Sociogénesis del conflicto: una interpretación desde la tradición oral y el imaginario turístico en la comunidad de Malinalco, Estado de México

El presente trabajo representa un avance de investigación acerca de la idea del imaginario turístico y su relación con el marco de referencia teórica del conflicto. En este sentido, se plantea la existencia de un conflicto que se genera socialmente entre el turista y el que recibe la comunidad, y en esta relación de origen se encuentra enraizado el imaginario colectivo de las comunidades expresado en situaciones de oralidad. La relación que guarda con el conflicto es en la medida de su resistencia y permanencia de los valores comunitarios manifestados en mitos, leyendas y tradiciones, en contraparte con lo que presenta el turista que encarna otros valores, produciéndose una reinvención de lo comunitario

Las características generales que presenta ahora la comunidad rural en México, es de una figura inacabada de la supuesta modernidad. De una expresión, que por más que se incorpora, forzada o voluntariamente a los procesos que se implementan gradualmente en todas las sociedades del mundo contemporáneo, no acaban por ser. Es decir, son muchas las manifestaciones en donde las sociedades y comunidades no se consolidan, sino que sus expresiones sólo son el tránsito hacia algo que no se define necesariamente como progreso, modernidad o desarrollo.Este estudio, ofrece una explicación acerca de la comunidad de Malinalco desde la categoría de la sociogénesis así como del conflicto, en el cual se desarrolla una interpretación de las manifestaciones culturales ligadas a la tradición oral. En esta argumentación, que cruza transversalmente todas las prácticas sociales y particularmente las religiosas, se desvela la importancia de un imaginario social basado en la recuperación de la oralidad y de la configuración en los procesos de identidad comunitaria. La sociogénesis del conflicto responde a manifestaciones del imaginario social que expresa la población de Malinalco, en sus prácticas culturales y celebraciones religiosas, recreadas por una fortalecida tradición oral. A través de mecanismos de permanencia, como son las imágenes, los objetos y su significado comunitario; de un sentido de resistencia manifiesto en sus recreaciones socioculturales y religiosas ligadas a las tensiones entre los estereotipos que genera la visita del turista y la construcción imaginaria del poblador. Así como los arquetipos del pasado colonial e indígena, y la exigencia de un reconocimiento comunitario entre los barrios integrantes de la cabecera municipal, se configura un proceso de reinvención comunitaria desde estas recreaciones y visiones del pasado, en un eje articulador para manifestarse en el presente sin romper el núcleo de origen, que corre paralelo a la historia oficial, pero que funda y recrea referentes en tiempos y espacios concebidos como sagrados con una narrativa discursiva de emancipación comunitaria

Las ciencias sociales y sus abordajes en los estudios avanzados

Este libro tiene como uno de sus objetivos principales impulsar un ejercicio académico de tipo interdisciplinario entre investigadores y egresados del programa de Doctorado en Ciencias Sociales de la Universidad Autónoma del Estado México (UAEM). Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

