    Injury Severity Score coding: Data analyst v. emerging m-health technology

    Background. The cost of Abbreviated Injury Scale (AIS) coding has limited its utility in areas of the world with the highest incidence of trauma. We hypothesised that emerging mobile health (m-health) technology could offer a cost-effective alternative to the current goldstandard AIS mechanism in a high-volume trauma centre in South Africa.Methods. A prospectively collected sample of consecutive patients admitted following a traumatic injury that required an operation during a 1-month period was selected for the study. AISs and Injury Severity Scores (ISSs) were generated by clinician-entered data using an m-health application (ISS eTHR) as well as by a team of AIS coders at Vancouver General Hospital, Canada (ISS VGH). Rater agreements for ISSs were analysed using Bland-Altman plots with 95% limits of agreement (LoA) and kappa statistics of the ISSs grouped into ordinal categories. Reliability was analysed using a two-way mixed-model intraclass correlation coefficient (ICC). Calibration and discrimination of univariate logistic regression models built to predict in-hospital complications using ISSs coded by the two methods were also compared.Results. Fifty-seven patients were managed operatively during the study period. The mean age of the cohort was 27.2 years (range 14 - 62), and 96.3% were male. The mechanism of injury was penetrating in 93.4% of cases, of which 52.8% were gunshot injuries. The LoA fell within –8.6 - 9.4. The mean ISS difference was 0.4 (95% CI –0.8 - 1.6). The kappa statistic was 0.53. The ICC of the individual ISS was 0.88 (95% CI 0.81 - 0.93) and the categorical ISS was 0.81 (95% CI 0.68 - 0.87). Model performance to predict in-hospital complications using either the ISS eTHR or the ISS VGH was equivalent.Conclusions. ISSs calculated by the eTHR and gold-standard coding were comparable.     Spatio-temporal changes of snowmelt in Greenland ice sheet based on SSM/I (SSMIS) data (1988-2016)

    359-365The snowmelt of the Greenland ice sheets is of great significance to the study of global climate change. This paper is based on the 19.35 GHz horizontal polarization data and 37.00 GHz vertical polarization data of the Special Sensor Microwave/ Imager Sounder (SSMIS) and Special Sensor Microwave/ Image (SSM/I) carried by National Defense Meteorological Satellite Program (DMSP) from 1988 to 2016, by cross-polarization ratio (XPGR) algorithm (threshold value is -0.0158). The inter-annual trends of snowmelt area, annual average snowmelt onset, end date and duration in Greenland were studied. The results showed that the maximum snowmelt area was 2,080,000 km2 in 2012, and the minimum was 1,115,000 km2 in 1992. From 1988 to 2016, the snowmelt area of the Greenland ice sheets was increased by 2.8×105 km2, with a growth rate of 9.66×103 km2/year. In the annual average change rate, there were earlier snowmelt onset date (0.16 days earlier each year), longer snowmelt duration (0.36 days longer each year) and later snowmelt end date (0.06 days later each year), and the snowmelt area was in the marginal region. The snowmelt area of the southern margin is the largest, and there are obvious regional differences. The snowmelt of Greenland ice sheets changes greatly and shows a periodic change rule in the annual mean snowmelt variation

    Transport through a quantum wire with a side quantum-dot array

    A noninteracting quantum-dot array side-coupled to a quantum wire is studied. Transport through the quantum wire is investigated by using a noninteracting Anderson tunneling Hamiltonian. The conductance at zero temperature develops an oscillating band with resonances and antiresonances due to constructive and destructive interference in the ballistic channel, respectively. Moreover, we have found an odd-even parity in the system, whose conductance vanishes for an odd number of quantum dots while becomes 2e2/h2e^2/h for an even number.     Targeted Delivery of Chemotherapy Agents Using a Liver Cancer-Specific Aptamer

