Die Aussagekraft von prĂ€- und postoperativen MRT-Untersuchungen fĂŒr den EinheilungsprozeĂ operativ rekonstruierter Rotatorenmanschettenrupturen
Die Magnetresonanztomographie wird in der derzeitigen Literatur als diagnostisches Mittel
der Wahl bei Pathologien der Rotatorenmanschette und als wichtigstes Diagnostikum in der
Evaluation von rekonstruierten Rotatorenmanschetten angesehen.
PrĂ€operativ kann die Magnetresonanztomographie Aussagen ĂŒber die Grösse und Lage der
Rotatorenmanschettenruptur und den qualitativen Zustand des Muskel- und Sehnengewebes
liefern.
In der vorliegenden Arbeit wurden bei 32 in die Studie eingeschlossenen Patienten mit 32
Schultern die kernspintomographisch prognostischen Faktoren ermittelt, welche zusammen
mit den erhobenen klinischen und operativen Befunden eine entscheidende Rolle fĂŒr eine
erfolgreiche Rekonstruktion der Rotatorenmanschette darstellen. Im postoperativen
Nachuntersuchungszeitraum, der insgesamt acht Monate betrug wurde jeweils drei und acht
Monate nach der Operation eine kernspintomographische und eine klinische
Kontrolluntersuchung durchgefĂŒhrt.
Durch die detailgetreue Darstellung der gesamten Rotatorenmanschette mittels
standardisierter MRT-Aufnahmen kann nicht nur eine exakte Diagnose gestellt werden,
sondern z.B. bei grossen RM-Defekten eine direkte therapeutische Konsequenz abgeleitet
werden. Bei Massendefekten wird die AbwÀgung zwischen rekonstruierenden
Therapieoptionen (Sehnennaht, Muskeltransposition) und palliativen Massnahmen
(arthroskopisches Debridement, Tuberkuloplastik) entscheidend erleichtert.
Auch im postoperativen Beobachtungszeitraum stellt die Magnetresonanztomographie ein
nichtinvasives, reproduzierbares diagnostisches Mittel von hoher SensitivitÀt und SpezifitÀt
dar, was in dieser Arbeit gezeigt werden konnte.
Durch die Anwendung von zwei postoperativen MR-Kontrolluntersuchungen drei und acht
Monate nach der Operation konnte so eine Aussage ĂŒber den Verlauf des
EinheilungsprozeĂes der rekonstruierten Rotatorenmanschette gemacht werden. Besonders
eignet sich die Magnetresonanztomographie im postoperativen Zeitraum zur Differenzierung
der Patienten mit Rerupturen von den Patienten mit intakten Rotatorenmanschetten, welche
klinisch symptomatisch sind. Dies ist allein durch eine klinische Untersuchung nicht möglich.
Die vorliegende Studie zeigte, daĂ vor allem die ersten Monate nach der Operation eine
entscheidende Phase fĂŒr die Einheilung der rekonstruierten Rotatorenmanschette darstellen.

Mit Hilfe der Magnetresonanztomographie können pathologische VerÀnderungen der
Rotatorenmanschette, welche mit klinischen Symptomen einhergehen, als auch
morphologische VerÀnderungen der Gewebe diagnostiziert werden, die noch klinisch
asymptomatisch sind. Obwohl diese subklinischen ZustÀnde meist noch keine klinische
Relevanz besitzen, sind sie doch von prognostischer Bedeutung fĂŒr die stufenweise
voranschreitende Pathogenese der Rotatorenmanschettenruptur.
Die Verwendung von zwei postoperativen Kontroll- untersuchungen drei und acht Monate
nach der Operation ermöglicht eine Verlaufsbeobachtung des EinheilungsprozeĂes der
rekonstruierten Rotatorenmanschette anhand der ermittelten prognostisch wichtigen MRParameter
in Korrelation mit den zugehörigen klinischen Befunden.
So konnten Rotatorenmanschettenrerupturen im postoperativen Zeitraum frĂŒhzeitig erkannt
werden und ein weiteres konservatives oder operatives Procedere rechtzeitig festgelegt
werden.
