    International Students, University Health Centers, and Memorable Messages About Health

    International students entering US universities often experience a variety of important socialization messages. One important message is learning about and using the US health system. International students often first encounter the US health system through their experiences with university health centers. The authors explore the memorable messages international students receive about using university health centers. Participants identified three important messages: (a) the insurance imperative, (b) triaging your own health, and (c) passing the TB “test”. These messages help to socialize international students, but also create confusion because of ambiguity and assumption of previous knowledge

    Waging a professional turf war: an examination of professionalization as a strategic communication practice used by registered dietitians

    In 2012, the American Dietetic Association changed its name to the Academy of Nutrition and Dietetics. The following year, the association allowed their members to select between two titles: registered dietitian (RD) or registered dietitian nutritionist. Based on interviews with dietitians, we argue that these semantic changes added fuel to a pre-existing discursive struggle for professional legitimacy. As of June 2017, there were over 100,000 registered dietetic practitioners, globally. Approximately 74% of RDs are members of the Academy, a notably high percent of representation, suggesting the influence of this professional organization on the occupation. Academy members work in a variety of occupational settings and fields, including hospitals, nonprofit organizations, and the food industry, and as such make significant contributions to patient health and public health outcomes.The Academy and its members have established a discursive turf war to strategically establish and defend boundaries within the field of nutrition and dietetics. Their discursive turf war has implications on the day-to-day work life of registered dietitians and other nutritional professionals as well as perceptions of professionalism made by audiences outside the field

    Storm-induced upwelling of high pCO2 waters onto the continental shelf of the western Arctic Ocean and implications for carbonate mineral saturation states

These processes are dynamically forcing the region, with unknown consequences for CO2 fluxes and carbonate mineral saturation states, particularly in the coastal regions where sensitive ecosystems are already under threat from multiple stressors. In October 2011, persistent wind-driven upwelling occurred in open water along the continental shelf of the Beaufort Sea in the western Arctic Ocean. During this time, cold (32.4) halocline water—supersaturated with respect to atmospheric CO2 (pCO2 > 550 μatm) and undersaturated in aragonite (Ωaragonite < 1.0) was transported onto the Beaufort shelf. A single 10-day event led to the outgassing of 0.18–0.54 Tg-C and caused aragonite undersaturations throughout the water column over the shelf. If we assume a conservative estimate of four such upwelling events each year, then the annual flux to the atmosphere would be 0.72–2.16 Tg-C, which is approximately the total annual sink of CO2 in the Beaufort Sea from primary production. Although a natural process, these upwelling events have likely been exacerbated in recent years by declining sea ice cover and changing atmospheric conditions in the region, and could have significant impacts on regional carbon budgets.     Secular sea level change in the Russian sector of the Arctic Ocean

It reflects changes in practically all dynamic and thermodynamic processes of terrestrial, oceanic, atmospheric, and cryospheric origin. The use of estimates of sea level rise as an indicator of climate change therefore incurs the difficulty that the inferred sea level change is the net result of many individual effects of environmental forcing. Since some of these effects may offset others, the cause of the sea level response to climate change remains somewhat uncertain. This paper is focused on an attempt to provide first-order answers to two questions, namely, what is the rate of sea level change in the Arctic Ocean, and furthermore, what is the role of each of the individual contributing factors to observed Arctic Ocean sea level change? In seeking answers to these questions we have discovered that during the period 1954–1989 the observed sea level over the Russian sector of the Arctic Ocean is rising at a rate of approximately 0.123 cm yr−1 and that after correction for the process of glacial isostatic adjustment this rate is approximately 0.185 cm yr−1. There are two major causes of this rise. The first is associated with the steric effect of ocean expansion. This effect is responsible for a contribution of approximately 0.064 cm yr−1 to the total rate of rise (35%). The second most important factor is related to the ongoing decrease of sea level atmospheric pressure over the Arctic Ocean, which contributes 0.056 cm yr−1, or approximately 30% of the net positive sea level trend. A third contribution to the sea level increase involves wind action and the increase of cyclonic winds over the Arctic Ocean, which leads to sea level rise at a rate of 0.018 cm yr−1 or approximately 10% of the total. The combined effect of the sea level rise due to an increase of river runoff and the sea level fall due to a negative trend in precipitation minus evaporation over the ocean is close to 0. For the Russian sector of the Arctic Ocean it therefore appears that approximately 25% of the trend of 0.185 cm yr−1, a contribution of 0.048 cm yr−1, may be due to the effect of increasing Arctic Ocean mass.This material is based upon work supported by the National Science Foundation under grant 0136432

