Modelling the Effects of Nifedipine on Ventricular and Myometrial Cells of Pregnant Rats
In this study we have used computational models to investigate the effects of nifedipine on two different cell types; the rat ventricular cell and the rat myometrial cell. Nifedipine is a calcium-channel blocker commonly used by health services around the world to treat both cardiovascular conditions (such as high blood pressure) and as a tocolytic to treat pre-term birth. The latter usage is prohibited in pregnant patients with pre-existing heart conditions. By applying discrete blocks to the L-Type calcium channels in each cell model we were able to simulate the presence of nifedipine at varying concentrations. Using the electrical and ionic responses to blocking these channels as indicators, we have been able to quantify and describe the effects of nifedipine in each cell type and compare them qualitatively. Although any level of block will reduce the maximum level of intracellular calcium in the myometrial cell, a 60% block or higher is required to produce a change in the morphology of the calcium transient. It remains to be shown if the dose required to achieve this could result in a patient with a pre-existing heart condition experiencing hypotension or other pathological cardiac conditions during labor, if nifedipine is used as a tocolytic
The widespread use of topical antimicrobials enriches for resistance in Staphylococcus aureus isolated from patients with atopic dermatitis.
BACKGROUND: Carriage rates of Staphylococcus aureus on affected skin in atopic dermatitis (AD) are approximately 70%. Increasing disease severity during flares and overall disease severity correlate with increased burden of S. aureus. Treatment in AD therefore often targets S. aureus with topical and systemic antimicrobials. OBJECTIVES: To determine whether antimicrobial sensitivities and genetic determinants of resistance differed in S. aureus isolates from the skin of children with AD and healthy child nasal carriers. METHODS: In this case-control study, we compared S. aureus isolates from children with AD (n = 50) attending a hospital dermatology department against nasal carriage isolates from children without skin disease (n = 49) attending a hospital emergency department for noninfective conditions. Using whole genome sequencing we generated a phylogenetic framework for the isolates based on variation in the core genome, then compared antimicrobial resistance phenotypes and genotypes between disease groups. RESULTS: Staphylococcus aureus from cases and controls had on average similar numbers of phenotypic resistances per isolate. Case isolates differed in their resistance patterns, with fusidic acid resistance (FusR ) being significantly more frequent in AD (P = 0·009). The genetic basis of FusR also differentiated the populations, with chromosomal mutations in fusA predominating in AD (P = 0·049). Analysis revealed that FusR evolved multiple times and via multiple mechanism in the population. Carriage of plasmid-derived qac genes, which have been associated with reduced susceptibility to antiseptics, was eight times more frequent in AD (P = 0·016). CONCLUSIONS: The results suggest that strong selective pressure drives the emergence and maintenance of specific resistances in AD
Healthcare Staff Wellbeing, Burnout, and Patient Safety: A Systematic Review
Objective
To determine whether there is an association between healthcare professionals’ wellbeing and burnout, with patient safety.
Design
Systematic research review.
Data Sources
PsychInfo (1806 to July 2015), Medline (1946 to July 2015), Embase (1947 to July 2015) and Scopus (1823 to July 2015) were searched, along with reference lists of eligible articles.
Eligibility Criteria for Selecting Studies
Quantitative, empirical studies that included i) either a measure of wellbeing or burnout, and ii) patient safety, in healthcare staff populations.
Results
Forty-six studies were identified. Sixteen out of the 27 studies that measured wellbeing found a significant correlation between poor wellbeing and worse patient safety, with six additional studies finding an association with some but not all scales used, and one study finding a significant association but in the opposite direction to the majority of studies. Twenty-one out of the 30 studies that measured burnout found a significant association between burnout and patient safety, whilst a further four studies found an association between one or more (but not all) subscales of the burnout measures employed, and patient safety.
Conclusions
Poor wellbeing and moderate to high levels of burnout are associated, in the majority of studies reviewed, with poor patient safety outcomes such as medical errors, however the lack of prospective studies reduces the ability to determine causality. Further prospective studies, research in primary care, conducted within the UK, and a clearer definition of healthcare staff wellbeing are needed.