Ein signifikanter RĂŒckgang der SignalintensitĂ€t der rekonstruierten Rotatorenmanschette in
der T1- und der T2-Wichtung (p<0,0001), eine ReversibilitÀt der fettigen Infiltration der
Muskulatur (p<0,0001), sowie eine ReversibilitÀt der Muskelatrophie (p<0,05) und eine MRtomographisch
gesicherte IntegritÀt der Rotatorenmanschette, in Verbindung mit steigenden
Constant-Scores kennzeichnen in der postoperativen Periode einen erfolgreichen
EinheilungsprozeĂ.
In der Rerupturgruppe zeigten die genannten MR-Parameter keine rĂŒcklĂ€ufige Tendenz und
unterschieden sich somit signifikant von der Intaktgruppe.
Durch die Anwendung standardisierter prÀoperativer und postoperativer MRT-Protokolle
konnte erstmals die Aussagekraft der prÀoperativen und der beiden postoperativen MRTBefunde
im Verlauf fĂŒr den EinheilungsprozeĂ der rekonstruierten Rotatorenmanschette
beurteilt und miteinander verglichen werden.
Da sich die prÀoperativen MRT-Befunde der T1/T2-SignalintensitÀten, der fettigen
Infiltration des Muskels, der muskulÀren Atrophie und der Rupturgrösse der postoperativ
intakten Rotatorenmanschetten nicht signifikant von denen der postoperativ rerupturierten
Rotatorenmanschetten unterschieden, konnte fĂŒr die prĂ€operativen MRT-Befunde lediglich
eine prognostische Wertigkeit fĂŒr den EinheilungsprozeĂ der Rotatorenmanschette
nachgewiesen werden.
Im Gegensatz dazu ermöglicht die signifikant unterschiedliche AusprÀgung der oben
genannten MRT-Befunde zwischen der Intaktgruppe und der Rerupturgruppe im

postoperativen Zeitraum in Korrelation mit den zugehörigen klinischen Befunden eine exakte
Aussage ĂŒber einen gelungenen EinheilungsprozeĂ der rekonstruierten Rotatorenmanschette.
Vor allem bei postperativ symptomatischen Patienten welche ĂŒber keine Reduktion der
Schmerzsymptomatik, eine anhaltende BewegungseinschrÀnkung oder ein persistierendes
Kraftdefizit berichten ist es empfehlenswert eine MRT-Untersuchung durchzufĂŒhren, um den
Status der rekonstruierten Rotatorenmanschette zu ĂŒberprĂŒfen. Alle kernspintomographischen
Befunde sollten allerdings stets in enger Korrelation mit den klinischen Befunden bewertet
werden
Ost-West-Unterschiede bei der Studien- und Hochschulwahl: Ergebnisse der StudienanfÀngerbefragung an der Martin-Luther- UniversitÀt Halle-Wittenberg im Wintersemester 2008/09
Mit Blick auf die demografischen Ungleichgewichte des deutschen Hochschulsystems stehen ostdeutsche Hochschulen vor der Herausforderung, Strategien zu entwickeln, mit denen es gelingt, sowohl westdeutsche StudienanfĂ€nger zu mobilisieren, als auch ostdeutsche StudienanfĂ€nger in der Region zu halten. An der Martin-Luther-UniversitĂ€t Halle-Wittenberg wird deshalb im Januar und Februar 2009 eine Studierendenbefragung durchgefĂŒhrt, um GrĂŒnde der Studien- und Hochschulwahl sowie die Bedeutung einzelner studienbezogener Informationsquellen in Erfahrung zu bringen. Zudem werden Erkenntnisse ĂŒber die Bewertung der Studienbedingungen in Halle gewonnen. Von besonderem Interesse ist dabei, ob die Gruppen der ostdeutschen und westdeutschen StudienanfĂ€nger durch bedeutsame Unterschiede gekennzeichnet sind. Der Forschungsbericht prĂ€sentiert Ergebnisse dieser Befragung und zieht einige Schlussfolgerungen fĂŒr die strategische Positionierung der Martin-Luther-UniversitĂ€t bzw. Halles als Hochschulstandort. Dabei gliedern sich die AusfĂŒhrungen in folgende Punkte: (1) Soziodemografische Merkmale der Befragten, (2) GrĂŒnde der Studienwahl, (3) GrĂŒnde der Hochschulwahl, (4) genutzte Informationsquellen und deren Bewertung, (5) Erwartungen vor Studienbeginn und Erfahrungen, (6) Bewertung der Studienentscheidung und Wechselabsichten sowie (7) WĂŒnsche fĂŒr Halle als Studienort. Zusammenfassend lĂ€sst sich sagen, dass die Ergebnisse insgesamt die Bedeutung der Martin-Luther-UniversitĂ€t fĂŒr Studienberechtigte in der Region unterstreichen. Zudem bezeugt die weitgehend positive Beurteilung der Studienbedingungen an der Martin-Luther-UniversitĂ€t eine relativ hohe Zufriedenheit mit dem Lehr- und Verwaltungspersonal aber auch mit der inhaltlichen und zeitlichen Abstimmung der Lehrveranstaltungen. Die Diskussion der Ergebnisse gibt darĂŒber hinaus aber auch einige Anhaltspunkte fĂŒr die Positionierung der Martin-Luther-UniversitĂ€t und der Stadt Halle im Wettbewerb um ost- und westdeutsche Studierende bzw. verweist auf interessante Ansatzpunkte fĂŒr vertiefende Studien. Open reduction and internal fixation versus casting for highly comminuted and intra-articular fractures of the distal radius (ORCHID): protocol for a randomized clinical multi-center trial
