    NuSTAR Spectroscopy of Multi-Component X-ray Reflection from NGC 1068

    We report on observations of NGC1068 with NuSTAR, which provide the best constraints to date on its >10>10~keV spectral shape. We find no strong variability over the past two decades, consistent with its Compton-thick AGN classification. The combined NuSTAR, Chandra, XMM-Newton, and Swift-BAT spectral dataset offers new insights into the complex reflected emission. The critical combination of the high signal-to-noise NuSTAR data and a spatial decomposition with Chandra allow us to break several model degeneracies and greatly aid physical interpretation. When modeled as a monolithic (i.e., a single N_H) reflector, none of the common Compton-reflection models are able to match the neutral fluorescence lines and broad spectral shape of the Compton reflection. A multi-component reflector with three distinct column densities (e.g., N_H~1.5e23, 5e24, and 1e25 cm^{-2}) provides a more reasonable fit to the spectral lines and Compton hump, with near-solar Fe abundances. In this model, the higher N_H components provide the bulk of the Compton hump flux while the lower N_H component produces much of the line emission, effectively decoupling two key features of Compton reflection. We note that ~30% of the neutral Fe Kalpha line flux arises from >2" (~140 pc), implying that a significant fraction of the <10 keV reflected component arises from regions well outside of a parsec-scale torus. Jeg har benyttet meg av relevante forskningsartikler, selvvalgt pensum og pensumlitteratur. Funn: Det ble funnet ti relevante artikler. De fleste artiklene handler om pasientens åndelige behov i møte med døden sett fra både pasientens og sykepleiers perspektiv, resten omhandler Islam, og muslimske ritualer. Drøfting: Drøftingen er basert på artiklene og annen relevant litteratur. Det blir drøftet pasientens behov for åndelig omsorg, og hva sykepleieren bør kjenne til i møte med den døende muslimske pasienten. Konklusjon: Det kommer frem at det er viktig at sykepleieren har kunnskap om den muslimske pasientens åndelige behov, og hvordan det kommer til uttrykk. Viktige aspekter å kjenne til er pasientens behov for bønn, rituell renhet, diett, og at familien har en viktig rolle for pasienten

    Patterns and Collective Behavior in Granular Media: Theoretical Concepts

    Granular materials are ubiquitous in our daily lives. While they have been a subject of intensive engineering research for centuries, in the last decade granular matter attracted significant attention of physicists. Yet despite a major efforts by many groups, the theoretical description of granular systems remains largely a plethora of different, often contradicting concepts and approaches. Authors give an overview of various theoretical models emerged in the physics of granular matter, with the focus on the onset of collective behavior and pattern formation. Their aim is two-fold: to identify general principles common for granular systems and other complex non-equilibrium systems, and to elucidate important distinctions between collective behavior in granular and continuum pattern-forming systems.Comment: Submitted to Reviews of Modern Physics.     Dips and eclipses in the X-ray binary Swift J1858.6-0814 observed with NICER

From these, we find an orbital period of P=76841.31.4+1.3P=76841.3_{-1.4}^{+1.3} s (21.3\approx21.3 hours) and an eclipse duration of tec=409818+17t_{\rm ec}=4098_{-18}^{+17} s (1.14\approx1.14 hours). We also find several absorption dips during the pre-eclipse phase. From the eclipse duration to orbital period ratio, the inclination of the binary orbit is constrained to i>70i>70^\circ. The most likely range for the companion mass suggests that the inclination is likely to be closer to this value than 9090^\circ. The eclipses are also consistent with earlier data, in which strong variability ('flares') and the long orbital period prevent clear detection of the period or eclipses. We also find that the bright flares occurred preferentially in the post-eclipse phase of the orbit, likely due to increased thickness at the disc-accretion stream interface preventing flares being visible during the pre-eclipse phase. This supports the notion that variable obscuration is responsible for the unusually strong variability in Swift J1858.6-0814.Peer reviewe

