PGA artrite reumatóide: formulação das questões e relação com DAS28
Trabalho final de mestrado integrado em Medicina (Reumatologia), apresentado à Faculdade de Medicina da Universidade de Coimbra.Introdução: A relevância da avaliação da atividade da Artrite Reumatóide segundo a
perspetiva do doente tem aumentado nos últimos tempos, integrando diferentes índices de
atividade da doença. Em relação à forma como o doente é questionado (Patient Global
Assessment – PGA), várias formulações da questão e período temporal ao qual se reporta esta
avaliação têm sido publicadas na literatura, podendo influenciar a avaliação de atividade e a
decisão terapêutica.
Objectivo: avaliar se a forma como a questão é formulada influencia a resposta do doente e
qual a sua correlação com as outras variáveis do DAS28 e com outras medidas de avaliação
do impacto da doença.
Material e Métodos: Cento e um doentes, com o diagnóstico de AR de acordo com os
Critérios de Classificação ACR 1987 ou ACR/EULAR 2010, seguidos em Consulta de
Reumatologia do Centro Hospitalar e Universitário de Coimbra, completaram de forma
autónoma um questionário que continha sete versões de PGA-VAS em escalas de 100 mm e
duas versões atribuindo um valor de 0 a 100. Avaliaram também fadiga, dor, deformidade e
mobilidade, utilizando escalas visuais 0-100 mm, e a capacidade funcional (HAQ). Pelo
médico assistente foram avaliados os componentes individuais do DAS28. Através do
coeficiente de correlação de Pearson foi avaliada a correlação entre as diferentes PGA-VAS e
entre estas e outras medidas de avaliação do impacto da doença. p<0,05 foi considerado
estatisticamente significativo em todas as análises.
Resultados: As PGA-VAS correlacionam-se fracamente com as outras variáveis do DAS28,
com exceção das articulações dolorosas, numa contagem de 28, em que correlação é
moderada (rho 0.370 a 0.438, p<0.01). Estas correlacionam-se moderada/fortemente com as
outras escalas visuais analógicas (fadiga, dor, deformidade e mobilidade), mas também com a
capacidade funcional do doente (HAQ). Todas as PGA-VAS intercorrelacionam-se moderada/fortemente (rho 0.452 a 0.786, p<0.01), excetuando-se as duas escalas
desenvolvidas pelos investigadores, com a questão realizada na positiva, que têm correlação
fraca ou inexistente com as outras. As diferenças do valor global do DAS28 calculada com as
diversas PGA-VAS é mínima (0.109±0.455).
Conclusão: As diferentes PGA-VAS correlacionam-se moderadamente entre si e com as
outras escalas de avaliação do impacto da doença. O impacto no DAS284vVS utilizando as
várias PGA-VAS foi mínimo. Para uma maior homogenização de resultados recomenda-se a
uniformização da questão da PGA-VAS e a sua validação.Background: Rheumatoid Arthritis´ activity´s evaluation according with the patient´s
perspective as assumed a big relevance, incorporating different rates of the disease’s activity.
In relation to how the patient is questioned (Patient Global Assessment – PGA), various
formulations of the question and the period of in which the evaluation is reported have been
published.
Objective: In this study, we intend to evaluate if the way the phrasing affects the answer given
by the patient and what its correlation with the other parameters of the DAS28 and other
measures to evaluate the disease´s impact.
Methods: One hundred and one patients, who were fulfilling the 1987 ACR or 2010
ACR/EULAR Diagnostic Criteria for the Rheumatoid Arthritis followed in Rheumatology
Department of the Hospital University of Coimbra, completed independently a questionnaire
which had seven versions of the PGA-VAS in a scale of 0-100mm and two versions assigning
a value from 0 to 100. They also evaluated fatigue, pain, deformity and mobility, using visual
scales 0-100mm, and the functional capacity (HAQ). Their medical assistant made an
evaluation, a determination of the activity of their disease (DAS28) and their individual components. All PGA-VAS were evaluated using Pearson's correlation coefficient. This has
also been used to correlate the PGA-VAS with other variables of the disease’s impact. In
allanalysis, p<0.05 was considered statistically significant.
Results: The PGA-VAS correlate weakly with the other variables of the DAS28, with the
exception of tender joints, score of 28, with which the correlation is moderate (rho 0.370 a
0.438, p<0.01). These correlate moderately/ strongly with other VAS (fatigue, pain, deformity
and mobility), but also with the patient's functional ability (HAQ). All PGA-VAS versions
correlate moderately/ strongly with each other (rho 0.452 to 0.786, p<0.01), with the
exception of the two scales developed by the researchers, where the issue is carried out in an
affirmative way, which correlation was weak or inexistent. The difference between the
DAS28 global scoring using all PGA-VAS versions was minimal (0.109±0.455).