<p>Abstract</p> <p>Background</p> <p>Fractures of the distal radius represent the most common fracture in elderly patients, and often indicate the onset of symptomatic osteoporosis. A variety of treatment options is available, including closed reduction and plaster casting, K-wire-stabilization, external fixation and open reduction and internal fixation (ORIF) with volar locked plating. The latter is widely promoted by clinicians and hardware manufacturers. Closed reduction and cast stabilization for six weeks is a simple, convenient, and ubiquitously available intervention. In contrast, ORIF requires hospitalization, but allows for functional rehabilitation.</p> <p>Given the lack of randomized controlled trials, it remains unclear whether ORIF leads to better functional outcomes one year after injury than closed reduction and casting.</p> <p>Methods/Design</p> <p>ORCHID (Open reduction and internal fixation versus casting for highly comminuted intra-articular fractures of the distal radius) is a pragmatic, randomized, multi-center, clinical trial with two parallel treatment arms. It is planned to include 504 patients in 15 participating centers throughout Germany over a three-year period. Patients are allocated by a central web-based randomization tool.</p> <p>The primary objective is to determine differences in the Short Form 36 (SF-36) Physical Component Score (PCS) between volar locked plating and closed reduction and casting of intraarticular, comminuted distal radius fractures in patients > 65 years of age one year after the fracture. Secondary outcomes include differences in other SF-36 dimensions, the EuroQol-5D questionnaire, the Disability of the Arm, Shoulder, and Hand (DASH) instrument. Also, the range of motion in the affected wrist, activities of daily living, complications (including secondary ORIF and revision surgery), as well as serious adverse events will be assessed. Data obtained during the trial will be used for later health-economic evaluations. The trial architecture involves a central statistical unit, an independent monitoring institute, and a data safety monitoring board. Following approval by the institutional review boards of all participating centers, conduct and reporting will strictly adhere to national and international rules, regulations, and recommendations (e.g., Good Clinical Practice, data safety laws, and EQUATOR/CONSORT proposals)</p> <p>Discussion</p> <p>To our knowledge, ORCHID is the first multicenter RCT designed to assess quality of life and functional outcomes following operative treatment compared to conservative treatment of complex, intra-articular fractures of the distal radius in elderly patients. Penerapan Pendekatan Kontekstual untuk Meningkatkan Prestasi Belajar Matematika pada Materi Lingkaran Bagi Siswa Kelas VIII C SMP Negeri 1 Karangawen Demak Tahun Pelajaran 2008/2009