    The variable hard x-ray emission of NGC 4945 as observed by NUSTAR

    We present a broadband (~0.5-79 keV) spectral and temporal analysis of multiple NuSTAR observations combined with archival Suzaku and Chandra data of NGC4945, the brightest extragalactic source at 100 keV. We observe hard X-ray (> 10 keV) flux and spectral variability, with flux variations of a factor 2 on timescales of 20 ksec. A variable primary continuum dominates the high energy spectrum (>10 keV) in all the states, while the reflected/scattered flux which dominates at E< 10 keV stays approximately constant. From modelling the complex reflection/transmission spectrum we derive a Compton depth along the line of sight of tau_Thomson ~ 2.9, and a global covering factor for the circumnuclear gas of ~ 0.15. This agrees with the constraints derived from the high energy variability, which implies that most of the high energy flux is transmitted, rather that Compton-scattered. This demonstrates the effectiveness of spectral analysis in constraining the geometric properties of the circumnuclear gas, and validates similar methods used for analyzing the spectra of other bright, Compton-thick AGN. The lower limits on the e-folding energy are between 200-300 keV, consistent with previous BeppoSAX, Suzaku and Swift BAT observations. The accretion rate, estimated from the X-ray luminosity and assuming a bolometric correction typical of type 2 AGN, is in the range ~0.1-0.3 lambda_Edd depending on the flux state. The substantial observed X-ray luminosity variability of NGC4945 implies that large errors can arise from using single-epoch X-ray data to derive L/L_Edd values for obscured AGNs.Comment: Accepted for publication in Ap

    Public involvement in the priority setting activities of a wait time management initiative: a qualitative case study

    <p>Abstract</p> <p>Background</p> <p>As no health system can afford to provide all possible services and treatments for the people it serves, each system must set priorities. Priority setting decision makers are increasingly involving the public in policy making. This study focuses on public engagement in a key priority setting context that plagues every health system around the world: wait list management. The purpose of this study is to describe and evaluate priority setting for the Ontario Wait Time Strategy, with special attention to public engagement.</p> <p>Methods</p> <p>This study was conducted at the Ontario Wait Time Strategy in Ontario, Canada which is part of a Federal-Territorial-Provincial initiative to improve access and reduce wait times in five areas: cancer, cardiac, sight restoration, joint replacements, and diagnostic imaging. There were two sources of data: (1) over 25 documents (e.g. strategic planning reports, public updates), and (2) 28 one-on-one interviews with informants (e.g. OWTS participants, MOHLTC representatives, clinicians, patient advocates). Analysis used a modified thematic technique in three phases: open coding, axial coding, and evaluation.</p> <p>Results</p> <p>The Ontario Wait Time Strategy partially meets the four conditions of 'accountability for reasonableness'. The public was not directly involved in the priority setting activities of the Ontario Wait Time Strategy. Study participants identified both benefits (supporting the initiative, experts of the lived experience, a publicly funded system and sustainability of the healthcare system) and concerns (personal biases, lack of interest to be involved, time constraints, and level of technicality) for public involvement in the Ontario Wait Time Strategy. Additionally, the participants identified concern for the consequences (sustainability, cannibalism, and a class system) resulting from the Ontario Wait Times Strategy.</p> <p>Conclusion</p> <p>We described and evaluated a wait time management initiative (the Ontario Wait Time Strategy) with special attention to public engagement, and provided a concrete plan to operationalize a strategy for improving public involvement in this, and other, wait time initiatives.</p

