Assessing hippocampal functional reserve in temporal lobe epilepsy:A multi-voxel pattern analysis of fMRI data
Assessing the functional reserve of key memory structures in the medial temporal lobes (MTL) of pre-surgical patients with intractable temporal lobe epilepsy (TLE) remains a challenge. Conventional functional MRI (fMRI) memory paradigms have yet to fully convince of their ability to confidently assess the risk of a post-surgical amnesia. An alternative fMRI analysis method, multi-voxel pattern analysis (MVPA), focuses on the patterns of activity across voxels in specific brain regions that are associated with individual memory traces. This method makes it possible to investigate whether the hippocampus and related structures contralateral to any proposed surgery are capable of laying down and representing specific memories. Here we used MVPA-fMRI to assess the functional integrity of the hippocampi and MTL in patients with long-standing medically refractory TLE associated with unilateral hippocampal sclerosis (HS). Patients were exposed to movie clips of everyday events prior to scanning, which they subsequently recalled during high-resolution fMRI. MTL structures were delineated and pattern classifiers were trained to learn the patterns of brain activity across voxels associated with each memory. Predictable patterns of activity across voxels associated with specific memories could be detected in MTL structures, including the hippocampus, on the side contralateral to the HS, indicating their functional viability. By contrast, no discernible memory representations were apparent in the sclerotic hippocampus, but adjacent MTL regions contained detectable information about the memories. These findings suggest that MVPA in fMRI memory studies of TLE can indicate hippocampal functional reserve and may be useful to predict the effects of hippocampal resection in individual patients

La recherche-action porte une double finalitĂ© : transformer la rĂ©alitĂ© et produire des connaissances concernant ces transformations. C’est selon cette mĂ©thodologie qu’une expĂ©rience a Ă©tĂ© menĂ©e au CĂ©gep Limoilou en vue de favoriser la persĂ©vĂ©rance et la rĂ©ussite des garçons aux Ă©tudes collĂ©giales. Des activitĂ©s de groupes de soutien, de tutorat, de formation des enseignants et enseignantes et de marketing social ont Ă©tĂ© mises sur pied au cours des annĂ©es 2002-2003 et 2003-2004. La recherche-action s’est dĂ©roulĂ©e selon un processus d’évaluation continu par un collectif de recherche qui retournait le fruit de ses analyses aux acteurs du milieu qui rajustaient le tir en amĂ©liorant les interventions rĂ©alisĂ©es. L’article prĂ©sente la problĂ©matique initiale, l’articulation de la mĂ©thodologie de recherche-action qu’a expĂ©rimentĂ©e l’équipe de recherche avec les acteurs du milieu ainsi qu’un aperçu des principaux rĂ©sultats sur la rĂ©ussite scolaire des garçons.Action research has a dual purpose – transforming reality and producing knowledge about these transformations. An experiment using this method was conducted at CĂ©gep Limoilou in view of improving perseverance and success among male college students. Support group activities, tutoring, teacher training and social marketing were undertaken over the course of 2002-2003 and 2003-2004. The action research involved continuous evaluation by a research group which conveyed the results of its analyses to the people working in the milieu, who then readjusted their approach and made improvements as they went along. The article presents the initial problem, the breakdown of the action research methodology with the milieu’s players, and a look at the principal results on the scholastic success of male students.La investigaciĂłn-acciĂłn tiene una doble finalidad: transformar la realidad y producir conocimientos relacionados con dicha transformaciĂłn. De acuerdo con esta metodologĂa se llevĂł a cabo una experiencia en el Colegio Limoilou con el fin de favorecer la perseverancia y el Ă©xito de los muchachos que cursan estudios colegiales. Se realizaron actividades de apoyo, de tutorĂa, de formaciĂłn de maestros y maestras y de marketing social entre 2002-2003 y 2003-2004. La investigaciĂłn-acciĂłn se realizĂł segĂşn un proceso de evaluaciĂłn permanente realizado por un colectivo de investigaciĂłn que retornaba el fruto de sus análisis a los actores, quienes reajustaban sus acciones con el fin de mejorar las intervenciones realizadas. El artĂculo presenta la problemática inicial, la articulaciĂłn de la metodologĂa de investigaciĂłn-acciĂłn experimentada por el equipo de investigaciĂłn con el concurso de los actores y presenta una visiĂłn sumaria de los principales resultados concernientes al Ă©xito escolar de los muchachos

Lateral Versus Dorsal Plating for Treating Metacarpal and Phalanx Fractures: A Retrospective Cohort Study
Background: The purpose of this study was to investigate the differences between the dorsal and lateral plate position for metacarpal and phalanx fractures.
Methods: We conducted a retrospective review of prospectively collected patient data for 186 fractures treated by a single surgeon between 2009 and 2011. Rates of plate removal, total arc of postoperative motion (TAM), and patient and injury demographics were tested for association with plating position.
Results: Increasing age, larger plates, and dorsal plating were found in univariate analysis to be associated with decreased TAM and increased plate removal in phalanx but not in metacarpal fractures.
Conclusions: These data suggest that dorsal plating results in decreased TAM and increased rates of plate removal in some fractures. TAM was greater in phalanx fractures treated with lateral plating and therefore this plating technique should be considered when treating these fractures