Sampai saat ini, pendidikan masih memegang peranan yang sangat penting. Dengan adanya pendidikan, sumber daya manusia dapat berkembang menuju ke arah yang lebih baik dan salah satunyanya adalah dapat dilihat dari prestasi belajar yang telah dicapai oleh peserta didik. Dalam perkembangannya, guru harus memiliki keahlian untuk memilih dan menggunakan metode pengajaran yang sesuai dengan mata pelajaran khususnya matematika serta mengetahui kondisi peserta didik disamping penguasaan keterampilan yang lain karena matematika merupakan mata pelajaran yang dianggap sulit oleh sebagian besar peserta didik. Untuk mengatasi permasalahan tersebut, diperlukan suatu metode pembelajaran yang berguna untuk meningkatkan minat agar aktivitas dan prestasi belajar peserta didik optimal yaitu dengan menggunakan pendekatan kontekstual. Penelitian ini ditempuh melalui 3 siklus, tiap siklus terdiri atas 4 tahapan, yaitu perencanaan, pelaksanaan/tindakan, pengamatan/observasi, dan refleksi. Metode pengambilan data dengan pemanfaatan Lembar Kerja, tes tertulis (evaluasi), lembar pengamatan guru dan lembar pengamatan peserta didik serta angket refleksi peserta didik. Hasil penelitian menunjukkan persentase ketuntasan belajar mengalami peningkatan yaitu dari 86,55 % pada siklus I menjadi 87,14 % pada siklus II dan meningkat lagi menjadi 90,81 % pada siklus III. Aktivitas peserta didik mengalami peningkatan dari persentase 72,5 % pada siklus I menjadi 88,75 % pada siklus II dan meningkat lagi menjadi 90 % pada siklus III. Aktivitas guru yang diperoleh lembar pengamatan aktivitas guru mengalami peningkatan dari persentase 71,6 % pada siklus I menjadi 87,5 % pada siklus II dan meningkat lagi menjadi 92 % pada siklus III. Dari hasil yang diperoleh dapat disimpulkan bahwa dengan penerapan pendekatan kontekstual pada materi lingkaran dapat meningkatkan prestasi belajar dan aktivitas peserta didik kelas VIII?Ă©?ĂĄ C SMP Negeri 1 Karangawen Demak sesuai dengan indikator yang ditentukan. ?Ă©?
Optimal regulatory strategies for metabolic pathways in Escherichia coli depending on protein costs
While previous studies have shed light on the link between the structure of metabolism and its transcriptional regulation, the extent to which transcriptional regulation controls metabolism has not yet been fully explored. In this work, we address this problem by integrating a large number of experimental data sets with a model of the metabolism of Escherichia coli. Using a combination of computational tools including the concept of elementary flux patterns, methods from network inference and dynamic optimization, we find that transcriptional regulation of pathways reflects the protein investment into these pathways. While pathways that are associated to a high protein cost are controlled by fineâtuned transcriptional programs, pathways that only require a small protein cost are transcriptionally controlled in a few key reactions. As a reason for the occurrence of these different regulatory strategies, we identify an evolutionary tradeâoff between the conflicting requirements to reduce protein investment and the requirement to be able to respond rapidly to changes in environmental conditions
Origin and fate of dissolved organic matter in four shallow Baltic Sea estuaries
Coastal waters have strong gradients in dissolved organic matter (DOM) quantity and characteristics, originating from terrestrial inputs and autochthonous production. Enclosed seas with high freshwater input therefore experience high DOM concentrations and gradients from freshwater sources to more saline waters. The brackish Baltic Sea experiences such salinity gradients from east to west and from river mouths to the open sea. Furthermore, the catchment areas of the Baltic Sea are very diverse and vary from sparsely populated northern areas to densely populated southern zones. Coastal systems vary from enclosed or open bays, estuaries, fjords, archipelagos and lagoons where the residence time of DOM at these sites varies and may control the extent to which organic matter is biologically, chemically or physically modified or simply diluted with transport off-shore. Data of DOM with simultaneous measurements of dissolved organic (DO) nitrogen (N), carbon (C) and phosphorus (P) across a range of contrasting coastal systems are scarce. Here we present data from the Roskilde Fjord, Vistula and Ăre estuaries and Curonian Lagoon; four coastal systems with large differences in salinity, nutrient concentrations, freshwater inflow and catchment characteristics. The C:N:P ratios of DOM of our data, despite high variability, show site specific significant differences resulting largely from differences residence time. Microbial processes seemed to have minor effects, and only in spring did uptake of DON in the Vistula and Ăre estuaries take place and not at the other sites or seasons. Resuspension from sediments impacts bottom waters and the entire shallow water column in the Curonian Lagoon. Finally, our data combined with published data show that land use in the catchments seems to impact the DOC:DON and DOC:DOP ratios of the tributaries most.peerReviewe
