    An exploration of young people’s narratives of hope following experience of psychosis

    Aims: To expand understandings of how young people with psychosis experience hope. This included to which factors young people attributed changes in their hopefulness and the role played by professionals and others with lived experience. Method: Ten young people recovering from an experience of psychosis were interviewed using narrative methodology. Results: The experience of hope as an overarching strand throughout the narratives had three common elements: sense of belonging, which included social inclusion, the importance of information and the significance of planning and occupation. Professionals played an important role in facilitating small steps forwards. Conclusions: The findings suggest the importance to young people of a sense of belonging and achieving small goals to facilitate hopeful thinking and, for clinicians, the value of supporting new peer relationships and meaningful occupation

    A three-dimensional boundary-layer code was developed for particular application to realistic hypersonic aircraft. It is very general and can be applied to a wide variety of boundary-layer flows. Laminar, transitional, and fully turbulent flows of compressible, reacting gases are efficiently calculated by use of the code. A body-oriented orthogonal coordinate system is used for the calculation and the user has complete freedom in specifying the coordinate system within the restrictions that one coordinate must be normal to the surface and the three coordinates must be mutually orthogonal

    Interactivity between protégés and scientists in an electronic mentoring program

    Interactivity is defined by Henri (1992) as a three-step process involving communication of information, a response to this information, and a reply to that first response. It is a key dimension of computer-mediated communication, particularly in the one-on-one communication involved in an electronic mentoring program. This report analyzes the interactivity between pairs of corporate research scientists (mentors) and university biology students (protégés) during two consecutive implementations of an electronic mentoring program. The frequency and structure of the interactions within each pair were examined to provide context: 542 messages were posted among the 20 mentors and 20 protégés. These messages were formed into 5-10 threads per pair, with 3-4 messages per thread, indicating a high level of interactivity (there were more responses posted than independent messages). Mentor-protégé pairs rated as effective by both mentors and protégés' posted more messages overall, had well-structured threads, had protégés and mentor postings that were similar in topic coverage and message length, and had little overt "management" behavior by mentors. However, there appears to be no clear recipe for successful interaction. Not only are there a variety of factors at play in developing an online relationship in this context, but mentor-protégés pairs can falter at various stages in the process and in various way

    In vivo biological activity of the components of haematoporphyrin derivative.

    The in vivo biological activity of various fractions and components of haematoporphyrin derivative (HpD) have been determined by measuring the depth of necrosis of implanted tumours in mice exposed to light after the administration of standard doses of porphyrins dissolved in alkali. In this assay, haematoporphyrin, hydroxyethylvinyldeuteroporphyrin and protoporphyrin are inactive, but the mono- and di-acetates of haematoporphyrin (which are major components of HpD) and acetoxyethylvinyldeuteroporphyrin are active. However, the situation appears to be more complex than this. The normal method for preparing HpD for injection involves an alkali treatment which causes hydrolysis and elimination of the acetoxy functions, and the only recognized products (haematoporphyrin, hydroxyethylvinyldeuteroporphyrin and protoporphyrin) are inactive in the in vivo assay. It is concluded that the active component here is a porphyrin, possibly a dimer or oligomer, which is retained on the column during the normal separation by HPLC. This conclusion is supported by the observations that (i) the crude material obtained from the spent column is active without further alkali treatment, and (ii) activity develops over 30 min, when HpD or the mono- or diacetates of haematoporphyrin are treated with sodium bicarbonate in aqueous DMSO.     Breakthrough Seizures - Further analysis of the Standard versus New Antiepileptic Drugs (SANAD) study

    Objectives To develop prognostic models for risk of a breakthrough seizure, risk of seizure recurrence after a breakthrough seizure, and likelihood of achieving 12-month remission following a breakthrough seizure. A breakthrough seizure is one that occurs following at least 12 months remission whilst on treatment. Methods We analysed data from the SANAD study. This long-term randomised trial compared treatments for participants with newly diagnosed epilepsy. Multivariable Cox models investigated how clinical factors affect the probability of each outcome. Best fitting multivariable models were produced with variable reduction by Akaike’s Information Criterion. Risks associated with combinations of risk factors were calculated from each multivariable model. Results Significant factors in the multivariable model for risk of a breakthrough seizure following 12-month remission were number of tonic-clonic seizures by achievement of 12-month remission, time taken to achieve 12-month remission, and neurological insult. Significant factors in the model for risk of seizure recurrence following a breakthrough seizure were total number of drugs attempted to achieve 12-month remission, time to achieve 12-month remission prior to breakthrough seizure, and breakthrough seizure treatment decision. Significant factors in the model for likelihood of achieving 12-month remission after a breakthrough seizure were gender, age at breakthrough seizure, time to achieve 12-month remission prior to breakthrough, and breakthrough seizure treatment decision. Conclusions This is the first analysis to consider risk of a breakthrough seizure and subsequent outcomes. The described models can be used to identify people most likely to have a breakthrough seizure, a seizure recurrence following a breakthrough seizure, and to achieve 12-month remission following a breakthrough seizure. The results suggest that focussing on achieving 12-month remission swiftly represents the best therapeutic aim to reduce the risk of a breakthrough seizure and subsequent negative outcomes. This will aid individual patient risk stratification and the design of future epilepsy trials

    An important element of all peatland restorationprojects is a programme of monitoring to checkresults and progress. Several peat projectworkshops identified a demand for technicalguidance on monitoring techniques. So NaturalEngland commissioned this study to: -Review the range of peatland restorationmonitoring techniques available. -Identify those that were consistent, informativeand easily applicable for peatland restorationprojects at a range of scales and budgets.Tables to identify appropriate monitoringtechniques for specific projects are published inthe Technical Information Note TIN097 -Guidelines for monitoring peatland restoration.Further information on these techniques isprovided in this report.The findings of this study have been used to: -Inform the JNCC project to design a researchprogramme on UK Peatland Green House Gasand Carbon Flux. -Develop thinking on monitoring peatlands inthe IUCN UK Peatland Programme. -Inform hydrological monitoring programmes forthe Dartmoor and Exmoor Mires Project.These findings are being disseminated to: -Encourage the use of balanced and consistentapproaches to peatland restoration monitoring. -Develop consistency in monitoring approachesso as to enable possible future collation ofpeatland monitoring data as a single databaseresource.A single database resource of peatlandrestoration would enable more robust analysesof monitoring data to support the developmentand implementation of future support andmanagement techniques for peatlandrestoration

    Prescribing Medications for Chronic Pain Management: The State of Education in Florida Medical Schools and Residency Programs

    The purpose of this study was to assess the state of chronic, non-cancer pain management education in Florida medical schools and residency programs. Surveys were developed and distributed to Florida allopathic and osteopathic medical schools. One survey was specific to students and residents and one to faculty. Confidence in their general education, as well as in treating patients with chronic, non-cancer pain, was measured. A majority of medical students and residents (59.5%), in addition to a majority of faculty (58.1%), agreed or strongly agree that more clinical time needs to be dedicated to chronic, non-cancer pain management education. Students, residents, and faculty suggested offering more lectures, online modules, workshops, and clinical scenarios as ways to improve the current educational environment. Results demonstrate that while Florida medical schools and residency programs provide education on chronic, non- cancer pain management, there is still room for improvement. Increased communication between institutions and standardizing competencies related to pain management may prove beneficial
