    Infectious diseases and vaccination strategies: how to protect the "unprotectable"?

    Introduction. The circulation of infectious diseases puts small infants too young to be vaccinated at risk of morbidity and mortality, often requiring prolonged hospitalization. Material and Methods. We have reviewed the medical records of children not eligible for vaccination because of age, admitted to hospital for pertussis, measles, or varicella from February 1, 2010, till February 1, 2012. Results. Of the case records scrutinized, 21 were hospitalized for pertussis, 18 for measles, and 32 for varicella. Out of them, 42%, 66%, and 78% diagnosed with, respectively, pertussis, measles, and varicella had a complicated course of the disease. Discussion. To avoid infectious disease circulation, childhood immunization strategies should be adopted, such as vaccination of healthcare givers, adult household contacts, and parents planning to have, or who have had, a newborn baby

    Bifid T waves in leads V2 and V3 in children: a normal variant

    <p>Abstract</p> <p>Introduction</p> <p>The T wave is rarely bifid, apart from patients with long QT syndrome or subjects treated with antiarrhythmic drugs. At times, a U wave partially superimposed upon the T wave is responsible for an apparently bifid T wave. Bifid T waves, in contrast, have been described in normal children in the past, but the phenomenon has not received any attention in recent years, to the extent that it is not mentioned in current textbooks of paediatric cardiology. Aim of the present study was to determine the incidence and clinical counterpart of bifid T waves in a paediatric population.</p> <p>Methods</p> <p>We selected 604 consecutive children free from clinically detectable heart disease; subjects whose electrocardiogram showed a bifid T wave underwent a complete clinical and echocardiographic examination. In addition, the electrocardiograms of 110 consecutive adults have also been analyzed. A T wave was considered as bifid whenever it was notched, being the 2 peaks separated from each other by a notch with duration ≥ 0.02 sec and voltage ≥ 0.05 mV. Moreover, in 7 children with bifid T wave in lead V2 further precordial recordings were obtained: a small electrode was gradually moved from V1 to V3, and 4 additional leads were recorded: 2 between V1 an V2, and 2 between V2 and V3.</p> <p>Results</p> <p>A bifid T wave was observed in 110 children (18,3%), with a relatively age-related incidence; the highest rate of bifid T waves (53%) occurred in the group of 5-year-old children. The bifid T wave was detected only in lead V2 in 51 cases (46,4%), only in lead V3 in 5 cases (4,6%), in both leads V2 and V3 in 50 cases (45,4%), and in leads other than V2 and V3 in 4 cases (3,6%). In the adult group, none of the examined electrocardiograms showed bifid T waves in any lead.</p> <p>In the bifid T wave paediatric population, the echocardiogram did not reveal any abnormality, apart from 3 subjects which had an asymptomatic mitral valve prolapse; a trivial mitral and/or tricuspid regurgitation detected by color Doppler, as well as a patent foramen ovale in infants, were not considered as abnormal findings. The QTc interval was normal in all of the subjects; the average QTc interval was not different in the bifid T wave population (402 ± 46 msec) with respect to the control group (407 ± 39 msec).</p> <p>Conclusion</p> <p>The incidence of bifid T waves in leads V2 and V3 in normal children is high, and awareness of this phenomenon avoids possible misinterpretations leading to a diagnosis of ECG abnormalities.</p

    The EARTHCRUISERS project (EARTH CRUst Imagery for investigating SEismicity, volcanism and marine natural Resources in the Sicilian offshore)

    The EARTHCRUISERS project was developed for the MIUR’s call “Progetti Premiali 2015” by the “Istituto Nazionale di Oceanografia e di Geofisica Sperimentale” (Trieste, Italy) in collaboration with the “Istituto Nazionale di Geofisica e Vulcanologia, Osservatorio Etneo” (Catania, Italy) and “Stazione Zoologica Anton Dohrn” (Naples, Italy). The main goals of the project are: (i) to identify and characterize the main crustal tectonic structures offshore Sicily and the Aeolian Islands, (ii) to better understand the geodynamic processes controlling seismicity and volcanism affecting this region, and (iii) to furnish a useful tool to estimate seismic, tsunamigenic and volcanic hazard in the highly populated coastal sectors. Furthermore, in order to contribute at the Blue Growth objectives, the project aims to analyze some relevant issues related to mineral prospecting offshore, such as the characterization of the hydrothermal systems in the Tyrrhenian Sea and the impact of the exploitation of oil and gas fields on the marine environment in the Sicily Channel. To achieve these objectives the acquisition of multibeam and sidescan sonar, multichannel seismic reflection, magnetic and gravimetric data is planned. Nearly 2500 km of multichannel seismic reflection lines will be acquired during the project in the Marsili Basin (Tyrrhenian Sea) and Mt. Etna offshore. This large amount of data will allow to: better understand the relationship between tectonics and evolution of volcanism; identify active faults and volcanic bodies; better constrain the seismostratigraphic and structural setting of the study areas, and investigate the eventual occurrence of unstable volcanic slopes which could lead to landslide and tsunami. Finally, the deployment offshore southeastern Sicily of a temporary Ocean Bottom Seismometer (OBS) network will carry out for monitoring the natural seismicity in the area of VEGA platform, the largest oil extraction site in Italian seas.     Seismological constraints for the dyke emplacement of the July-August 2001 lateral eruption at Mt. Etna volcano, Italy

