Efeitos de Anestésico Local no Tempo Entre Bólus e na Duração do Trabalho de Parto em Patient-Controlled Epidural Analgesia: Estudo Prospectivo de Dois Regimes de Doses Ultra-Baixas com Ropivacaina e Sufentanil
BACKGROUND: Patient-controlled epidural analgesia with low concentrations of anesthetics is effective in reducing labor pain. The aim of this study was to assess and compare two ultra-low dose regimens of ropivacaine and sufentanil (0.1% ropivacaine plus 0.5 ÎŒg.ml-1 sufentanil vs. 0.06% ropivacaine plus 0.5 ÎŒg.ml-1 sufentanil) on the intervals between boluses and the duration of labor.
MATERIAL AND METHODS:
In this non-randomized prospective study, conducted between January and July 2010, two groups of parturients received patient-controlled epidural analgesia: Group I (n = 58; 1 mg.ml-1 ropivacaine + 0.5 ÎŒg.ml-1 sufentanil) and Group II (n = 57; 0.6 mg.ml-1 ropivacaine + 0.5 ÎŒg.ml-1 sufentanil). Rescue doses of ropivacaine at the concentration of the assigned group without sufentanil were administered as necessary. Pain, local anesthetic requirements, neuraxial blockade characteristics, labor and neonatal outcomes, and maternal satisfaction were recorded.
RESULTS:
The ropivacaine dose was greater in Group I (9.5 [7.7-12.7] mg.h-1 vs. 6.1 [5.1-9.8 mg.h-1], p < 0.001). A time increase between each bolus was observed in Group I (beta = 32.61 min, 95% CI [25.39; 39.82], p < 0.001), whereas a time decrease was observed in Group II (beta = -1.40 min, 95% CI [-2.44; -0.36], p = 0.009). The duration of the second stage of labor in Group I was significantly longer than that in Group II (78 min vs. 65 min, p < 0.001).
CONCLUSIONS:
Parturients receiving 0.06% ropivacaine exhibited less evidence of cumulative effects and exhibited faster second stage progression than those who received 0.1% ropivacaine
Properties of Galaxies in and around Voids
Two surveys for intrinsically faint galaxies towards nearby voids have been
conducted at the MPI f\"ur Astronomie, Heidelberg. One selected targets from a
new diameter limited () catalog with morphological criteria while
the other used digitized objective prism Schmidt plates to select mainly HII
dwarf galaxies. For some 450 galaxies, redshifts and other optical data were
obtained. We studied the spatial distribution of the sample objects, their
luminosity function, and their intrinsic properties. Most of the galaxies
belong to already well known sheets and filaments. But we found about a dozen
highly isolated galaxies in each sample (nearest neighborhood distance ). These tend to populate additional structures and are not
distributed homogeneously throughout the voids. As our results on 'void
galaxies' still suffer from small sample statistics, I also tried to combine
similar existing surveys of nearby voids to get further hints on the larger
structure and on the luminosity function of the isolated galaxies. No
differences in the luminosity function of sheet and void galaxies could be
found. The optical and infrared properties of both samples are in the normal
range for samples dominated by late-type dwarfs. Follow-up HI studies show that
the isolated dwarfs in both samples have unusual high amount of neutral gas for

Undifferentiated connective tissue disease: a seven-center cross-sectional study of 184 patients
The purpose of this study was to characterize the clinical and serological features of a large cohort of patients with antinuclear antibody (ANA) positive undifferentiated connective tissue disease (UCTD). Consecutive patients with UCTD, followed up at the Rheumatology Clinic of the participating centers, were included. Data from these patients were obtained by clinical evaluation and chart review. All patients were diagnosed as having UCTD on basis of the following criteria: positive ANA plus at least one clinical feature of connective tissue disease, but not fulfilling classification criteria for any differentiated connective tissue disease. One hundred eighty-four patients were studied (female patients-94.5%; mean age at time of evaluation-47 years). The most prevalent manifestations were arthralgia (66%), arthritis (32%), Raynaud's phenomenon (30%), sicca symptoms (30%), and leukopenia (19%). The prevalence of ANA was 100%, anti-SSA 20%, anti-dsDNA 14%, and anti-SSB 7%. Patients with anti-dsDNA/anti-Sm, anticentromere/anti-Scl70, or anti-SSA/anti-SSB antibodies more frequently presented a set of manifestations close to systemic lupus erythematosus (SLE), systemic sclerosis, or Sjögren syndrome, respectively. We analyze a large cohort of UCTD. Seventy-two percent of these UCTD patients present lupus-, scleroderma-, or Sjögren-like features but do not fulfill classification criteria and mostly present a mild disease
