    Introduction/Purpose: Patients with foot and ankle conditions have been shown to demonstrate improvement in their generic outcomes using the Patient Reported Outcome Measurement Information System (PROMIS). However, the responsiveness to change and the impact change may have on quality of life has not been explored following non-operative care with physical therapy. The effect size (ES) is an assessment of the magnitude of change while the impact of change in physical function can be assed with quality of life utility scores. Therefore, the purpose of this analysis was to investigate the responsiveness of the PROMIS physical function (PF) scale on changes in quality of life

    The Relationship Between Ankle, Hindfoot, and Forefoot Position and Posterior Tibial Muscle Excursion

    Background: The purpose of this study was to examine the relationship of forefoot position in the transverse plane (abduction/adduction), hindfoot position in the frontal plane (eversion/inversion), and ankle position in the sagittal plane (plantarflexion/dorsiflexion) with posterior tibialis (PT) muscle excursion using an in vitro cadaver model. Methods: Seven fresh-frozen cadaver specimens were potted and mounted on a frame. The PT tendon was dissected 15 cm proximal to the medial malleolus, and a 5-kg weight was sutured to the tendon. A six-camera motion analysis system (Optotrak, Northern Digital, Inc.) was used to track three-dimensional (3D) motion of the tibia, calcaneus (hindfoot) and first metatarsal (forefoot) using bone pins. The ankle, hindfoot, and forefoot were manually placed in 24 different ankle and foot positions. A stepwise regression analysis was used to examine the relationship among ankle, hindfoot, and forefoot kinematics and PT muscle excursion. Results: Hindfoot eversion/inversion and forefoot abduction/adduction accounted for 77% of the variance in PT muscle excursion, with small contributions from ankle plantarflexion/dorsiflexion (5.7%) and forefoot plantarflexion/dorsiflexion (1.9%). A combined regression equation applied to individual specimens resulted in average errors of less than 2.5 mm. Conclusions: This study supports the hypothesis that PT muscle excursion can be estimated using specific foot and ankle kinematic variables. Further, these data suggest that hindfoot eversion and forefoot abduction account for most of the variance in PT muscle excursion and are theorized to be important to control clinically altering the length of the posterior tibial muscle

    Omeprazole compared with misoprostol for ulcers associated with nonsteroidal antiinflammatory drugs

    Background: Misoprostol is effective for ulcers associated with the use of nonsteroidal antiinflammatory drugs (NSAIDs) but is often poorly tolerated because of diarrhea and abdominal pain. We compared the efficacy of omeprazole and misoprostol in healing and preventing ulcers associated with NSAIDs. Methods: in a double-blind study, we randomly assigned 935 patients who required continuous NSAID therapy and who had ulcers or more than 10 erosions in the stomach or duodenum (or both) to receive 20 mg or 40 mg of omeprazole orally in the morning or 200 microg of misoprostol orally four times daily. Patients were treated for four weeks or, in the absence of healing, eight weeks. Treatment success was defined as the absence of ulcers and the presence of fewer than five erosions at each site and not more than mild dyspepsia. We then randomly reassigned 732 patients in whom treatment was successful to maintenance therapy with 20 mg of omeprazole daily, 200 microg of misoprostol twice daily, or placebo for six months. Results: at eight weeks, treatment was successful in 76 percent of the patients given 20 mg of omeprazole (233 of 308), 75 percent of those given 40 mg of omeprazole (237 of 315), and 71 percent of those given misoprostol (212 of 298). The rates of gastric-ulcer healing were significantly higher with 20 mg of omeprazole (but not 40 mg of omeprazole) than with misoprostol. Healing rates among patients with duodenal ulcers were higher with either dose of omeprazole than with misoprostol, whereas healing rates among patients with erosions alone were higher with misoprostol. More patients remained in remission during maintenance treatment with omeprazole (61 percent) than with misoprostol (48 percent, P=0.001) and with either drug than with placebo (27 percent, P<0.001). There were more adverse events during the healing phase in the misoprostol group than in the groups given 20 mg and 40 mg of omeprazole (59 percent, 48 percent, and 46 percent, respectively). Conclusions: the overall rates of successful treatment of ulcers, erosions, and symptoms associated with NSAIDs were similar for the two doses of omeprazole and misoprostol. Maintenance therapy with omeprazole was associated with a lower rate of relapse than misoprostol. Omeprazole was better tolerated than misoprostol

In Europe and around the world, the approach to management of the marine environment has developed from the management of single issues (e.g., species and/or pressures) toward holistic Ecosystem Based Management (EBM) that includes aims to maintain biological diversity and protect ecosystem functioning. Within the European Union, this approach is implemented through the Marine Strategy Framework Directive (MSFD, 2008/56/EC). Integrated Ecosystem Assessment is required by the Directive in order to assess Good Environmental Status (GES). Ecological modeling has a key role to play within the implementation of the MSFD, as demonstrated here by case studies covering a range of spatial scales and a selection of anthropogenic threats. Modeling studies have a strong role to play in embedding data collected at limited points within a larger spatial and temporal scale, thus enabling assessments of pelagic and seabed habitat. Furthermore, integrative studies using food web and ecosystem models are able to investigate changes in food web functioning and biological diversity in response to changes in the environment and human pressures. Modeling should be used to: support the development and selection of specific indicators; set reference points to assess state and the achievement of GES; inform adaptive monitoring programs and trial management scenarios. The modus operandi proposed shows how ecological modeling could support the decision making process leading to appropriate management measures and inform new policy

    Using ecological models to assess ecosystem status in support of the European Marine Strategy Framework Directive

