    TricycloDNA-modified oligo-2′-deoxyribonucleotides reduce scavenger receptor B1 mRNA in hepatic and extra-hepatic tissues—a comparative study of oligonucleotide length, design and chemistry

    We report the evaluation of 20-, 18-, 16- and 14-mer phosphorothioate (PS)-modified tricycloDNA (tcDNA) gapmer antisense oligonucleotides (ASOs) in Tm, cell culture and animal experiments and compare them to their gap-matched 20-mer 2′-O-methoxyethyl (MOE) and 14-mer 2′,4′-constrained ethyl (cEt) counterparts. The sequence-matched 20-mer tcDNA and MOE ASOs showed similar Tm and activity in cell culture under free-uptake and cationic lipid-mediated transfection conditions, while the 18-, 16- and 14-mer tcDNA ASOs were moderate to significantly less active. These observations were recapitulated in the animal experiments where the 20-mer tcDNA ASO formulated in saline showed excellent activity (ED50 3.9 mg/kg) for reducing SR-B1 mRNA in liver. The tcDNA 20-mer ASO also showed better activity than the MOE 20-mer in several extra-hepatic tissues such as kidney, heart, diaphragm, lung, fat, gastrocnemius and quadriceps. Interestingly, the 14-mer cEt ASO showed the best activity in the animal experiments despite significantly lower Tm and 5-fold reduced activity in cell culture relative to the 20-mer tcDNA and MOE-modified ASOs.     Commercial Bioinoculants Increase Root Length Colonization and Improve Petiole Nutrient Concentration of Field-grown Grapevines

    Agricultural bioinoculants containing arbuscular mycorrhizal fungi represent a potential opportunity to reduce the dependence of grapevines (Vitis) on agrochemicals. This field study assessed the ability of four commercial bioinoculants to colonize grapevine roots and their effects on petiole nutrient concentration, berry composition, and root morphology of ‘Pinot noir’ (Vitis vinifera) grafted onto rootstock ‘Couderc 3309’ (Vitis riparia × Vitis rupestris) and ‘Riesling’ (V. vinifera) grafted onto ‘Couderc 3309’ and Selection Oppenheim four (Vitis berlandieri × V. riparia). Three bioinoculants increased root mycorrhizal colonization; however, regardless of the treatment, mycorrhizal fungal structures were enhanced. Grapevine petiole nutrient concentration was improved by bioinoculants. Root diameter, root length density, and specific root length increased with greater mycorrhizal root colonization. Using bioinoculants to reduce chemical fertilizers may be a good strategy to improve grapevine productivity and health in cool climates; however, the impact of mycorrhizal bioinoculants in the vineyard may differ among scion–rootstocks, edaphoclimatic conditions, and vineyard soil microbiomes

    Acute Pelvic Ischemia: a Fatal Outcome after Endovascular Aortoiliac Aneurysm Repair with Iliac Branch Device

    Introdução: A oclusão da artéria hipogástrica pode ser necessária na reparação endovascular de aneurismas da aorta abdominal (EVAR). A oclusão intencional da hipogástrica pode ter complicações isquémicas. As endopróteses de bifurcação ilíaca (IBD) surgiram como alternativa endovascular à oclusão da hipogástrica em doentes com elevado risco para isquemia pélvica. Os autores descrevem um caso de oclusão precoce do ramo hipogástrico de IBD com graves consequências clínicas. Caso clínico: Sexo masculino, de 74 anos, com aneurisma da aorta abdominal (diâmetro máximo de 55 mm) com envolvimento de ambas as bifurcações ilíacas e segmentos proximais das hipogástricas (diâmetro máximo de 31 e 32 mm), submetido a EVAR com revascularização hipogástrica esquerda via IBD (Cook Zenith®) e coiling+overstenting da artéria hipogástrica contralateral. O procedimento decorreu sem complicações e a angiografia final mostrava permeabilidade da hipogástrica revascularizada e escassa colateralidade pélvica. O pós-operatório imediato complicou-se de dor lombar e glútea bilateral associada a manifestações cutâneas isquémicas e monoparesia do membro inferior esquerdo. Por agravamento progressivo nas primeiras 24h e angioTC com oclusão do stent da hipogástrica esquerda, procedeu-se novamente a revascularização da hipogástrica, com bom resultado na angiografia final. Apesar da revascularização bem-sucedida, houve agravamento progressivo do estado geral, com isquemia pélvica irreversível e rabdomiólise. Óbito ao 5.◦dia pós-operatório. Conclusão: A isquemia pélvica aguda é uma complicação grave e frequentemente fatal que pode advir da oclusão bilateral das artérias hipogástricas. A falência da revascularização por IBD pode ser fatal, pelo que os autores aconselham um cuidado redobrado no controlo angiográfico final e um baixo limiar para investigação na suspeita de complicações pós-operatórias. Se maior risco de falência técnica, embolização ou escassa colateralidade pélvica, a preservação bilateral de fluxo nas artérias hipogástricas pode estar recomendada

    Isatuximab plus pomalidomide and dexamethasone in relapsed/refractory multiple myeloma patients with renal impairment: ICARIA-MM subgroup analysis