    A randomised controlled trial to evaluate the efficacy of a 6 month dietary and physical activity intervention for prostate cancer patients receiving androgen deprivation therapy

    <p>Abstract</p> <p>Background</p> <p>Treatment with Androgen Deprivation Therapy (ADT) for prostate cancer is associated with changes in body composition including increased fat and decreased lean mass; increased fatigue, and a reduction in quality of life. No study to date has evaluated the effect of dietary and physical activity modification on the side-effects related to ADT. The aim of this study is to evaluate the efficacy of a 6-month dietary and physical activity intervention for prostate cancer survivors receiving ADT to minimise the changes in body composition, fatigue and quality of life, typically associated with ADT.</p> <p>Methods</p> <p>Men are recruited to this study if their treatment plan is to receive ADT for at least 6 months. Men who are randomised to the intervention arm receive a home-based tailored intervention to meet the following guidelines a) ≥ 5 servings vegetables and fruits/day; b) 30%-35% of total energy from fat, and < 10% energy from saturated fat/day; c) 10% of energy from polyunsaturated fat/day; d) limited consumption of processed meats; e) 25-35 gm of fibre/day; f) alcoholic drinks ≤ 28 units/week; g) limited intake of foods high in salt and/or sugar. They are also encouraged to include at least 30 minutes of brisk walking, 5 or more days per week. The primary outcomes are change in body composition, fatigue and quality of life scores. Secondary outcomes include dietary intake, physical activity and perceived stress. Baseline information collected includes: socio-economic status, treatment duration, perceived social support and health status, family history of cancer, co-morbidities, medication and supplement use, barriers to change, and readiness to change their health behaviour. Data for the primary and secondary outcomes will be collected at baseline, 3 and 6 months from 47 intervention and 47 control patients.</p> <p>Discussion</p> <p>The results of this study will provide detailed information on diet and physical activity levels in prostate cancer patients treated with ADT and will test the feasibility and efficacy of a diet and physical activity intervention which could provide essential information to develop guidelines for prostate cancer patients to minimise the side effects related to ADT.</p> <p>Trial registration</p> <p>ISRCTN trial number ISCRTN75282423</p

    Video-assisted thoracic surgery (VATS) for resection of metastatic adenocarcinoma as an acceptable alternative

