Multimorbidity clusters in patients with chronic obstructive airway diseases in the EpiChron Cohort
Chronic obstructive airway diseases such as chronic obstructive pulmonary disease (COPD), asthma, rhinitis, and obstructive sleep apnea (OSA) are amongst the most common treatable and preventable chronic conditions with high morbidity burden and mortality risk. We aimed to explore the existence of multimorbidity clusters in patients with such diseases and to estimate their prevalence and impact on mortality. We conducted an observational retrospective study in the EpiChron Cohort (Aragon, Spain), selecting all patients with a diagnosis of allergic rhinitis, asthma, COPD, and/or OSA. The study population was stratified by age (i.e., 15–44, 45–64, and = 65 years) and gender. We performed cluster analysis, including all chronic conditions recorded in primary care electronic health records and hospital discharge reports. More than 75% of the patients had multimorbidity (co-existence of two or more chronic conditions). We identified associations of dermatologic diseases with musculoskeletal disorders and anxiety, cardiometabolic diseases with mental health problems, and substance use disorders with neurologic diseases and neoplasms, amongst others. The number and complexity of the multimorbidity clusters increased with age in both genders. The cluster with the highest likelihood of mortality was identified in men aged 45 to 64 years and included associations between substance use disorder, neurologic conditions, and cancer. Large-scale epidemiological studies like ours could be useful when planning healthcare interventions targeting patients with chronic obstructive airway diseases and multimorbidity
Chronic Obstructive Pulmonary Disease (COPD) as a disease of early aging: Evidence from the EpiChron Cohort
Background Aging is an important risk factor for most chronic diseases. Patients with COPD develop more comorbidities than non-COPD subjects. We hypothesized that the development of comorbidities characteristically affecting the elderly occur at an earlier age in subjects with the diagnosis of COPD. Methods and findings We included all subjects carrying the diagnosis of COPD (n = 27, 617), and a similar number of age and sex matched individuals without the diagnosis, extracted from the 727, 241 records of individuals 40 years and older included in the EpiChron Cohort (Aragon, Spain). We compared the cumulative number of comorbidities, their prevalence and the mortality risk between both groups. Using network analysis, we explored the connectivity between comorbidities and the most influential comorbidities in both groups. We divided the groups into 5 incremental age categories and compared their comorbidity networks. We then selected those comorbidities known to affect primarily the elderly and compared their prevalence across the 5 age groups. In addition, we replicated the analysis in the smokers'' subgroup to correct for the confounding effect of cigarette smoking. Subjects with COPD had more comorbidities and died at a younger age compared to controls. Comparison of both cohorts across 5 incremental age groups showed that the number of comorbidities, the prevalence of diseases characteristic of aging and network''s density for the COPD group aged 56-65 were similar to those of non-COPD 15 to 20 years older. The findings persisted after adjusting for smoking. Conclusion Multimorbidity increases with age but in patients carrying the diagnosis of COPD, these comorbidities are seen at an earlier age
Zooplankton Biomass Depletion Event Reveals the Importance of Small Pelagic Fish Top-Down Control in the Western Mediterranean Coastal Waters
The influence of hydrochemistry and trophic conditions on the coastal zooplankton community’s biomass and metabolic activities was investigated along the Spanish Mediterranean coastal waters, from Algeciras Bay to Barcelona, from autumn 2011 to autumn 2012. Two hydrographic regions were differentiated: NW Alboran (ALB) and W Mediterranean (MED). Zooplankton metabolism was assessed from measurements of the electron transport system (ETS) and aminoacyl-tRNA synthetases (AARS) activities, as proxies for potential respiration and somatic growth, respectively. Zooplankton showed three to fivefold higher biomass in ALB than in MED during autumn 2011 and spring 2012. However, in autumn 2012, a drastic decrease in biomass standing stock was observed in ALB, with no significant differences between the two regions. This biomass depletion event was not associated with environmental variables, food availability or zooplankton metabolic rates, but coincided with a twofold peak of Sardina pilchardus landings in ALB. A reduced standing stock coupled with high zooplankton growth rates suggests mortality by predation as the main cause for the low zooplankton biomass typically observed in MED, and in ALB during autumn 2012.Versión del edito