EquiFACS: the Equine Facial Action Coding System
Although previous studies of horses have investigated their facial expressions in specific contexts, e.g. pain, until now there has been no methodology available that documents all the possible facial movements of the horse and provides a way to record all potential facial configurations. This is essential for an objective description of horse facial expressions across a range of contexts that reflect different emotional states. Facial Action Coding Systems (FACS) provide a systematic methodology of identifying and coding facial expressions on the basis of underlying facial musculature and muscle movement. FACS are anatomically based and document all possible facial movements rather than a configuration of movements associated with a particular situation. Consequently, FACS can be applied as a tool for a wide range of research questions. We developed FACS for the domestic horse (Equus caballus) through anatomical investigation of the underlying musculature and subsequent analysis of naturally occurring behaviour captured on high quality video. Discrete facial movements were identified and described in terms of the underlying muscle contractions, in correspondence with previous FACS systems. The reliability of others to be able to learn this system (EquiFACS) and consistently code behavioural sequences was highâand this included people with no previous experience of horses. A wide range of facial movements were identified, including many that are also seen in primates and other domestic animals (dogs and cats). EquiFACS provides a method that can now be used to document the facial movements associated with different social contexts and thus to address questions relevant to understanding social cognition and comparative psychology, as well as informing current veterinary and animal welfare practices
Sparsest factor analysis for clustering variables: a matrix decomposition approach
We propose a new procedure for sparse factor analysis (FA) such that each variable loads only one common factor. Thus, the loading matrix has a single nonzero element in each row and zeros elsewhere. Such a loading matrix is the sparsest possible for certain number of variables and common factors. For this reason, the proposed method is named sparsest FA (SSFA). It may also be called FA-based variable clustering, since the variables loading the same common factor can be classified into a cluster. In SSFA, all model parts of FA (common factors, their correlations, loadings, unique factors, and unique variances) are treated as fixed unknown parameter matrices and their least squares function is minimized through specific data matrix decomposition. A useful feature of the algorithm is that the matrix of common factor scores is re-parameterized using QR decomposition in order to efficiently estimate factor correlations. A simulation study shows that the proposed procedure can exactly identify the true sparsest models. Real data examples demonstrate the usefulness of the variable clustering performed by SSFA
REALMS Study: Real-World Effectiveness and Safety of Fingolimod in Patients with Relapsing-Remitting Multiple Sclerosis in Portugal
Background: Fingolimod, an oral sphingosine 1-phosphate receptor modulator, is approved by EMA for relapsing-remitting multiple sclerosis (RRMS).
Objectives: To assess the effectiveness and safety of fingolimod in patients with RRMS in real-world clinical practice in Portugal.
Methods: Retrospective, multicentre, non-interventional study, reporting 3 years follow-up of data collected from October 2015 to July 2016. Sociodemographic data and previous treatments at baseline and data regarding disease evolution, including number of relapses, annualised relapse rates (ARR) and Expanded Disability Status Scale (EDSS), were collected.
