    Leading competitor masters athletes: identities and the road to success

    This project sought to explore identities used by leading competitor master level runners in order for them to continue down the road of success. The following research question was explored: How do leading competitor masters level long distance runners describe their life as a runner, in relation to today, and the context they are training in? The participants were three leading competitor master level athletes (age 50+), who all reside in Sudbury, Ontario. Artefacts brought the project to a deeper level. An inductive thematic analysis was carried out. Themes and sub-themes highlighted specific ways each masters’ athlete navigated their running journey to allow them to maintain their competitiveness as a leading competitor. In conclusion, the study discussed the importance of the athletic identity: how it functioned, shifted, and changed over time to allow the athletes to navigate their other identities and go through processes such as identity management.Master of Arts (MA) in Psycholog

    Intra- and interobserver reproducibility of pelvic ultrasound for the detection and measurement of endometriotic lesions

    STUDY QUESTION: What is the interobserver and intraobserver reproducibility of pelvic ultrasound for the detection of endometriotic lesions? SUMMARY ANSWER: Pelvic ultrasound is highly reproducible for the detection of pelvic endometriotic lesions. WHAT IS KNOWN ALREADY: Transvaginal ultrasound (TVS) has been widely adopted as the first-line assessment for the diagnosis and assessment of pelvic endometriosis. Severity of endometriosis as assessed by ultrasound has been shown to have good concordance with laparoscopy (kappa 0.79). The reproducibility of TVS for assessment of ovarian mobility and pouch of Douglas obliteration using the 'sliding sign' has already been described in the literature. However, there is no available data in the literature to demonstrate the intraobserver repeatability of measurements for endometriotic cysts and nodules. STUDY DESIGN SIZE DURATION: This was a prospective observational cross-sectional study conducted over a period of 12 months. We included 50 consecutive women who were all examined by two operators (A and B) during their clinic attendance. PARTICIPANTS/MATERIALS SETTING METHODS: The study was carried out in a specialist endometriosis centre. We included all consecutive women who had ultrasound scans performed independently by two experienced operators during the same visit to the clinic. The outcomes of interest were the inter- and intraobserver reproducibility for the detection of endometriotic lesions. We also assessed repeatability of the measurements of lesion size. MAIN RESULTS AND THE ROLE OF CHANCE: There was a good level of agreement between operator A and operator B in detecting the presence of pelvic endometriotic lesions (k = 0.72). There was a very good level of agreement between operators in identifying endometriotic cysts (k = 0.88) and a good level of agreement in identifying endometriotic nodules (k = 0.61). The inter- and intraobserver repeatability of measuring endometriotic cysts was excellent (intra-class correlation (ICC) ≥ 0.98). There was good interobserver measurement repeatability for bowel nodules (ICC 0.88), but the results for nodules in the posterior compartment were poor (ICC 0.41). The intraobserver repeatability for nodule size measurements was good for both operators (ICC ≥0.86). LIMITATIONS REASONS FOR CAUTION: Within this cohort, there was insufficient data to perform a separate analysis for nodule size in the anterior compartment. All examinations were performed within a specialised unit with a high prevalence of deep endometriosis. Our findings may not apply to operators without intensive ultrasound training in the diagnosis of pelvic endometriosis. WIDER IMPLICATIONS OF THE FINDINGS: These findings are important because ultrasound has been widely accepted as the first-line investigation for the diagnosis of pelvic endometriosis, which often determines the need for future investigations and treatment. The detection and measurement of bowel nodules is essential for anticipation of surgical risk and planning surgical excision.     The Connected World - Opening Electronic Doors at the National Library of Canada

    For hundreds of years, libraries have been the custodians of the accumulated knowledge of human civilization - collecting it, preserving it, and making it available and accessible to those who need it or want to use it. This tradition, supported by a variety of legislation, is continuing. With the advent of technology, libraries have changed how they do their work but not why they do it. As we move into the 21st century, the National Library of Canada's basic mandate to collect, preserve and provide access to Canada's published heritage remains unchanged. What has changed over the years is the nature of the published heritage. It now includes electronic publications. Technological progress has expanded (and made more ambitious) our information needs, having placed at our disposal a greater access to national and international information resources. Consequently, the National Library of Canada has broadened its mandate. It not only provides access to the country's published heritage. It also endeavours to facilitate access for all Canadians to world-wide networks of information resources. There are many complex challenges related to providing or facilitating an equitable and universal access to these networks. Creating and managing an electronic collection, preservation or archiving of this electronic collection, and investing in a technical infrastructure required to support a large national heritage collection are the three challenges that will be highlighted. The world becomes connected at an economic expense. Some of the costs are related to the lack of standards, others to the continued inoperability of systems. With digitization costs ranging from 2to2 to 6 per page and given the ever decreasing support for cultural agencies, other financial solutions have been sought. The National Library has become engaged in various (government and private industry) partnerships for its multiple digitization activities that include digitizing unique manuscript collections, creating electronic versions of its current and past exhibits, and making available in electronic format some unique bibliographic tools. These activities will be briefly reviewed. World Wide Web, the final destination for the National Library's digital information, is gradually becoming the backbone of the National Library's services and operations and many resources are dedicated to making this happen. The Library's publishing program is now driven by technology and more than one format may be used when publishing electronically. Based on user requirements, some publications are retained in print or at least are available on a print-on-demand basis. Some of the web-related issues that will be reviewed are: equitable access, including accessibility for persons with disabilities and bilingualism (English and French)

    Formation and stability of self-assembled coherent islands in highly mismatched heteroepitaxy