    Using antibody/aptamer-drug conjugates can be a promising method for decreasing toxicity, while increasing the efficiency of chemotherapy.In this study, the antitumor agent Doxorubicin (Dox) was incorporated into the modified DNA aptamer TLS11a-GC, which specifically targets LH86, a human hepatocellular carcinoma cell line. Cell viability tests demonstrated that the TLS11a-GC-Dox conjugates exhibited both potency and target specificity. Importantly, intercalating Dox into the modified aptamer inhibited nonspecific uptake of membrane-permeable Dox to the non-target cell line. Since the conjugates are selective for cells that express higher amounts of target proteins, both criteria noted above are met, making TLS11a-GC-Dox conjugates potential candidates for targeted delivery to liver cancer cells.Considering the large number of available aptamers that have specific targets for a wide variety of cancer cells, this novel aptamer-drug intercalation method will have promising implications for chemotherapeutics in general

    A Global Community of Courts? Modelling the Use of Persuasive Authority as a Complex Network

    There is a growing discussion in the legal literature of an emerging global community of courts composed of a network of increasing judicial dialogue across national borders. We investigate the use of foreign persuasive authority in common law countries by analyzing the network of citations to case law in a corpus of over 1.5 million judgments given by the senior courts of twenty-six common law countries. Our corpus of judgments is derived from data available in the vLex Justis database. In this paper we aim to quantify the flow of jurisprudence across the countries in our corpus and to explore the factors that may influence a judge’s selection of foreign jurisprudence. Utilization of foreign case law varies across the countries in our data, with the courts of some countries presenting higher engagement with foreign jurisprudence than others. Our analysis shows that there has been an upward trend in the use of foreign case law over time, with a marked increase in citations across national borders from the 1990s onward, potentially indicating that increased digital access to foreign judgments has served to facilitate and promote comparative analysis. Not only has the use of foreign case law generally increased over time, the factors that may influence the selection of case law have also evolved, with judges gradually casting their research beyond the most influential and well-known foreign authorities. Notwithstanding that judgments emanating from the United Kingdom (chiefly from the courts of England and Wales) constitute the most frequently consulted body of jurisprudence, we find evidence that domestic courts favor citing the case law of countries that are geographically proximal

    Mass changes of southern and northern inylchek glacier, Central Tian Shan, kyrgyzstan, during ∼1975 and 2007 derived from remote sensing data

    Glacier melt is an essential source of freshwater for the arid regions surrounding the Tian Shan. However, the knowledge about glacier volume and mass changes over the last decades is limited. In the present study, glacier area, glacier dynamics and mass changes are investigated for the period ∼1975-2007 for Southern Inylchek Glacier (SIG) and Northern Inylchek Glacier (NIG), the largest glacier system in Central Tian Shan separated by the regularly draining Lake Merzbacher. The area of NIG increased by 2.0 ± 0.1 km2 (∼1.3%) in the period ∼1975-2007. In contrast, SIG has shrunk continuously in all investigated periods since ∼1975. Velocities of SIG in the central part of the ablation region reached ∼100-120 m a-1 in 2002/2003, which was slightly higher than the average velocity in 2010/2011. The central part of SIG flows mainly towards Lake Merzbacher rather than towards its terminus. The measured velocities at the distal part of the terminus downstream of Lake Merzbacher were below the uncertainty, indicating very low flow with even stagnant parts. Geodetic glacier mass balances have been calculated using multioral digital elevation models from KH-9 Hexagon (representing the year 1975), SRTM3 (1999), ALOS PRISM (2006) and SPOT-5 high-resolution geometrical (HRG) data (2007). In general, a continuous mass loss for both SIG and NIG could be observed between ∼1975 and 2007. SIG lost mass at a rate of 0.43 ± 0.10 m w.e. a-1 and NIG at a rate of 0.25 ± 0.10 m w.e. ag-1 within the period ∼1975-1999. For the period 1999-2007, the highest mass loss of 0.57 ± 0.46 m w.e. ag-1 was found for NIG, whilst SIG showed a potential moderate mass loss of 0.28 ± 0.46 m w.e. a-1. Both glaciers showed a small retreat during this period. Between ∼1975 and 1999, we identified a thickening at the front of NIG with a maximum surface elevation increase of about 150 m as a consequence of a surge event. In contrast significant thinning (>0.5 m a-1) and comparatively high velocities close to the dam of Lake Merzbacher were observed for SIG, indicating that Lake Merzbacher enhances glacier mass loss.Publisher PDFPeer reviewe
