    Gestão acadêmica exercida por professores universitários:: um estudo de caso

    No Brasil é muito comum que o professor universitário seja designado ao exercício de cargos administrativos sem que ele tenha sido previamente treinado ou orientado em relação às funções administrativas. Especificamente na área da saúde, não existem trabalhos que relatem ou questionem este processo de transição de professor de área técnica para gestor. O presente artigo trata-se de uma análise qualitativa sobre a perspectiva que professores com formação acadêmica em área da saúde ou biológicas possuem sobre o fato de assumirem cargo de gerência em uma universidade federal no estado de Minas Gerais. Com o objetivo de fazer uma reflexão sobre a importância de um treinamento prévio a estes gestores, o trabalho consiste numa descrição qualitativa dos principais questionamentos desses docentes durante a condução das atividades relativas ao trabalho de coordenação de colegiado de graduação ou pós graduação. Após comparação dos resultados das entrevistas com outros trabalhos da literatura com propósitos semelhantes, foi possível evidenciar que os professores acreditam não possuir formação qualificada e específica para assumirem cargos de gerência dentro da universidade e que seu trabalho, quando bem-sucedido, decorre de práticas empírica

    Genotyping And Descriptive Proteomics Of A Potential Zoonotic Canine Strain Of Giardia Duodenalis, Infective To Mice

The genetic assemblages A and B of Giardia duodenalis are frequently isolated from human and canine hosts. Most of the assemblage A strains are not infective to adult mice, which can limit the range of studies regarding to biology of G. duodenalis, including virulence factors and the interaction with host immune system. This study aimed to determine the infectivity in mice of an assemblage A Giardia duodenalis strain (BHFC1) isolated from a dog and to classify the strain in sub-assemblages (Al, All, AIII) through the phylogenetic analysis of beta-giardin (bg), triose phosphate isomerase (tpi) and glutamate dehydrogenase (gdh) genes. In addition, the proteomic profile of soluble and insoluble protein fractions of trophozoites was analyzed by 2D-electrophoresis. Accordingly, trophozoites of BHFC1 were highly infective to Swiss mice. The phylogenetic analysis of tpi and gdh revealed that BHFC1 clustered to sub-assemblage Al. The proteomic map of soluble and insoluble protein fractions led to the identification of 187 proteins of G. duodenalis, 27 of them corresponding to hypothetical proteins. Considering both soluble and soluble fractions, the vast majority of the identified proteins (n = 82) were classified as metabolic proteins, mainly associated with carbon and lipid metabolism, including 53 proteins with catalytic activity. Some of the identified proteins correspond to antigens while others can be correlated with virulence.     Microglia dysfunction caused by the loss of Rhoa disrupts neuronal physiology and leads to neurodegeneration

Using conditional gene targeting in mice, we demonstrate that genetic ablation of the small GTPase Rhoa in adult microglia is sufficient to trigger spontaneous microglia activation, producing a neurological phenotype (including synapse and neuron loss, impairment of long-term potentiation [LTP], formation of β-amyloid plaques, and memory deficits). Mechanistically, loss of Rhoa in microglia triggers Src activation and Src-mediated tumor necrosis factor (TNF) production, leading to excitotoxic glutamate secretion. Inhibiting Src in microglia Rhoa-deficient mice attenuates microglia dysregulation and the ensuing neurological phenotype. We also find that the Rhoa/Src signaling pathway is disrupted in microglia of the APP/PS1 mouse model of Alzheimer disease and that low doses of Aβ oligomers trigger microglia neurotoxic polarization through the disruption of Rhoa-to-Src signaling.     Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial

    Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients.     Analysis of inhibition of neuronal nitric oxide synthase in vasopressin secretion during experimental sepsis.

