    Effects of Educating Local Government Officers and Healthcare and Welfare Professionals in Suicide Prevention

    Suicide is a major public health issue. In Japan, local governments are responsible for suicide prevention, and local government officers are therefore expected to act as gatekeepers for suicide prevention. In this study, through a questionnaire survey, the authors examined the current knowledge and attitudes concerning suicide prevention among local government officers and healthcare and welfare professionals, and the effects of providing suicide prevention education on their knowledge of and attitudes toward suicide and its prevention. One hundred eighty-three local government officers and 432 healthcare/welfare professionals completed the survey before and after a single education session. Before the session, the local government officers and healthcare/welfare professionals showed mainly positive attitudes toward suicide prevention efforts, with little difference between the two groups. After the training, knowledge and attitudes were further improved for most questionnaire items. Respondents with one or more experiences of suicide prevention training showed significantly more knowledge and positive attitudes before the training than those with no such experience. Moreover, knowledge of depression and having a sympathetic attitude were found to be especially associated with the overall attitude that “suicide can be prevented”. Training in suicide prevention was shown to be effective in promoting appropriate knowledge and attitudes among local government officers and healthcare/welfare professionals who are gatekeepers for preventing suicide. Our findings confirm the importance of suicide prevention education, and will contribute to creating a standard educational program on suicide prevention in Japan

    Acinar Cell Carcinoma of the Pancreas with Colon Involvement

    We report a case of acinar cell carcinoma of the pancreas with colon involvement that was difficult to distinguish from primary colon cancer. A 60-year-old man was admitted with a 1-month history of diarrhea. Contrast-enhanced computed tomography (CT) revealed a large tumor (10.6×11.6 cm) at the splenic flexure of the colon. Colonoscopy showed completely round ulcerative lesions, and biopsy revealed poorly differentiated adenocarcinoma. Left hemicolectomy, resection of the jejunum and pancreas body and tail, and splenectomy were performed based on a diagnosis of descending colon cancer (cT4N0M0, stage IIB), and surgery was considered to be curative. Diagnosis was subsequently confirmed as moderately differentiated acinar cell carcinoma of the pancreas by immunohistochemical staining (pT3N0M0, stage IIA). Multiple liver metastases with portal thrombosis were found 8 weeks postoperatively. Despite combination chemotherapy with oral S-1 and gemcitabine, the patient died of hepatic failure with no effect of chemotherapy 14 weeks postoperatively. Correct diagnosis was difficult to determine preoperatively from the clinical, CT, and colonoscopy findings. Moreover, the disease was extremely aggressive even after curative resection. Physicians should consider pancreatic cancer in the differential diagnosis of similar cases

    Constraining cosmic scatter in the Galactic halo through a differential analysis of metal-poor stars

    © ESO 2017.Context. The chemical abundances of metal-poor halo stars are important to understanding key aspects of Galactic formation and evolution. Aims. We aim to constrain Galactic chemical evolution with precise chemical abundances of metal-poor stars (−2.8 ≤ [Fe/H] ≤ −1.5). Methods. Using high resolution and high S/N UVES spectra of 23 stars and employing the differential analysis technique we estimated stellar parameters and obtained precise LTE chemical abundances. Results. We present the abundances of Li, Na, Mg, Al, Si, Ca, Sc, Ti, V, Cr, Mn, Co, Ni, Zn, Sr, Y, Zr, and Ba. The differential technique allowed us to obtain an unprecedented low level of scatter in our analysis, with standard deviations as low as 0.05 dex, and mean errors as low as 0.05 dex for [X/Fe]. Conclusions. By expanding our metallicity range with precise abundances from other works, we were able to precisely constrain Galactic chemical evolution models in a wide metallicity range (−3.6 ≤ [Fe/H] ≤ −0.4). The agreements and discrepancies found are key for further improvement of both models and observations. We also show that the LTE analysis of Cr II is a much more reliable source of abundance for chromium, as Cr I has important NLTE effects. These effects can be clearly seen when we compare the observed abundances of Cr I and Cr II with GCE models. While Cr I has a clear disagreement between model and observations, Cr II is very well modeled. We confirm tight increasing trends of Co and Zn toward lower metallicities, and a tight flat evolution of Ni relative to Fe. Our results strongly suggest inhomogeneous enrichment from hypernovae. Our precise stellar parameters results in a low star-to-star scatter (0.04 dex) in the Li abundances of our sample, with a mean value about 0.4 dex lower than the prediction from standard Big Bang nucleosynthesis; we also study the relation between lithium depletion and stellar mass, but it is difficult to assess a correlation due to the limited mass range. We find two blue straggler stars, based on their very depleted Li abundances. One of them shows intriguing abundance anomalies, including a possible zinc enhancement, suggesting that zinc may have been also produced by a former AGB companion.Peer reviewedFinal Published versio

