Outcomes of specialist discharge coordination and intermediate care schemes for patients who are homeless: analysis protocol for a population-based historical cohort
Introduction People who are homeless often experience poor hospital discharge arrangements, reflecting ongoing care and housing needs. Specialist integrated homeless health and care provision (SIHHC) schemes have been developed and implemented to facilitate the safe and timely discharge of homeless patients from hospital. Our study aims to investigate the health outcomes of patients who were homeless and seen by a selection of SIHHC services. Methods and analysis Our study will employ a historical population-based cohort in England. We will examine health outcomes among three groups of adults: (1) homeless patients seen by specialist discharge schemes during their hospital admission; (2) homeless patients not seen by a specialist scheme and (3)admitted patients who live in deprived neighbourhoods and were not recorded as being homeless. Primary outcomes will be: time from discharge to next hospital inpatient admission; time from discharge to next accident and emergency attendance and 28-day emergency readmission. Outcome data will be generated through linkage to hospital admissions data (Hospital Episode Statistics) and mortality data for November 2013 to November 2016. Multivariable regression will be used to model the relationship between the study comparison groups and each of the outcomes. Ethics and dissemination Approval has been obtained from the National Health Service (NHS) Confidentiality Advisory Group (reference 16/CAG/0021) to undertake this work using unconsented identifiable data. Health Research Authority Research Ethics approval (REC 16/EE/0018) has been obtained in addition to local research and development approvals for data collection at NHS sites. We will feedback the results of our study to our advisory group of people who have lived experience of homelessness and seek their suggestions on ways to improve or take this work further for their benefit. We will disseminate our findings to SIHHC schemes through a series of regional workshops
Endovascular thrombectomy in patients with large core ischemic stroke: a cost-effectiveness analysis from the SELECT study
Background It is unknown whether endovascular thrombectomy (EVT) is cost effective in large ischemic core infarcts.
Methods In the prospective, multicenter, cohort study of imaging selection study (SELECT), large core was defined as computed tomography (CT) ASPECTS(CTP) ischemic core volume (rCBF Results From 361 patients enrolled in SELECT, 105 had large core on CT or CTP (EVT 62, MM 43). 19 (31%) EVT vs 6 (14%) MM patients achieved modified Rankin Scale (mRS) score 0–2 (OR 3.27, 95% CI 1.11 to 9.62, P=0.03) with a shift towards better mRS (cOR 2.12, 95% CI 1.05 to 4.31, P=0.04). Over the projected lifetime of patients presenting with large core, EVT led to incremental costs of 24 665 per QALY. EVT has a higher NMB compared with MM at lower (EVT -76 740) and upper (EVT 57 134) WTP thresholds. PSA confirmed the results and CEAC showed 77% and 92% acceptability of EVT at the WTP of 100 000, respectively. EVT was associated with an increment of 37 901, 22 501 and a gain of 1.62, 2.36 and 2.21 QALYs, respectively. Conclusions In a non-randomized prospective cohort study, EVT resulted in better outcomes in large core patients with higher QALYs, NMB and high cost-effectiveness acceptability rates at current WTP thresholds. Randomized trials are needed to confirm these results. Clinical trial registration NCT0244658
Genome to Phenome: Improving Animal Health, Production, and Well-Being – A New USDA Blueprint for Animal Genome Research 2018–2027
In 2008, a consortium led by the Agricultural Research Service (ARS) and the National Institute for Food and Agriculture (NIFA) published the “Blueprint for USDA Efforts in Agricultural Animal Genomics 2008–2017,” which served as a guiding document for research and funding in animal genomics. In the decade that followed, many of the goals set forth in the blueprint were accomplished. However, several other goals require further research. In addition, new topics not covered in the original blueprint, which are the result of emerging technologies, require exploration. To develop a new, updated blueprint, ARS and NIFA, along with scientists in the animal genomics field, convened a workshop titled “Genome to Phenome: A USDA Blueprint for Improving Animal Production” in November 2017, and these discussions were used to develop new goals for the next decade. Like the previous blueprint, these goals are grouped into the broad categories “Science to Practice,” “Discovery Science,” and “Infrastructure.” New goals for characterizing the microbiome, enhancing the use of gene editing and other biotechnologies, and preserving genetic diversity are included in the new blueprint, along with updated goals within many genome research topics described in the previous blueprint. The updated blueprint that follows describes the vision, current state of the art, the research needed to advance the field, expected deliverables, and partnerships needed for each animal genomics research topic. Accomplishment of the goals described in the blueprint will significantly increase the ability to meet the demands for animal products by an increasing world population within the next decade
Detection of intrinsic source structure at ~3 Schwarzschild radii with Millimeter-VLBI observations of SAGITTARIUS A*
We report results from very long baseline interferometric (VLBI) observations
of the supermassive black hole in the Galactic center, Sgr A*, at 1.3 mm (230
GHz). The observations were performed in 2013 March using six VLBI stations in
Hawaii, California, Arizona, and Chile. Compared to earlier observations, the
addition of the APEX telescope in Chile almost doubles the longest baseline
length in the array, provides additional {\it uv} coverage in the N-S
direction, and leads to a spatial resolution of 30 as (3
Schwarzschild radii) for Sgr A*. The source is detected even at the longest
baselines with visibility amplitudes of 4-13% of the total flux density.