    A fisiopatologia da sepse se caracteriza por hipotensão acompanhada de aumento da secreção de vasopressina (AVP) na fase inicial e diminuição numa fase mais tardia. Essa hipotensão é em parte devido ao aumento da quantidade de óxido nítrico, que juntamente com outros mediadores tem sua produção aumentada durante a sepse. A óxido nítrico sintase (NOS) é responsável pela síntese deste mediador, e sua isoforma neuronal (nNOS) está presente no músculo esquelético, pulmões, testículos, próstata, pele e também nos neurônios vasopressinérgicos do hipotálamo. O presente trabalho avaliou a participação do óxido nítrico produzido pela isoforma neuronal de NOS sobre a secreção temporal de AVP durante a sepse experimental. Ratos Wistar receberam injeção i.p. de 7-nitroindazol (50mg/kg ou 80mg/kg), inibidor específico da NOS neuronal, ou DMSO 10% + óleo de gergelim na proporção 1:9 (veículo) e após 30 minutos foram submetidos ao estímulo séptico por ligadura e perfuração cecal (CLP) ou à operação fictícia (OF). Em um grupo de animais, a sobrevida foi avaliada durante 5 dias. Em outro grupo, os animais foram decapitados 0, 4, 6, 18 e 24 horas após a cirurgia e o sangue processado para determinação do hematócrito, sódio sérico, osmolalidade, proteínas, glicose, creatinina, nitrato sérico e AVP plasmática. A neurohipófise foi removida para a determinação do conteúdo de AVP, e o hipotálamo dissecado para a determinação da atividade da NOS total. A mortalidade observada após CLP não foi modificada com o pré-tratamento com 7-NI (50mg/kg), assim como os aumentos temporais de hematócrito, glicose e nitrato sérico observados. As proteínas plasmáticas e o sódio sérico apresentaram diminuição após CLP e o pré-tratamento com 7-NI antecipou a perda proteica e postergou a diminuição do sódio sérico. Os animais após CLP não apresentaram alterações de creatinina e osmolalidade, entretanto quando prétratados com 7-NI, apresentaram aumento em 6 e 18 horas e diminuição a partir de 4hs, respectivamente. A atividade da NOS total no hipotálamo aumentou nos tempos determinados de 4 e 24 horas após CLP e este aumento foi reduzido com o prétratamento com o 7-NI nas doses de 50 e 80mg/kg, respectivamente. O conteúdo neurohipofisário de AVP diminuiu em 4, 6 e 18 horas após CLP e o pré-tratamento com 7-NI reduziu os estoques apenas em 0 e 6 horas. As concentrações plasmáticas de vasopressina apresentaram-se aumentadas sómente 6 horas após CLP e o pré-tratamento não alterou essas concentrações. Esses resultados permitem concluir que o NO produzido pela NOS neuronal não teria uma tarefa substancial na secreção de vasopressina durante sepse experimental.The pathophysiology of sepsis is caracterized by hypotension accompanied by increase of vasopressin secretion (AVP) in early phase and decrease during late phase. This hypotension is due, in part, to the increase of nitric oxide (NO) production, that, like other mediators, shows high production during sepsis. Nitric oxide synthase (NOS) is responsible by synthesis of NO. The neuronal isoform of NOS is present in skeletic muscle, testicles, prostate, skin and vasopressinergics neurons of hypothalamus. The present work evaluated the participation of nitric oxide produced by neuronal NOS in temporal vasopressin secretion during experimental sepsis. Rats Wistar received intraperitoneal injection of 7-nitroindazole (50 or 80 mg/kg), an inhibitor of neuronal nitric oxide synthase activity, or DMSO 10% + sesame oil in the proportion 1:9 (vehicle) and after 30 minutes, they were submited to septic stimulus by cecal ligation and puncture (CLP) or to sham operation. In one of the groups, the survival rate was evaluated during 5 days. In other group, the animals were decapited 0, 4, 6, 18 and 24 hours after CLP and the blood was processed to determinate haematocrit, serum sodium, osmolality, proteins, glucose, creatinine, serum nitrate, and plasma AVP. Neurohypophysis was removed to determination of vasopressin content, and hypotalamus was dissected to determinate total NOS activity. Mortality observed after CLP was not affected by periferal injection of 7-nitroindazole (50 mg/kg) as well as haematocrit, glucose and nitrate increase. Serum sodium and plasma protein decreased after CLP and the treatment antecipated the loss protein, and delayed serum sodium decrease. CLP animals didn\'t show creatinine and osmolality alterations, but when treated with 7-nitroindazole, showed increase 6 and 18 hours, and decrease 4 hours, respectively. NOS activity in hypothalamus increased 4 and 24 hours after CLP, and was reduced with 7-NI pretreatment (50 and 80 mg/kg respectively). AVP neurohypophysis content diminished 4, 6 and 18 hours after CLP and 7-NI reduced the content just at 0 and 6 hours. Vasopressin plasma concentration increased just 6 hours after CLP and 7-NI pretreatment didn\'t alter this parameter. We concluded that NO produced by neuronal NOS doesn\'t have a substantial role in vasopressin secretion during experimental sepsis