Neuron-specific alterations in signal transduction pathways associated with Alzheimer's disease
The hallmarks of sporadic Alzheimer's disease (AD) are extracellular amyloid deposits, intracellular neurofibrillary tangles (NFTs), and neuronal death. Hyperphosphorylation of tau is a key factor in the generation of NFTs. Mitogen activated protein kinase 1 (MAPK1) and protein kinase C beta (PRKCB) are thought to play a role in hyperphosphorylation, and PRCKB is thought to be involved in hypoxic stress and vascular dysfunction, and to trigger MAPK phosphorylation pathways. We performed single-cell analyses of neurons with different vulnerabilities to AD-related changes. Using quantitative PCR (qPCR), we measured the levels of MAPK1 and PRKCB transcript in CA1 (high vulnerability), CA2 pyramidal cells from the hippocampus, granule cells from the cerebellum (low vulnerability), and neurons from the brain stem (nucleus tractus spinalis nervi trigemini, characterized by early neurophysiological deficits) at progressive Braak stages compared to age-matched controls. The highly vulnerable CA1 pyramidal neurons were characterized by age- and disease-unrelated increases in PRCKB levels and by age- and disease-related increases in MAPK1 levels. In contrast, low PRKCB levels were found in CA2 pyramidal neurons, and MAPK1 levels were elevated in controls and intermediate AD stages. Both PRKCB and MAPK1 were increased in the late AD stages. MAPK1 and PRKCB levels were low in the brainstem and cerebellum. We propose that alterations in the expression of these two genes occur early in the pathogenesis of AD in a region-specific manner. Tumor Growth Rate Estimates Are Independently Predictive of Therapy Response and Survival in Recurrent High-Grade Serous Ovarian Cancer Patients
This study aimed to assess the predictive value of tumor growth rate estimates based on serial cancer antigen-125 (CA-125) levels on therapy response and survival of patients with recurrent high-grade serous ovarian cancer (HGSOC). In total, 301 consecutive patients with advanced HGSOC (exploratory cohort: n = 155, treated at the Medical University of Vienna; external validation cohort: n = 146, from the Ovarian Cancer TherapyâInnovative Models Prolong Survival (OCTIPS) consortium) were enrolled. Tumor growth estimates were obtained using a validated two-phase equation model involving serial CA-125 levels, and their predictive value with respect to treatment response to the next chemotherapy and the prognostic value with respect to disease-specific survival and overall survival were assessed. Tumor growth estimates were an independent predictor for response to second-line chemotherapy and an independent prognostic factor for second-line chemotherapy use in both univariate and multivariable analyses, outperforming both the predictive (second line: p = 0.003, HR 5.19 [1.73â15.58] vs. p = 0.453, HR 1.95 [0.34â11.17]) and prognostic values (second line: p = 0.042, HR 1.53 [1.02â2.31] vs. p = 0.331, HR 1.39 [0.71â2.27]) of a therapy-free interval (TFI) < 6 months. Tumor growth estimates were a predictive factor for response to third- and fourth-line chemotherapy and a prognostic factor for third- and fourth-line chemotherapy use in the univariate analysis. The CA-125-derived tumor growth rate estimate may be a quantifiable and easily assessable surrogate to TFI in treatment decision making for patients with recurrent HGSOC
