The Effect of Spironolactone on the Pathogenesis of Ligatureinduced Alveolar Bone Loss in Wistar Rats
Tumor necrosis factor (TNF) is a pro-inflammatory cytokine that has a straight relationship with tissue destruction in the pathogenesis of periodontitis. Inhibitory effects of TNF production have been attributed to spironolactone. The aim of this study was to evaluate the effect of spironolactone on the pathogenesis of ligature-induced alveolar bone loss in rats. Experimental periodontitis was induced in 38 Wistar rats by ligature placement in the left second maxillary molar. The contra-lateral maxillary molar served as intragroup control. Animals were randomly divided into 4 groups and treated with spironolactone (50, 100, 200 mg·kg-1) or saline. Morphometrical registration of maxillary alveolar bone was performed after 28 days of experimental periodontitis. Intra-group comparisons showed significantly higher alveolar bone loss mean values in maxillary sides with ligature (paired sample t test, p<0.05). Mean alveolar bone loss was not significantly different between groups, independently of the dosage (range: 0.63 – 0.66 mm, one-way ANOVA, p>0.05). Although spironolactone has recognized TNF-inhibitory properties, the possibility of its use on modulation of host immune-inflammatory response in periodontal disease was not confirmed.

Penelitian ini bertujuan untuk mengetahui bagaimana suatu media membingkai dan mengkonstruksikan realitas berita mengenai konflik KPK dan Polri di vivanews.co.id dan detik.com. framing yang digunakan dalam penelitian adalah framing milik Robert Entman yang berfokus pada bagaimana medefinisikan suatu masalah, apa/siapa penyebab masalah, keputusan moral apa yang terdapat dalam berita tersebut dan apa penyelesaian yang ditwarkan oleh wartawan. Hasil dari penelitian ini menunjukkan bahwa dalam mengkonstruksi dan membingkai konflik KPK dan Polri baik vivanews.co.id dan detik.com memiliki kesan yang berbeda. Dimana vivanews.co.id cenderung memojokkan Jokowi bahwa keputusannya dalam mengajukan Budi Gunawan sebagai calon Kapolri adalah awal dari konflik tersebut, namun berbeda detiknews.com yang mana meminta presiden Jokowi mengakhiri konflik tersebut dengan mempertimbangkan masukkan dari tim independennya.

Relationships Between Chemoreflex Responses, Sleep Quality, and Hematocrit in Andean Men and Women.
Andean highlanders are challenged by chronic hypoxia and many exhibit elevated hematocrit (Hct) and blunted ventilation compared to other high-altitude populations. While many Andeans develop Chronic Mountain Sickness (CMS) and excessive erythrocytosis, Hct varies markedly within Andean men and women and may be driven by individual differences in ventilatory control and/or sleep events which exacerbate hypoxemia. To test this hypothesis, we quantified relationships between resting ventilation and ventilatory chemoreflexes, sleep desaturation, breathing disturbance, and Hct in Andean men and women. Ventilatory measures were made in 109 individuals (n = 63 men; n = 46 women), and sleep measures in 45 of these participants (n = 22 men; n = 23 women). In both men and women, high Hct was associated with low daytime SpO2 (p < 0.001 and p < 0.002, respectively) and decreased sleep SpO2 (mean, nadir, and time <80%; all p < 0.02). In men, high Hct was also associated with increased end-tidal PCO2 (p < 0.009). While ventilatory responses to hypoxia and hypercapnia did not predict Hct, decreased hypoxic ventilatory responses were associated with lower daytime SpO2 in men (p < 0.01) and women (p < 0.009) and with lower nadir sleep SpO2 in women (p < 0.02). Decreased ventilatory responses to CO2 were associated with more time below 80% SpO2 during sleep in men (p < 0.05). The obstructive apnea index and apnea-hypopnea index also predicted Hct and CMS scores in men after accounting for age, BMI, and SpO2 during sleep. Finally, heart rate response to hypoxia was lower in men with higher Hct (p < 0.0001). These data support the idea that hypoventilation and decreased ventilatory sensitivity to hypoxia are associated with decreased day time and nighttime SpO2 levels that may exacerbate the stimulus for erythropoiesis in Andean men and women. However, interventional and longitudinal studies are required to establish the causal relationships between these associations