    Giardiasis is an intestinal infection that affects more than two hundred million people annually worldwide; it is caused by the flagellated protozoan Giardia duodenalis. In tropical countries and in low or middle-income settings, like Brazil, its prevalence can be high. There is currently no systematic review on the presence of G. duodenalis in patients, animals or water sources in Brazil. This systematic review was performed according to recommendations established by Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA). As databases for our searches, we have used PubMed, Embase, Scopus and the Brazilian database SciELO using the keywords «Giardia*» and «Brazil». This systematic review identified research studies related to G. duodenalis in water, giardiasis in animals, prevalence of giardiasis across Brazilian regions, genotyping of strains isolated in humans, and giardiasis in indigenous populations. We also propose a network of G. duodenalis transmission in Brazil based on genotypes analyses. This is the first time within the last twenty years that a review is being published on the occurrence of G. duodenalis in Brazil, addressing relevant issues such as prevalence, molecular epidemiology and analytical methods for parasite detection. || Giardiasis is an intestinal disease that affect millions of people worldwide, including children. Its main route of transmission is by ingestion of food or water contaminated with the protozoan G. duodenalis. Transmission does not require an animal host, although transmission from animals to human (zoonotic transmission) has been confirmed as an important vector of human giardiasis. This study is a comprehensive description of the impact of giardiasis in Brazil based on studies published in the country from the past 20 years. We describe Giardia prevalence in humans (including indigenous populations), animals and water supplies. In addition, we create a transmission network model for the disease, based on genotype data previously identified in animal and human hosts as well as in environmental samples.     A La microbiota adquirida de acuerdo con la vía de nacimiento: una revisión integradora

    Objetivo: analizar las evidencias científicas existentes en la literatura sobre la relación de la vía de nacimiento con la microbiota adquirida por el recién nacido. Método: se trata de una revisión integradora sobre la influencia de la vía de nacimiento en la colonización de la microbiota en el recién nacido. Fue realizada una búsqueda en la literatura en las bases de datos Medical Literature Analysis and Retrieval System Online/PubMed y Biblioteca Virtual en Salud, teniendo como estrategia de búsqueda la selección de artículos basados en los descriptores desarrollados en el Medical Subject Headings (términos MeSH) o Descriptores en Ciencia de la Salud (DeCS). Resultados: los recién nacidos por vía vaginal presentan, en los primeros días de vida, mayor concentración de Bacteroides, Bifidobacterias y Lactobacillus; y, con el pasar de las semanas muestran mayor variabilidad de la microbiota. Los recién nacidos por cesárea presentan microbioma semejante a la piel materna y al ambiente hospitalario, poseyendo menor diversidad y siendo principalmente constituida de Staphylococcus, Streptococcus y Clostridium. Conclusión: la microbiota vaginal materna proporciona al neonato una mayor variedad de microorganismos colonizadores que son responsables por auxiliar en la capacitación y mejor adecuación de su sistema inmunológico. Se evidencia que el parto vaginal es la vía ideal y que la cesárea debe ser realizada apenas cuando existen indicaciones reales.Objetivo: analisar as evidências científicas existentes na literatura sobre a relação da via de nascimento com a microbiota adquirida pelo recém-nascido. Method: this integrative review addresses the role of the type of delivery on newborns’ microbial colonization. A search was conducted in the Medical Literature Analysis and Retrieval System Online/PubMed and Virtual Health Library databases using the descriptors provided by Medical Subject Headings (MeSH) and Health Science Descriptors (DeCS). Results: infants born vaginally presented a greater concentration of Bacteroides, Bifidobacteria, and Lactobacillus in the first days of life and more significant microbial variability in the following weeks. The microbiome of infants born via C-section is similar to the maternal skin and the hospital setting and less diverse, mainly composed of Staphylococcus, Streptococcus, and Clostridium. Conclusion: the maternal vaginal microbiota provides newborns with a greater variety of colonizing microorganisms responsible for boosting and preparing the immune system. Vaginal birth is the ideal birth route, and C-sections should only be performed when there are medical indications
