    A tunable microwave cavity containing ionizable metallic vapor or gases and an apparatus for precisely positioning a microwave coupling tip in the cavity and for precisely adjusting at least one dimension of the cavity are disclosed. With this combined structure, resonance may be achieved with various types of ionizable gases. A coaxial probe extends into a microwave cavity through a tube. One end of the tube is retained in a spherical joint attached in the cavity wall. This allows the coaxial probe to be pivotally rotated. The coaxial probe is slideable within the tube thus allowing the probe to be extended toward or retracted from the center of the cavity

    Three point lead screw positioning apparatus for a cavity tuning plate

    Three lead screws are provided for adjusting the position of a traversing plate. Each of the three lead screws is threaded through a collar that is press fitted through the center of one of three pinion gears. A sun gear meshes with all three pinion gears and transversely moves the three lead screws upon actuation of a drive gear. The drive gear meshes with the sun gear and is driven by a handle or servomotor. When the handle or servomotor rotates the drive gear, the sun gear rotates causing the three pinion gears to rotate, thus, causing transverse movement of the three lead screws and, accordingly, transverse movement of the transversing plate. When the drive gear rotates, the traversing plate is driven in and out of a microwave cavity. Thus, the length or size of the cavity can be tuned while maintaining the traversing plate in an exact parallel relationship with an opposing plate on another end of the cavity

    The hepatoadrenal syndrome: A common yet unrecognized clinical condition

    Objective: Adrenal failure is common in critically ill patients, particularly those with sepsis. As liver failure and sepsis are both associated with increased circulating levels of endotoxin and proinflammatory mediators and reduced levels of apoprotein-1/ high-density lipoprotein, we postulated that adrenal failure may be common in patients with liver disease. Design: Clinical study. Setting: Liver transplant intensive care unit. Patients: The study cohort included 340 patients with liver disease. Interventions: Based on preliminary observational data, all patients admitted to our 28-bed liver transplant intensive care unit (LTICU) undergo adrenal function testing. An honest broker system was used to extract clinical, hemodynamic, medication, and laboratory data on patients admitted to the LTICU from March 2002 to March 2004. A random (stress) cortisol level <20 μg/dL in a highly stressed patient (respiratory failure, hypotension) was used to diagnose adrenal insufficiency. In all other patients, a random cortisol level <15 μg/dL or a 30-min level <20 μg/dL post-low-dose (1 μg) cosyntropin was considered diagnostic of adrenal insufficiency. Patients were grouped as follows: a) chronic liver failure; b) fulminant hepatic failure; c) patients immediately status post-orthotopic liver transplantation receiving a steroid-free protocol of immunosuppression; and d) patients status post-remote liver transplant (≥6 months). The decision to treat patients with stress doses of hydrocortisone was at the discretion of the treating intensivist and transplant surgeon. Measurements and Main Results: Two-hundred and forty-five (72%) patients met our criteria for adrenal insufficiency (the hepatoadrenal syndrome). Eight (33%) patients with fulminant hepatic failure, 97 (66%) patients with chronic liver disease, 31(61%) patients with a remote history of liver transplantation, and 109 (92%) patients who had undergone liver transplantation under steroid-free immunosuppression were diagnosed with adrenal insufficiency. The high-density lipoprotein level at the time of adrenal testing was the only variable predictive of adrenal insufficiency (p < .0001). In vasopressor-dependent patients with adrenal insufficiency, treatment with hydrocortisone was associated with a significant reduction (p = .02) in the dose of norepinephrine at 24 hrs, whereas the dose of norepinephrine was significantly higher (p = .04) in those patients with adrenal failure not treated with hydrocortisone. In vasopressor-dependent patients without adrenal insufficiency, treatment with hydrocortisone did not affect vasopressor dose at 24 hrs. One hundred and forty-one patients (26.4%) died during their hospitalization. The baseline serum cortisol was 18.8 ± 16.2 μg/dL in the nonsurvivors compared with 13.0 ± 11.8 μg/dL in the survivors (p < .001). Of those patients with adrenal failure who were treated with glucocorticoids, the mortality rate was 26% compared with 46% (p = .002) in those who were not treated. In those patients receiving vasopressor agents at the time of adrenal testing, the baseline cortisol was 10.0 ± 4.8 μg/dL in those with adrenal insufficiency compared with 35.6 ± 21.2 μg/dL in those with normal adrenal function. Vasopressor-dependent patients who did not have adrenal failure had a mortality rate of 75%. Conclusions: Patients with liver failure and patients post-liver transplantation have an exceedingly high incidence of adrenal failure, which may be pathophysiologically related to low levels of high-density lipoprotein. Treatment of patients with adrenal failure may improve outcome. High baseline serum cortisol levels may be a maker of disease severity and portend a poor prognosis.     Phospholipid Scramblase-1-Induced Lipid Reorganization Regulates Compensatory Endocytosis in Neuroendocrine Cells