    We study the energetics of island formation in Stranski-Krastanow growth within a parameter-free approach. It is shown that an optimum island size exists for a given coverage and island density if changes in the wetting layer morphology after the 3D transition are properly taken into account. Our approach reproduces well the experimental island size dependence on coverage, and indicates that the critical layer thickness depends on growth conditions. The present study provides a new explanation for the (frequently found) rather narrow size distribution of self-assembled coherent islands.Comment: 4 pages, 5 figures, In print, Phys. Rev. Lett.     Microtesla MRI of the human brain combined with MEG

    One of the challenges in functional brain imaging is integration of complementary imaging modalities, such as magnetoencephalography (MEG) and functional magnetic resonance imaging (fMRI). MEG, which uses highly sensitive superconducting quantum interference devices (SQUIDs) to directly measure magnetic fields of neuronal currents, cannot be combined with conventional high-field MRI in a single instrument. Indirect matching of MEG and MRI data leads to significant co-registration errors. A recently proposed imaging method - SQUID-based microtesla MRI - can be naturally combined with MEG in the same system to directly provide structural maps for MEG-localized sources. It enables easy and accurate integration of MEG and MRI/fMRI, because microtesla MR images can be precisely matched to structural images provided by high-field MRI and other techniques. Here we report the first images of the human brain by microtesla MRI, together with auditory MEG (functional) data, recorded using the same seven-channel SQUID system during the same imaging session. The images were acquired at 46 microtesla measurement field with pre-polarization at 30 mT. We also estimated transverse relaxation times for different tissues at microtesla fields. Our results demonstrate feasibility and potential of human brain imaging by microtesla MRI. They also show that two new types of imaging equipment - low-cost systems for anatomical MRI of the human brain at microtesla fields, and more advanced instruments for combined functional (MEG) and structural (microtesla MRI) brain imaging - are practical.Comment: 8 pages, 5 figures - accepted by JM

    Cryptococcal Meningitis Screening and Community-based Early Adherence Support in People With Advanced Human Immunodeficiency Virus Infection Starting Antiretroviral Therapy in Tanzania and Zambia: A Cost-effectiveness Analysis

    BACKGROUND: A randomized trial demonstrated that among people living with late-stage human immunodeficiency virus (HIV) infection initiating antiretroviral therapy, screening serum for cryptococcal antigen (CrAg) combined with adherence support reduced all-cause mortality by 28%, compared with standard clinic-based care. Here, we present the cost-effectiveness. METHODS: HIV-infected adults with CD4 count <200 cells/μL were randomized to either CrAg screening plus 4 weekly home visits to provide adherence support or to standard clinic-based care in Dar es Salaam and Lusaka. The primary economic outcome was health service care cost per life-year saved as the incremental cost-effectiveness ratio (ICER), based on 2017 US dollars. We used nonparametric bootstrapping to assess uncertainties and univariate deterministic sensitivity analysis to examine the impact of individual parameters on the ICER. RESULTS: Among the intervention and standard arms, 1001 and 998 participants, respectively, were enrolled. The annual mean cost per participant in the intervention arm was US339(95339 (95% confidence interval [CI], 331-347),resultinginanincrementalcostoftheinterventionofUS347), resulting in an incremental cost of the intervention of US77 (95% CI, 6666-88). The incremental cost was similar when analysis was restricted to persons with CD4 count <100 cells/μL. The ICER for the intervention vs standard care, per life-year saved, was US70(9570 (95% CI, 43-211)forallparticipantswithCD4countupto200cells/μLandUS211) for all participants with CD4 count up to 200 cells/μL and US91 (95% CI, 4949-443) among those with CD4 counts <100 cells /μL. Cost-effectveness was most sensitive to mortality estimates. CONCLUSIONS: Screening for cryptococcal antigen combined with a short period of adherence support, is cost-effective in resource-limited settings

    Rapid, B1B_1-insensitive, dual-band quasi-adiabatic saturation transfer with optimal control for complete quantification of myocardial ATP flux

    Purpose: Phosphorus saturation-transfer experiments can quantify metabolic fluxes non-invasively. Typically, the forward flux through the creatine-kinase reaction is investigated by observing the decrease in phosphocreatine (PCr) after saturation of γ\gamma-ATP. The quantification of total ATP utilisation is currently under-explored, as it requires simultaneous saturation of inorganic phosphate (Pi) and PCr. This is challenging, as currently available saturation pulses reduce the already-low γ\gamma-ATP signal present. Methods: Using a hybrid optimal-control and Shinnar-Le-Roux method, a quasi-adiabatic RF pulse was designed for the dual-saturation of PCr and Pi to enable determination of total ATP utilisation. The pulses were evaluated in Bloch equation simulations, compared with a conventional hard-cosine DANTE saturation sequence, before application to perfused rat hearts at 11.7 Tesla. Results: The quasi-adiabatic pulse was insensitive to a >2.5>2.5-fold variation in B1B_1, producing equivalent saturation with a 53% reduction in delivered pulse power and a 33-fold reduction in spillover at the minimum effective B1B_1. This enabled the complete quantification of the synthesis and degradation fluxes for ATP in 30-45 minutes in the perfused rat heart. While the net synthesis flux (4.24±0.84.24\pm0.8 mM/s, SEM) was not significantly different from degradation flux (6.88±26.88\pm2 mM/s, p=0.06p=0.06) and both measures are consistent with prior work, nonlinear error analysis highlights uncertainties in the Pi-to-ATP measurement that may explain a trend suggesting a possible imbalance. Conclusion: This work demonstrates a novel quasi-adiabatic dual-saturation RF pulse with significantly improved performance that can be used to measure ATP turnover in the heart in vivo.Comment: 26 pages, Accepted at Magnetic Resonance in Medicine, 24/11/2020 [This version post reviews
