    Mycophenolic acid formulations in adult renal transplantation – update on efficacy and tolerability

    The description more than 30 years ago of the role of de novo purine synthesis in T and B lymphocytes clonal proliferation opened the possibility for selective immunosuppression by targeting specific enzymatic pathways. Mycophenolic acid (MPA) blocks the key enzyme inosine monophosphate dehydrogenase and the production of guanosine nucleotides required for DNA synthesis. Two MPA formulations are currently used in clinical transplantation as part of the maintenance immunosuppressive regimen. Mycophenolate mofetil (MMF) was the first MPA agent to be approved for the prevention of acute rejection following renal transplantation, in combination with cyclosporine and steroids. Enteric-coated mycophenolate sodium (EC-MPS) is an alternative MPA formulation available in clinical transplantation. In this review, we will discuss the clinical trials that have evaluated the efficacy and safety of MPA in adult kidney transplantation for the prevention of acute rejection and their use in new combination regimens aiming at minimizing calcineurin inhibitor toxicity and chronic allograft nephropathy. We will also discuss MPA pharmacokinetics and the rationale for therapeutic drug monitoring in optimizing the balance between efficacy and safety in individual patients

    A Computational Efficient Nyquist Shaping Approach for Short-Reach Optical Communications

    [EN] Recently, Half-Cycle Nyquist Subcarrier Modulation (HC-SCM) was proposed to achieve high spectral efficiency in intensity-modulator direct-detection optical links. This paper shows that the HC-SCM scheme has a high computational load and proposes the rational Oversampled Subcarrier Modulation (OVS-SCM) as a computational efficient alternative that, furthermore, improves the spectral efficiency.     Use of the atmospheric generators for capnophilic bacteria Genbag-CO2 for the evaluation of in vitro Plasmodium falciparum susceptibility to standard anti-malarial drugs

    Background: The aim of this study was to evaluate the cultivation system in which the proper atmospheric conditions for growing Plasmodium falciparum parasites were maintained in a sealed bag. The Genbag (R) system associated with the atmospheric generators for capnophilic bacteria Genbag CO2 (R) was used for in vitro susceptibility test of nine standard anti-malarial drugs and compared to standard incubator conditions. Methods: The susceptibility of 36 pre-identified parasite strains from a wide panel of countries was assessed for nine standard anti-malarial drugs (chloroquine, quinine, mefloquine, monodesethylamodiaquine, lumefantrine, dihydroartemisinin, atovaquone and pyrimethamine) by the standard 42-hour H-3-hypoxanthine uptake inhibition method using the Genbag CO2 (R) system and compared to controlled incubator conditions (5% CO2 and 10% O-2). Results: The counts per minute values in the control wells in incubator atmospheric conditions (5% CO2 and 10% O-2) were significantly higher than those of Genbag (R) conditions (2738 cpm vs 2282 cpm, p < 0.0001). The geometric mean IC50 estimated under the incubator atmospheric conditions was significantly lower for atovaquone (1.2 vs 2.1 nM, p = 0.0011) and higher for the quinolines: chloroquine (127 vs 94 nM, p < 0.0001), quinine (580 vs 439 nM, p < 0.0001), monodesethylamodiaquine (41.4 vs 31.8 nM, p < 0.0001), mefloquine (57.5 vs 49.7 nM, p = 0.0011) and lumefantrine (23.8 vs 21.2 nM, p = 0.0044). There was no significant difference of IC50 between the 2 conditions for dihydroartemisinin, doxycycline and pyrimethamine. To reduce this difference in term of anti-malarial susceptibility, a specific cut-off was estimated for each drug under Genbag (R) conditions by regression. The cut-off was estimated at 77 nM for chloroquine (vs 100 nM in 10% O-2), 611 nM for quinine (vs 800 nM), 30 nM for mefloquine (vs 30 nM), 61 nM for monodesethylamodiaquine (vs 80 nM) and 1729 nM for pyrimethamine (vs 2000 nM). Conclusions: The atmospheric generators for capnophilic bacteria Genbag CO2 (R) is an appropriate technology that can be transferred to the field for epidemiological surveys of drug-resistant malaria. The present data suggest the importance of the gas mixture on in vitro microtest results for anti-malarial drugs and the importance of determining the microtest conditions before comparing and analysing the data from different laboratories and concluding on malaria resistance