Implications
This review illustrates the need for healthcare organisations to consider improving employees’ mental health as well as creating safer work environments when planning interventions to improve patient safety
How external and internal resources influence user action: the case of infusion devices
Human error can have potentially devastating consequences in contexts such as healthcare, but there is a rarely a simple dichotomy between errors and correct behaviour. Furthermore, there has been little consideration of how the activities of users (erroneous and otherwise) relate to the conceptual fit between user and device, despite the fact that healthcare technologies are becoming increasingly prevalent and complex. In this article, we present a study in which nurses’ conceptions of infusion device practice were elicited to identify misfits. By focusing on key concepts that users work with when setting up infusions and the extent to which the system supports them, our analysis highlights how actions are influenced by the different resources available to users including: the device itself; supporting artefacts; the conceptual understanding of the user; and the community of practice the user is part of. The findings reveal the ways in which users are resourceful in their day-to-day activities and also suggest potential vulnerabilities within the wider system that could threaten patient safety. Our approach is able to make previously under-explored aspects of practice visible, thus enabling insight into how users act and why

The influence of the accessory genome on bacterial pathogen evolution
Bacterial pathogens exhibit significant variation in their genomic content of virulence factors. This reflects the abundance of strategies pathogens evolved to infect host organisms by suppressing host immunity. Molecular arms-races have been a strong driving force for the evolution of pathogenicity, with pathogens often encoding overlapping or redundant functions, such as type III protein secretion effectors and hosts encoding ever more sophisticated immune systems. The pathogens’ frequent exposure to other microbes, either in their host or in the environment, provides opportunities for the acquisition or interchange of mobile genetic elements. These DNA elements accessorise the core genome and can play major roles in shaping genome structure and altering the complement of virulence factors. Developing a Collaboration with the Houston Independent School District: Testing the Generalizability of a Partnership Model
Moving evidence-based practices into real-world settings is a high priority for education and public health. This paper describes the development of a partnership among the Houston Independent School District, the American Institutes of Research, and the Houston Federation of Teachers to support research on and program sustainability for the Good Behavior Game, a team-based classroom behavior management strategy that has shown positive impact in randomized field trials. The conceptual framework guiding partnership development is presented, followed by an application of the framework in Houston. Lessons learned and implications for the next stage of research and practice are then discussed

Strategies for conducting situated studies of technology use in hospitals
Ethnographic methods are widely used for understanding situated practices with technology. When authors present their data gathering methods, they almost invariably focus on the bare essentials. These enable the reader to comprehend what was done, but leave the impression that setting up and conducting the study was straightforward. Text books present generic advice, but rarely focus on specific study contexts. In this paper, we focus on lessons learnt by non-clinical researchers studying technology use in hospitals: gaining access; developing good relations with clinicians and patients; being outsiders in healthcare settings; and managing the cultural divide between technology human factors and clinical practice. Drawing on case studies across various hospital settings, we present a repertoire of ways of working with people and technologies in these settings. These include engaging clinicians and patients effectively, taking an iterative approach to data gathering and being responsive to the demands and opportunities provided by the situation. The main contribution of this paper is to make visible many of the lessons we have learnt in conducting technology studies in healthcare, using these lessons to present strategies that other researchers can take up
The population biology of the living coelacanth studied over 21 years
Between 1986 and 2009 nine submersible and
remote-operated vehicle expeditions were carried out to
study the population biology of the coelacanth Latimeria
chalumnae in the Comoro Islands, located in the western
Indian Ocean. Latimeria live in large overlapping home
ranges that can be occupied for as long as 21 years. Most
individuals are confined to relatively small home ranges,
resting in the same caves during the day. One hundred and
forty five coelacanths are individually known, and we
estimate the total population size of Grande Comore as
approximately 300–400 adult individuals. The local population
inhabiting a census area along an 8-km section of
coastline remained stable for at least 18 years. Using
LASER-assisted observations, we recorded length frequencies
between 100 and 200 cm total length and did not
encounter smaller-bodied individuals (\100 cm total
length). It appears that coelacanth recruitment in the
observation areas occur mainly by immigrating adults. We
estimate that the mean numbers of deaths and newcomers
are 3–4 individuals per year, suggesting that longevity may
exceed 100 years. The domestic fishery represents a threat
to the long-term survival of coelacanths in the study area.
Recent changes in the local fishery include a decrease in
the abundance of the un-motorized canoes associated with
exploitation of coelacanths and an increase in motorized
canoes. Exploitation rates have fallen in recent years, and
by 2000, had fallen to lowest ever reported. Finally, future