Longitudinal changes of cardiotocographic parameters throughout pregnancy: a prospective cohort study comparing small-for-gestational-age and normal fetuses from 24 to 40 weeks
Objective: To compare longitudinal trends of cardiotocographic (CTG) parameters between small-for-gestational-age (SGA) and normal fetuses, from 24 to 41 weeks of pregnancy. Methods: A prospective cohort study was carried out in singleton pregnancies without fetal malformations. At least one CTG was performed in each of the following intervals: 24-26 weeks+6 days, 27-29 weeks+6 days, 30-32 weeks+6 days, 33-35 weeks+6 days, 36-38 weeks+6 days and >= 39 weeks. Tracings were analyzed using the Omniview-SisPorto (R) 3.6 system. Cases with a normal - pregnancy outcome, including a birthweight >= 10th percentile for gestational age, were compared with two groups of SGA fetuses: with birthweight < 10th percentile (SGA < p10) and < 3rd percentile (SGA < p3; a subgroup of the latter). Generalized linear mixed-effects models were used for analysis. Results: A total of 176 fetuses (31 SGA) and 1256 tracings (207 from SGA fetuses) were evaluated. All CTG parameters changed significantly throughout pregnancy in the three groups, with a decreasing baseline and probability of decelerations, and an increasing average long-term variability (LTV), average short-term variability (STV) and accelerations. Baseline showed a more pronounced decrease (steeper slope) in SGA fetuses, being higher in these cases at earlier gestational ages and lower later in pregnancy. Average LTV was significantly lower in SGA < p3 fetuses, but a parallel increase occurred in all groups. There was a considerable inter-fetal variability within each group. Conclusion: A unique characterization of CTG trends throughout gestation in SGA fetuses was provided. A steeper descent of the baseline was reported for the first time. The findings raise the possibility of clinical application of computerized CTG analysis in screening and management of fetal growth restriction
COVID-19 Vaccines and Atrial Fibrillation: Analysis of the Post-Marketing Pharmacovigilance European Database
Atrial fibrillation (AF) has been described in COVID-19 patients. Recently, some case reports and US pharmacovigilance analyses described AF onset as a rare adverse event following COVID-19 vaccination. The possible correlation is unclear. We systematically analyzed the reports of AF related to COVID-19 vaccines collected in the European pharmacovigilance database, EudraVigilance (EV), from 2020 to November 2022. We carried out descriptive and disproportionality analyses. Moreover, we performed a sensitivity analysis, excluding the reports describing other possible alternative AF causes (pericarditis, myocarditis, COVID-19, or other drugs that may cause/exacerbate AF). Overall, we retrieved 6226 reports, which represented only 0.3% of all those related to COVID-19 vaccines collected in EV during our study period. AF reports mainly referred to adults (in particular, >65 years old), with an equal distribution in sex. Reports were mainly related to tozinameran (54.04%), elasomeran (28.3%), and ChAdOx1-S (14.32%). The reported AF required patient hospitalization in 35% of cases and resulted in a life-threatening condition in 10% of cases. The AF duration (when reported) was highly variable, but the majority of the events had a short duration (moda = 24 h). Although an increased frequency of AF reporting with mRNA vaccines emerges from our study, other investigations are required to investigate the possible correlation between COVID-19 vaccination and the rare AF occurrence
Associations between a posteriori defined dietary patterns and bone mineral density in adolescents
Purpose: Dietary pattern analysis may uncover the joint effects of multiple dietary components on bone health, but such research is scarce and targets mostly adults.
Methods: We quantified prospective associations between dietary patterns and bone mineral density (BMD) in 1,007 adolescents of a cohort born in 1990 and recruited at schools in Porto during the 2003/2004 school year. Forearm BMD was measured using dual-energy X-ray absorptiometry. Participants’ dietary patterns were classified “Healthier”, “Dairy products”, “Fast food and sweets” and “Lower intake” according to previously identified patterns obtained in a larger sample of 1,489 participants using the K-means method. Using dietary patterns at 13 years old as the main exposure, associations were estimated cross-sectionally (with BMD at the age of 13) and prospectively (with annual BMD variation between 13 and 17 years), using linear regression coefficients adjusted for height, weight, energy intake and, in girls, for menarche age.
Results: No significant associations between the a posteriori dietary patterns identified and mean BMD at 13 were found. However, among girls, adherence to a pattern characterized by low intake of energy and all food groups was negatively associated with annual BMD variation between 13 and 17 years [adjusted coefficient (95 % CI) −0.451 (−0.827; −0.074) mg·cm−2·year−1].
Conclusions: Although results showed that, in girls, adherence to a “Lower intake” dietary pattern is associated with lower annual BMD variation throughout adolescence, overall, there were no consistent associations between dietary patterns and forearm BMD in adolescents
Periodontal disease affects oral cancer progression in a surrogate animal model for tobacco exposure
For decades, the link between poor oral hygiene and the increased prevalence of oral cancer has been suggested. Most recently, emerging evidence has suggested that chronic inflammatory diseases from the oral cavity (e.g., periodontal disease), to some extent, play a role in the development of oral squamous cell carcinoma (OSCC). The present study aimed to explore the direct impact of biofilm-induced periodontitis in the carcinogenesis process using a tobacco surrogate animal model for oral cancer. A total of 42 Wistar rats were distributed into four experimental groups: Control group, periodontitis (Perio) group, 4-nitroquinoline 1-oxide (4-NQO) group and 4NQO/Perio group. Periodontitis was stimulated by placing a ligature subgingivally, while oral carcinogenesis was induced by systemic administration of 4NQO in the drinking water for 20 weeks. It was observed that the Perio, 4NQO and 4NQO/Perio groups presented with significantly higher alveolar bone loss compared with that in the control group. Furthermore, all groups receiving 4NQO developed lesions on the dorsal surface of the tongue; however, the 4NQO/Perio group presented larger lesions compared with the 4NQO group. There was also a modest overall increase in the number of epithelial dysplasia and OSCC lesions in the 4NQO/Perio group. Notably, abnormal focal activation of cellular differentiation (cytokeratin 10-positive cells) that extended near the basal cell layer of the mucosa was observed in rats receiving 4NQO alone, but was absent in rats receiving 4NQO and presenting with periodontal disease. Altogether, the presence of periodontitis combined with 4NQO administration augmented tumor size in the current rat model and tampered with the protective mechanisms of the cellular differentiation of epithelial cells