The European Union's Marine Strategy Framework Directive (MSFD) seeks to achieve, for all European seas, "Good Environmental Status" (GEnS), by 2020. Ecological models are currently one of the strongest approaches used to predicting and understanding the consequences of anthropogenic and climate-driven changes in the natural environment. We assess the most commonly used capabilities of the modelling community to provide information about indicators outlined in the MSFD, particularly on biodiversity, food webs, non-indigenous species and seafloor integrity descriptors. We built a catalogue of models and their derived indicators to assess which models were able to demonstrate: (1) the linkages between indicators and ecosystem structure and function and (2) the impact of pressures on ecosystem state through indicators. Our survey identified 44 ecological models being implemented in Europe, with a high prevalence of those that focus on links between hydrodynamics and biogeochemistry, followed by end-to-end, species distribution/habitat suitability, bio-opt ical (remote sensing) and multispecies models. Approximately 200 indicators could be derived from these models, the majority of which were biomass and physical/hydrological/chemical indicators. Biodiversity and food webs descriptors, with ∼49% and ∼43% respectively, were better addressed in the reviewed modelling approaches than the non-indigenous species (0.3%) and sea floor integrity (∼8%) descriptors. Out of 12 criteria and 21 MSFD indicators relevant to the abovementioned descriptors, currently only three indicators were not addressed by the 44 models reviewed. Modelling approaches showed also the potential to inform on the complex, integrative ecosystem dimensions while addressing ecosystem fundamental properties, such as interactions between structural components and ecosystems services provided, despite the fact that they are not part of the MSFD indicators set. The cataloguing of models and their derived indicators presented in this study, aim at helping the planning and integration of policies like the MSFD which require the assessment of all European Seas in relation to their ecosystem status and pressures associated and the establishment of environmental targets (through the use of indicators) to achieve GEnS by 2020

    New genetic loci implicated in fasting glucose homeostasis and their impact on type 2 diabetes risk.

    Levels of circulating glucose are tightly regulated. To identify new loci influencing glycemic traits, we performed meta-analyses of 21 genome-wide association studies informative for fasting glucose, fasting insulin and indices of beta-cell function (HOMA-B) and insulin resistance (HOMA-IR) in up to 46,186 nondiabetic participants. Follow-up of 25 loci in up to 76,558 additional subjects identified 16 loci associated with fasting glucose and HOMA-B and two loci associated with fasting insulin and HOMA-IR. These include nine loci newly associated with fasting glucose (in or near ADCY5, MADD, ADRA2A, CRY2, FADS1, GLIS3, SLC2A2, PROX1 and C2CD4B) and one influencing fasting insulin and HOMA-IR (near IGF1). We also demonstrated association of ADCY5, PROX1, GCK, GCKR and DGKB-TMEM195 with type 2 diabetes. Within these loci, likely biological candidate genes influence signal transduction, cell proliferation, development, glucose-sensing and circadian regulation. Our results demonstrate that genetic studies of glycemic traits can identify type 2 diabetes risk loci, as well as loci containing gene variants that are associated with a modest elevation in glucose levels but are not associated with overt diabetes

    Job requirements compared to medical school education: differences between graduates from problem-based learning and conventional curricula

    Background: Problem-based Learning (PBL) has been suggested as a key educational method of knowledge acquisition to improve medical education. We sought to evaluate the differences in medical school education between graduates from PBL-based and conventional curricula and to what extent these curricula fit job requirements. Methods: Graduates from all German medical schools who graduated between 1996 and 2002 were eligible for this study. Graduates self-assessed nine competencies as required at their day-to-day work and as taught in medical school on a 6-point Likert scale. Results were compared between graduates from a PBL-based curriculum (University Witten/Herdecke) and conventional curricula. Results: Three schools were excluded because of low response rates. Baseline demographics between graduates of the PBL-based curriculum (n = 101, 49% female) and the conventional curricula (n = 4720, 49% female) were similar. No major differences were observed regarding job requirements with priorities for "Independent learning/working" and "Practical medical skills". All competencies were rated to be better taught in PBL-based curriculum compared to the conventional curricula (all p < 0.001), except for "Medical knowledge" and "Research competence". Comparing competencies required at work and taught in medical school, PBL was associated with benefits in "Interdisciplinary thinking" (Δ + 0.88), "Independent learning/working" (Δ + 0.57), "Psycho-social competence" (Δ + 0.56), "Teamwork" (Δ + 0.39) and "Problem-solving skills" (Δ + 0.36), whereas "Research competence" (Δ - 1.23) and "Business competence" (Δ - 1.44) in the PBL-based curriculum needed improvement. Conclusion: Among medical graduates in Germany, PBL demonstrated benefits with regard to competencies which were highly required in the job of physicians. Research and business competence deserve closer attention in future curricular development

    A haemagglutination test for rapid detection of antibodies to SARS-CoV-2

    Serological detection of antibodies to SARS-CoV-2 is essential for establishing rates of seroconversion in populations, and for seeking evidence for a level of antibody that may be protective against COVID-19 disease. Several high-performance commercial tests have been described, but these require centralised laboratory facilities that are comparatively expensive, and therefore not available universally. Red cell agglutination tests do not require special equipment, are read by eye, have short development times, low cost and can be applied at the Point of Care. Here we describe a quantitative Haemagglutination test (HAT) for the detection of antibodies to the receptor binding domain of the SARS-CoV-2 spike protein. The HAT has a sensitivity of 90% and specificity of 99% for detection of antibodies after a PCR diagnosed infection. We will supply aliquots of the test reagent sufficient for ten thousand test wells free of charge to qualified research groups anywhere in the world