    Apuntes sobre algunas aves nidificantes en el macizo del Penyagolosa (Castellón)

    El macizo del Penyagolosa es una de las zonas más ricas en especies de aves de la Comunidad Valenciana, ya que cuenta con una importante masa boscosa en muy buen estado de conservación con una alta diversidad. A pesar de esta riqueza, no hay prácticamente trabajos que describan su ornitocenosis o que den detalles sobre las poblaciones de aves que lo habitan. Para estudiar esta ornitocenosis se puso en marcha una campaña de anillamiento durante 7 primaveras en la fuente del mas de la Cambreta y se plantearon una serie de censos en esa misma área. Las campañas de anillamiento recogieron datos de 29 especies, siendo las más abundantes el petirrojo europeo, el reyezuelo listado y el carbonero garrapinos. Se aportan parámetros de la población reproductora como fechas de cría, sex-ratio, tasa de recaptura interanual y biometría. Los datos apuntan a una temporada de nidificación tardía con una única nidada. En base a los transectos, se calculan algunos índices ecológicos que indican un medio con una alta diversidad y una buena estructura de las poblaciones de aves. Aparecen especies poco comunes en la provincia como la curruca mosquitera, el mosquitero común, el zorzal común o el verderón serrano.The Penyagolosa massif is one of the bird species richest areas in the Comunitat Valenciana region, since it has an important forested area with high plant diveristy. Despite this richness, there is almost no works describing its ornithocenosis or that give detalis of the bird populations that inhabit here. To study this ornithocenosis we conducted a ringing campaign along 7 springs at the Font del mas de la Cambreta and conducted some censues in this same area. Ringing campaigns gathered data on 29 species, the most abundant were the European Robin, Common Firecrest and Coal Tit. We provide parameters of the breeding population as breeding dates, sex-ratio, interannual capture rates and biometry. Data gathered point to a delayed breeding season and to a single breeding attempt. Based on transects, we calculate some ecological indexes that point to an area with high diversity and a good structure of the bird populations. We detected some scarce species in the Province as Garden Warbler, Common Chiffchaff, Song Trhush or Citril Finch

    La presente investigación titulada “Nivel de satisfacción laboral de los trabajadores de la Sede Central del Ministerio de Transportes y Comunicaciones del Perú – Lima, 2014, se llevó a cabo con la finalidad de determinar el nivel de satisfacción laboral de los trabajadores de la Sede Central del Ministerio de Transportes y Comunicaciones. Debido a las características de los aspectos a investigar se consideró apropiado emplear la investigación de tipo básica y de nivel descriptivo. En la investigación se siguió el diseño no experimental, transversal. La población estuvo conformada por los 3000 Trabajadores de la sede central del Ministerio de Transporte y Comunicaciones del Perú, siendo la muestra conformada por 150 Trabajadores. La técnica que se utilizó para la recolección de datos es la encuesta; El instrumento que se utilizó fue el cuestionario que Consta de 33 ítems. Llegando a concluir que el nivel de satisfacción laboral de los trabajadores de la sede central del Ministerio de Transportes y Comunicaciones del Perú – Lima, 2014 es alta; así lo confirman el 58% de los encuestados

However, this relationship is not exempt from methodological problems that may bias the establishment of direct effects between the variables studied. Thus, further research is necessary to investigate the role of socioeconomic variables, their composition and distribution according to health status, particularly on non-communicable diseases

