifying and treating latently-infected patients constitutes one of the most important impediments to TB control efforts. Those individuals can remain undiagnosed for decades serving as potential reservoirs for disease reactivation. Tests for the accurate diagnosis of latent infection currently are unavailable. HspX protein (α-crystallin), encoded by Rv2031c gene, is produced in vitro by M. tuberculosis during stationary growth phase and hypoxic or acidic culture conditions. In this study, using standard, and Luminex xMAP® bead capture ELISA, respectively, we report on detection of anti-HspX IgG and IgM antibodies and HspX protein in sera from acute and latent TB patients. For the antibody screen, levels of IgG and IgM antibodies were similar between non-infected and active TB patients; however, individuals classified into the group with latent TB showed higher values of anti-HspX IgM (p = 0.003) compared to active TB patients. Using the bead capture antigen detection assay, HspX protein was detected in sera from 56.5% of putative latent cases (p< 0.050) compared to the background median with an average of 9,900 pg/ml and a range of 1,000 to 36,000 pg/ml. Thus, presence of anti-HspX IgM antibodies and HspX protein in sera may be markers of latent TB

    THE 7.8 Mw EARTHQUAKE AND TSUNAMI OF 16th April 2016 IN ECUADOR: Seismic Evaluation, Geological Field Survey and Economic Implications

    We evaluated the recent earthquake and tsunami responsible for considerable damage and 663 deaths due to a 7.8Mw movement on the 16th of April 2016. The seismic event filled tens of thousands in refugee camps and affected some two million persons directly. The potential of high losses and damage with a total of 29,672 properties, including family houses, is also given by the fact that the infrastructure of the fishing, tourism and other industries and the movement to live along the beaches, have been highly developed within the last decades along the Ecuadorian coasts. The geological survey and determination of field data were performed three days after the main seismic event, allowing to obtain 290 data coseismic effects on the ground that allowed to evaluate the maximum macroseismic intensities as well as the predominant geomorphological features.The results of these sampling stations allowed to reconstruct a geological map with isoseismals fields of intensities. With all the compiled and recorded coseismic data in the field of higher macroseismic intensities, we proceeded to produce a map of intensities applying the definitions and degrees of the ESI 2007 scale. We also evaluated the distribution and intensities of the aftershocks demonstrating the spatial- temporal affinities. The occurred tsunami, although less destructive than previous in the same region has been documented with all details available. The economical assessment included in this study concludes that this earthquake impacted a large part of a variety of coastal cities destroying between 70 and 80% of some close-by villages and cities with a distance of 140-150 km of the epicenter, which suffered damages of their buildings within 40 to 55%, in which lines of electricity transmission, infrastructure of water supply, hospitals, schools, private and public buildings, main roads and highways have been severely affected or even completely destroyed. The costs of the damages of the mentioned infrastructure are summing up an approximate loss of some 3.3 billion USD, being equivalent to 3.31 % of the Ecuadorian GDP. In addition to losses in infrastructure and properties, over 28 thousand jobs were lost and about 300 million US$ in trade and businesses