    Adenocarcinomas commonly metastasize to the lungs and can be resected using open thoracotomy or video-assisted thoracic surgery (VATS). This study reviews metastatic resections in primary adenocarcinoma patients, using both thoracotomy and VATS. We aim to compare long-term prognoses to test the efficacy and viability of VATS. A retrospective review of primary adenocarcinoma patients who underwent resection of pulmonary metastases from 1990 to 2006 was carried out. Information was obtained by chart review. Endpoints analyzed were disease-free interval (DFI), survival time, and recurrence-free survival (RFS). In a total of 42 (16 male, 26 female; median age 58.5 years) primary adenocarcinoma patients, 21 patients underwent first pulmonary metastatic resection using VATS (7 male, 14 female; median age 57 years) and 21 using thoracotomy (9 male, 12 female; median age 59 years). Primary adenocarcinomas were mainly 27 colorectal (64%) and 11 breast (26%). Two VATS (10%) and three open patients (14%) had local recurrences of the original cancer. Median postoperative follow was 13.3 months [interquartile range (IQR) 4.5–32.8 months] for VATS and 36.9 months (IQR 19.3–48.6 months) after thoracotomy. Median DFI–1 was 22.3 months (IQR 13.5–40.6 months) for VATS patients and 35.6 months (IQR 26.7–61.3 months) for open patients. Second thoracic occurrences were noted in six VATS patients (median DFI–2 9.2 months), and in seven open patients (median DFI-2 21.5 months). Third thoracic occurrences were noted in one VATS patient (DFI-3 18.7 months) and in one thoracotomy patient (DFI-3 21.8 months). Odds ratio of recurrence showed 12.5% less chance of developing recurrence in VATS patients. Five-year RFS was 53% in VATS and 57% in thoracotomy patients. VATS has become a viable alternative to open thoracotomy for resection of pulmonary metastases. In cases of primary adenocarcinoma, VATS showed no increase in number of thoracic recurrences, and comparable RFS. Short-term follow-up is encouraging; long-term follow-up will be needed to confirm these results

    Usefulness and engagement with a guided workbook intervention (WorkPlan) to support work related goals among cancer survivors

    Background: Returning to work after cancer is associated with improved physical and psychological functioning, but managing this return can be a challenging process. A workbook based intervention (WorkPlan) was developed to support return-to-work among cancer survivors. The aim of this study was to explore how participants using the workbook engaged with the intervention and utilised the content of the intervention in their plan to return-to-work. Methods: As part of a feasibility randomised controlled trial, 23 participants from the intervention group were interviewed 4-weeks post intervention. Interviews focussed on intervention delivery and data was analysed using Framework analysis. Results: Participants revealed a sense of empowerment and changes in their outlook as they transitioned from patient to employee, citing the act of writing as a medium for creating their own return-to-work narrative. Participants found the generation of a return-to-work plan useful for identifying potential problems and solutions, which also served as a tool for aiding discussion with the employer on return-to-work. Additionally, participants reported feeling less uncertain and anxious about returning to work. Timing of the intervention in coordination with ongoing cancer treatments was crucial to perceived effectiveness; participants identified the sole or final treatment as the ideal time to receive the intervention. Conclusions: The self-guided workbook supports people diagnosed with cancer to build their communication and planning skills to successfully manage their return-to-work. Further research could examine how writing plays a role in this process

    Ocean currents shape the microbiome of Arctic marine sediments

    Prokaryote communities were investigated on the seasonally stratified Alaska Beaufort Shelf (ABS). Water and sediment directly underlying water with origin in the Arctic, Pacific or Atlantic oceans were analyzed by pyrosequencing and length heterogeneity-PCR in conjunction with physicochemical and geographic distance data to determine what features structure ABS microbiomes. Distinct bacterial communities were evident in all water masses. Alphaproteobacteria explained similarity in Arctic surface water and Pacific derived water. Deltaproteobacteria were abundant in Atlantic origin water and drove similarity among samples. Most archaeal sequences in water were related to unclassified marine Euryarchaeota. Sediment communities influenced by Pacific and Atlantic water were distinct from each other and pelagic communities. Firmicutes and Chloroflexi were abundant in sediment, although their distribution varied in Atlantic and Pacific influenced sites. Thermoprotei dominated archaea in Pacific influenced sediments and Methanomicrobia dominated in methane-containing Atlantic influenced sediments. Length heterogeneity-PCR data from this study were analyzed with data from methane-containing sediments in other regions. Pacific influenced ABS sediments clustered with Pacific sites from New Zealand and Chilean coastal margins. Atlantic influenced ABS sediments formed another distinct cluster. Density and salinity were significant structuring features on pelagic communities. Porosity co-varied with benthic community structure across sites and methane did not. This study indicates that the origin of water overlying sediments shapes benthic communities locally and globally and that hydrography exerts greater influence on microbial community structure than the availability of methane