Construcción de Servicios Turísticos a Nivel Local en Toluca, Estado de México

En los últimos años, en el mu nicipio de Toluca, Estado de México, se viene impulsando la creación de servicios turísticos. Esto ha dado lugar a una dinámica de desarrollo que, paulatinamente, está modificando las condiciones de vida de la sociedad toluquense. Se supone que una de las formas en que este municipio se está desarrollando es a partir del turismo. Esta investigación se planteó con el obje tivo de establecer si, efectivamente, se está dando lugar a ese tipo de desarrollo municipal. Se trata de un acercamiento que se lleva a cabo haciendo uso de los instrumentos contenidos en la propuesta teórico - metodológica denominada desarrollo local . A pa rtir de ella, se analiza la dinámica del desarrollo turístico en cuatro delegaciones del municipio de Toluca. En primer lugar se exponen brevemente los principios generales del modelo de desarrollo que se viene aplicando en México desde hace ya varios años y se recupera lo relativo al desarrollo local y al desarrollo sustentable dentro de ese modelo. Luego se establece la relación que, dentro del modelo, se supone debe existir entre esos tipos de desarrollo y el turismo. Finalmente, a manera de ejemplos, se exponen los resultados de cuatro casos estudiados, llevándose a cabo la identificación de los recursos susceptibles de utilizarse con fines turísticos, o bien, que ya pueden ser considerados recursos turísticos; se establece si se está dando lugar o no, a la generación de procesos autogestivos a largo plazo para que pueda hablarse de desarrollo local y de sustentabilidad. La conclusión general apunta a reconocer que, en los cuatro casos estudiados, para la identificación de recursos turísticos no se ha tom ado en cuenta a las poblaciones locales y que la finalidad con la que se está llevando a cabo es para establecer desarrollos turísticos en los que los empresarios locales serán los únicos beneficiados. do M&eacute;xico, vem promovendo a cria&ccedil;&atilde;o de servi&ccedil;os tur&iacute;sticos. Isto levou a uma din&acirc;mica de desenvolvimento que, gradualmente, est&aacute; mudando a vida dos moradores da cidade de Toluca. Sup&otilde;e-se que uma das maneiras em que este concelho est&aacute; a ser desenvolvido &eacute; de turismo. Esta pesquisa foi concebida com o objectivo de estabelecer se &eacute; que est&aacute; a dar origem ao desenvolvimento municipal tal. Esta &eacute; uma abordagem que &eacute; levada a cabo usando as ferramentas contidos no te&oacute;rico-metodol&oacute;gico desenvolvimento chamada local. A partir dele, podemos analisar a din&acirc;mica de desenvolvimento do turismo em quatro munic&iacute;pio local de Toluca. Primeiro descreve brevemente os princ&iacute;pios gerais do modelo de desenvolvimento tem sido aplicadas no M&eacute;xico por v&aacute;rios anos e se recuperou em rela&ccedil;&atilde;o ao desenvolvimento local e desenvolvimento sustent&aacute;vel dentro desse modelo. Depois de estabelecer a rela&ccedil;&atilde;o dentro do modelo, &eacute; suposto existir entre esses tipos de desenvolvimento e turismo. Finalmente, como exemplos, apresentamos resultados de quatro estudos de caso, realizando a identifica&ccedil;&atilde;o dos recursos que possam ser utilizadas para fins tur&iacute;sticos, ou que podem ser considerados como recursos tur&iacute;sticos, &eacute; definido se est&aacute; ocorrendo ou n&atilde;o, para a gera&ccedil;&atilde;o de longo prazo de auto-gest&atilde;o de processos para que haja desenvolvimento local e sustentabilidade. A conclus&atilde;o geral &eacute; reconhecer que, nos quatro casos estudados, para identificar recursos tur&iacute;sticos n&atilde;o foi tida em conta a popula&ccedil;&atilde;o local e do prop&oacute;sito para o qual est&aacute; a realizar &eacute; o de estabelecer o desenvolvimento do turismo em que empres&aacute;rios locais ser&atilde;o os &uacute;nicos benefici&aacute;rios. Como se observa a gera&ccedil;&atilde;o de auto-gest&atilde;o de processos, segue-se que nenhuma quest&atilde;o de desenvolvimento local do turismo, nestes casos.</span></p

Association between convalescent plasma treatment and mortality in COVID-19: a collaborative systematic review and meta-analysis of randomized clinical trials