    Nucleosynthesis Yields of Core-Collapse Supernovae and Hypernovae, and Galactic Chemical Evolution

    We present new nucleosynthesis yields as functions of the stellar mass, metallicity, and explosion energy (corresponding to normal supernovae and Hypernovae). We apply the results to the chemical evolution of the solar neighborhood. Our new yields are based on the new developments in the observational/theoretical studies of supernovae (SNe) and extremely metal-poor (EMP) stars in the halo, which have provided excellent opportunities to test the explosion models and their nucleosynthesis. We use the light curve and spectra fitting of individual SN to estimate the mass of the progenitor, explosion energy, and produced 56Ni mass. Comparison with the abundance patterns of EMP stars has made it possible to determine the model parameters of core-collapse SNe, such as mixing-fallback parameters. More specifically, we take into account the two distinct new classes of massive SNe: 1) very energetic Hypernovae, whose kinetic energy (KE) is more than 10 times the KE of normal core-collapse SNe, and 2) very faint and low energy SNe (Faint SNe). These two new classes of SNe are likely to be ``black-hole-forming'' SNe with rotating or non-rotating black holes. Nucleosynthesis in Hypernovae is characterized by larger abundance ratios (Zn,Co,V,Ti)/Fe and smaller (Mn,Cr)/Fe than normal SNe, which can explain the observed trends of these ratios in EMP stars. Nucleosynthesis in Faint SNe is characterized by a large amount of fall-back, which explains the abundance pattern of the most Fe-poor stars. These comparisons suggest that black-hole-forming SNe made important contributions to the early Galactic (and cosmic) chemical evolution.Comment: To appear in Nuclear Physics A (Special Issue on Nuclear Astrophysics) eds.     Association of melanocortin 1 receptor gene (MC1R) polymorphisms with skin reflectance and freckles in Japanese.

    Most studies on the genetic basis of human skin pigmentation have focused on people of European ancestry and only a few studies have focused on Asian populations. We investigated the association of skin reflectance and freckling with genetic variants of melanocortin 1 receptor (MC1R) gene in Japanese. DNA samples were obtained from a total of 653 Japanese individuals (ages 19-40 years) residing in Okinawa; skin reflectance was measured using a spectrophotometer and freckling status was determined for each individual. Lightness index (L*) and freckling status were not correlated with age, body mass index or ancestry (Ryukyuan or Main Islanders of Japan). Among the 10 nonsynonymous variants that were identified by direct sequencing of the coding region of MC1R, two variants--R163Q and V92M--with the derived allele frequencies of 78.6 and 5.5%, respectively, were most common. Multiple regression analysis showed that the 163Q allele and the presence of nonsynonymous rare variants (allele frequencies <5%) were significantly associated with an increase in sex-standardized skin lightness (L* of CIELAB (CIE 1976 (L*a*b*) color space)) of the inner upper arm. Relative to the 92V allele, the 92M allele was significantly associated with increased odds of freckling. This is the first study to show an association between the 163Q allele and skin reflectance values; this association indicated that light-toned skin may have been subjected to positive selection in East Asian people