    COVERED DISTANCES OF HANDBALL PLAYERS OBTAINED BY AN AUTOMATIC TRACKING METHOD

    The aim of this work is to obtain the distances covered by handball players and their velocities during a match using a new approach based on automatic tracking method described in Figueroa et. al. (2006a, 2006b) and the Adaboost detector (Okuma, 2004). A whole game of a Brazilian regional handball championship for players under age of 21 was recorded. Applying the mentioned automatic tracking, the accumulated covered distances and the velocities were calculated for all the players. The results of average covered distances (±SD) in the 1st and 2 nd halves were 2199(±230) and 2453(±214). The results of covered distances and the velocities allow individual and collective analyses of the players by the team staff. The proposed method revealed to be a powerful tool to improve physical analysis of the handball players

    Baroclinic M2 circulation in Algeciras Bay and its implications for the water exchange with the Strait of Gibraltar: Observational and 3-D model results

    The M2 tidal circulation in Algeciras Bay (Strait of Gibraltar) is analyzed using a 3-D, nonlinear, baroclinic, hydrodynamic model, in conjunction with observed data series. Results show the influence of the density stratification on the vertical structure of the M2 currents in Algeciras Bay, although its tidal dynamics shows major differences with respect to the Strait of Gibraltar. Whereas the M2 currents in the Strait present mainly barotropic behavior, the baroclinic effects prevail in Algeciras Bay. A notable finding is the presence of a tidal M2 counter-current system between the upper Atlantic and the lower Mediterranean water layers within the Bay, with amplitudes of up to 25 cm s−1. The interface between the two layers oscillates in antiphase relation with respect to the free-surface elevation, with amplitudes of almost 20 m. The presence of the submarine Algeciras Canyon was found to be determinant in the three-dimensional structure of tidal currents within the Bay, strengthening the baroclinic tidal regime of currents. This situation has quantitative consequences for the flow-exchange processes between Algeciras Bay and the outer Strait, with rates 20 times higher than those obtained when considering only the barotropic behavior, as well as inflow/outflow lateral recirculation volumes during half a tidal cycle that account for more than 20% of the net accumulated volume. This flow-exchange system was found to be affected by the nonlinear interaction processes between the first baroclinic period of resonance of Algeciras Bay and the M2 tide

    Influence of the substrate on the bulk properties of hybrid lead halide perovskite films

    In addition to the known effect of the substrate on the interfacial properties of perovskite films, here we show that the bulk properties of hybrid lead halide perovskite films depend on the type of substrate used for film growth. Despite the relative large film thickness, ∼600 nm, the roughness and nature of the substrate layer (glass, FTO, TiO2 and PEDOT:PSS) affect not just the degree of preferential orientation and crystal grain size but also the lattice parameters of CH3NH3PbI3 films synthesized from the PbCl2 precursor. The obtained changes in lattice parameters indicate that the Pb–Pb distance varies by around 0.7%. We suggest that the substrate roughness and chemical nature determine the concentration of defects mainly by varying the chlorine content and probably by the incorporation of oxygen and iodine vacancies during film nucleation and growth. These differences also have consequences in the observed light induced transformations. Upon laser illumination, the formation of additional defects, most probably related to oxygen, is revealed by 110 and 165 cm−1 Raman peaks. With increasing laser power the chemical transformation into PbOx is clearly identified by the 140 and 275 cm−1 Raman peaks. The irreversible photoluminescence enhancement observed at low power with illumination time, also dependent on the substrate nature, is proposed to be due to the localization of the electron–hole excitons created in the vicinity of the light generated defects.     Early treatment failure during treatment of Plasmodium falciparum malaria with atovaquone-proguanil in the Republic of Ivory Coast

    The increased spread of drug-resistant malaria highlights the need for alternative drugs for treatment and chemoprophylaxis. The combination of atovaquone‐proguanil (Malarone®) has shown high efficacy against Plasmodium falciparum with only mild side-effects. Treatment failures have been attributed to suboptimal dosages or to parasite resistance resulting from a point mutation in the cytochrome b gene. In this paper, a case of early treatment failure was reported in a patient treated with atovaquone-proguanil; this failure was not associated with a mutation in the parasite cytochrome b gene, with impaired drug bioavailability, or with re-infection