Conclusion: The different PGA-VAS correlate each other moderately and with other scales in
which patients self-evaluates of disease impact. The impact in DAS284vVS was minimal. To
increase homogenization in the results, we recommend standardizing the question of PGAVAS
and its validation
Effect of the ingestion of a mousse with cinnamon C. Burmannii on the postprandial blood glucose response of healthy subjects and its antioxidant power
Dissertação para obtenção do grau de Mestre em Nutrição ClínicaBackground: Cinnamon has been shown to reduce postprandial glycaemia and enhance
insulin sensitivity in healthy adults.Aims: To study the effect of C. burmannii on the postprandial blood glucose response of
healthy subjects and its antioxidant capacity in a semi-solid food.Design: Twenty four apparently healthy subjects participated in this study. They were
randomly assigned in group A (reference meal) or group B (test meal). The blood glucose
concentrations were measured before the ingestion of the meals and 30, 60, 90 and 120
minutes after the start of the meal. The test meal used consisted of 100 g of mousse mixed
with 3 g of cinnamon.Results: The addition of 3 g of cinnamon to the mousse had no significant effect in blood
glucose response in terms of the areas under the curve (AUC) and in the different
postprandial times (p>0,05). The mean Cmax was significantly lower after the ingestion of
the reference meal than after the ingestion of the mousse with 3 g of C.burmannii (96
mg/dl VS 104,42 mg/dl; p=0,011). The chemical analysis showed that the mousse with 3 g
of cinnamon has a much higher phenolic content and antioxidant capacity than the mousse
without cinnamon.Conclusions: The inclusion of cinnamon in the mousse increased the antioxidant capacity
of this semi-solid food, however it did not reduce the postprandial glucose response in
healthy subjects
Can family ownership influence firms' capital structure decisions?
The purpose of the present study is to endeavor the explanatory capacity of family ownership in
determining the capital structure of the firms. Having this purpose in mind, we collect and
analyze information for the period 2005-2013, regarding a sample of 194 family and non-family
businesses, whose headquarters' location is in either European or North American countries. We
obtain empirical evidence to conclude that i) non-family firms present higher leverage ratios
compared to their family peers; and ii) non-family firms rely more on long-term debt than their
family peers. Our study adds value to previous research since it compares companies from two
different continents, (North) America and Europe, it analyzes how different firm characteristics
may influence both leverage ratio and long-term debt to total debt and finally because it studies
the impact of the financial crisis on the firms' financial results

O presente trabalho tem como objetivo desenvolver um plano de marketing estratégico verde para lançar no mercado a Xwatts, uma empresa digital 100% Eco-friendly que integra na sua filosofia corporativa um conjunto de políticas ambientais sólidas e de confiança que visam poupar os recursos energéticos e conduzir os consumidores a soluções menos nocivas para a sua saúde e para o meio ambiente. Para tal é necessário perceber quem são os consumidores verdes e o que procuram quando adquirem os produtos. A articulação entre as necessidades de mercado e a oferta de uma empresa é fundamental para o desenho de um plano de marketing estratégico.

The Indirect Effect of the Mindful Parenting of Mothers and Fathers on the Child’s Perceived Well-Being Through the Child’s Attachment to Parents
Portuguese Familial Hypercholesterolaemia Study
A Hipercolesterolemia Familiar (FH) é uma doença genética associada a
um elevado risco cardiovascular. Doentes com FH possuem valores muito
elevados de colesterol no plasma, desde o nascimento. Até à data 3 genes
foram associados à FH: LDLR (85-90%), APOB (5-8%) e PCSK9 (1-2%). Em
1999 foi estabelecido, no Instituto Nacional de Saúde Doutor Ricardo Jorge,
o Estudo Português de Hipercolesterolemia Familiar (EPHF). Este estudo de
investigação tem como objetivo principal identificar a causa genética da
dislipidemia em doentes com critérios clínicos de FH. O EPHF identificou
molecularmente 718 doentes heterozigotos com uma variante patogénica
ou provavelmente patogénica (segundo as diretrizes da ACMG) num dos 3
genes associados à FH. Adicionalmente, 90 indivíduos possuem uma das
35 variantes de significado incerto cuja patogenicidade necessita de ser
avaliada através de estudos funcionais. Foram também identificados 10 homozigotos
(3 homozigotos verdadeiros e 7 heterozigotos compostos), com
variantes patogénicas nos genes LDLR e PCSK9. O EPHF conseguiu identificar
3,8% dos portugueses que se calcula terem FH, colocando Portugal
entre os dez países com mais doentes identificados. O risco cardiovascular
dos doentes com FH é determinado pelos valores elevados de colesterol
que os doentes apresentam desde o nascimento, mas também pela patogenicidade
da variante identificada. A identificação precoce dos doentes
com FH, através do diagnóstico genético, permite ao clínico implementar
medidas terapêuticas adequadas e mais agressivas, de modo a diminuir o
risco cardiovascular inerente a estes doentes.Familial Hypercholesterolemia (FH) is a genetic disorder associated with
a high cardiovascular risk. Patients with FH have very high plasma cholesterol
values since birth. Until now three genes have been associated to FH:
LDLR (85-90%), APOB (5-8%) and PCSK9 (1-2%). In 1999 the Portuguese
FH (PFHS) Study was established at the National Institute of Health. This
study aims to identify the genetic cause of dyslipidemia in patients with
clinical diagnosis of FH. The Portuguese FH Study genetically identified 718
heterozygous patients with a pathogenic or probably pathogenic variant
(according to ACMG guidelines) in one of the 3 genes associated to FH. In
addition, 90 individuals have one of 35 variants of uncertain significance
whose pathogenicity needs to be assessed through functional studies. Ten
homozygotes (3 true homozygotes and 7 heterozygous compounds) were
also identified, with pathogenic variants in the LDLR and PCSK9 genes.