Poor sleep quality decreases concurrent training benefits in markers of metabolic syndrome and quality of life of morbidly obese patients
Indexación: Scopus.Background: Sleep quality (SQ) plays a role in multiple activities of daily living, but little is known about its role in concurrent training [CT, high-intensity interval (HIIT) plus resistance training (RT)] adaptations for metabolic syndrome (MetS) and health-related quality of life (HRQoL) markers. The aim of the present study was to determine the effects of a 20-week CT programme on MetS and HRQoL markers according to the SQ of morbidly obese patients. Methods: Twenty-nine morbidly obese patients were allocated to one of two groups: good sleep quality (GSQ, n = 15, 38.07 ± 12.26 years) and poor sleep quality (PSQ, n = 14, 40.79 ± 11.62 years). HRQoL, body mass index, waist circumference (WC), systolic and diastolic blood pressure (SBP and DBP, respectively), and plasma outcomes were measured. Results: The GSQ group reported significant changes (pre-vs. post-intervention) in WC (114.0 ± 3.1 vs. 110.4 ± 3.4 cm, p = 0.012), SBP (137.0 ± 4.3 vs. 125.6 ± 1.8 mmHg, p = 0.006), and HRQoL general health (51.33 ± 21.08 vs. 64.33 ± 16.24, p = 0.020). By contrast, the PSQ group showed significant changes only in SQ (9.00 ± 2.42 vs. 5.36 ± 2.84, p = 0.004). Conclusions: Morbidly obese PSQ patients showed a lower response for improving MetS and HRQoL markers after a 20-week CT programme than GSQ peers. However, there was a greater effect size for decreasing WC and SBP in favour of the GSQ compared with the PSQ group, suggesting that there are limitations to CT benefits on these outcomes in the PSQ group. Unfolding of differential energy spectra in the MAGIC experiment
The paper describes the different methods, used in the MAGIC experiment, to
unfold experimental energy distributions of cosmic ray particles (gamma-rays).
Questions and problems related to the unfolding are discussed. Various
procedures are proposed which can help to make the unfolding robust and
reliable. The different methods and procedures are implemented in the MAGIC

Implementation of the Random Forest Method for the Imaging Atmospheric Cherenkov Telescope MAGIC
The paper describes an application of the tree classification method Random
Forest (RF), as used in the analysis of data from the ground-based gamma
telescope MAGIC. In such telescopes, cosmic gamma-rays are observed and have to
be discriminated against a dominating background of hadronic cosmic-ray
particles. We describe the application of RF for this gamma/hadron separation.
The RF method often shows superior performance in comparison with traditional
semi-empirical techniques. Critical issues of the method and its implementation
are discussed. An application of the RF method for estimation of a continuous
parameter from related variables, rather than discrete classes, is also

Probing quantum gravity using photons from a flare of the active galactic nucleus Markarian 501 observed by the MAGIC telescope
We analyze the timing of photons observed by the MAGIC telescope during a
flare of the active galactic nucleus Mkn 501 for a possible correlation with
energy, as suggested by some models of quantum gravity (QG), which predict a
vacuum refractive index \simeq 1 + (E/M_{QGn})^n, n = 1,2. Parametrizing the
delay between gamma-rays of different energies as \Delta t =\pm\tau_l E or
\Delta t =\pm\tau_q E^2, we find \tau_l=(0.030\pm0.012) s/GeV at the 2.5-sigma
level, and \tau_q=(3.71\pm2.57)x10^{-6} s/GeV^2, respectively. We use these
results to establish lower limits M_{QG1} > 0.21x10^{18} GeV and M_{QG2} >
0.26x10^{11} GeV at the 95% C.L. Monte Carlo studies confirm the MAGIC
sensitivity to propagation effects at these levels. Thermal plasma effects in
the source are negligible, but we cannot exclude the importance of some other

Olber's Paradox for Superluminal Neutrinos: Constraining Extreme Neutrino Speeds at TeV-ZeV Energies with the Diffuse Neutrino Background
The only invariant speed in special relativity is c; therefore, if some
neutrinos travel at even tiny speeds above c, normal special relativity is
incomplete and any superluminal speed may be possible. I derive a limit on
superluminal neutrino speeds v >> c at high energies by noting that such speeds
would increase the size of the neutrino horizon. The increased volume of the
Universe visible leads to a brighter astrophysical neutrino background. The
nondetection of "guaranteed" neutrino backgrounds from star-forming galaxies
and ultrahigh energy cosmic rays (UHECRs) constrains v/c at TeV--ZeV energies.
I find that v/c <= 820 at 60 TeV from the nondetection of neutrinos from
star-forming galaxies. The nondetection of neutrinos from UHECRs constrains v/c
to be less than 2500 at 0.1 EeV in a pessimistic model and less than 4.6 at 4
EeV in an optimistic model. The UHECR neutrino background nondetection is
strongly inconsistent with a naive quadratic extrapolation of the OPERA results
to EeV energies. The limits apply subject to some caveats, particularly that
the expected pionic neutrino backgrounds exist and that neutrinos travel faster
than c when they pass the detector. They could be improved substantially as the
expected neutrino backgrounds are better understood and with new experimental
neutrino background limits. I also point out that extremely subluminal speeds
would result in a much smaller neutrino background intensity than expected.Comment: 13 pages, 2 figures, fixed titl