    Calcium-regulated exocytosis in neuroendocrine cells and neurons is accompanied by the redistribution of phosphatidylserine (PS) to the extracellular space, leading to a disruption of plasma membrane asymmetry. How and why outward translocation of PS occurs during secretion are currently unknown. Immunogold labeling on plasma membrane sheets coupled with hierarchical clustering analysis demonstrate that PS translocation occurs at the vicinity of the secretory granule fusion sites. We found that altering the function of the phospholipid scramblase-1 (PLSCR-1) by expressing a PLSCR-1 calcium-insensitive mutant or by using chromaffin cells from PLSCR-1−/−mice prevents outward translocation of PS in cells stimulated for exocytosis. Remarkably, whereas transmitter release was not affected, secretory granule membrane recapture after exocytosis was impaired, indicating that PLSCR-1 is required for compensatory endocytosis but not for exocytosis. Our results provide the first evidence for a role of specific lipid reorganization and calcium-dependent PLSCR-1 activity in neuroendocrine compensatory endocytosis

    We review the history of the parallel tempering simulation method. From its origins in data analysis, the parallel tempering method has become a standard workhorse of physiochemical simulations. We discuss the theory behind the method and its various generalizations. We mention a selected set of the many applications that have become possible with the introduction of parallel tempering and we suggest several promising avenues for future research.Comment: 21 pages, 4 figure

    Contribution à la valorisation des lactosérums industriels à l'aide des techniques de séparation par membranes

    Non disponible / Not availableLes techniques de séparation par membranes sont de plus en plus utilisées pour valoriser les constituants des lactosérums, coproduits de la filière laitière. Une meilleure connaissance de ces techniques de fractionnement, ainsi que le développement de procédés spécifiques à chacun des constituants des lactosérums, contribuent à la recherche de nouvelles voies de valorisations. Dans cette optique, l'effet d'un paramètre physicochimique important, le ph, a été étudié lors de l'ultrafiltration de lactosérum doux non clarifié. Les flux de perméation, les niveaux de colmatage et le transfert des protéines majeures à travers différentes membranes varient de manière significative selon le ph d'ultrafiltration, mais aussi selon la nature de la membrane. Les différences observées, selon le ph d'ultrafiltration, peuvent notamment s'expliquer par des changements d'hydrophilie, de charge et de polymérisation des protéines, mais aussi par des modifications de solubilisation des sels de phosphate de calcium. Si la fraction protéique des lactosérums est bien valorisée, peu de travaux portent sur la valorisation de la fraction minérale. Or, les lactosérums acides de fromagerie sont particulièrement riches en calcium et en phosphate. Le traitement technologique, mis au point dans cette étude à partir de lactosérum acide de fromagerie à 6% d'extrait sec, permet de précipiter 48% du calcium et 67% du phosphate initial. La poudre de phosphate de calcium biologique obtenue est constituée uniquement de brushite cristallisée avec une pureté de 85%. Une étape de microfiltration, optimisée à l'échelle pilote, permet d'accroitre cette pureté a 90%. Même s'il reste encore à optimiser l'extraction industrielle de ce sel de phosphate, ces travaux répondent parfaitement au souci et à la nécessité de valorisation de la fraction minérale des lactosérums