Results: Two-hundred and seventy-five participants were enrolled in the REALMS study. Results showed that the main reason to switch to fingolimod was failure of previous treatment (56.7%) and only 3.6% were naĂŻve patients. In the total population, there was a significant decrease in ARR of 64.6% in the first year of treatment, 79.7% in the second year and 82.3% in the third year, compared with baseline. More than 67.0% of patients had no relapses during the 3 years after switching to fingolimod. EDSS remained stable throughout the study.
Conclusions: Therapy with fingolimod showed a sustained effectiveness and safety over the 3 years, particularly on patients switched from first-line drugs (BRACE). No new safety issues were reported.info:eu-repo/semantics/publishedVersio
Medical audit: prenatal care program in a health center in southern Brazil
INTRODUCTION: A medical audit on the prenatal care program in the Vila Municipal Health Center, Pelotas, RS, Brazil, was described with the purpose of verifying the aspects of the medical process and improving the program's effectiveness. METHODS: Data from prenatal specific records were collected. Pregnant women with delivery due date in 1997 and in the first semester of 1998 were included in the study. Women registered in the program when they were 4-month pregnant and who had had at least 5 visits were also enrolled. Bivariate analysis was used to detect health care indicators. RESULTS: A total of 73 pregnant women were registered in the program in 1997 and 75 in 1998. In 1997, the average number of medical visits was 5.2, while in 1998 this average was 6.2. Objetivo : analisar os fatores que interferem no desempenho de usuĂĄrios de implante coclear em testes de percepção de fala.MĂ©todos : foram aplicados questionĂĄrios e testes de percepção de fala em 25 indivĂduos, de ambos os sexos, portadores de perda auditiva neurossensorial e usuĂĄrios de IC unilateral.Resultados : - 48% dos participantes atingiram bom desempenho nos resultados dos testes de percepção de fala, apresentando porcentagens maiores ou iguais a 50 % de acertos. - Os participantes que obtiveram melhores resultados nĂŁo sĂŁo necessariamente os que possuem maior idade auditiva. - Dos 12 participantes que atingiram desempenho acima de 50%, 10 (83,3%) foram implantados mais precocemente (atĂ© 5 anos). - O uso do AASI na orelha contralateral influenciou significantemente nos Ăndices de reconhecimento das palavras ou sentenças. - Os participantes que iniciaram o uso de AASI mais cedo nĂŁo foram necessariamente os que apresentaram melhores resultados nos testes.ConclusĂŁo : a ativação mais precoce dos IC mostrou-se uma variĂĄvel importante no desempenho dos testes. - O uso do AASI na orelha contralateral influenciou significantemente nos Ăndices de reconhecimento das palavras ou sentenças
A single low-energy, iron-poor supernova as the source of metals in the star SMSS J 031300.36-670839.3
The element abundance ratios of four low-mass stars with extremely low
metallicities indicate that the gas out of which the stars formed was enriched
in each case by at most a few, and potentially only one low-energy, supernova.
Such supernovae yield large quantities of light elements such as carbon but
very little iron. The dominance of low-energy supernovae is surprising, because
it has been expected that the first stars were extremely massive, and that they
disintegrated in pair-instability explosions that would rapidly enrich galaxies
in iron. What has remained unclear is the yield of iron from the first
supernovae, because hitherto no star is unambiguously interpreted as
encapsulating the yield of a single supernova. Here we report the optical
spectrum of SMSS J031300.36- 670839.3, which shows no evidence of iron (with an
upper limit of 10^-7.1 times solar abundance). Based on a comparison of its
abundance pattern with those of models, we conclude that the star was seeded
with material from a single supernova with an original mass of ~60 Mo (and that
the supernova left behind a black hole). Taken together with the previously
mentioned low-metallicity stars, we conclude that low-energy supernovae were
common in the early Universe, and that such supernovae yield light element
enrichment with insignificant iron. Reduced stellar feedback both chemically
and mechanically from low-energy supernovae would have enabled first-generation
stars to form over an extended period. We speculate that such stars may perhaps
have had an important role in the epoch of cosmic reionization and the chemical
