    Tétanos compliqué de septicémie sur hémoglobinopathie majeure

    Rapporter un cas de tétanos compliqué de septicémie chez un enfant souffrant d'une hémoglobinopathie majeure. Enfant de 9 ans drépanocytaire (SC), non vacciné contre le tétanos, a été admis au CHU YO pour raideur de la nuque, difficulté à l'ouverture de la bouche et hyperthermie. L'examen à son admission notait un syndrome infectieux avec une hyperthermie (température à 39°1C), Pouls=100/mn, Fréquence cardiaque=100batt/mn, fréquence respiratoire=30cycles/mn, poids =22Kg, un trismus, une contracture des muscles para vertébraux, des paroxysmes toniques à la palpation (stade II de Mollaret), un syndrome méningé. L'examen n'avait pas retrouvé une porte d'entrée. La ponction lombaire avait ramené un liquide céphalo rachidien clair ; l'hémogramme avait montré une hyperleucocytose à 10 200/mm3, et l'hémoculture a permis d'isoler Staphylococcus aureus. Sous antibiothérapie et sous sédatifs, l'évolution a été favorable. Les porteurs d'hémoglobinopathie majeure sont plus exposés aux infections. La physiopathologie de ces infections s'explique par l'immunodépression et les troubles de la phagocytose.Un dépistage précoce de ces hémoglobinopathies, un bon suivi et une bonne couverture vaccinale des enfants drépanocytaires sont essentiels pour prévenir le tétanos et ses complications

    Effect of temperature variations on the bond behavior of FRCM applied to masonry

    In the last decades, Fiber Reinforced Cementitious Matrix (FRCM) composites were successfully introduced to repair and strengthen existing masonry structures. The good mechanical performances of these materials determined their efficiency as a strengthening technique; however, their durability is still an open issue. As a matter of fact, FRCM composites may be exposed to a combination of different environmental conditions and, additionally, to temperature variations due to solar radiation. The objective of this research was to study the effects of temperature variations on the bond behavior of a FRCM composite, constituted by a basalt grid and a lime-based mortar matrix, applied to masonry. For this purpose, an experimental investigation on thermally conditioned FRCM-strengthened masonry wallets is presented, in which 14 single-lap shear tests were performed. Before testing, samples were exposed to different target temperatures inside a climatic chamber: 32, 40, 50, 60 and 80 degrees C. Thermocouples were embedded within the FRCM reinforcing layers at two different depths to detect the inner temperature profiles and to control the conditioning process. The single-lap shear tests were then carried out inside the same climatic chamber, while maintaining the target temperature constant. A decrease in terms of peak-axial stress was observed by increasing temperature, along with a progressive change in the failure mode, from fiber rupture outside the bonded area to fiber slippage within the mortar matrix layers

    A Wideband Tightly Coupled Array for Omnidirectional Pattern Synthesis

    This work presents the design of a wideband circular tightly coupled array for omnidirectional pattern generation. The proposed array works over an 8:1 bandwidth, providing very high performance in terms of active reflection coefficient, realized gain, and omnidirectional patterns with reduced ripples. The array is composed of 32 low-profile, self-complementary, bow-tie elements displaced around a metallic mast. The capacitive coupling between neighbouring elements and the implementation of a properly designed frequency selective surface allow for increasing the electrical length of each antenna while compensating for the inductive effect of the metallic mast, according to the tightly coupled and connected arrays theory. A cover layer is then inserted to improve the average gain on the azimuthal plane. Time-domain simulation results confirm the validity of the proposed approach and implementation

    MicroRNA-155 influences B-cell function through PU.1 in rheumatoid arthritis

    MicroRNA-155 (miR-155) is an important regulator of B cells in mice. B cells have a critical role in the pathogenesis of rheumatoid arthritis (RA). Here we show that miR-155 is highly expressed in peripheral blood B cells from RA patients compared with healthy individuals, particularly in the IgD-CD27- memory B-cell population in ACPA+ RA. MiR-155 is highly expressed in RA B cells from patients with synovial tissue containing ectopic germinal centres compared with diffuse synovial tissue. MiR-155 expression is associated reciprocally with lower expression of PU.1 at B-cell level in the synovial compartment. Stimulation of healthy donor B cells with CD40L, anti-IgM, IL-21, CpG, IFN-α, IL-6 or BAFF induces miR-155 and decreases PU.1 expression. Finally, inhibition of endogenous miR-155 in B cells of RA patients restores PU.1 and reduces production of antibodies. Our data suggest that miR-155 is an important regulator of B-cell activation in RA