We argue that such flux densities cannot result from interstellar refractive
scattering alone, but indicate the presence of compact intrinsic source
structure on scales of 3 Schwarzschild radii. The measured nonzero
closure phases rule out point-symmetric emission. We discuss our results in the
context of simple geometric models that capture the basic characteristics and
brightness distributions of disk- and jet-dominated models and show that both
can reproduce the observed data. Common to these models are the brightness
asymmetry, the orientation, and characteristic sizes, which are comparable to
the expected size of the black hole shadow. Future 1.3 mm VLBI observations
with an expanded array and better sensitivity will allow a more detailed
imaging of the horizon-scale structure and bear the potential for a deep

Scientists\u27 Warning on Invasive Alien Species
Biological invasions are a global consequence of an increasingly connected world and the rise in human population size. The numbers of invasive alien species - the subset of alien species that spread widely in areas where they are not native, affecting the environment or human livelihoods - are increasing. Synergies with other global changes are exacerbating current invasions and facilitating new ones, thereby escalating the extent and impacts of invaders. Invasions have complex and often immense long-term direct and indirect impacts. In many cases, such impacts become apparent or problematic only when invaders are well established and have large ranges. Invasive alien species break down biogeographic realms, affect native species richness and abundance, increase the risk of native species extinction, affect the genetic composition of native populations, change native animal behaviour, alter phylogenetic diversity across communities, and modify trophic networks. Many invasive alien species also change ecosystem functioning and the delivery of ecosystem services by altering nutrient and contaminant cycling, hydrology, habitat structure, and disturbance regimes. These biodiversity and ecosystem impacts are accelerating and will increase further in the future. Scientific evidence has identified policy strategies to reduce future invasions, but these strategies are often insufficiently implemented. For some nations, notably Australia and New Zealand, biosecurity has become a national priority. There have been long-term successes, such as eradication of rats and cats on increasingly large islands and biological control of weeds across continental areas. However, in many countries, invasions receive little attention. Improved international cooperation is crucial to reduce the impacts of invasive alien species on biodiversity, ecosystem services, and human livelihoods. Countries can strengthen their biosecurity regulations to implement and enforce more effective management strategies that should also address other global changes that interact with invasions
Unraveling Twisty Linear Polarization Morphologies in Black Hole Images
We investigate general relativistic magnetohydrodynamic simulations (GRMHD)
to determine the physical origin of the twisty patterns of linear polarization
seen in spatially resolved black hole images and explain their morphological
dependence on black hole spin. By characterising the observed emission with a
simple analytic ring model, we find that the twisty morphology is determined by
the magnetic field structure in the emitting region. Moreover, the dependence
of this twisty pattern on spin can be attributed to changes in the magnetic
field geometry that occur due to the frame dragging. By studying an analytic
ring model, we find that the roles of Doppler boosting and lensing are
subdominant. Faraday rotation may cause a systematic shift in the linear
polarization pattern, but we find that its impact is subdominant for models
with strong magnetic fields and modest ion-to-electron temperature ratios.
Models with weaker magnetic fields are much more strongly affected by Faraday
rotation and have more complicated emission geometries than can be captured by
a ring model. However, these models are currently disfavoured by the recent EHT
observations of M87*. Our results suggest that linear polarization maps can
provide a probe of the underlying magnetic field structure around a black hole,
which may then be usable to indirectly infer black hole spins. The generality
of these results should be tested with alternative codes, initial conditions,
and plasma physics prescriptions.Comment: 25 pages, 19 figure
The XMM Cluster Survey: X-ray analysis methodology
The XMM Cluster Survey (XCS) is a serendipitous search for galaxy clusters
using all publicly available data in the XMM-Newton Science Archive. Its main
aims are to measure cosmological parameters and trace the evolution of X-ray
scaling relations. In this paper we describe the data processing methodology
applied to the 5,776 XMM observations used to construct the current XCS source
catalogue. A total of 3,675 > 4-sigma cluster candidates with > 50
background-subtracted X-ray counts are extracted from a total non-overlapping
area suitable for cluster searching of 410 deg^2. Of these, 993 candidates are
detected with > 300 background-subtracted X-ray photon counts, and we
demonstrate that robust temperature measurements can be obtained down to this
count limit. We describe in detail the automated pipelines used to perform the
spectral and surface brightness fitting for these candidates, as well as to
estimate redshifts from the X-ray data alone. A total of 587 (122) X-ray
temperatures to a typical accuracy of < 40 (< 10) per cent have been measured
to date. We also present the methodology adopted for determining the selection
function of the survey, and show that the extended source detection algorithm
is robust to a range of cluster morphologies by inserting mock clusters derived
from hydrodynamical simulations into real XMM images. These tests show that the
simple isothermal beta-profiles is sufficient to capture the essential details
of the cluster population detected in the archival XMM observations. The
redshift follow-up of the XCS cluster sample is presented in a companion paper,