Abstract Background Convalescent plasma has been widely used to treat COVID-19 and is under investigation in numerous randomized clinical trials, but results are publicly available only for a small number of trials. The objective of this study was to assess the benefits of convalescent plasma treatment compared to placebo or no treatment and all-cause mortality in patients with COVID-19, using data from all available randomized clinical trials, including unpublished and ongoing trials (Open Science Framework, https://doi.org/10.17605/OSF.IO/GEHFX ). Methods In this collaborative systematic review and meta-analysis, clinical trial registries (ClinicalTrials.gov, WHO International Clinical Trials Registry Platform), the Cochrane COVID-19 register, the LOVE database, and PubMed were searched until April 8, 2021. Investigators of trials registered by March 1, 2021, without published results were contacted via email. Eligible were ongoing, discontinued and completed randomized clinical trials that compared convalescent plasma with placebo or no treatment in COVID-19 patients, regardless of setting or treatment schedule. Aggregated mortality data were extracted from publications or provided by investigators of unpublished trials and combined using the Hartung–Knapp–Sidik–Jonkman random effects model. We investigated the contribution of unpublished trials to the overall evidence. Results A total of 16,477 patients were included in 33 trials (20 unpublished with 3190 patients, 13 published with 13,287 patients). 32 trials enrolled only hospitalized patients (including 3 with only intensive care unit patients). Risk of bias was low for 29/33 trials. Of 8495 patients who received convalescent plasma, 1997 died (23%), and of 7982 control patients, 1952 died (24%). The combined risk ratio for all-cause mortality was 0.97 (95% confidence interval: 0.92; 1.02) with between-study heterogeneity not beyond chance (I2 = 0%). The RECOVERY trial had 69.8% and the unpublished evidence 25.3% of the weight in the meta-analysis. Conclusions Convalescent plasma treatment of patients with COVID-19 did not reduce all-cause mortality. These results provide strong evidence that convalescent plasma treatment for patients with COVID-19 should not be used outside of randomized trials. Evidence synthesis from collaborations among trial investigators can inform both evidence generation and evidence application in patient care

Incidence of severe critical events in paediatric anaesthesia (APRICOT): a prospective multicentre observational study in 261 hospitals in Europe

Background Little is known about the incidence of severe critical events in children undergoing general anaesthesia in Europe. We aimed to identify the incidence, nature, and outcome of severe critical events in children undergoing anaesthesia, and the associated potential risk factors. Methods The APRICOT study was a prospective observational multicentre cohort study of children from birth to 15 years of age undergoing elective or urgent anaesthesia for diagnostic or surgical procedures. Children were eligible for inclusion during a 2-week period determined prospectively by each centre. There were 261 participating centres across 33 European countries. The primary endpoint was the occurence of perioperative severe critical events requiring immediate intervention. A severe critical event was defined as the occurrence of respiratory, cardiac, allergic, or neurological complications requiring immediate intervention and that led (or could have led) to major disability or death. This study is registered with ClinicalTrials.gov, number NCT01878760. Findings Between April 1, 2014, and Jan 31, 2015, 31â127 anaesthetic procedures in 30â874 children with a mean age of 6·35 years (SD 4·50) were included. The incidence of perioperative severe critical events was 5·2% (95% CI 5·0â5·5) with an incidence of respiratory critical events of 3·1% (2·9â3·3). Cardiovascular instability occurred in 1·9% (1·7â2·1), with an immediate poor outcome in 5·4% (3·7â7·5) of these cases. The all-cause 30-day in-hospital mortality rate was 10 in 10â000. This was independent of type of anaesthesia. Age (relative risk 0·88, 95% CI 0·86â0·90; p<0·0001), medical history, and physical condition (1·60, 1·40â1·82; p<0·0001) were the major risk factors for a serious critical event. Multivariate analysis revealed evidence for the beneficial effect of years of experience of the most senior anaesthesia team member (0·99, 0·981â0·997; p<0·0048 for respiratory critical events, and 0·98, 0·97â0·99; p=0·0039 for cardiovascular critical events), rather than the type of health institution or providers. Interpretation This study highlights a relatively high rate of severe critical events during the anaesthesia management of children for surgical or diagnostic procedures in Europe, and a large variability in the practice of paediatric anaesthesia. These findings are substantial enough to warrant attention from national, regional, and specialist societies to target education of anaesthesiologists and their teams and implement strategies for quality improvement in paediatric anaesthesia.