    Identification of the Rostral Migratory Stream in the Canine and Feline Brain

    In the adult rodent brain, neural progenitor cells migrate from the subventricular zone of the lateral ventricle towards the olfactory bulb in a track known as the rostral migratory stream (RMS). To facilitate the study of neural progenitor cells and stem cell therapy in large animal models of CNS disease, we now report the location and characteristics of the normal canine and feline RMS. The RMS was found in Nissl-stained sagittal sections of adult canine and feline brains as a prominent, dense, continuous cellular track beginning at the base of the anterior horn of the lateral ventricle, curving around the head of the caudate nucleus and continuing laterally and ventrally to the olfactory peduncle before entering the olfactory tract and bulb. To determine if cells in the RMS were proliferating, the thymidine analog 5-bromo-2-deoxyuridine (BrdU) was administered and detected by immunostaining. BrdU-immunoreactive cells were present throughout this track. The RMS was also immunoreactive for markers of proliferating cells, progenitor cells and immature neurons (Ki-67 and doublecortin), but not for NeuN, a marker of mature neurons. Luxol fast blue and CNPase staining indicated that myelin is closely apposed to the RMS along much of its length and may provide guidance cues for the migrating cells. Identification and characterization of the RMS in canine and feline brain will facilitate studies of neural progenitor cell biology and migration in large animal models of neurologic disease

    AbstractIn view of the increasing complexity of both cardiovascular implantable electronic devices (CIEDs) and patients in the current era, practice guidelines, by necessity, have become increasingly specific. This document is an expert consensus statement that has been developed to update and further delineate indications and management of CIEDs in pediatric patients, defined as ≤21 years of age, and is intended to focus primarily on the indications for CIEDs in the setting of specific disease categories. The document also highlights variations between previously published adult and pediatric CIED recommendations and provides rationale for underlying important differences. The document addresses some of the deterrents to CIED access in low- and middle-income countries and strategies to circumvent them. The document sections were divided up and drafted by the writing committee members according to their expertise. The recommendations represent the consensus opinion of the entire writing committee, graded by class of recommendation and level of evidence. Several questions addressed in this document either do not lend themselves to clinical trials or are rare disease entities, and in these instances recommendations are based on consensus expert opinion. Furthermore, specific recommendations, even when supported by substantial data, do not replace the need for clinical judgment and patient-specific decision-making. The recommendations were opened for public comment to Pediatric and Congenital Electrophysiology Society (PACES) members and underwent external review by the scientific and clinical document committee of the Heart Rhythm Society (HRS), the science advisory and coordinating committee of the American Heart Association (AHA), the American College of Cardiology (ACC), and the Association for European Paediatric and Congenital Cardiology (AEPC). The document received endorsement by all the collaborators and the Asia Pacific Heart Rhythm Society (APHRS), the Indian Heart Rhythm Society (IHRS), and the Latin American Heart Rhythm Society (LAHRS). This document is expected to provide support for clinicians and patients to allow for appropriate CIED use, appropriate CIED management, and appropriate CIED follow-up in pediatric patients

    Magnetic Resonance Thermometry at 7T for Real-Time Monitoring and Correction of Ultrasound Induced Mild Hyperthermia

    While Magnetic Resonance Thermometry (MRT) has been extensively utilized for non-invasive temperature measurement, there is limited data on the use of high field (≥7T) scanners for this purpose. MR-guided Focused Ultrasound (MRgFUS) is a promising non-invasive method for localized hyperthermia and drug delivery. MRT based on the temperature sensitivity of the proton resonance frequency (PRF) has been implemented in both a tissue phantom and in vivo in a mouse Met-1 tumor model, using partial parallel imaging (PPI) to speed acquisition. An MRgFUS system capable of delivering a controlled 3D acoustic dose during real time MRT with proportional, integral, and derivative (PID) feedback control was developed and validated. Real-time MRT was validated in a tofu phantom with fluoroptic temperature measurements, and acoustic heating simulations were in good agreement with MR temperature maps. In an in vivo Met-1 mouse tumor, the real-time PID feedback control is capable of maintaining the desired temperature with high accuracy. We found that real time MR control of hyperthermia is feasible at high field, and k-space based PPI techniques may be implemented for increasing temporal resolution while maintaining temperature accuracy on the order of 1°C