    In this paper we report seismological evidence regarding the emplacement of the dike that fed the July 18 - August 9, 2001 lateral eruption at Mt. Etna volcano. The shallow intrusion and the opening of the eruptive fracture system, which mostly occurred during July 12, and July 18, were accompanied by one of the most intense seismic swarms of the last 20 years. A total of 2694 earthquakes (1 ÂŁ Md ÂŁ 3.9) were recorded from the beginning of the swarm (July 12) to the end of the eruption (August 9). Seismicity shows the upward migration of the dike from the basement to the relatively thin volcanic pile. A clear hypocentral migration was observed, well constraining the upwards propagation of a near-vertical dike, oriented roughly N-S, and located a few kilometers south of the summit region. Earthquake distribution and orientation of the P-axes from focal mechanisms indicate that the swarm was caused by the local stress source related to the dike intrusion

    Inter-society consensus for the use of inhaled corticosteroids in infants, children and adolescents with airway diseases

    Background: In 2019, a multidisciplinary panel of experts from eight Italian scientific paediatric societies developed a consensus document for the use of inhaled corticosteroids in the management and prevention of the most common paediatric airways disorders. The aim is to provide healthcare providers with a multidisciplinary document including indications useful in the clinical practice. The consensus document was intended to be addressed to paediatricians who work in the Paediatric Divisions, the Primary Care Services and the Emergency Departments, as well as to Residents or PhD students, paediatric nurses and specialists or consultants in paediatric pulmonology, allergy, infectious diseases, and ear, nose, and throat medicine. Methods: Clinical questions identifying Population, Intervention(s), Comparison and Outcome(s) were addressed by methodologists and a general agreement on the topics and the strength of the recommendations (according to the GRADE system) was obtained following the Delphi method. The literature selection included secondary sources such as evidence-based guidelines and systematic reviews and was integrated with primary studies subsequently published. Results: The expert panel provided a number of recommendations on the use of inhaled corticosteroids in preschool wheezing, bronchial asthma, allergic and non-allergic rhinitis, acute and chronic rhinosinusitis, adenoid hypertrophy, laryngitis and laryngospasm. Conclusions: We provided a multidisciplinary update on the current recommendations for the management and prevention of the most common paediatric airways disorders requiring inhaled corticosteroids, in order to share useful indications, identify gaps in knowledge and drive future research

    Analysis and Evaluation of Ramp Metering: From Historical Evolution to the Application of New Algorithms and Engineering Principles

    In the modern era, characterized by intense urbanization and frequent travel between interconnected communities, the constant expansion of cities, associated with high densities and growing need for traveling, has led to a significant increase in road traffic volumes. More than ever, road traffic today requires effort to be managed effectively in order to improve performance and safety conditions, given the greater probability of unpleasant events such as accidents or road congestion with related delays and the increased stress levels of the user and infrastructure. Fortunately, there are already various engineering tools, such as ramp metering, that can be used for this purpose. Ramp metering allows for achieving the aforementioned desired benefits, including improving mobility, reliability, efficiency, and safety, and even reducing environmental impact. It also has been shown to be cost-effective from the existing literature. Further research will be necessary to strengthen the quality, efficacy, and efficiency of ramp metering, especially considering the fast-paced progress in technology (e.g., connected autonomous vehicles and drones used for surveys) and new challenging scenarios (e.g., congested industrial areas and emergency vehicles). This review’s scope is to present a general overview of principal ramp metering solutions, focusing on current research studies in the last couple of years and highlighting some of the main algorithms used for this purpose, depending on diverse scenarios. With this article, the authors desire to present the subject of ramp metering, providing a general overview of its story, evolution, and recent analytical models

    Autonomous Vehicles: An Analysis Both on Their Distinctiveness and the Potential Impact on Urban Transport Systems

    Autonomous driving is a technological innovation that involves the use of Artificial Intelligence (AI) in the automotive area, representing the future of transport and whose applications will influence the concept of driving and many other features of modern society. Indeed, the introduction of Autonomous Vehicles (AVs) on the market, along with the development of related technologies, will have a potential impact not only on the automotive industry but also on urban transport systems. New mobility-related businesses will emerge, whereas existing ones will have to adapt to changes. There are various aspects that affect urban transport systems: in this work, we highlight how road markings, intersections, and pavement design upgradings have a key role for AVs operation. This work aims to describe how contemporary society will be influenced by Autonomous Vehicles’ spread in regard to urban transport systems. A comprehensive analysis of the expected developments within urban transport systems is hereby presented, and some crucial issues concerning benefits and drawbacks are also discussed. From our studies, it emerges that the detection performed by vehicles is mainly affected by road markings characteristics, especially at intersections. Indeed, the need for a new cross-sections type arise, since vehicles wandering phenomena will be reduced due to AVs position-keeping systems