The PFHS was able to identify 3.8% of the Portuguese that are estimated to
have FH, placing Portugal among the ten countries with the most patients
identified. The cardiovascular risk of FH patients is determined by the
high cholesterol values present since birth, but also by the pathogenicity
of the identified variant. The early identification of these patients through
genetic diagnosis allows the clinician to implement appropriate and more
aggressive therapeutic measures in order to reduce the cardiovascular risk

Organização e gestão da formação num centro da indústria farmacêutica : o sistema de acreditação de entidades formadoras
Relatório de estágio de mestrado, Educação e Formação (Área de especialidade em Organização e Gestão da Educação e da Formação), Universidade de Lisboa, Instituto de Educação, 2018O presente relatório de estágio, no âmbito do Mestrado em Educação e Formação, na especialidade de em Organização e Gestão da Educação e da Formação, evidencia o trabalho desenvolvido no estágio curricular no Centro de Estudos Superiores da Indústria Farmacêutica (CESIF).
A estrutura do relatório está subdividida em cinco diferentes momentos: o primeiro remete para um enquadramento teórico relativo à investigação realizada, o segundo foca-se essencialmente na caracterização do CESIF, o terceiro apresenta o estudo desenvolvido, o quatro descreve todas as atividades desenvolvidas no estágio curricular e, no último, inclui-se a apresentação dos resultados obtidos.
O CESIF é uma entidade privada dedicada à formação de cursos de formação contínua na indústria farmacêutica e tem como principal objetivo promover a formação de profissionais especializados em diferentes técnicas de gestão integradas que trabalham ou desejam trabalhar em diversas entidades do mercado de trabalho.
O estágio teve a duração de oito meses e visou a realização de um estudo sobre a organização e gestão da formação num centro de formação da indústria farmacêutica, considerando o sistema de acreditação aplicado na mesma.
A análise apresentada nos diferentes capítulos surgiu de um conjunto de processos metodológicos, nomeadamente a pesquisa bibliográfica e documental utilizada para a análise de conceitos que estão enquadrados no primeiro capítulo e para recolha de documentos da empresa, que auxiliaram mais tarde na discussão de resultados; a observação participante que suportou a caraterização do funcionamento quotidiano da organização; duas entrevistas utilizadas para a caracterização da organização, bem como para a compreensão do sistema de acreditação utilizado, e por último, a análise de conteúdo dos resultados do estudo.
Através dos diferentes registos anteriormente mencionados e com experiência adquirida ao longo do tempo, foi possível conhecer com algum pormenor todo o sistema de certificação de empresas formadoras, bem como o próprio sistema de certificação utilizado pelo CESIF. Foi possível também refletir de um modo crítico sobre todo o trabalho desenvolvido referente ao processo de certificação na instituição de estágio.This report, accomplished in the Education and Training Master, specialisation in Education and Training Organisation and Management, reflects the work developed during a student internship at Centro de Estudos Superiores da Indústria Farmacêutica (CESIF).
The structure of this report is divided in five distinct sections: the first refers to the theoretical framework in regards to the research made, the second focuses essentially on the characterisation of CESIF, the third presents the study developed, the fourth describes all the implemented during the internship and, in the last section, the presentation of the final results is included.
CESIF is a private entity dedicated to the development of continuing education and training courses in pharmaceutical issues and its main objective is to promote the qualification of professionals specialised in different management techniques integrated in various entities that constitute the job market.
The internship occurred in a period of eight months, with the aim of developing a study on the training organisation and management in a training pharmaceutical industry centre, while considering the accreditation system applied.
The analysis presented in the different chapters originated from a set of methodological processes, namely bibliographical and documentary research used for the analysis of concepts that were elaborated in the first chapter as well as for the retrieval of the company’s documents, which subsequently supported the presentation of the final results; the participant observation that supported the description of the regular processes that occured in the company; two interviews used not only to characterise the company, but also to understand the accreditation system used, and finally, the content analysis of the study results.
Through the different methods mentioned above and with the experience acquired over time, it was possible to discern with sufficient detail the entire certification system of training companies, as well as the certification system used by CESIF itself. It was also possible to analyse and reflect on all the work developed regarding the certification process in the internship’s establishment