    The randomized, phase 3 ICARIA-MM study investigated isatuximab (Isa) with pomalidomide and dexamethasone (Pd) versus Pd in patients with relapsed/refractory multiple myeloma and ?2 prior lines. This prespecified subgroup analysis examined efficacy in patients with renal impairment (RI; estimated glomerular filtration rate <60 mL/min/1.73 m²). Isa 10 mg/kg was given intravenously once weekly in cycle 1, and every 2 weeks in subsequent 28-day cycles. Patients received standard doses of Pd. Median progression-free survival (PFS) for patients with RI was 9.5 months with Isa-Pd (n = 55) and 3.7 months with Pd (n = 49; hazard ratio [HR] 0.50; 95% confidence interval [CI], 0.30-0.85). Without RI, median PFS was 12.7 months with Isa-Pd (n = 87) and 7.9 months with Pd (n = 96; HR 0.58; 95% CI, 0.38-0.88). The overall response rate (ORR) with and without RI was higher with Isa-Pd (56 and 68%) than Pd (25 and 43%). Complete renal response rates were 71.9% (23/32) with Isa-Pd and 38.1% (8/21) with Pd; these lasted ?60 days in 31.3% (10/32) and 19.0% (4/21) of patients, respectively. Isa pharmacokinetics were comparable between the subgroups, suggesting no need for dose adjustment in patients with RI. In summary, the addition of Isa to Pd improved PFS, ORR and renal response rates

    Ixazomib-lenalidomide-dexamethasone in routine clinical practice: Effectiveness in relapsed/refractory multiple myeloma

    [Aim]: To evaluate the effectiveness and safety of ixazomib-lenalidomide-dexamethasone (IRd) in relapsed/refractory multiple myeloma in routine clinical practice. Patients & methods: Patient-level data from the global, observational INSIGHT MM and the Czech Registry of Monoclonal Gammopathies were integrated and analyzed.[Results]: At data cut-off, 263 patients from 13 countries were included. Median time from diagnosis to start of IRd was 35.8 months; median duration of follow-up was 14.8 months. Overall response rate was 73%, median progression-free survival, 21.2 months and time-to-next therapy, 33.0 months. Ixazomib/lenalidomide dose reductions were required in 17%/36% of patients; 32%/30% of patients discontinued ixazomib/lenalidomide due to adverse events.[Conclusion]: The effectiveness and safety of IRd in routine clinical practice are comparable to those reported in TOURMALINE-MM1.This work was supported by Millennium Pharmaceuticals, Inc., Cambridge, MA, USA, a wholly owned subsidiary of Takeda Pharmaceutical Company Limited

    Treatment of relapsed and refractory multiple myeloma: recommendations from the International Myeloma Working Group

    This Policy Review presents the International Myeloma Working Group's clinical practice recommendations for the treatment of relapsed and refractory multiple myeloma. Based on the results of phase 2 and phase 3 trials, these recommendations are proposed for the treatment of patients with relapsed and refractory disease who have received one previous line of therapy, and for patients with relapsed and refractory multiple myeloma who have received two or more previous lines of therapy. These recommendations integrate the issue of drug access in both low-income and middle-income countries and in high-income countries to help guide real-world practice and thus improve patient outcomes

    Transcriptomic and biochemical investigations support the role of rootstock-scion interaction in grapevine berry quality

    Background In viticulture, rootstock genotype plays a critical role to improve scion physiology, berry quality and to adapt grapevine (Vitis viniferaL.) to different environmental conditions. This study aimed at investigating the effect of two different rootstocks (1103 Paulsen - P - and Mgt 101-14 - M) in comparison with not grafted plants - NGC - on transcriptome (RNA-seq and small RNA-seq) and chemical composition of berry skin inPinot noir, and exploring the influence of rootstock-scion interaction on grape quality. Berry samples, collected at veraison and maturity, were investigated at transcriptional and biochemical levels to depict the impact of rootstock on berry maturation. Results RNA- and miRNA-seq analyses highlighted that, at veraison, the transcriptomes of the berry skin are extremely similar, while variations associated with the different rootstocks become evident at maturity, suggesting a greater diversification at transcriptional level towards the end of the ripening process. In the experimental design, resembling standard agronomic growth conditions, the vines grafted on the two different rootstocks do not show a high degree of diversity. In general, the few genes differentially expressed at veraison were linked to photosynthesis, putatively because of a ripening delay in not grafted vines, while at maturity the differentially expressed genes were mainly involved in the synthesis and transport of phenylpropanoids (e.g. flavonoids), cell wall loosening, and stress response. These results were supported by some differences in berry phenolic composition detected between grafted and not grafted plants, in particular in resveratrol derivatives accumulation. Conclusions Transcriptomic and biochemical data demonstrate a stronger impact of 1103 Paulsen rootstock than Mgt 101-14 or not grafted plants on ripening processes related to the secondary metabolite accumulations in berry skin tissue. Interestingly, theMYB14gene, involved in the feedback regulation of resveratrol biosynthesis was up-regulated in 1103 Paulsen thus supporting a putative greater accumulation of stilbenes in mature berries