    We introduce our methodological study with a short review of the main literature on embodied language, including some recent studies in neuroscience. We investigated this component of natural language using Recurrence Quantification Analysis (RQA). RQA is a relatively new statistical methodology, particularly effective in complex systems. RQA provided a reliable quantitative description of recurrences in text sequences at the orthographic level. In order to provide examples of the potential impact of this methodology, we used RQA to measure structural coupling and synchronization in natural and clinical verbal interactions.     Dirofilaria repens in Vietnam: Detection of 10 Eye and Subcutaneous Tissue Infection Cases Identified by Morphology and Molecular Methods

    From 2006 to 2010, hospitals in Hanoi treated 10 human patients for dirofilariasis. The worms were collected from parasitic places, and identification of the species was completed by morphology and molecular methods. Ten parasites were recovered either from the conjunctiva (n=9) or subcutaneous tissue (n=1). The parasites were 4.0-12.5 cm in length and 0.5-0.6 mm in width. Morphological observations suggested all parasites as Dirofilaria repens. Three of the 10 parasites (1 from subcutaneous tissue and 2 from eyes) were used for molecular confirmation of the species identification. A portion of the mitochondrial cox1 (461 bp) was amplified and sequenced. Nucleotide and amino acid homologies were 95% and 99-100%, respectively, when compared with D. repens (Italian origin, GenBank AJ271614; DQ358814). This is the first report of eye dirofilariasis and the second report of subcutaneous tissue dirofilariasis due to D. repens in Vietnam

    O presente artigo visa contribuir com a formulação de um modelo de análise dentro da perspectiva conceitual que considera os elementos da esfera pública habermasiana como fundamentos para o conceito de gestão social. O trabalho se propõe a analisar o processo, ainda incompleto, de criação de uma reserva extrativista no litoral sul do Estado de Santa Catarina. Essa análise será realizada utilizando os critérios para a avaliação de processos decisórios participativos deliberativos propostos por Tenório (2008). O artigo busca dar sustentação teórica para a utilização de tais critérios concebidos para avaliação de intervenções formais, em uma situação de intervenção ainda informal, por meio do conceito de esfera pública literária presente na obra “Mudança Estrutural da Esfera Pública” de Jürgen Habermas. O método proposto permitiu a inferência de que o grupo promotor da política pública atua como um espaço de raciocínio público em processo de autocompreensão

    Carbohydrate Mouth Rinse Mitigates Mental Fatigue Effects on Maximal Incremental Test Performance, but Not in Cortical Alterations

    Detrimental mental fatigue effects on exercise performance have been documented in constant workload and time trial exercises, but effects on a maximal incremental test (MIT) remain poorly investigated. Mental fatigue-reduced exercise performance is related to an increased effort sensation, likely due to a reduced prefrontal cortex (PFC) activation and inhibited spontaneous behavior. Interestingly, only a few studies verified if centrally active compounds may mitigate such effects. For example, carbohydrate (CHO) mouth rinse potentiates exercise performance and reduces effort sensation, likely through its effects on PFC activation. However, it is unknown if this centrally mediated effect of CHO mouth rinse may mitigate mental fatigue-reduced exercise performance. After a proof-of-principle study, showing a mental fatigue-reduced MIT performance, we observed that CHO mouth rinse mitigated MIT performance reductions in mentally fatigued cyclists, regardless of PFC alterations. When compared to placebo, mentally fatigued cyclists improved MIT performance by 2.24–2.33 when rinsing their mouth with CHO during MIT. However, PFC and motor cortex activation during MIT in both CHO and placebo mouth rinses were greater than in mental fatigue. Results showed that CHO mouth rinse mitigated the mental fatigue-reduced MIT performance, but challenged the role of CHO mouth rinse on PFC and motor cortex activation
